Serious Combination of Chronic Non Tamponade Uremic Pericardial Effusion, Hypotension, and Atrial Fibrillation on Living Donor Renal Transplantation: A Case Report and Review of Literature.

Although pericardial effusions are not uncommon in patients with end-stage renal disease, uremic pericardial effusion (UPE) frequently remains unrecognized in the absence of clinical signs and symptoms. We present a case of post-living donor renal transplantation delayed graft function due to asymptomatic undiagnosed chronic nontamponade UPE. The patient developed dramatic intraoperative severe hypotension, electrolyte abnormalities, and atrial fibrillation. Prolonged intraoperative hypotension and allograft hypoperfusion caused mild acute tubular necrosis and postoperative delayed graft funct

Impact of donor age on renal allograft function and survival.

BACKGROUND: The lack of cadaveric donors coupled with a rapidly growing number of potential recipients have stimulated the implementation of several strategies, including the acceptance of older donors, to increase the organ pool and reduce the waiting list for kidney transplantation. However several studies have demonstrated higher incidences of delayed graft function and poor graft outcomes among kidneys harvested from older donors. OBJECTIVE: The objective of this study was to evaluate the influence of donor age on the function and long-term survival of renal allografts. PATIENTS: We perf

Impact of pretransplant body mass index on early kidney graft function.

BACKGROUND: An increase in the number of obese patients on transplantation waiting lists can be observed. There are conflicting results regarding the influence of body mass index (BMI) on graft function. METHODS: We performed a single-center, retrospective study of 859 adult patients who received a renal graft from deceased donors. BMI (kg/m(2)) was calculated from patients' height and weight at the time of transplantation. Kidney recipients were subgrouped into 4 groups, according to their BMI: Groups A (<18.5; n = 57), B (18.6-24.9; n = 565), C (25-29.9; n = 198) and D (>30; n = 39). Primar

Intraoperative Intravenous Administration in Renal Transplantation: A Meta-Analysis of Randomized Controlled Trials.

The effects of intraoperative intravenous (IV) fluids choices on kidney function during renal transplantation remain unclear. This meta-analysis compared the effectiveness of normal saline with that of alternative colloids and crystalloids in end-stage renal disease (ESRD) patients who underwent renal transplantation. We searched PubMed, Embase, and Cochrane Library to April 2025. Randomized control trials comparing normal saline with IV fluids were included. The primary outcome was delayed graft function. Secondary outcomes were total fluid volume, renal function, and perioperative ion change

Does multiorgan versus kidney-only cadaveric organ procurement affect graft outcomes?

INTRODUCTION: The majority of kidney grafts in most European countries still come from deceased donors who provide other organs. We analyzed whether multiorgan procurement portends a worse functional outcome compared with kidney-only harvesting. METHODS: We performed a retrospective analysis of 1043 consecutive brain-dead donor kidney transplantations performed at a single academic institution from September 2002 to June 2011. The graft outcomes using kidney-only donors (n = 243) were compared with multiorgan donor grafts (n = 800) analyzing donor age, gender, cause of death, duration of mech

Hypothermic Machine Perfusion Allows Safe Delay in Kidney Transplantation After Cold Storage.

Background/Objectives: Static cold storage (SCS) remains the standard method of kidney preservation. As a referral transplant center, we frequently receive kidneys initially preserved with SCS and subsequently initiate prolonged hypothermic machine perfusion (HMP) to extend allocation time and optimize recipient matching. The clinical impact of this sequential preservation strategy remains incompletely defined. To compare outcomes between kidneys preserved with SCS followed by prolonged HMP (SCS+HMP) and SCS alone. Methods: This single-center retrospective study included 200 adult recipients o

Time dependency of factors affecting renal allograft survival.

The function of renal transplants can deteriorate at any time posttransplant, but the risks and mechanisms may differ at different times posttransplant. Survival of 522 consecutive cadaveric renal transplant recipients followed for at least 6 mo were analyzed, with patient death censored. The overall risk factors in univariate analysis were acute rejection requiring antibody therapy (AR), delayed graft function, elevated serum creatinine at 6 mo, high panel-reactive antibodies, and donor age > or =55 yr, with borderline effects of recipient age and female gender. These risks were studied in ea

Cold Ischemia Time, Kidney Donor Profile Index, and Kidney Transplant Outcomes: A Cohort Study.

RATIONALE & OBJECTIVE: An average of 3,280 recovered deceased donor kidneys are discarded annually in the United States. Increased cold ischemia time is associated with an increased rate of organ decline and subsequent discard. Here we examined the effect of prolonged cold ischemia time on kidney transplant outcomes. STUDY DESIGN: Retrospective observational study. SETTING & PARTICIPANTS: Recipients of deceased donor kidney transplants in the United States from 2000 to 2018. EXPOSURE: Recipients of deceased donor kidneys were divided based on documented cold ischemia time: ≤16, 16-24, 24-32

Impact of intraoperative goal-directed therapy on perioperative outcomes in kidney transplantation: a multicenter randomized controlled trial.

Appropriate fluid management is crucial in anesthesiologic management during kidney transplantation (KT). Traditional parameters such as blood pressure and central venous pressure are unreliable and weakly supported by guidelines. Goal-directed fluid therapy (GDT) has emerged as a technique for administering fluids and vasoactive drugs based on algorithms to ensure adequate tissue perfusion. Current data suggest GDT may reduce tissue edema and respiratory complications in KT recipients. This multicenter, single-blind randomized controlled trial compared conventional fluid management strategies

Late calcineurin inhibitor withdrawal as a strategy to prevent graft loss in patients with suboptimal kidney transplant function.

UNLABELLED: Chronic allograft nephropathy is a major cause of progressive renal failure in renal transplant recipients. Its etiology is multifactorial and may include both immunologic and nonimmunologic causes. In this observational cohort study we set out to see if calcineurin inhibitor withdrawal would reduce the likelihood of graft loss. METHODS: One hundred and five renal transplant recipients with impaired kidney function (mean serum creatinine 3.0 +/- 0.1 mg/dl) and biopsy-proven chronic allograft nephropathy had the dose of their calcineurin inhibitors, cyclosporine (CSA), or tacrolimu

Relaxin Positively Influences Ischemia-Reperfusion Injury in Solid Organ Transplantation: A Comprehensive Review.

In recent decades, solid organ transplantation (SOT) has increased the survival and quality of life for patients with end-stage organ failure by providing a potentially long-term treatment option. Although the availability of organs for transplantation has increased throughout the years, the demand greatly outweighs the supply. One possible solution for this problem is to extend the potential donor pool by using extended criteria donors. However, organs from such donors are more prone to ischemia reperfusion injury (IRI) resulting in higher rates of delayed graft function, acute and chronic gr

The impact of pretransplantation serum albumin levels on long-term renal graft outcomes.

BACKGROUND: The association between pretransplantation serum albumin concentrations and renal graft outcomes is unclear. Hypoalbuminemia is a strong predictor of cardiovascular and all-cause mortality in chronic renal disease. Protein-energy wasting in dialysis patients is associated with irreversible damage and an increased risk of adverse outcomes. In this retrospective study, we investigated the association between preoperative albumin levels and short-term as well as long-term renal graft outcomes. MATERIALS AND METHODS: We included 375 renal transplant recipients in our center between 19

Long-term allograft survival after kidney transplantation.

BACKGROUND: Technical and medical advances over the past few years have produced an important increase in the functionality of renal allografts. The aim of this study was to identify the factors associated with allograft survival 15 years after transplantation in our series. METHODS: A retrospective study of kidney transplantations was carried out at Reina Sofia Hospital in Cordoba from February 1979 to December 1997, with follow-up through June 2012. A subanalysis of the series was undertaken, and Kaplan-Meier analysis and Cox proportional hazards model regression used to achieve the main ob

Hypothermia for expanded criteria organ donors in kidney transplantation in France (HYPOREME): a multicentre, randomised controlled trial.

BACKGROUND: Expanded criteria donors help to increase graft availability, but provide organs with an increased risk of delayed graft function. We aimed to investigate whether donor hypothermia decreases the risk of delayed graft function compared with normothermia. METHODS: We did this multicentre, randomised, controlled, parallel-arm trial at 53 intensive care units and transplant centres in France. We included expanded criteria donors in whom death was diagnosed based on neurological criteria, in compliance with French law, and the recipients of their kidney grafts. Eligible expanded criter

Machine learning to predict transplant outcomes: helpful or hype? A national cohort study.

An increasing number of studies claim machine learning (ML) predicts transplant outcomes more accurately. However, these claims were possibly confounded by other factors, namely, supplying new variables to ML models. To better understand the prospects of ML in transplantation, we compared ML to conventional regression in a "common" analytic task: predicting kidney transplant outcomes using national registry data. We studied 133 431 adult deceased-donor kidney transplant recipients between 2005 and 2017. Transplant centers were randomly divided into 70% training set (190 centers/97 787 recipien

Donor Hemodynamics as a Predictor of Outcomes After Kidney Transplantation From Donors After Cardiac Death.

Donation after cardiac death is an important source of transplantable organs, but evidence suggests donor warm ischemia contributes to inferior outcomes. Attempts to predict recipient outcome using donor hemodynamic measurements have not yielded statistically significant results. We evaluated novel measures of donor hemodynamics as predictors of delayed graft function and graft failure in a cohort of 1050 kidneys from 566 donors. Hemodynamics were described using regression line slopes, areas under the curve, and time beyond thresholds for systolic blood pressure, oxygen saturation, and shock

Early rejection episodes.

1. Early rejection episodes had an overriding effect on graft survival. One-year graft survival decreased by 18% in LRD transplants and by 27% in cadaver-donor transplants when rejection occurred. Early rejection-free patients had 96% (LRD) and 87% (cadaver-donor) 1-year graft survival. 2. Among rejection-free recipients of first cadaver transplants, 1-year graft survival decreased by 5% when function was delayed for 8-14 days and by 10% when function was delayed more than 14 days, suggesting some rejections were masked by early nonfunction. 3. Rejections occurred in 30% of first cadaver donor

Machine Perfusion as a Temporal and Resuscitative Bridge in Emergent Renal Autotransplantation: A Novel Case Report.

BACKGROUND Renal autotransplantation is a well-described but infrequently used alternative to nephrectomy, complex renal or ureteral reconstruction, and allograft transplantation. While commonly performed and reported in the elective setting, it can be limited by ischemia, hemodynamic stability, and operative timing. Its use in the emergent setting can be limited by the patient's clinical stability and by the inability to safely perform immediate reimplantation. Hypothermic machine perfusion has been well described as a tool to improve outcomes following deceased donor transplantation. However
