Large Vallecula Epiglottica Lipoma: A Rare but Fatal Cause of Dysphagia.

Lipoma is a benign mesenchymal tumor with 13% occurrence in the head and neck region. Despite being the commonest type of tumor, it only accounts for 0.6% of the upper aerodigestive tract. We report a case of a 41-year-old gentleman who presented with progressively worsening dysphagia and a muffled voice. A flexible nasopharyngeal endoscope showed a solitary well-lobulated pedunculated cystic-looking mass occupying the oropharynx arising from the vallecular space and lingual surface of the epiglottis with a partially seen mobile posterior vocal cord. CT of the neck showed a single lesion with

[Pentax AWS airway scope].

Pentax AWS Airway Scope (Pentax, Japan) is a new videolaryngoscope. We successfully used the device in 15 patients and in other two patients with difficult airways. In a 51-year-old woman, after induction of anesthesia and neuromuscular blockade, laryngoscopy using a Macintosh blade showed only the tip of the epiglottis and tracheal intubation failed. Pressure on the neck and the use of gum elastic bougie did not solve the problem. When the Airway Scope was used, a full view of the glottis was obtained and the trachea was easily intubated. In a 71-year-old man, after induction of anesthesia an

Successful reversal of remimazolam anesthesia in a "cannot intubate, can ventilate" situation: a case report.

BACKGROUND: Compared to other intravenous anesthetics, availability of a specific antagonist flumazenil is a clear advantage of remimazolam. We report a patient who could be rapidly woken up when laryngoscopy and tracheal intubation were unexpectedly difficult. CASE PRESENTATION: A 62-year-old man was scheduled to have resection of a small gingival tumor. Preoperative airway examination was unremarkable except for an omega-shaped epiglottis. Anesthesia was induced with remifentanil/remimazolam infusion and rocuronium. A small omega-shaped edematous epiglottis precluded identification of glott

Large Vallecular Masses; Differential Diagnosis and Imaging Features.

Large vallecular masses are an uncommon disease entity which includes a wide spectrum of rare pathologies. These masses may present as purely vallecular in location or appear as an extension from adjacent anatomical sites like tongue base including lingual tonsils, epiglottis, palatine tonsils, epiglottis, and the remainder of supraglottis. Literature review reveals very rare conditions presenting as vallecular mass lesions. The imaging features are generally diagnostic for characterization. However, these masses can exhibit indeterminate appearance at imaging. We present a series of five case

Morphometric Examination of the Larynx in Turkish Population.

OBJECTIVE: The aim of this study was to measure laryngeal dimensions in a sample of Turkish cadavers, including males and females of various ages and heights. MATERIALS AND METHODS: Morphological measurement was performed on 102 laryngeal specimens. Hyoid bone and laryngeal cartilages were removed from human cadavers. All dimensions of the cartilages were measured using a thread and vernier calipers. Age, sex, and height were recorded. All the parameters for male and female were compared. Statistical comparisons were made using Student's t test and Pearson's correlation. RESULTS: The followi

Evaluation of Oropharyngeal Structure and DISE Findings for Successful Soft Palatal Webbing Flap Pharyngoplasty.

PURPOSE: Lateral pharyngeal wall collapse, linked to weak pharyngeal wall stability, is characteristic of severe obstructive sleep apnea (OSA) patients. Soft palate webbing flap palatopharyngoplasty (SPWFPP) has been introduced to enhance the stability of the lateral pharyngeal wall with minimal postoperative complications by eliminating the need for tonsillectomy. This study analyzed the anatomic findings of oropharynx for determination of outcome in OSA patients requiring SPWFPP. METHODS: Twenty-eight OSA patients who underwent SPWFPP combined with septoturbinoplasty were included in this s

Differential diagnosis and management of adult-onset laryngomalacia.

OBJECTIVE: Adult-onset laryngomalacia is a rare clinical entity that has been infrequently reported. This study aims to evaluate the clinical presentation, diagnosis, and management of adult-onset laryngomalacia through literature review and report of a case. METHODS: PubMed and Google Scholar databases were queried for articles published from 1960 to 2019 including only patients aged 18 years and older. Included keywords were: 'laryngomalacia', 'adult laryngomalacia', 'acquired laryngomalacia', 'idiopathic laryngomalacia', 'laryngeal obstruction', 'floppy epiglottis', 'floppy epiglottis', an

Airflow Shape Is Associated With the Pharyngeal Structure Causing OSA.

BACKGROUND: OSA results from the collapse of different pharyngeal structures (soft palate, tongue, lateral walls, and epiglottis). The structure involved in collapse has been shown to impact non-CPAP OSA treatment. Different inspiratory airflow shapes are also observed among patients with OSA. We hypothesized that inspiratory flow shape reflects the underlying pharyngeal structure involved in airway collapse. METHODS: Subjects with OSA were studied with a pediatric endoscope and simultaneous nasal flow and pharyngeal pressure recordings during natural sleep. The mechanism causing collapse was

Incidentally Detected Asymptomatic Epiglottic Capillary Hemangioma in a 66-Year-Old Female: A Case Report.

Laryngeal hemangiomas are benign vascular tumors that occur predominantly in children. Occurrence in adults is rare, particularly when developing in the epiglottis. This report describes a rare case of a pedunculated capillary hemangioma of the epiglottis in a 66-year-old female. The lesion was incidentally discovered during general anesthesia and was completely asymptomatic. Laryngeal endoscopy revealed a bluish-red, pedunculated mass with prominent vascularity, and MRI findings were consistent with a superficial vascular lesion without evidence of infiltration. Due to the potential risk of a

Spontaneous epiglottic hematoma secondary to direct oral anticoagulant.

Spontaneous hemorrhage is a known risk for patients on anticoagulation therapy. Most previous spontaneous airway hemorrhage cases reported involve warfarin, and of the few that involved a direct oral anticoagulant, none involved the epiglottis. The following case describes a spontaneous epiglottic hematoma in a patient one week after starting a direct oral anticoagulant. An 85-year-old man presented to the emergency department with acute onset of neck swelling, odynophagia and sublingual ecchymosis. Evaluation in the emergency department included advanced imaging of the neck and consultation w

[Endolaryngeal resection of the larynx for adenocystic cancer].

Adenocystic carcinoma accounts for 3-5% of all malignant neoplasms of the head and neck organs. Due to the rarity of detecting this cancer in the larynx, treatment options are still controversial: surgery with postoperative radiation therapy or surgery alone. Patient A., 64 years old, turned to the City Clinical Oncological Hospital No. 1 in October 2019 with complaints of moderate pain when swallowing, sensation of a foreign body. Fibrolaryngoscopy revealed an exophytic form of growth bright red color formation with a small tuberous surface up to 12 mm in size in the area of the free left edg

[Efficacy of a Bronchofiberscope in Combination with the GlideScopee in a Difficult Airway Patient].

We present a case of securing a difficult airway in which intubation with a fiberoptic bronchoscope (FOB) assisted by the GlideScope® (GS) was effective. A 69-year-old man was scheduled to undergo laparoscopic hernia repair under general anesthesia. He had a small jaw and Grade 3 upper lip bite test in the preoperative assessment. Mask ventilation was possible following the induction of general anesthesia. Given that the glottis was not observable with the Macintosh laryngoscope (Cormack-Lehane Grade 4), laryngoscopy was per- formed with the GS, which allowed visualization of the tip of the ep

[Epiglottic anteversion in the etiology of globus pharyngeus and its surgical management].

OBJECTIVES: To define the role of epiglottic anteversion in the etiology of globus pharyngeus (GP) and to evaluate the effectiveness of laser partial epiglottectomy in its treatment. PATIENTS AND METHODS: Four patients who admitted to our clinic between 2001 and 2003 with the complaint of GP underwent laryngoscopic examination, which revealed a structurally anteverted epiglottis that was in contact with the tongue base and vallecula. After ruling out other etiologic factors of GP, the patients were operated on under general anesthesia and laser partial epiglottectomy was carried out. In this

The current status of corrective surgery for laryngomalacia.

When the airway obstruction related to laryngomalacia becomes significant, surgical correction of the underlying laryngeal deformity is indicated. The three elements of the supraglottic soft tissue that prolapses, namely, the corniculate mounds on the arytenoid cartilages, the shortened ary-epiglottic folds, and the omega shaped epiglottis, can all be appropriately "trimmed" using either conventional instruments or the surgical laser. Relief of symptoms is dramatic and, provided surgery is performed precisely and not during episodes of infection, complications should not occur. The aid of spec

What prevents Struthio camelus and Dromaius novaehollandiae (Palaeognathae) from choking? A novel anatomical mechanism in ratites, the linguo-laryngeal apparatus.

BACKGROUND: The avian glottis channels air from the oropharynx to the trachea and is situated on an elevated structure, the laryngeal mound. It is imperative that the glottis be protected and closed during swallowing, which in mammals is achieved by covering the glottis with the epiglottis, as well as by adduction of the arytenoid cartilages. An epiglottis, however, is reportedly absent in birds. Ratites such as Struthio camelus and Dromaius novaehollandiae possess a very wide glottis in comparison to other birds. The question therefore arises as to how these large birds avoid inhalation of in

Larynx-preserving reconstruction after extended base of the tongue resection.

BACKGROUND: As the base of the tongue (BOT) plays essential roles in speech and swallowing, surgical resection of BOT cancer is typically avoided. Moreover, standard reconstructive procedures for larynx-preserving BOT defects have not yet been established. We performed immediate flap reconstruction after wide resection of BOT cancer with laryngeal preservation. Herein, the functional and oncological results of our strategy were analysed. METHODS: We retrospectively evaluated patients who underwent extended BOT resection (including the oral tongue, upper/lateral oropharyngeal wall, epiglottis
