Outcomes and function of conoid ligament on the basis of postoperative radiographic findings of arthroscopic stabilization for the distal clavicle fractures.

PURPOSE: Distal clavicle fractures are divided into three types according to Neer's classification. These fractures are usually treated with a sling to immobilize the upper extremity, however, the treatment of type 2 fractures remain controversial. We focused on the anatomical basis of these fractures in which disruptions in the conoid ligament led to the distraction between the two bony fragments. In this study, we report an arthroscopic procedure for conoid ligament reconstruction and its therapeutic outcomes, and discuss the function of the reconstructed conoid ligament. HYPOTHESIS: Arthro

Clinically significant skeletal variations of the shoulder and the wrist: role of MR imaging.

Several skeletal variations of the upper extremity may cause painful conditions or precipitate early degenerative changes, either spontaneously or in response to overuse and trauma. Magnetic resonance imaging has proved particularly useful for accurate interpretation of many of these clinically significant skeletal variations; however, the widespread use of MR imaging may have contributed to over-emphasizing their clinical importance, which is still controversial in many cases. We review, illustrate, and discuss clinically significant skeletal variations of the upper extremity as seen on MR im

The morphology of the subacromial and related shoulder bursae. An anatomical and histological study.

Shoulder bursae are essential for normal movement and are also implicated in the pathogenesis of shoulder pain and dysfunction. The subacromial bursa (SAB), within the subacromial space, is considered a primary source of shoulder pain. Several other bursae related to the subcoracoid space, including the coracobrachial (CBB), subcoracoid (SCB) and subtendinous bursa of subscapularis (SSB), are also clinically relevant. The detailed morphology and histological characteristics of these bursae are not well described. Sixteen embalmed cadaveric shoulders from eight individuals (five females, three

Morphological Analysis of the Latissimus Dorsi Tendon Insertion: Implications for Optimizing Tendon Transfer Surgeries in Rotator Cuff Repair.

Latissimus dorsi (LD) tendon transfer is widely used to treat irreparable rotator cuff tears, particularly in cases with posterosuperior and anterosuperior tear patterns. We conducted a comprehensive anatomical analysis of the attachment of the LD tendon to the humerus, focusing on its morphological patterns and morphometric features, which are critical for optimizing the surgery. Dissection of 32 LD tendons in 16 Korean cadavers revealed three insertion patterns: fully combined (40.6%), partially combined (34.4%), and fully separated (25.0%). The mean width, length, and thickness of the tendo

The distal acromial tip fusion: a novel procedure to mitigate acromial stress fractures in high-risk patients undergoing reverse shoulder arthroplasty.

Acromion and scapular spine fractures (ASFs) pose significant challenges in the management of patients undergoing reverse total shoulder arthroplasty and there is a pressing need for effective preventive measures. Current predictive factors for ASF include female sex, osteoporosis, older age, and anatomical considerations such as glenoid medialization and acromial morphology. However, surgical options to mitigate ASF risk are limited. In this study, we propose a novel technique: incorporation of distal acromial tip fusion (DATF) during primary reverse total shoulder arthroplasty procedures. DA

Myositis ossificans around shoulder following military training programme.

The myositis ossificans around shoulder in military recruits are not reported yet. Three young male soldiers presented with complaints of palpable mass at the anterior aspect of shoulder; tenderness around the superior part of deltopectoral groove close to acromioclavicular joint; and restriction of shoulder motion. They also noticed ecchymosis and pain around the coracoid process and anterior shoulder region during regular firing exercises. Plain X-rays and computerized tomography showed extra-capsular, dense, irregular structure in the space between pectoralis and deltoid muscles which corre

Coracoclavicular stabilization using a suture anchor technique.

Multiple fixation options exist for coracoclavicular stabilization, but many are technically demanding and require hardware removal. In the study reported here, we reviewed a specific fixation technique that includes suture anchors moored in the base of the coracoid process. We retrospectively reviewed 24 consecutive cases of patients who underwent coracoclavicular stabilization with a suture anchor for a type III or type V acromioclavicular (AC) joint separation or a group II, type II or type V distal clavicle fracture. Eighteen of the 22 patients had full strength and painless range of motio

Anatomy of the axillary nerve and its relation to inferior capsular shift.

Axillary nerve injury is a recognized complication of the capsular slide procedure for multidirectional instability of the shoulder. Axillary nerve dissection followed by an anterior or posterior capsular shift procedure was carried out on 12 autopsy subjects to observe: (1) the normal relationships of the nerve; (2) its proximity to structures dissected in the procedure; and (3) the effects upon it of both anterior and posterior capsular shift procedures. The axillary nerve arises immediately posterior to the coracoid process and conjoint tendon. It crosses the inferolateral border of the sub

[Anterior recurrent luxation of the shoulder. Postoperative recurrences].

Seventy-nine recurrences have been seen after surgical procedures for recurrent dislocation of the shoulder. There were 58 "true" recurrences, 17 shoulders with anterior instability and 4 with instability in all directions. The "true" recurrences were more frequent in young people with a mean age of 20 years at the time of the first surgical procedure. Two-thirds of them occurred on the dominant side. Most of the cases had been treated by distal displacement or lengthening of the coracoid process (38 Trillat procedures, 8 Latarjet procedures and 8 Oudart procedures). Only two had been treated

Arthroscopic Subscapularis Augmentation Using the Long Head of the Biceps Tendon for Anterior Shoulder Instability.

The limitations of transferring the coracoid process along with the conjoined tendon are coracoacromial arch damage, technical difficulty, and nerve injury. The long head of the biceps tendon (LHBT) proximal transposition technique has a weaker sling effect and a risk of nerve injury. The arthroscopic subscapularis augmentation technique may have risks of shoulder external rotation restriction and subscapularis transection. Herein, we introduce an arthroscopic technique for the transfer of the LHBT for subscapularis augmentation to address these risks. Indications of this technique were patien

Magnetic resonance imaging for supraspinatus muscle atrophy after cuff repair.

Between 1998 to 2000, the shoulders of 17 patients with rotator cuff tears were examined using magnetic resonance imaging preoperatively, and 6 and 12 months after open surgical repair. To assess the course of the supraspinatus muscle atrophy, the easily reproducible magnetic resonance imaging parameters occupation ratio and tangent sign in the Y-shaped view of the supraspinous fossa were evaluated and compared with the clinical examination. Occupation ratio is a quantitative parameter that reflects the area ratio, expressed as a percentage of the supraspinatus muscle belly to the supraspinous

Radiographic Analysis of Glenoid Size and Shape After Arthroscopic Coracoid Autograft Versus Distal Tibial Allograft in the Treatment of Anterior Shoulder Instability.

BACKGROUND: The Latarjet procedure for autograft transposition of the coracoid to the anterior rim of the glenoid remains the most common procedure for reconstruction of the glenoid after shoulder instability. The anatomic glenoid reconstruction using distal tibial allograft has gained popularity and is suggested to better match the normal glenoid size and shape. However, concerns about decreased healing and increased resorption arise when an allograft bone is used. PURPOSE: To use radiological findings to evaluate the arthroscopic reconstruction of the glenoid with respect to the size, shape

Anatomical ligament reconstruction for Neer type IIB distal clavicular fractures using suture buttons without acromioclavicular joint fixation.

BACKGROUND: Coracoclavicular (CC) ligament reconstruction has been reported as an option for distal clavicle fractures with ligament injuries. However, most of the reported techniques use single-bundle reconstruction which is not fully anatomical and sometimes associated with complications such as nonunion, coracoid fractures, or discomfort on the clavicle due to large-diameter drill holes and bulky implants. We therefore developed an anatomical double-bundle reconstruction of the CC ligaments for distal clavicle fractures. The conoid ligament is reconstructed using a suture button, while the

The Arthroscopically Guided Bristow-Latarjet Procedure With Cortical Button Fixation: A Minimum 10-Year Follow-up.

BACKGROUND: Despite improved visualization, the use of arthroscopic surgery to perform the Latarjet procedure has not decreased the rates of complications and glenohumeral osteoarthritis (OA) in the long term. Many of the reported complications are related to the use of screws for bone block fixation with freehand drilling. PURPOSE: To evaluate the long-term (at a minimum 10-year follow-up) clinical and radiological outcomes of the arthroscopic Bristow-Latarjet procedure using a posterior guided drilling technique and suture button for coracoid bone graft fixation.
