Endocrine and multiple sclerosis outcomes in patients with autoimmune thyroid events in the alemtuzumab CARE-MS studies.

BACKGROUND: Alemtuzumab is an effective therapy for relapsing multiple sclerosis. Autoimmune thyroid events are a common adverse event. OBJECTIVE: Describe endocrine and multiple sclerosis outcomes over 6 years for alemtuzumab-treated relapsing multiple sclerosis patients in the phase 3 CARE-MS I, II, and extension studies who experienced adverse thyroid events. METHODS: Endocrine and multiple sclerosis outcomes were evaluated over 6 years. Thyroid event cases, excluding those pre-existing or occurring after Year 6, were adjudicated retrospectively by expert endocrinologists independently of

Pituitary and adrenal disorders induced by immune checkpoint inhibitors.

Over the past decade, the development of ICI (immune checkpoint inhibitors) has constituted a revolution in the treatment of many cancers, but with a specific toxicity profile including endocrine IRAEs (immune-related adverse events). As the indications for these molecules are constantly increasing due to their efficacy, it is important that endocrinologists and oncologists know how to detect, manage and monitor this type of toxicity. Many guidelines and recommendations have been proposed in the last few years for the management of endocrinopathies. French guidelines on immunotherapy-related e

Thyroid: Year-in-Review 2024-2025.

At the 34th meeting of the American Association of Clinical Endocrinology, held on May 15-17 2025 in Orlando, Florida, the "Thyroid Year in Review," presented by Dr. Malini Gupta, summarized fourteen carefully selected peer-reviewed articles published between May 2024 and March 2025 relevant for the practicing endocrinologist. These articles were chosen for addressing unanswered questions or highlighting new directions that shape the field of thyroidology, acknowledging that many other outstanding studies have also advanced the discipline. While the Thyroid Year-in-Review was presented by a si

The no-man's land of nontubal ectopic and cesarean section scar pregnancies.

Nontubal ectopic pregnancy (NTEP) and cesarean section scar pregnancy (CCSP) are a relatively uncommon but potentially very dangerous conception event. If diagnosed early, they can be treated very effectively. Cesarean section scar ectopic is unique in that expectant management may allow them to persist longer, but with overwhelmingly catastrophic consequences. Reproductive endocrinologists are uniquely positioned to diagnose and treat NTEP/CCSP early, thereby avoiding long-term catastrophic consequences.

What is causing this patient's urticaria?

This article describes a teenage patient who was referred to a pediatric endocrinologist after her workup for recurring urticaria revealed a suppressed thyroid-stimulating hormone level and positive microsomal thyroid peroxidase antibodies. The patient's laboratory results revealed an autoimmune cause for the urticaria as a result of new-onset autoimmune thyroid disease.

Impact of coronavirus disease 2019 on medical practice in endocrine and metabolic diseases in Japan: a nationwide surveillance study conducted by the Japan Endocrine Society.

We investigated the impact of the Coronavirus disease 2019 (COVID-19) pandemic on the management of endocrine and metabolic disorders in Japan. We conducted a cross-sectional nationwide questionnaire survey targeting board-certified endocrinologists under the auspices of the Japan Endocrine Society. The questionnaire consisted of multiple-choice questions and open-ended responses. Out of approximately 2,700 specialists, 528 (19.5%) opted to participate, suggesting a high level of interest in COVID-19 management among endocrinologists. The study found that almost half of participants had encoun

[TRANSGENDER ENDOCRINOLOGY].

Clinical encounters with transgender people seeking hormonal treatment for gender reaffirmation, provide an opportunity, besides engagement in the endocrine dilemmas, to challenge the binary pattern of thinking and reveal many fascinating and complex matters of transgender medicine: human, philosophical and political. The current review of the subject is presented by an endocrinologist engaged in hormonal treatment of transgender people.

The routinization of lay expertise: A diachronic account of the invention and stabilization of an open-source artificial pancreas.

Embodied health movements (EHMs) advance their agendas by mediating the production, circulation, and revision of biomedical knowledge. To do this, their constituents become lay experts by blending their embodied experience of illness with self-taught technical knowledge. However, it is unclear how lay expertise is routinized within EHMs, and consequently, to what extent it can be made durable in long-term partnerships with credentialed experts. I follow the OpenAPS community-a group of people with type one diabetes who engineered an open-source 'artificial pancreas'-from their inception in the

[Radioactive iodine in the treatment of Graves' disease: history and modern concept of radionuclide therapy].

Radioactive iodine 131I is a theranostic isotope used both for diagnosis and therapy of benign thyroid diseases and thyroid cancer for 85 years. The formation of nuclear medicine is closely linked with the use of 131I. The history of radioiodine therapy began in 1941, when endocrinologist Saul Hertz for the first time used 131I to treat patients with Graves' disease. Since 1946 radioactive iodine 131I became widely available, and its effectiveness became public knowledge after reports on thyrotoxicosis treatment published in the Journal of the American Medical Association by multidisciplinary

[Pathogenetic, clinical and therapeutical features of ophthalmologic involvement in Basedow's disease].

A review of the recent literature concerning the pathogenetic, anatomical, pathological, clinical and therapeutical features of endocrine ophthalmopathy serves to lay stress on the fact that, whereas ocular signs are predominantly associated with hyperthyroidism pictures, they may also be encountered in other dysendocrinopathies. Satisfactory local and general treatment can be significantly improved by cooperation between the oculist and the endocrinologist.

Factors impacting couples' decision-making between vasectomy reversal versus sperm retrieval/in vitro fertilization/intracytoplasmic sperm injection.

The purpose of this study was to identify factors which impact a couples' decision-making between the options of vasectomy reversal vs. sperm retrieval/in vitro fertilization/intracytoplasmic sperm injection when counseled both by a reproductive urologist and a reproductive endocrinologist. A retrospective chart review was performed of couples who wish to achieve a pregnancy with a male partner with a history of prior vasectomy, in a couples' private fertility center. Of patients presenting for fertility options with a history of vasectomy, 175 couples elected to be counseled by both a reprodu

[Clinical guideline of «congenital hypothyroidism»].

Congenital hypothyroidism is an important issue of pediatric endocrinology at which timely diagnosis and treatment can prevent the development of severe cases of the disease. The developed clinical guidelines are a working tool for a practicing physician. The target audience is pediatric endocrinologists and pediatricians. They briefly and logically set out the main definition of the disease, epidemiology, classification, methods of diagnosis and treatment, based on the principles of -evidence-based medicine.

Pituitary disorders during pregnancy.

The pituitary gland undergoes much anatomic and physiologic variation during pregnancy. Pituitary disease may have a significant impact on a patient prior to conception as well as throughout her pregnancy. It is imperative to provide care to patients affected by pituitary disease with a multidisciplinary approach involving endocrinologists, obstetricians and, when appropriate, neurosurgical care, as this group of disorders can represent a substantial level of morbidity and mortality for both mother and fetus.

[Decision-making in gender reassignment surgery].

The prescribing of hormones and the performing of gender reassignment surgery on a healthy body are dangerous and irreversible acts. To ensure patients' safety, endocrinologists and surgeons must use the skills of experienced psychiatrists to create a differential diagnosis, evaluate any frailties and weigh up the risk-benefit ratio. Therapeutic decisions are made as part of multidisciplinary consultation meetings. A minimum two-year observation period must be respected for the treatment to be covered by social security.

Cushing's syndrome.

Cushing's syndrome remains one of the great diagnostic challenges to internists and endocrinologists. Urologists have the primary role in the treatment of many of the forms of Cushing's syndrome, especially adrenal tumors and micronodular hyperplasia. As we continue to gain experience with Cushing's disease treated with pituitary surgery and with etopic ACTH syndrome and the other forms of Cushing's syndrome, urologists are likely to become even more involved in the therapy of all types of Cushing's syndrome.

Does atherosclerosis contribute the development of Fahr's Syndrome?

Fahr's syndrome is a rare neurological condition in which calcification of vessels in the basal ganglia and cerebellum usually occurs. Impaired glucose tolerance (IGT) may be present, causing multiple organ complications by means of atherosclerosis. We report a case of Fahr's syndrome associated with hypoparathyroidism and IGT. We present the case of a 52-year-old man with Fahr's syndrome and IGT. The patient's neurological examination was normal except for bilateral tremor. Laboratory investigations showed that the level of serum Ca (calcium) was 5.9 mg/dl, the parathormone (PTH) level was 1.

The molecular pathology of pituitary hormone deficiency and resistance.

In this chapter, we have reviewed the fast-moving area of the molecular pathology of pituitary hormone deficiencies and resistance. Examples have been described affecting all levels of pituitary function, i.e. the releasing hormone, its receptor, the pituitary hormone and its receptor, and the development of the pituitary gland. Other examples in these genes, and in those in which no mutation has yet been found, will undoubtedly be discovered in the next few years, throwing light on the structural basis of the gene product's function and allowing a greater understanding of endocrine physiology

Pharmacist involvement in a thyroid clinic.

The patient-care activities of a pharmacist in a thyroid clinic are described. Since 1978 a pharmacist has been an active member of the staff of a thyroid clinic associated with a 500-bed university hospital. More than 1500 patients are referred to the clinic each year. The pharmacist initiates, maintains, or modifies the drug therapy of selected patients with physician-diagnosed thyroid disorders under the guidance of written protocols approved by the chief endocrinologist. Most patients treated by the pharmacist are receiving thyroid-suppression therapy, antithyroid drugs for Graves' disease
