Experience of the use of trabectedin (ET-743, Yondelis) in 21 patients with pre-treated advanced sarcoma from a single centre.

AIMS: Trabectedin (ET-743, Yondelis) is a marine-derived alkaloid that has two actions. It binds in the minor groove of DNA resulting in a conformational change; thus potentially altering interactions with transcription factors and other DNA binding proteins and it also interacts with the transcription-coupled nucleotide excision repair machinery to induce lethal double-stranded DNA breaks. In recent phase II trials it has shown considerable activity in the treatment of sarcomas. Here the use of trabectedin in patients with advanced refractory sarcoma from a single institution is presented. M

Subcutaneous veltuzumab, a humanized anti-CD20 antibody, in the treatment of refractory pemphigus vulgaris.

IMPORTANCE B-cell depletion with the anti-CD20 antibody rituximab is highly effective for pemphigus vulgaris (PV) treatment. However, most patients experience relapse, and intravenous rituximab infusions are expensive. Therefore, cost-effective anti-CD20 therapies are desirable.OBSERVATIONS A compassionate-use investigational new drug protocol was approved to administer veltuzumab, a second-generation humanized anti-CD20 antibody, to a patient with refractory PV. Veltuzumab was administered as two 320-mg (188mg/m2) subcutaneous doses 2 weeks apart, resulting in complete remission of disease off

Remdesivir: treatment of COVID-19 in special populations.

Remdesivir (RDV) is the mainstay antiviral therapy for moderate to severe COVID-19. Although remdesivir was the first drug approved for COVID-19, information about its efficacy and safety profile is limited in a significant segment of the population, such as people with underlying diseases, the elderly, children, and pregnant and lactating women. The efficacy and safety profile of RDV in disease progression, renal impairment, liver impairment, immunosuppression, geriatrics, pediatrics, pregnancy, and breastfeeding in COVID-19 patients was evaluated. The databases searched included Embase, Scop

Mitochondrial augmentation of hematopoietic stem cells in children with single large-scale mitochondrial DNA deletion syndromes.

Patients with single large-scale mitochondrial DNA (mtDNA) deletion syndromes (SLSMDs) usually present with multisystemic disease, either as Pearson syndrome in early childhood or as Kearns-Sayre syndrome later in life. No disease-modifying therapies exist for SLSMDs. We have developed a method to enrich hematopoietic cells with exogenous mitochondria, and we treated six patients with SLSMDs through a compassionate use program. Autologous CD34+ hematopoietic cells were augmented with maternally derived healthy mitochondria, a technology termed mitochondrial augmentation therapy (MAT). All pati

Leptin deficiency: clinical implications and opportunities for therapeutic interventions.

The discovery of leptin has significantly advanced our understanding of the metabolic importance of adipose tissue and has revealed that both leptin deficiency and leptin excess are associated with severe metabolic, endocrine, and immunological consequences. We and others have shown that a prominent role of leptin in humans is to mediate the neuroendocrine adaptation to energy deprivation. Humans with genetic mutations in the leptin and leptin receptor genes have deregulated food intake and energy expenditure leading to a morbidly obese phenotype and a disrupted regulation in neuroendocrine an

Current experience in treating invasive zygomycosis with posaconazole.

The treatment of zygomycosis has two cornerstones, namely, surgery and antifungal drugs. In many patients, both need to be applied to achieve treatment success; without treatment, the mortality rate of zygomycosis approaches 100%. Because treatment options are limited, no well-designed randomized clinical trial has been conducted and data are predominantly derived from compassionate-use programmes or case reports. Amphotericin B (AmB) lipid complex (ABLC) was clinically evaluated for efficacy against zygomycosis in a single series and resulted in cure or improvement in 52% and in the stabilizi

Recombinant activated factor VII as a general haemostatic agent: evidence-based efficacy and safety.

BACKGROUND: Recombinant activated factor VII (rfVIIa) is widely used as a prohemostatic agent for patients with congenital and acquired haemophilia. In the last few years it has been increasingly used 'off label' in medical and surgical patients to minimise or control severe or refractory bleeding, in particular when conventional therapy failed or was judged to be insufficient. METHODS: A computer-assisted search of the MEDLINE, EMBASE, and CENTRAL (Cochrane Central Register of Controlled Trials) electronic databases up to July 2006 to identify published randomised controlled trials (RCTs) th

Angiotensin II - A Brief Review and Role in Severe SARS-COV-2 Sepsis.

The renin-angiotensin-aldosterone system (RAAS), whose major vasopressor effector is angiotensin II (ATII), has multiple activities and regulates sodium-water homeostasis and fluid and blood pressure homeostasis. RAAS plays a crucial role in cardiocirculatory shock because it counteracts hypotension and hypovolemia by activating different physiologic responses. Based on the encouraging results of the ATHOS-3 trial, the US Food and Drug Administration and the European Medicines Agency approved the use of ATII for catecholamine-resistant vasodilatory shock. More recently, ATII was used for the c

Stakeholders' Views on Barriers to Research on Controversial Controlled Substances.

Many diseases and disease symptoms still lack effective treatment. At the same time, certain controversial Schedule I drugs, such as heroin and cannabis, have been reputed to have considerable therapeutic potential for addressing significant medical problems. Yet, there is a paucity of U.S. clinical studies on the therapeutic uses of controlled drugs. For example, people living with HIV/AIDS experience a variety of disease- and medication-related symptoms. Their chronic pain is intense, frequent, and difficult to treat. Nevertheless, clinical trials of compassionate management for their chroni

BACKGROUND: Treatment options for patients with urothelial cancer (UC) refractory to platinum and immunotherapy are limited and survival is short. Enfortumab vedotin (EV) is a monoclonal anti-NECTIN4 antibody conjugated to monomethyl auristatin. It was recently approved because of superior survival in comparison to standard-of-care (SOC) chemotherapy. Real-world patients, however, often have worse characteristics than patients included in clinical trials. OBJECTIVE: To analyze the efficacy and safety of EV in a cohort of real-world patients. DESIGN SETTING AND PARTICIPANTS: Retrospective dat

Randomised controlled trial to investigate an online compassionate imagery intervention for improving veterinarian mental wellbeing.

BACKGROUND: Veterinarians are at increased risk of psychological distress due to the regular experience of moral dilemmas, leading to compassion fatigue and burnout. This study explored the efficacy of a 2-week, modified online compassionate imagery intervention for improving veterinarian mental wellbeing. METHODS: A randomised controlled trial design was used with two parallel arms (intervention vs. wait-list control group). One hundred and sixty-two students and qualified veterinarians were randomly allocated to each arm. The intervention group watched a 10-15-minute video every day for 14

Xenotransplantation in living human recipients: Where we stand in 2026: Vital Insights into Xenotransplantation.

Xenotransplantation has emerged as a potential solution to the persistent shortage of donor organs for patients with end-stage organ failure. After decades of preclinical work, recent advances in genetic engineering, immunosuppression, and perioperative management have enabled the first clinical xenotransplant procedures in living human recipients. This inaugural AJT XenoPulse review summarized the current peer-reviewed experience with cardiac, renal, and hepatic xenografts performed under compassionate-use or early clinical trial frameworks. Two porcine heart transplants demonstrated technica
