Bilateral brachial artery emboli presenting as aortic dissection.

Loss of peripheral pulses in a patient with chest pain suggests the diagnosis of aortic dissection. An 80-year-old woman presented with an episode of chest pain and acute bilateral loss of upper extremity pulses that was initially treated as aortic dissection. Findings of physical examination and echocardiography were consistent with mitral stenosis. Angiography revealed bilateral brachial artery emboli, which were treated by embolectomy. To our knowledge, this case represents the first report of simultaneous brachial artery emboli in association with mitral stenosis.

[Diagnosis of acute pulmonary artery embolism].

The clinical picture and the electrocardiographic and rheopneumographic findings were compared with the data obtained during operation or angiopneumography in 31 patients with embolism of the pulmonary artery. In massive embolism of the pulmonary artery, the dominating clinical symptoms were sudden dyspnea, tachycardia and cyanosis. Collapse and cardiac arrest were encountered in most patients. Moderate embolism is characterized by advancing dyspnea but cyanosis and tachycardia are inconsistant symptoms. The results of electrocardiography and rheopneumography are in correlation with the data o

Reperfusion Therapy and Outcomes in High-Risk Pulmonary Embolism: Multicenter Registry Study From the Tokyo Cardiovascular Care Unit Network.

BACKGROUND: High-risk pulmonary embolism (PE) is associated with substantial mortality, and contemporary guidelines recommend prompt reperfusion therapy when feasible. However, real-world data describing how reperfusion is used in practice, and its outcomes, remain limited. METHODS AND RESULTS: We conducted an observational analysis of patients with high-risk PE enrolled in the Tokyo Cardiovascular Care Unit Network registry between 2020 and 2022. High-risk PE was defined as a presentation complicated with shock, sustained hypotension, or cardiac arrest. Reperfusion therapies included systemi

Acute limb ischaemia: not an uncommon complication of COVID-19 infection.

A new variant of coronavirus (2019-nCoV) causing acute respiratory distress in humans was identified for the first time in 2019, in Wuhan, China. One of the many complications of infection with this coronavirus is hypercoagulopathy, resulting in acute thrombosis; often leading to acute limb ischaemia. Herein, we report 20 cases of COVID-19 with peripheral arterial thrombosis involving either upper or lower limbs. Some patients underwent vascular procedures and most had to undergo amputation at some level. All the cases (n=20) were referred to us during the 8-month period June 2020 to March 202

Failure of Major Upper Extremity Replantation Due to COVID-19-Related Arterial Thrombosis.

Coronavirus disease of 2019 (COVID-19), caused by the new severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that emerged in Wuhan, was declared a pandemic by the World Health Organization. COVID-19 has many different clinical manifestations. One of them is arterial hypercoagulopathy. Although its mechanism is not fully explained, acute thrombosis and thromboembolism can be seen in patients. In this study, we present a case who was amputated due to the development of arterial thrombosis on the 10th day following infection with coronavirus, despite successful replantation after trauma

Distal arteriovenous fistula to preserve patency in COVID-19-associated acute limb ischemia.

The purpose of this study was to report our institutional experience with patients with COVID-19 who developed acute limb ischemia during hospitalization and to determine the characteristics and clinical outcomes. Between March 2020 and January 2021, we treated 3 patients who were COVID-19-positive and developed acute limb ischemia after they received thromboprophylaxis. We performed an embolectomy by exposing the popliteal artery below the knee to treat an occlusion of the popliteal and tibial arteries. An infusion of unfractionated heparin was initiated immediately after surgery, maintaining

Successful Treatment of Refractory Massive Pulmonary Embolism with Repeated Administration of Systemic Thrombolysis.

BACKGROUND: We report a case series of two patients in the intensive care unit with massive pulmonary embolism and obstructive shock who had resolution of shock after repeated administration of alteplase. CASE SUMMARIES: Both patients were initially dosed 10 mg of alteplase followed by infusion of 90 mg over 2 hours, but remained in obstructive shock requiring significant inotropic and vasopressor support. Both patients were deemed poor candidates for embolectomy. The first patient received repeated doses reaching an accumulative dose of 200 mg alteplase over 15 hours. The second patient rece

Pulmonary embolism during and after pregnancy.

BACKGROUND: Venous thromboembolic disease is among the most common causes of morbidity and mortality during pregnancy. The clinical evaluation alone is insufficient for the diagnosis of venous thromboembolic disease, and the normal pregnant state makes this evaluation even more challenging. DIAGNOSIS: Objective testing is the mainstay of diagnosis, including compression ultrasound, impedance plethysmography, ventilation-perfusion scanning, computed tomography scanning, and pulmonary angiography. All of these tests can be safety performed during pregnancy. TREATMENT: If deep vein thrombosis o

[Thrombolytic therapy of pulmonary embolism--a therapeutic alternative?].

The spontaneous prognosis of pulmonary embolism is mainly dependent on the degree of pulmonary artery obstruction. As for the cause of spontaneous lysis, submassive pulmonary embolism generally will be survived with complete restitution of the pulmonary artery trunk. On the other hand, the hallmark of massive pulmonary embolism is a tremendously high early mortality. In the decision for thrombolysis in patients with pulmonary embolism, major attention must therefore be paid to the severity of pulmonary obstruction. Up to now, neither a reduction in mortality nor in secondary morbidity by throm

Acute aortic occlusion presenting with lower limb paralysis.

In a two year period eight patients have presented with acute aortic occlusion and a poor outcome in seven. Initial failure to diagnose aortic occlusion, with a mean delay from presentation to diagnosis of 24 hours, was mainly responsible. All patients had varying degrees of paralysis on presentation which misled clinicians although other findings of acute ischaemia (pain, absent pulses, colour change and anaesthesia) were always present. Two patients were initially referred to a neurologist, another to a neurosurgeon, and the fourth to an orthopaedic surgeon. Even after diagnosis had been est

Pulmonary embolism. Prophylaxis diagnosis and treatment.

The natural history diagnosis and immediate treatment of patients suffering from pulmonary embolism has been discussed. Anaesthetists should use their influence to bring about a high standard of prophylactic care against deep venous thrombosis and consequently of pulmonary embolism. They are likely to be involved in the resuscitation and treatment in intensive care units of those cases who suffer from major symptoms and massive emboli and some of them will rarely be involved in anaesthetising for pulmonary embolectomy aided by cardiopulmonary by-pass and, less rarely, for IVC ligation or plica

Lower limb embolus: a near-lethal disease after age 75 years.

Because it has been suggested that embolectomy increases morbidity and mortality, we reviewed our experience with lower-extremity embolectomy in an elderly high-risk population. Of 153 consecutive patient records reviewed, 69 were sufficiently complete for analysis. Of these patients, 45 were men with a mean age of 80.7 years (range, 75 to 91 years); 24 were women with a mean age of 76.9 years (range, 75 to 81 years). Records were reviewed for patient age, sex, race, acute myocardial infarction or atrial fibrillation upon admission, history of smoking, diabetes, and cardiac disease (acute and

Surgical thrombus removal in acute pulmonary embolism. Where do we stand today?

Acute pulmonary embolism (PE) remains a major cause of cardiovascular morbidity and mortality, with clinical presentations ranging from asymptomatic disease to obstructive shock and sudden death. Although anticoagulation is effective in most hemodynamically stable patients, a substantial proportion present with or progress to right ventricular (RV) failure and require reperfusion therapy. Surgical embolectomy (SE), first introduced in the early 20th century, was historically associated with prohibitive mortality, and largely abandoned following the advent of systemic thrombolysis. However, adv

[Vascular surgical emergency in advanced age: arterial embolism--iliofemoral venous thrombosis].

Acute embolic arterial occlusion is the most frequent emergency in vascular surgery; the patients usually have significant heart disease and additional risk factors due to high age. Since 1966 acute embolic occlusions of major vessels of the lower extremity have been operated in 1496 patients; 82% of patients were over 65 years of age--18% even over 80 years. Standardized embolectomy from a small groin incision in local anaesthesia after preoperative check of general conditions has the best prognosis concerned limb salvage. Postoperative long term anticoagulation is performed routinely to prev

Management of a Patient With Acute Abdomen and Thyroid Storm.

BACKGROUND: Acute abdomen often requires urgent surgical intervention. In certain instances, thyrotoxicosis can contribute to the onset of acute abdomen. This relationship complicates both diagnosis and management. Therefore, understanding the interplay between thyrotoxicosis and acute abdomen is essential for implementing effective treatment strategies. This case report aims to illustrate the diagnostic and therapeutic challenges encountered in managing a patient with mesenteric ischemia due to thyroid storm, emphasizing the importance of a multidisciplinary approach in achieving favorable ou

Reappraisal of Microsurgical Revascularization for Anterior Circulation Ischemia in Patients with Progressive Stroke.

BACKGROUND: Intravenous thrombolysis using tissue plasminogen activator and endovascular treatment for acute ischemic stroke is becoming an established standard therapy. However, there is no consensus in the treatment of patients who are suffering from progressive neurologic symptoms in the later stages. The purpose of this study was to evaluate the safety and efficacy of microsurgical revascularization in such patients with progressive stroke. METHODS: We retrospectively reviewed the clinical and radiological records of 14 consecutive patients with progressive stroke who underwent emergency

Debulking the tricuspid valve with FlowTriever aspiration: A case series.

Intracardiac masses adhering to the tricuspid valve can occur as a result of right-sided infective endocarditis, malignancy, clot formation in the right atrium, or clots-in-transit passing through the right atrium. Early surgical intervention is recommended for tricuspid valve vegetation in some patients, although open heart surgery is not always an option. Treatment options for right heart thrombi include anticoagulation, thrombolysis, surgical embolectomy, or mechanical aspiration. We present a case series of tricuspid valve debulking using aspiration with the FlowTriever System.

Thrombus straddling a patent foramen ovale and massive pulmonary embolism: Venous arterial extracorporeal membrane oxygenation as a valuable support tool.

Thrombus straddling a patent foramen ovale and massive pulmonary embolism is a very rare and life-threatening condition. Optimal management can be controversial because different therapeutic options are available and individual approach based in individual risk is needed. We present a case of a thrombus straddling the patent foramen ovale with massive pulmonary embolism, hemodynamic instability, and upper extremity embolism. We performed surgical pulmonary embolectomy, and venous arterial extracorporeal membrane oxygenation was needed to successfully overcome severe right ventricular impairmen

[Acute pulmonary embolism and cardiac arrest treated with thrombolysis and an automatic chest compression device].

Survival after pulseless electrical activity cardiac arrest is poor. In this case report we describe a patient who had acute massive pulmonary embolism and was treated with thrombolysis and an automatic mechanical chest compression device, Lund University Cardiac Arrest System (LUCAS 2). Chest compressions were effectively provided for two hours during a prolonged resuscitation and transferral for pulmonary embolectomy. The patient was extubated one day after the operation and discharged with normal cerebral function nine days after the cardiac arrest.

[Application of extracorporeal membrane oxygenation in catastrophic pulmonary embolism].

Acute high-risk pulmonary embolism (HRPE), particularly in patients progressing to catastrophic pulmonary embolism (PE), is associated with extremely high mortality. As a cardiopulmonary support technique, extracorporeal membrane oxygenation (ECMO) provides critical circulatory and respiratory support. Whether used as a standalone treatment or a bridge to definitive therapy (such as surgical embolectomy or catheter-based intervention), ECMO plays a pivotal role in the management of catastrophic PE. This review summarizes the role of ECMO in combination with various therapeutic strategies for c
