Neuroapraxia of Trigeminal Nerve Controlled by Neuromonitoring During Microvascular Decompression in Multiple Sclerosis Patients Affected by Drug-Resistant Trigeminal Neuralgia Recurrent After Previous Operations.

BACKGROUND AND IMPORTANCE: Trigeminal neuralgia (TN) can complicate the clinical course of multiple sclerosis (MS) and can be very difficult to treat. Usually, these patients experience multiple recurrences after surgical procedures with a poor overall outcome. To the best of our knowledge, we report the first 3 cases of drug-resistant MS-related TN recurrent after previous operations in which intraoperative neuromonitoring controlled neuroapraxia of trigeminal nerve was performed. We describe the surgical technique, report the clinical outcomes of patients, and review the pertinent literature

Human immunodeficiency virus type 1 protease inhibitor drug-resistant mutants give discordant results when compared in single-cycle and multiple-cycle fitness assays.

The replication fitness of HIV-1 drug-resistant mutants has been measured using either multiple-cycle or single-cycle assays (MCAs or SCAs); these assays have not been systematically compared. We developed an MCA and an SCA that utilized either intact or env-deleted recombinant viral vectors, respectively, in which virus-infected cells were detected by flow cytometry of a reporter gene product. Fitness was measured using each assay for 11 protease mutants, 9 reverse transcriptase mutants, and two mutants with mutations in gag p6, which is important for the release of virus particles from the c

Innowave MTB/RIF/INH facilitates timely and accurate diagnosis of multiple-drug resistant tuberculosis as a near POCT technique: a multicenter prospective on-site performance evaluation study.

BACKGROUND: Many rifampicin (RIF)-resistant (RR) tuberculosis (TB) patients remain sensitive to isoniazid (INH), which challenges the strategy of using RR as an instant indicator of multiple-drug resistance tuberculosis (MDR-TB). A molecular test capable of concurrently detecting RIF and INH resistance is urgently needed. METHODS: The performance of a novel rapid molecular test, Innowave MTB/RIF/INH (InnowaveDX) was evaluated prospectively in three tertiary hospitals. Its capability of detecting resistance to RIF and INH was assessed. RESULTS: In 767 pulmonary tuberculosis (PTB) patients, In

Fucoxanthin attenuates rifampin-induced cytochrome P450 3A4 (CYP3A4) and multiple drug resistance 1 (MDR1) gene expression through pregnane X receptor (PXR)-mediated pathways in human hepatoma HepG2 and colon adenocarcinoma LS174T cells.

Pregnane X receptor (PXR) has been reported to regulate the expression of drug-metabolizing enzymes, such as the cytochrome P450 3A (CYP3A) family and transporters, such as multiple drug resistance 1 (MDR1). Fucoxanthin, the major carotenoid in brown sea algae, is a putative chemopreventive agent. In this study, we determined whether fucoxanthin could overcome drug resistance through attenuation of rifampin-induced CYP3A4 and MDR1 gene expression by PXR-mediated pathways in HepG2 hepatoma cells. We found that fucoxanthin (1-10 μM) significantly attenuated rifampin (20 μM)-induced CYP3A4, MDR1

Realization of Osteolysis, Angiogenesis, Immunosuppression, and Drug Resistance by Extracellular Vesicles: Roles of RNAs and Proteins in Their Cargoes and of Ectonucleotidases of the Immunosuppressive Adenosinergic Noncanonical Pathway in the Bone Marrow Niche of Multiple Myeloma.

Angiogenesis and immunosuppression promote multiple myeloma (MM) development, and osteolysis is a primary feature of MM. Although immunomodulatory drugs and proteasome inhibitors (PIs) markedly improve the survival of patients with MM, this disease remains incurable. In the bone marrow niche, a chain of ectoenzymes, including CD38, produce immunosuppressive adenosine, inhibiting T cell proliferation as well as immunosuppressive cells. Therefore, anti-CD38 antibodies targeting myeloma cells have the potential to restore T cell responses to myeloma cells. Meanwhile extracellular vesicles (EVs) c

Crisis averted: a world united against the menace of multiple drug-resistant superbugs -pioneering anti-AMR vaccines, RNA interference, nanomedicine, CRISPR-based antimicrobials, bacteriophage therapies, and clinical artificial intelligence strategies to safeguard global antimicrobial arsenal.

The efficacy of antibiotics and other antimicrobial agents in combating bacterial infections faces a grave peril in the form of antimicrobial resistance (AMR), an exceedingly pressing global health issue. The emergence and dissemination of drug-resistant bacteria can be attributed to the rampant overuse and misuse of antibiotics, leading to dire consequences such as organ failure and sepsis. Beyond the realm of individual health, the pervasive specter of AMR casts its ominous shadow upon the economy and society at large, resulting in protracted hospital stays, elevated medical expenditures, an

Multi-Drug Resistance in Bacterial Genomes-A Comprehensive Bioinformatic Analysis.

Antimicrobial resistance is presently one of the greatest threats to public health. The excessive and indiscriminate use of antibiotics imposes a continuous selective pressure that triggers the emergence of multi-drug resistance. We performed a large-scale analysis of closed bacterial genomes to identify multi-drug resistance considering the ResFinder antimicrobial classes. We found that more than 95% of the genomes harbor genes associated with resistance to disinfectants, glycopeptides, macrolides, and tetracyclines. On average, each genome encodes resistance to more than nine different class

Antimicrobial Multidrug Resistance and Mechanisms of Action: An Overview.

Antimicrobial multidrug resistance is the ability of microorganisms to withstand the effects of several antimicrobial agents, presenting a major challenge to modern healthcare systems worldwide. Although considerable research has been conducted, the molecular and evolutionary mechanisms underlying resistance are still not completely understood. This review brings together current knowledge to explain how resistance originates, spreads, and persists in different pathogens. Microorganisms may show primary resistance, which arises naturally without prior exposure to drugs, or acquired resistance,

[Penicillin resistance of pneumococci from invasive pediatric infections].

Susceptibility to antibiotics of 199 isolates of Streptococcus pneumoniae from blood and cerebrospinal fluid of children hospitalized during 1987-1993 was tested. Of 119 isolates, 18% were resistant to penicillin. The prevalence in 1992-93 was double that in 1987-89. The increase in minimal inhibitory concentrations (MIC) of penicillin was associated with an increase in MIC of other beta-lactams. There was also resistance to other antibiotics: trimethoprim-sulfamethoxazole, erythromycin and chloramphenicol. Most of the resistant isolates belonged to serogroups 6, 14, 19 and 23. [The present and future problems of resistance in staphylococci, streptococci and M. tuberculosis].

Mechanisms of resistance of strains of Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pneumoniae, Enterococcus faecalis, Enterococcus faecium, Neisseria meningitidis and Mycobacterium tuberculosis to antibiotics are presented and discussed. The resistance to methicillin-oxacillin in S. aureus and S. epidermidis is chromosomally-mediated by sequence of DNA, namely by gene mecA. This gene codes altered PBP-2A protein. Strains of S.pneumoniae contain PBP-1A, PBP-2B and PBP-2X proteins. They cause resistance to many beta-lactam antibiotics. Resistance to vankomycin, respectively

Molecular epidemiological characteristics of Shigella spp. isolated from river Narmada during 2005-2006.

Shigellosis is an acute gastroenteritis caused by Shigella species. Forty isolates of Shigella spp. were obtained from the river Narmada during 2005-2006. Twenty-three isolates were identified as S. flexneri, 10 as S. sonnei, and seven as S. dysenteriae on the basis of biochemical tests and serotyping. All the isolates harbored at least one plasmid (range: 1-4) and exhibited 12 distinct plasmid profile patterns. Out of 40 isolates, 90% were found to be resistant against more than two antibiotics. All of the isolates were resistant to ciprofloxacin. It is noteworthy that all of the S. dysenteri

Multidrug Resistance (MDR): A Widespread Phenomenon in Pharmacological Therapies.

Multidrug resistance is a leading concern in public health. It describes a complex phenotype whose predominant feature is resistance to a wide range of structurally unrelated cytotoxic compounds, many of which are anticancer agents. Multidrug resistance may be also related to antimicrobial drugs, and is known to be one of the most serious global public health threats of this century. Indeed, this phenomenon has increased both mortality and morbidity as a consequence of treatment failures and its incidence in healthcare costs. The large amounts of antibiotics used in human therapies, as well as

Multiple resistance mechanisms acting in unison in an Escherichia coli clinical isolate.

The emergence of multiple-resistant isolates poses a serious problem in the hospital environment making it important to evaluate the responsible factors. This work ascertains the mechanisms responsible for the development of resistance in enterobacterial clinical isolates. The major resistance mechanisms have been explored. The presence of target mutations, drug hydrolyzing enzymes, active efflux pump, and drug-resistance genes were elucidated experimentally employing standard methods. One of the clinical isolates was resistant to five classes of structurally unrelated antibiotics and showed i

Treatment of complicated intra-abdominal infections in the era of multi-drug resistant bacteria.

The management of severe intra-abdominal infections remains a major challenge facing surgeons and intensive care physicians, because of its association with high morbidity and mortality. Surgical management and intensive care medicine have constantly improved, but in the recent years a rapidly continuing emergence of resistant pathogens led to treatment failure secondary to infections with multi-drug resistant bacteria. In secondary peritonitis the rate of resistant germs at the initial operation is already 30 %. The lack of effective antibiotics against these pathogens resulted in the develop
