Complex fractures of the distal humerus in the elderly: is primary total elbow arthroplasty a valid treatment alternative? A series of 20 cases.

INTRODUCTION: Distal humerus fractures are fairly rare. But as our population ages, these fractures become more complex and the choice of treatment more delicate. Poor bone quality results in many technical problems and the fixation hardware stability remains at risk. The goal of this study was to evaluate the functional recovery and morbidity of complex distal humerus fractures in elderly patients when treated with elbow prosthesis. HYPOTHESIS: Good functional recovery can be achieved with a total joint replacement. PATIENTS AND METHODS: This series consisted of 20 patients (18 women and tw

Complications of radial head prostheses.

Radial head prostheses are indicated for treatment of complex radial head fractures not amenable for fixation. After the initial experience with silastic implants, metallic or pyrocarbon arthroplasty have been used for 20 years. Little is known about complications related to these implants. Main complications are related to loosening whether they are cemented or not cemented. Hypotheses have been proposed like inadequate stem design, insufficient cement technique, stress shielding, and foreign body reactions secondary to polyethylene wear. Pain and stiffness are other common complications ofte

Is systematic 1-stage exchange a valid attitude in chronic infection of total elbow arthroplasty?

INTRODUCTION: Surgical treatment for chronic infection of total elbow arthroplasty (TEA) generally involves 2-stage exchange. In the lower limb, 1-stage strategies are increasingly implemented, but few cases have been reported for the elbow. We present results in a preliminary series, with the aim of: (1) assessing control of infection in systematic 1-stage exchange for chronic TEA infection, (2) detailing clinical and radiological results, and (3) analyzing intra- and post-operative complications. HYPOTHESIS: Systematic 1-stage exchange for chronic TEA prosthetic joint infection provides sat

Radial head replacement with a bipolar system: an average 10-year follow-up.

BACKGROUND: We report the long-term results of a cohort of patients after radial head replacement with a bipolar design and a smooth cementless stem at a mean follow-up of 10.4 years. METHODS: Of 17 possible patients from a previous minimum 2-year follow-up study, 16 were available for review. Patients were assessed using clinical and radiographic examination and with standardized outcome measures. Range of motion, stability, and radiographic evaluation of implant loosening and joint degeneration were assessed. Comparisons were performed using the Wilcoxon signed rank test for unequal groups.

[Therapy for radial head fractures].

The treatment of radial head fractures is challenging for the trauma surgeon due to the complex functional anatomy of the elbow. Satisfactory results can only be achieved if the function and stability of the humeroradial joint are entirely restored. Thus, depending on the fracture type, ranging from non-displaced Mason type I fractures to comminuted Mason type III fractures, an individual treatment concept has to be established. Treatment protocols range from conservative treatment with short-term cast immobilization to open reduction and internal fixation according to AO-principles or to the

Outcomes in total elbow arthroplasty in patients with haemophilia at the University of California, San Francisco: a retrospective review.

Severe haemophilic arthropathy of the elbow is a significant cause of morbidity among adults with haemophilia. However, previous reports of total elbow arthroplasty (TEA) in the haemophilic population have been based on small numbers of patients with relatively short-term follow-up. The records of seven total elbow arthroplasties in six adult men with haemophilia at the University of California, San Francisco who underwent TEA over a period of 25 years were retrospectively reviewed. Type of haemophilia, age at time of TEA, HIV infection status, pre- and postoperative range-of-motion (ROM) scor

Measuring residual limb health in persons with upper limb amputation: Modifications of the Prosthetic Evaluation Questionnaire residual limb health scale.

BACKGROUND: Assessing the user perspective on residual limb health problems is particularly important for amputation care, given the relationship between residual limb health and prosthetic satisfaction. Only 1 measure, the Residual Limb Health scale of the Prosthetic Evaluation Questionnaire (PEQ) has been validated for use in lower limb amputation, but not examined in persons with upper limb amputation (ULA). OBJECTIVES: The objective of this study was to examine the psychometric properties of a modified PEQ Residual Limb Health scale in a sample of persons with ULA. STUDY DESIGN: The stud

Treatment of Radial Head Fractures with a Modular Metallic Radial Head Replacement.

INTRODUCTION: Radial head arthroplasty with a smooth-stemmed metallic modular implant is a reliable treatment option for patients with acute unreconstructible radial head fractures, and good clinical outcomes may be expected beyond 5 years of follow-up (Video 1). STEP 1 PREOPERATIVE PLANNING: Obtain a careful history and perform a physical examination along with appropriate imaging to facilitate appropriate treatment decisions. STEP 2 OPERATING ROOM SETUP AND PATIENT POSITIONING: Perform proper operating room setup and patient positioning, as they are required to gain access to all affected

Implant Survivorship, Radiographic Evaluation, and Clinical Outcomes of Radial Head Arthroplasty Using The Depuy Synthes Radial Head Replacement System.

BACKGROUND: Radial head arthroplasty (RHA) implants are designed either to promote osseo-integration with an anatomic stem or function as a loose-fitting spacer. Anatomic implants have shown painful loosening leading to revision or removal, making the loose-fitting design an appealing alternative. This study evaluates short-term survivorship and clinical, radiographic, and patient reported outcomes following RHA using the DePuy Synthes monoblock loose fit radial head replacement system. METHODS: Nineteen patients (mean age 54 SD 17 years), who underwent RHA were enrolled, with a mean follow-u

Flail Arm Reconstruction After Failed Nerve Transfers: A Case Report.

CASE: A 53-year-old, right-hand-dominant gentleman suffered a traumatic right scapulothoracic dissociation with severe soft tissue damage and complete right arm paralysis following a high-speed all-terrain vehicle accident. Subsequent burns led to digital amputations. He underwent elbow disarticulation with humeral shortening osteotomy, glenohumeral arthrodesis, and a vascularized radial osteocutaneous flap for clavicular reconstruction. Postoperatively, he achieved stable limb positioning and functional improvement with a prosthesis. CONCLUSION: This case underscores the importance of a mult

Periprosthetic humeral fractures after shoulder and elbow arthroplasty.

Due to rapidly increasing numbers of arthroplasty surgeries of the upper extremity, periprosthetic humeral fractures after shoulder and elbow arthroplasty, formerly described as rare, may hence increase in the near future. Therefore the aim of the present work was to give an overview of the existing literature including possible classifications as well as an update on treatment concepts and experiences with own cases. After a literature research have been done, existing prevalence, classifications and treatment options, mostly described in case series, were processed to create an overview of t

Limb Salvage After Grenade-Related Brachial Artery Injury: A Case Report.

Since war injuries are not predictable, each patient presents as a new and unique case. Before deciding on amputation for upper extremity injuries, it is necessary to evaluate all limb-sparing vascular interventions that will keep the amputation level as distal as possible, in order to improve the patient's quality of life and enable the use of more functional prostheses. While amputation may be a faster and more life-saving option for a surgeon, limb-sparing and mobile joint-preserving surgical procedures should be the preferred choice, provided the patient's overall condition permits. This c

Functional Capacity Evaluation in Upper Limb Reduction Deficiency and Amputation: Development and Pilot Testing.

Purpose To develop and pilot test a functional capacity evaluation (FCE) for individuals with upper limb absence (ULA) due to reduction deficiency or amputation, and to examine the relationship between FCE results and presence of musculoskeletal complaints (MSC). Method Five tests (overhead lifting, overhead working, repetitive reaching, fingertip dexterity, and handgrip strength) were selected and adapted if necessary. The newly developed FCE, called FCE-One-Handed (FCE-OH), was pilot tested in 20 adults individuals with ULA, and 20 matched controls. MSC were assessed via a questionnaire. Res

[Locking plate osteosynthesis of the radial head fractures : clinical and radiological results].

BACKGROUND: Therapy of radial head fractures is still controversially discussed. Especially comminuted fractures are at risk of complications such as radial head necrosis, nonunion and secondary loss of reduction after open reduction and internal fixation. The aim of this study was to evaluate clinical and radiographic results of ORIF (open reduction internal fixation) of radial head fractures using a new radial head-specific locking plate system. PATIENTS AND METHODS: A total of 21 patients (13 men and 8 women) were treated with locking plate osteosynthesis of radial head fractures. Mean age
