An atypical form of infantile meningococcal meningitis.

We describe a rare case of infantile meningococcal (serotype B) meningitis in a 3-month-old Chinese boy with an atypical indolent presentation with prolonged persistent fever despite appropriate antimicrobial therapy likely due to drug fever. The case highlights the need for continued vigilance in identifying similar cases in the future.

Rocky Mountain spotted fever in children.

Rocky Mountain spotted fever is typically undifferentiated from many other infections in the first few days of illness. Treatment should not be delayed pending confirmation of infection when Rocky Mountain spotted fever is suspected. Doxycycline is the drug of choice even for infants and children less than 8 years old.

Enhanced efficacy of liposome-encapsulated ribavirin against Rift Valley fever virus infection in mice.

Administration of liposome-encapsulated ribavirin to mice led to ribavirin concentrations in the liver, the primary site of Rift Valley fever virus proliferation, that were fivefold greater than those attained with the same doses of free ribavirin. Liposomal ribavirin given at a dose of either 25 or 50 mg of drug per kg of body weight protected mice against a rapidly lethal high-titer challenge with Rift Valley fever virus, whereas similar doses of free drug or empty liposomes had no detectable benefit. Hence, tissue targeting of ribavirin with liposomes substantially increased the therapeutic

Clinical manifestations and microbiological features between indigenous and imported enteric fever in Taiwan, 2010-2020.

Previous studies had showed that indigenous clones of Salmonella Typhi and S. Paratyphi were originally imported from other countries in Taiwan. We presented the clinical manifestations and laboratory findings of indigenous and imported enteric fever cases in Taiwan in the current decade. We retrospectively reviewed typhoid and paratyphoid fever cases in two medical centers of Chang Gung Memorial Hospitals in 2010-2020. A total of 37 enteric fever cases including 24 typhoid fever and 13 paratyphoid fever were recorded. There were 20 indigenous cases, 16 imported cases, and one indetermined cas

DRESS syndrome in the setting of oxacillin therapy-a call for better patient preparedness: a case report.

BACKGROUND: Drug reaction with eosinophilia and systemic symptoms syndrome is a rare but severe and potentially life-threatening hypersensitivity reaction, with significant morbidity and mortality. The clinical presentation of drug reaction with eosinophilia and systemic symptoms may include extensive skin rash, fever, lymphadenopathy, internal organ involvement, eosinophilia, and atypical lymphocytosis, most commonly due to drug-induced reaction. Our case is a rare occurrence of drug reaction with eosinophilia and systemic symptoms syndrome in the setting of oxacillin therapy. CASE PRESENTAT

Typhoid fever: case report and literature review.

Typhoid fever remains a major health problem worldwide, in contrast to Chile, where this disease is an isolated finding. Clinical presentation is varied, mainly presenting with fever, malaise, abdominal discomfort, and nonspecific symptoms often confused with other causes of febrile syndrome. We report a six-year-old, male patient presenting with fever of two weeks associated with gastrointestinal symptoms, malaise, hepatomegaly and elevated liver enzymes. Differential diagnoses were considered and a Widal reaction and two blood cultures were requested; both came back positive, confirming the

A personal perspective on clinical research in Enteric Fever.

With the global spread of enteric fever, the emergence of Salmonella enterica serovar Paratyphi as a major pathogen (particularly in Asia), the spread of drug resistance, and the global increase in the incidence of non-Typhi salmonella, particularly in patients coinfected with human immunodeficiency virus, there is now more than ever a need for clinical research in enteric fever. The work of Ted Woodward 60 years ago remains relevant today, and his holistic approach to clinical research inspired many of us to follow in his footsteps. There remains healthy discussion among clinicians about the

Dengue Fever-Triggered Malignant Hyperthermia.

Malignant hyperthermia (MH) is a genetic skeletal muscle disorder characterized by hypermetabolic crisis usually triggered by anesthetic drugs. Non-anesthesia-triggered or awake MH is rare or under-reported. Other than anesthetic drugs, identified common triggers are exercise, fever, and viral infection. The literature does not report any awake MH cases triggered by dengue fever. We report a possible case of dengue fever-triggered awake MH. The main aim of reporting this case is to raise awareness of possible malignant hyperthermia in dengue patients and of a clinical grading system (CGS) for

Protracted febrile myalgia syndrome in a kidney transplant recipient with familial Mediterranean fever.

Drug-induced toxic myopathy is a complication of familial Mediterranean fever in patients who receive colchicine, especially when combined with cyclosporine. Protracted febrile myalgia syndrome is a severe form of familial Mediterranean fever. A 34-year-old man who had familial Mediterranean fever for > 15 years developed kidney failure because of secondary amyloidosis. He received living-unrelated-donor kidney transplant that functioned normally. He was on colchicine prophylaxis that was continued after transplant, and he received immuno-suppression induction with antithymocyte globulin and m

A 14-year-old female with fever, rash, lymphadenopathy, and pancytopenia: a case report.

BACKGROUND: Anticonvulsant hypersensitivity syndrome is a rare adverse drug reaction associated with aromatic anticonvulsant drugs. This syndrome can range from mild cutaneous rash to drug reaction with eosinophilia and systemic symptoms that include fever, rash, lymphadenopathy, pancytopenia, and involvement of multiple internal organs. We aimed to report this case in the literature and make physicians aware of the uncommon symptoms of this syndrome when they prescribe antiepileptic medications in particular. Omadacycline in the treatment of severe Q fever pneumonia during an influenza epidemic: a case report with literature review.

Q fever, caused by Coxiella burnetii (Q fever rickettsiae), is a zoonotic disease with a natural reservoir and has been reported in many countries and regions. Its clinical presentation is non-specific and easily confused with other infectious or non-infectious diseases. Conventional diagnostic methods, such as respiratory specimen culture, often fail to yield conclusive results, increasing the risk of misdiagnosis. This case involves a 78-year-old male patient from Zhejiang Province, China, who presented with fever as the primary complaint and developed severe pneumonia. The diagnosis of H1N1

Yellow fever virus is susceptible to sofosbuvir both in vitro and in vivo.

Yellow fever virus (YFV) is a member of the Flaviviridae family. In Brazil, yellow fever (YF) cases have increased dramatically in sylvatic areas neighboring urban zones in the last few years. Because of the high lethality rates associated with infection and absence of any antiviral treatments, it is essential to identify therapeutic options to respond to YFV outbreaks. Repurposing of clinically approved drugs represents the fastest alternative to discover antivirals for public health emergencies. Other Flaviviruses, such as Zika (ZIKV) and dengue (DENV) viruses, are susceptible to sofosbuvir,

Double Jeopardy: Lamotrigine-Induced Drug Reaction With Eosinophilia and Systemic Symptoms (DRESS) Complicated by Valproate Hepatotoxicity.

A 17-year-old boy with primary generalized epilepsy developed a high fever, a widespread rash, swollen lymph nodes, and an enlarged liver and spleen a few weeks after starting lamotrigine. Blood tests revealed eosinophilia and elevated liver enzymes, meeting the Registry of Severe Cutaneous Adverse Reaction (RegiSCAR) criteria for drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome. Lamotrigine was discontinued, and corticosteroid therapy was initiated, while antiepileptic treatment with valproate and levetiracetam was continued. Although his fever and rash resolved, liver f

Case Report: Drug-induced pneumonia caused by moxifloxacin and a literature review.

Drug-induced pneumonia is a rare and potentially life-threatening adverse drug reaction. Moxifloxacin is a fluoroquinolone antibiotic with broad-spectrum antimicrobial activity. Despite reports of moxifloxacin-related side effects such as interstitial nephritis, recurrent tendinitis, and pseudoallergic reactions, moxifloxacin-induced pneumonia is exceedingly rare. We report the case of a 45-year-old male who developed fever and cough, and progressed to hypersensitivity syndrome related to drug-induced pneumonia following moxifloxacin therapy. Discontinuation of moxifloxacin led to resolution o

[Fever in rheumatological diseases].

Fever is a frequent and important symptom in patients with rheumatological diseases and can be an expression of activity of the underlying rheumatological disease. There is great variability in the incidence of fever as a symptom of the disease between individual diseases. The growing understanding of the molecular signatures of the diseases can help to explain these discrepancies: A genetic overactivation of potently pyrogenic cytokines is the reason why fever is nearly always present in autoinflammatory syndromes. In contrast, fever is less common in polyarthritis and myositis and mostly lim
