Outcomes of surgical paraclavicular thoracic outlet decompression.

BACKGROUND: Thoracic outlet syndrome (TOS) is a constellation of signs and symptoms caused by compression of the neurovascular structures in the thoracic outlet. These structures include the brachial plexus, the subclavian vein, and the subclavian artery, resulting in neurogenic (NTOS), venous (VTOS), and arterial (ATOS) types of TOS, respectively. The purpose of this study was to evaluate the outcomes of paraclavicular surgical decompression for TOS. METHODS: A prospective analysis of patients who underwent surgical decompression for TOS at a newly established center was performed. Diagnosis

Lower extremity nerve decompression in burn patients.

INTRODUCTION: Generalized neuropathy after burn injury is quite common, but the diagnosis and management of peripheral nerve compression, late after injury, can be difficult. Although the release of upper extremity nerves has been reported, the indications, timing, and outcomes of lower extremity nerve decompression, after burn injury, are not known. METHODS: We performed a descriptive, retrospective, 10-year review of elective peripheral nerve decompression in 107 burn patients, at a regional burn center. Data collected included age, injury type, total body surface area, prior fasciotomy/esc

Active chest compression-decompression for cardiopulmonary resuscitation.

BACKGROUND: Active compression-decompression cardiopulmonary resuscitation (ACDR CPR) uses a hand-held suction device, applied mid-sternum, to compress the chest then actively decompress the chest after each compression. Randomised controlled trials testing this device have shown discordant results. OBJECTIVES: To determine the effect of active chest compression-decompression CPR compared to standard chest compression CPR on mortality and neurological function in adults with cardiac arrest treated either in-hospital or out-of-hospital. SEARCH METHODS: We updated the searches of CENTRAL in Th

Mild procedure for lumbar decompression: a review.

Lumbar spinal stenosis (LSS) is characterized by narrowing of the spinal canal with impingement of the spinal cord by surrounding tissues of bones. Current management options for LSS include rest, medications, physical therapy, epidural steroid injections, alternative medicine, and surgical decompression. Because each modality of treatment has its own set of limitations, there is a need for a safe, effective, and cost-saving treatment for LSS. mild is a minimally invasive procedure for treatment of degenerative LSS with ligamentum flavum hypertrophy through percutaneous decompression of the hy

[Treatment of trigeminal neuralgia with microvascular decompression].

Pure microvascular decompression (MVD) can cure (that is, no pain, no medication) primary trigeminal neuralgia (TN) caused by vascular compression in 75% of patients (90% when compression is pronounced), according to a Kaplan-Meier survival study at 15 years. MRI with high resolution evidences neurovascular conflicts with good reliability. The results were found to be significantly better when the prosthesis implanted to maintain the compressive vessel away was not touching the nerve. This argues in favor of a real decompressive mechanism of the MVD procedure, rather than a conduction block. B

Minimally invasive orbital decompression for Graves' ophthalmopathy.

We reviewed a 7-year experience at a tertiary-care, academic medical center with balanced, minimally invasive decompression for Graves' ophthalmopathy, in an effort to define the goals, risks, and outcomes of surgical intervention. Endoscopic medial decompression was performed in 26 patients; 23 underwent lateral decompression as well, and 13 also had inferior decompression. Septoplasty, turbinate reduction, and orbital rim augmentation were performed as needed. The indications for surgery were threat to vision (n = 10) and proptosis with a desire to return to the predisease state (rehabilitat

Radius decompression for treatment of kienböck disease.

The authors present a new concept in the treatment of Kienböck disease. They believe that the favorable outcome obtained with these different procedures (ie, either shortening of the radius or lengthening of the ulna) is due to the surgical decompression on the distal forearm. The procedure presented is called metaphyseal core decompression and it consists of curettage of the distal radius and ulna (the ulna has been excluded for the past 12 years) through a small cortical window. From 1976 to 2000, 48 patients were operated and retrospectively reviewed. Thirty-five were men and 13 were women,
