Efficacy and safety of a low-flow veno-venous carbon dioxide removal device: results of an experimental study in adult sheep.

INTRODUCTION: Extracorporeal lung assist, an extreme resource in patients with acute respiratory failure (ARF), is expanding its indications since knowledge about ventilator-induced lung injury has increased and protective ventilation has become the standard in ARF. METHODS: A prospective study on seven adult sheep was conducted to quantify carbon dioxide (CO2) removal and evaluate the safety of an extracorporeal membrane gas exchanger placed in a veno-venous pump-driven bypass. Animals were anaesthetised, intubated, ventilated in order to reach hypercapnia, and then connected to the CO2 remo

Short-term oral estriol for cervical stenosis, labial adhesion, and challenging intrauterine contraceptive device removal in postmenopausal women: A case series.

Cervical stenosis and labial adhesion associated with vulvovaginal atrophy (VVA) obstruct access to the uterine cavity and reduce quality of life. Removal of intrauterine contraceptive devices (IUD) is challenging without adequate cervical ripening in postmenopausal women. Local estrogen administration and mechanical dilatation are performed to improve these conditions; however, alternatives are necessary for those who have difficulty with vaginal medication. We report five cases of short-term oral estriol administration in postmenopausal women. Case 1 was a 60-year-old woman with an endometri

Five-Year Remission of Hidradenitis Suppurativa Following the Removal of a Levonorgestrel Intrauterine Device and Disease Overview for Obstetrics and Gynecology Providers.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease that is characterized by recurrent, painful nodules, sinus tract formation, and scarring. We report the case of a 20-year-old female with the abrupt onset of Hurley stage I HS following the implantation of a levonorgestrel intrauterine device (IUD) and complete remission of disease following the removal of the device one year later. To our knowledge, this is the first documented case of long-term remission of HS after the removal of levonorgestrel IUD. We also provide a concise overview of HS and suggested primary interventio

Study on the removal characteristics of different air pollution control devices for condensable particulate matter in coal-fired power plants.

This study reports the emissions of condensable particulate matter (CPM) and filterable particulate matter (FPM) in two coal-fired power plants with different air pollution control devices (APCDs). The mechanisms of CPM removed by existing APCDs in coal-fired power plants were explored, and a series of analyses were also carried out on the composition and characteristics of CPM. The results show that the removal efficiencies to CPM by electrostatic-bag-precipitator (EBP) and ESP are 77.34% and 79.23%, respectively, so the difference is not obvious because the interception filtration mechanisms

An ingested mobile phone in the stomach may not be amenable to safe endoscopic removal using current therapeutic devices: A case report.

INTRODUCTION: This case report is intended to inform clinicians, endoscopists, policy makers and industry of our experience in the management of a rare case of mobile phone ingestion. PRESENTATION OF CASE: A 29-year-old prisoner presented to the Emergency Department with vomiting, ten hours after he claimed to have swallowed a mobile phone. Clinical examination was unremarkable. Both initial and repeat abdominal radiographs eight hours later confirmed that the foreign body remained in situ in the stomach and had not progressed along the gastrointestinal tract. Based on these findings, upper e

Non-invasive removal of the Smart tracheal occlusion device for fetal congenital diaphragmatic hernia: a single-arm, open-label, phase 1 study.

BACKGROUND: One of the drawbacks of fetoscopic endoluminal tracheal occlusion (FETO) for congenital diaphragmatic hernia (CDH) is the need for a second intervention to establish airway patency. In this first-in-human study, we aimed to evaluate a novel tracheal occlusion device (Smart-TO) that spontaneously deflates when in close proximity to a strong magnetic field. METHODS: We conducted two parallel single-centre, single-arm, open-label trials in Paris, France, and Leuven, Belgium. Eligibility criteria included patient age 18 years or more, singleton pregnancy with CDH, and severe or modera

The "Subtle" connection between development of cardiac implantable electrical device infection and survival after complete system removal: An observational prospective multicenter study.

BACKGROUND: Despite the improvements in transvenous lead extraction (TLE), patients with cardiac implantable device related infection (CIEDI) have a poor prognosis at long term. We explored the possible role of factors associated with development of CIEDI as predictors of post-TLE survival. METHODS: We performed a multi-center prospective observational study in a population of consecutive patients referred for TLE for CIEDI. We adopted a previously developed 10-point scale for CIEDI risk stratification and assessed its performance in predicting post-TLE survival. RESULTS: We enrolled 169 con

A single center's experience with Candida parapsilosis related long-term central venous access device infections: the port removal decision and its outcomes.

Pediatric cancer patients have an increased risk of potentially life-threatening fungal infections such as Candida parapsilosis, associated with long-term CVADs. The Infectious Diseases Society of America (IDSA) guidelines on Candida catheter-related bloodstream infections recommend systemic antifungal therapy and catheter removal. In this study, we focused on our experience with antifungal failure due to totally implanted catheter-associated C. parapsilosis bloodstream infections. We investigated cases leading to port removal in pediatric malignancy patients and the associated patient outcome

Secondary residual shunt after atrial septal defect closure with an amplatzer occluder: surgical removal and evaluation of device biocompatibility after 7 years.

Interventional occluder implantation has become the preferred mode of treatment in children with clinically significant atrial septal defect (ASD). The continuous increase in the number of patients treated calls for long-term data on device integrity and biocompatibility. We report a child who underwent successful interventional ASD closure at age 5 years. Secondary residual shunt occurred after 3 years of follow-up, and surgical ASD closure became necessary at age 12 years. The Amplatzer occluder device, which was removed after 7 years in vivo, showed no signs of mechanical failure, corrosion

Bacterial endocarditis presenting with unilateral renal artery occlusion treated with the Rinspirator Thrombus Removal System rinsing and thrombectomy device and suction thrombectomy.

We report a 36-year-old man who presented with unilateral flank pain caused by renal artery occlusion with ischemia and infarction from septic emboli secondary to bacterial endocarditis. We treated the occlusion with a novel rinsing and aspiration device, the Rinspirator Thrombus Removal System (ev3, Plymouth, Minn) and suction thrombectomy, which resulted in significant revascularization of the kidney and relief of symptoms. Postprocedural imaging demonstrated marked improvement in renal vascularization, with only small areas of infarction. This technique may be useful in patients where the e

The SaRePo protocol: A seven-step strategy to minimize complications potentially related to the removal of totally implanted central venous access devices.

Removal of totally implanted central venous access devices (brachial ports, chest-ports, femoral ports) is potentially associated with the risk of untoward events, some of them negligible (prolonged maneuver time due to technical difficulties), some relevant (hematoma), and some severe (embolization of catheter fragments into the circulation). The removal technique suitable for minimizing such complications has been described only in few manuals, but it has never been standardized. This paper describes a standardized protocol (SaRePo: Safe Removal of Ports) which consists of seven basic strate

A novel surgical technique for removal of intramedullary nails when there is a failure of the proximal extraction device: Report of two cases and review of literature.

Diaphyseal fractures of the tibia is a common injury with an incidence of 16.9/100,000 population. Surgical treatment with intramedullary interlocking nail is the treatment of choice in closed diaphyseal tibial injuries. Removal of the nail is required in non-union, infected implant, implant failure or during exchange nailing procedure. Implant removal surgery presents some unique challenges to the Orthopaedic surgeons like broken nails, broken screws and failure of the extraction devices to engage with the threads of the nail due to stripping of the threads in the proximal part of the nail.
