A higher than expected prevalence of AV nodal reentrant tachycardia in patients receiving implantable cardioverter-defibrillators.

BACKGROUND: Epidemiologic studies have indicated that the prevalence of paroxysmal supraventricular tachycardia (SVT) is approximately two to three of 1000 persons, of whom 50-60% have atrioventricular node reentrant tachycardia (AVNRT). Although SVT has been reported to account for a significant portion of inappropriate shocks in patients receiving implantable cardioverter-defibrillators (ICDs), the incidence of AVNRT is unknown. OBJECTIVE: To define the incidence of AVNRT in patients with ICDs. METHODS AND RESULTS: Of 426 patients followed with an ICD, 15 patients with AVNRT were identifie

Implantable cardioverter defibrillators: do women fare worse than men? Gender comparison in the INTRINSIC RV trial.

INTRODUCTION: Due to limited enrollment of women in previous trials, there is a paucity of data comparing outcome and arrhythmic events in men versus women with implantable cardioverter defibrillators (ICDs). METHODS AND RESULTS: We analyzed outcome of patients in the INTRINSIC RV (Inhibition of Unnecessary RV Pacing with AV Search Hysteresis in ICDs) trial based on gender. Women comprised 19% (293/1530) of the INTRINSIC RV population. Compared with men, women were less likely to have coronary disease, ischemic cardiomyopathy, and hyperlipidemia, and were more likely to have congestive heart

Automatic impedance monitoring and patient alert feature in implantable cardioverter defibrillators: being alert for the unexpected!

Recent advances in implantable cardioverter defibrillator (ICD) technology have enabled implementation of an automatic monitoring feature in ICDs that provides daily measurements of several technical parameters such as battery status, pacing, and high-voltage impedance. The system alerts the patient with an audible alarm to contact the physician in case the measured parameters are not within normal limits. Early detection of intermittent and potentially serious complications justifies routine use of this feature. This report describes a patient with an ICD who died suddenly due to ventricular

Detection of atrial fibrillation during sinus tachycardia induced by exercise in patients with implantable atrial defibrillators.

Accurate detection of atrial fibrillation (AF) is essential for appropriate operation of an implantable atrial defibrillator (IAD). However, during episodes of sinus tachycardia, distinction between AF and sinus rhythm (SR) using the "quiet interval" and "baseline crossing" analysis in the detection algorithm of the IAD may be difficult. The efficacy of this AF detection algorithm was tested in five patients implanted with an IAD (METRIX, Model 3000 or 3020, InControl Inc.) during treadmill exercise testing. The IADs were programmed to Monitor Mode with a wake up cycle of 1 minute for AF detec

Are more complex implantable cardioverter defibrillators associated with an increased mortality? Lessons learned from clinical trials.

We compared 1-year survival in patients receiving implantable cardioverter defibrillators (ICDs) that provide only shock therapy with more advanced ICDs that provide antitachycardia pacing, bradycardia pacing, low-energy cardioversion, and advanced detection algorithms. Outcome in patients with advanced-generation ICD systems was similar or improved compared with outcome in patients receiving ICDs with only monophasic shock.

Management of complications associated with a first-generation endocardial defibrillation lead system for implantable cardioverter-defibrillators.

An automatic cardioverter-defibrillator could be implanted using an endocardial defibrillation lead system (consisting of a tripolar defibrillation electrode catheter in conjunction with an epicostal patch electrode) in 9 of 10 patients with sustained ventricular tachycardia or ventricular fibrillation. Six lead system complications were observed during a follow-up period of 51 +/- 36 weeks. Three catheter electrode conductor fractures occurred and manifested as oversensing and subsequent delivery of inappropriate shocks (1 patient), inability to defibrillate during electrophysiologic testing

MAGNETOcardiography Parameters to Predict Sudden Cardiac Death (MAGNETO-SCD) or Ventricular Events From Implantable Cardioverter Defibrillators.

BACKGROUND: Sudden cardiac death (SCD) caused by ventricular arrhythmias remains a leading cause of mortality worldwide. Current risk stratification for implantable-cardioverter defibrillator (ICD) insertion relies heavily on left ventricular ejection fraction, which has limited discriminatory power. OBJECTIVES: The goal of this study was to evaluate whether established/novel magnetocardiography (MCG) parameters (Rotation Score and Angular Dynamics) were associated with subsequent ICD therapies. METHODS: Magneto-SCD (Magnetocardiography [MCG] Parameters in the Prediction of Future ICD Therap

Different Effects of Defibrillator Shocks on the Myocardial Damage Between Subcutaneous and Transvenous Implantable Cardioverter-Defibrillators.

BACKGROUND: Although the implantable cardioverter defibrillators (ICD) are the only established treatment to prevent sudden cardiac death, previous studies reported that those who receive shock therapies, whether appropriate or inappropriate, have a higher risk of death. However, the difference in the influence of shock therapies between via Transvenous-ICD (TV-ICD) and Subcutaneous-ICD (S-ICD) has not been adequately examined. We aimed to investigate the influence of shocks on myocardium in patients with TV-ICD and S-ICD. METHODS: We enrolled consecutive patients who received a defibrillator

Comparison of subcutaneous and transvenous implantable cardioverter-defibrillators: Insights from a single-center cohort in Taiwan.

BACKGROUND: Transvenous implantable cardioverter-defibrillators (TV-ICDs) are associated with complications from intravenous leads. Subcutaneous ICDs (S-ICDs) were developed as a safer, effective alternative. This study evaluated the efficacy and safety of S-ICDs in an Asian population. METHODS: In this single-center retrospective study, we reviewed adult patients with indications for single-chamber ICDs who did not require pacing or have pace-terminable ventricular tachycardia. Propensity score matching in a 1:2 ratio was applied to compare patients with S-ICDs and TV-ICDs. The primary outco

Safety and Clinical Outcome of Subcutaneous Implantable Cardioverter-Defibrillators in an Asian Population: A Prospective Study.

BACKGROUND: There is limited data on long-term safety and efficacy of subcutaneous implantable cardioverter defibrillator (S-ICD) in Asian patients with a lower body mass index (BMI) due to smaller body build. METHODS: We performed a 2 year prospective study in 5 Asian cities from May 2017 to May 2018 to investigate the safety and feasibility of S-ICD for primary or secondary prevention of sudden cardiac death in an Asian population. RESULTS: We enrolled 75 S-ICD recipients (77% males; age 49.6 ± 16.8 years) with a mean BMI of 24.6 ± 5.0 (range 15.9-40.9) with 67% (50/75) with BMI ≤ 23. The

A refined, minimally invasive, reproducible ovine ischaemia-reperfusion-infarction model using implantable defibrillators: Methodology and validation.

Ischaemic heart disease remains a leading cause of premature mortality and morbidity. Understanding the associated pathophysiological mechanisms of cardiac dysfunction arising from ischaemic heart disease and the identification of sites for new therapeutic interventions requires a preclinical model that reproduces the key clinical characteristics of myocardial ischaemia, reperfusion and infarction. Here, we describe and validate a refined and minimally invasive translationally relevant approach to induce ischaemia, reperfusion and infarction in the sheep. The novelty and refinement in the proc

Do Implantable Cardioverter-Defibrillators Prevent Sudden Cardiac Death in End-Stage Renal Disease Patients on Dialysis?

Chronic kidney disease patients appear to be predisposed to heart rhythm disorders, including atrial fibrillation/atrial flutter, ventricular arrhythmias, and supraventricular tachycardias, which increase the risk of sudden cardiac death. The pathophysiological factors underlying arrhythmia and sudden cardiac death in patients with end-stage renal disease are unique and include timing and frequency of dialysis and dialysate composition, vulnerable myocardium, and acute proarrhythmic factors triggering asystole. The high incidence of sudden cardiac deaths suggests that this population could ben

The Uncertain Benefit from Implantable Cardioverter-Defibrillators in Nonischemic Cardiomyopathy: How to Guide Clinical Decision-Making?

Life-threatening dysrhythmias remain a significant cause of mortality in patients with nonischemic cardiomyopathy (NICM). Implantable cardioverter-defibrillators (ICD) effectively reduce mortality in patients who have survived a life-threatening arrhythmic event. The evidence for survival benefit of primary prevention ICD for patients with high-risk NICM on guideline-directed medical therapy is not as robust, with efficacy questioned by recent studies. In this review, we summarize the data on the risk of life-threatening arrhythmias in NICM, the recommendations, and the evidence supporting the

Usefulness of remote monitoring for the early detection of back-up mode in implantable cardioverter defibrillators.

BACKGROUND: Reversion of an implantable cardioverter defibrillator (ICD) to back-up mode degrades the operating capabilities of the device, puts patients at risk and requires rapid intervention by a manufacturer's technician. AIM: To illustrate the usefulness of remote monitoring of ICDs for the early detection of reversion to back-up mode. METHODS: In our centre, all patients implanted with an ICD, with or without resynchronisation, were offered remote monitoring as soon as the technology became available. Alerts triggered by the remote monitoring system were included prospectively in a reg

Subcutaneous implantable cardioverter defibrillators in patients with left ventricular assist devices: case report and comprehensive review.

BACKGROUND: Left ventricular assist devices (LVAD) are increasingly used in patients with advanced heart failure, many of whom have been or will be implanted with an implantable cardioverter defibrillator (ICD). Interaction between both devices is a matter of concern. Subcutaneous ICD (S-ICD) obtains its signals through subcutaneous vectors, which poses special challenges with regards to adequate performance following LVAD implantation. CASE SUMMARY: We describe the case of a 24-year-old man implanted with an S-ICD because of idiopathic dilated cardiomyopathy, severe biventricular dysfunction

Ventricular tachycardia prediction in patients with implantable cardioverter-defibrillators for primary prevention of sudden cardiac death.

Clinical data analysis of 83 patients with implantable cardioverter-defibrillators (ICDs) for sudden cardiac death (SCD) primary prevention has been done. We revealed 5 parameters associated with the detection of life-threatening ventricular arrhythmias. These parameters formed the basis for constructing a logistic regression model. The model makes it possible to obtain the probability of occurrence of a specific event depending on the severity of the predictive parameters and the degree of its influence (risk of true ventricular arrhythmias detection). Estimating the potential risk of the lif

Inappropriate shocks in single-chamber and subcutaneous implantable cardioverter-defibrillators: a systematic review and meta-analysis.

AIMS: Single-chamber (VR-ICD) and subcutaneous (S-ICD) implantable cardioverter-defibrillators are effective to protect patients against sudden death but expose them to higher risk of inappropriate shock (IS). We sought to quantify the annual rate and influencing factors of ISs in VR- and S-ICDs from the literature. METHODS AND RESULTS: PubMed, Embase, and Cochrane Library were searched for full text articles with IS rates. Poisson distribution estimated proportion of patients with ISs; rates were annualized based on follow-up duration. Random effects meta-analysis accounted for study-to-stud
