Short-Interval Two-Stage Treatment of a Comminuted Open Tibial Diaphyseal Fracture.

Open comminuted tibial fractures caused by gunshot injuries remain challenging due to the high risk of infection, bone loss, and prolonged functional impairment. We report the case of a young male patient who sustained a severe open comminuted tibial diaphyseal fracture with a large segmental bone defect following a gunshot injury. The patient was managed using a short-interval two-stage approach, consisting of early distraction osteogenesis with an external fixator followed by timely conversion to intramedullary fixation combined with autologous bone grafting. This strategy allowed effective

The Biomechanical Behavior of Fixation Options in Short Segment Radius and Ulna Diaphyseal Fractures.

PURPOSE: Plate fixation of radius and ulna shaft fractures has evolved due to a more complete understanding of the biology and mechanics. Screw fixation across six cortices has been adopted as the standard, purported to provide the optimal biological and mechanical environment for healing; however, recent investigations have proposed that four cortices of fixation may be sufficient. These recommendations are intended for segments of optimal length; however, it is less clear how these options perform in short segment fixation. Our objective was the determine the biomechanical behavior of fixati

Classification of diaphyseal tumors based on residual medullary cavity length for prosthetic reconstruction.

Diaphyseal prosthetic reconstruction for bone tumors continues to face challenges with failures due to the lack of evidence-based classification systems. This study aimed to develop and validate a novel classification framework to optimize reconstruction strategies through a retrospective analysis of 112 patients undergoing diaphyseal prosthetic reconstruction between 2010-2021. Diaphyseal tumors were classified into five types according to anatomical location and residual medullary cavity length, with each category corresponding to a tailored prosthetic reconstruction strategy. Clinical outco

Chondroblastoma with secondary aneurysmal bone cyst of the diaphyseal ulna: A rare case report.

Chondroblastomas are rare benign cartilaginous neoplasms that primarily affect young patients. Aneurysmal bone cysts (ABCs) are also rare, accounting for 1-2% of all bone tumors, and can arise either idiopathically or as secondary lesions associated with other tumors, such as giant cell tumors or chondroblastomas. Here, we present a rare case of a secondary aneurysmal bone cyst arising in conjunction with a chondroblastoma located in the left diaphysis of the ulna.

Successful Reimplantation of Extruded Ulnar Diaphyseal Segment: A Case Report and Review of Literature.

CASE: A 32-year-old man presented with a type II open both-bone forearm fracture and segmental bone loss because of complete extrusion of a diaphyseal fragment (3 cm) of ulna. The patient presented to our level 1 trauma center after a motor vehicle collision. The extruded segment underwent sterilization and immediate reimplantation with internal fixation approximately 6 hours after arrival. Our patient achieved union by 7-month follow-up, demonstrated excellent functional outcomes, and was free from infection at 1-year follow-up. CONCLUSION: In select cases, successful reimplantation can be a

A 40-year Study of the Factors Associated with Diaphyseal Forearm Fractures in Skiers and Snowboarders.

BACKGROUND: Skiing and snowboarding are popular sports that are associated with a high number of orthopaedic injuries. Diaphyseal forearm fractures are an important subset of these injuries. To our knowledge, factors associated with these injuries, the mechanisms that cause them, and their relative frequencies in skiers and snowboarders have not been described. In addition, it has been proposed that the use of wrist guards may increase the risk of sustaining a diaphyseal forearm fracture; therefore, we sought to explore the relationship between wrist guard use and diaphyseal forearm fractures.

Unplanned Return to the Operating Room (UpROR) After Pediatric Diaphyseal Femoral Fractures.

BACKGROUND: Femoral fractures are the most common cause of fracture-related admissions. Unplanned return to the operating room (UpROR) is a major event and imposes a substantial burden on the family and health care system. The purpose of this study was to determine the incidence of complications and early UpROR during the first 6 months following treatment of femoral fractures and their risk factors. METHODS: In an institutional review board-approved study, a retrospective review of all patients who were treated for a femoral fracture during a 10-year period at a pediatric tertiary care cente

Diaphyseal Fracture of the Humerus during Elective Cesarean Section - A Rare Case Report.

INTRODUCTION: Humeral fractures are the second common long bone fractures of the neonatal period after clavicle. Most cases of birth-related humerus fracture are reported during a vaginal breech delivery. The cesarean section does not eliminate the risk of long bone fractures. A humerus fracture is less common but still can happen due to forceful maneuvers like traction during cesarean which can go unnoticed to operating obstetrician or misinterpreted as brachial plexus injury by an inexperienced orthopedic surgeon. CASE REPORT: We received a call from the neonatal intensive care unit (NICU)

Defining the Mean Angle of Diaphyseal Long Bone Nonunions-Does Shear Prevail?

OBJECTIVES: To define the mean angle of a series of diaphyseal nonunions based on radiographic analysis. DESIGN: A retrospective cohort study. SETTING: Two level-1 trauma centers. PATIENTS: One hundred twenty patients presenting with nonunion. INTERVENTION: A mean nonunion angle was calculated from a series of AP and lateral X-rays using a standardized technique. The nonunion angle was then estimated in a single plane by considering the greater of the 2 measured angles. Additional data collected included patient age, sex, nonunion site, initial fracture angle, and original fracture pattern

Chronic femoral diaphyseal osteomyelitis with radiographs initially concerning for Paget disease of the bone.

Chronic osteomyelitis can be difficult to diagnose given its similar radiographic appearance to other lesions. This case report describes a 48-year-old woman, who presented with left thigh pain and on radiography a large disorganized sclerotic lesion involving nearly the entire femoral diaphysis, concerning for Paget disease or malignancy. Biopsy suggested chronic osteomyelitis but did not identify a causative organism. Treatment with antibiotics led to resolution of pain and improvement of biochemical markers. This case exemplifies the role of radiographic imaging in the diagnosis of chronic

Management of spiral diaphyseal fractures of the fifth metatarsal: A case series and a review of literature.

INTRODUCTION: Spiral diaphyseal fractures of the fifth metatarsal can present with significant displacement. It is considered that non-operative management is sufficient in most cases but there is no clear consensus as to what this may be. This study reports the functional outcome of this injury in a small patient cohort and is the first study to report on outcomes of different non-operative measures. MATERIAL AND METHODS: This is a retrospective study of 33 consecutive patients presenting to a central London teaching hospital who were managed by a variety of treatment modalities depending on

To evaluate the role of platelet-rich plasma in healing of acute diaphyseal fractures of the femur.

PURPOSE: New research is focusing on the use of autologous growth factors to increase the effect of bone fracture healing while decreasing the amount of healing time for the patient. Platelets have been demonstrated to be the natural storage vessel for several growth factors and cytokines that promote blood coagulation, tissue repair, and the process of bone mineralization. The present study aims to evaluate the role of platelet-rich plasma (PRP) in healing of acute femoral shaft fractures radiologically. We hypothesize that it provides artificial hematoma and releases various growth factors.

Prognostic potential of markers of bone turnover in delayed-healing tibial diaphyseal fractures.

INTRODUCTION: Clinical and radiographic examinations detect delayed or nonunion only after the event has occurred. Biochemical markers of bone turnover (BTMs) are promising laboratory tools that offer an early insight into the likelihood of delayed union. We hypothesized that BTMs display temporal variations following fractures and the behavior of BTMs differ between normal and delayed union of fractures. METHODS: This was a prospective study of patients with closed fracture of tibia treated with intramedullary, interlocking nailing. BTM assays (NTX, BSAP, P1NP and osteocalcin) and clinical a
