Biliary therapy: are we ready for EUS-guidance?

Endoscopic retrograde cholangiopancreaticography is the standard therapy for the therapy of biliary obstruction. However, the success rate is not 100%, depending on various patient and physician related factors. In these cases, where endoscopic drainage is not possible, either percutaneous drainage or surgery are established alternatives. Both modalities carry a higher complication rate and are more invasive than endoscopic drainage. With linear echo-endoscopes, left intrahepatic bile ducts as well as the distal common bile duct can be visualized from the stomach or the duodenal bulb respectiv

Endoscopic Retrograde Cholangiopancreatography-Related Complications and Their Management Strategies: A "Scoping" Literature Review.

Endoscopic retrograde cholangiopancreatography (ERCP) is a well-known procedure with both diagnostic and therapeutic utilities in managing pancreaticobiliary conditions. With the advancements of endoscopic techniques, ERCP has become a relatively safe and effective procedure. However, as ERCP is increasingly being utilized for different advanced techniques, newer complications have been noticed. Post-ERCP complications are known, and mostly include pancreatitis, infection, hemorrhage, and perforation. The risks of these complications vary depending on several factors, such as patient selection

Outcome of access sphincterotomy using a needle knife converted from a standard biliary sphincterotome.

Cannulation fails in up to 10 % of all endoscopic retrograde cholangiopancreatographies (ERCPs). A standard sphincterotome can be converted to a needle knife to perform precut sphincterotomy (PCS). In this retrospective study, we analyzed cannulation rates, adverse events, and the percentage of patients requiring a second sphincterotome using a converted needle knife. Over a 7-year period, 3322 ERCPs were performed by one experienced therapeutic endoscopist; 1487 sphincterotomies were performed, 78 precut sphincterotomies using a converted needle knife. Successful cannulation using the convert

PTCD and Peroral Electronic Cholangioscopy with Holmium Laser Lithotripsy for CBD Stones in a Super-Elderly Patient Post-Billroth II Gastrojejunostomy.

The management of impacted common bile duct stones in patients with prior Billroth II gastrectomy poses unique challenges. Altered gastrointestinal anatomy renders duodenoscope passage and biliary cannulation during conventional ERCP exceedingly difficult, resulting in a low success rate. Super‑elderly (over 90 years old) patients often have multiple comorbidities such as cardiovascular diseases and require rapid relief of biliary obstruction and infection control to create a subsequent therapeutic window. Meanwhile, traditional open biliary surgery and general anesthesia carry prohibitive ris

Endoscopic management of internally migrated pancreatic duct stents (with video).

BACKGROUND AND AIMS: Spontaneous intestinal migration of pancreatic stents is a known phenomenon. However, retrieval of a proximally migrated pancreatic stent (PMPS) poses a therapeutic challenge. The primary aim of this study was to evaluate technical success of endoscopic retrograde of cholangiopancreatography (ERCP) for extraction of PMPS, including number of sessions, need of surgery for failures and intervention-induced adverse events. The secondary outcome was to evaluate long-term effects of PMPS on the ductal morphology. METHODS: Data of patients undergoing pancreatic stenting since J

Bile Duct Stone Size May Influence the Efficacy of Endoscopic Sphincterotomy With or Without Large-Balloon Dilation: A Meta-Analysis.

Purpose: Endoscopic sphincterotomy (EPT) combined with endoscopic papillary large balloon dilatation (EPBD) are used to remove large common bile duct (CBD) stones. This meta-analysis compared the efficacy and safety of EPT+EPBD versus EPT alone in the removal of stones based on stone size. Materials and Methods: Twenty-two studies (11 randomized control trials [RCTs] and 11 non-RCTs) were identified and reviewed based on searches of Embase, PubMed, and Web of Science. CBD stone's size was measured with reference to diameter of the duodenoscope (13 mm) and size of the large dilatation balloon (

A new method of preventing bile duct injury in laparoscopic cholecystectomy.

AIM: Of all the complications of laparoscopic cholectecystomy, bile duct injury (BDI) is the most serious complication. The prevention of injury to the common bile duct (CBD) remains a significant concern in laparoscopic cholecystectomy (LC). Different kinds of methods have been advanced to avoid this injury but no single method has gained wide acceptance. Because of various limitations of current methodologies we began a study using cold light illumination of the extrahepatic biliary system (light cholangiography LCP) to better visualize this area and thereby reduce the risk of bile duct inju

Laparoscopic transgastric endoscopic retrograde cholangiopancreatography (ERCP) after gastric bypass: case series and a description of technique.

BACKGROUND: Roux-en-Y gastric bypass excludes the biliary and pancreatic tree from traditional endoscopic evaluation and treatment. As the number of former bypass patients accrues, the need to assess and treat this subset of patients for biliary and pancreatic disease will increase. The authors describe their technique, indications, and outcomes for this group of patients. METHODS: Data were collected by a retrospective chart review of the experience two surgeons had with laparoscopically assisted transgastric endoscopic retrograde cholangiopancreatography (ERCP) from July 2004 to October 200

[Propofol sedation in endoscopic manometry of Oddi's sphincter].

Endoscopic manometry of the sphincter Oddi (SO) is a sophisticated method which requires a cooperative patient. Therefore, during endoscopic manometry sufficient i.v. sedation is crucial, and additionally must no affect SO-motility. In a pilot trial SO-motility was determined in ten patients with suspected SO-dysfunction (SOD) under initial sedation with 4.8 +/- 1 mg midazolam (baseline), and 3 min after an i.v. bolus of 50 mg of propofol. In addition, endoscopic manometry was performed in 57 consecutive patients with suspected SOD from 10/94-9/95 under sedation with midazolam (6.2 +/- 1.6 mg)

Colonoscopy-assisted preplacement of guide tube for endoscopic retrograde cholangiopancreatography in patients with complex surgically altered anatomy.

BACKGROUND: Balloon-assisted enteroscopy is limited by a small working channel and few compatible accessories. Herein, we assessed a novel endoscopic retrograde cholangiopancreatography (ERCP) involving a long-type single-balloon enteroscope (200 cm length; 2.8 mm channel), requiring dual-operator manipulation. Although standard in our centre, this technique's clinical limitations compared to single-operator long-colonoscopy ERCP remain unclear. METHODS: This retrospective study enrolled patients with complex surgically altered anatomy who underwent ERCP from 2021 to 2024, divided into the si

Case Report: Pancreatic sclerosing epithelioid mesenchymal neoplasm (SEMN) with novel pancreatic duct obstruction in a middle-aged female.

Pancreatic sclerosing epithelioid mesenchymal neoplasms (SEMN) are extremely rare mesenchymal tumors, with only approximately 10 cases reported globally to date, resulting in limited understanding of their clinical, imaging, and pathological characteristics. We present a 55-year-old female admitted to our hospital following the incidental detection of a pancreatic mass during a routine health check-up. The patient had no specific clinical symptoms or relevant medical history. Preoperative multimodal imaging-including computed tomography (CT), dynamic contrast-enhanced MRI (dynamic contrast MRI

EUS-Directed Transgastric Endoscopic Retrograde Cholangiopancreatography (EDGE): The First Learning Curve.

INTRODUCTION: Endoscopic ultrasound (EUS)-directed transgastric endoscopic retrograde cholangiopancreatography (EDGE) is a minimally invasive option for pancreaticobiliary access in patients with Roux-en-Y anatomy. The procedure involves creating a fistulous tract between the remnant stomach or jejunum and the bypassed stomach with the deployment of a lumen-apposing metal stent (LAMS), followed by the advancement of an endoscope through the LAMS to perform an ERCP or EUS. It is a technically challenging procedure, requiring skills in EUS, fluoroscopy, and LAMS deployment. The aim of this study
