Locked Metacarpophalangeal Joint of the Little Finger in Extension Position due to the Incarceration of a Chronic Fracture Fragment: A Case Report.

We managed a case of locked metacarpophalangeal joint of the little finger in the extension position. Incarceration of a chronic osteochondral fracture fragment led to this unique condition. The fracture fragment partially adhered to the volar plate and ulnar collateral ligament on the joint side, which is supposed to have resulted in manually irreducible locking of the joint. We performed open reduction and achieved release of the locked joint by excising the fracture fragment.

Simultaneous Regional Fasciectomy, Skin Grafting, and Distraction Arthrolysis of the Proximal Interphalangeal Joint for Dupuytren's Contracture of the Little Finger.

We present a 58-year-old right-handed man, who consulted us with an 11-year history of Dupuytren's disease. To correct contracture of the little finger, we performed regional fasciectomy, skin grafting, and distraction arthrolysis of the proximal interphalangeal (PIP) joint using an external fixator. Preoperative or postoperative skeletal traction has been advocated to treat potential or residual stiffness of the PIP joint in Dupuytren's contracture, but its intraoperative use has not been reported before. Our method has the advantage of treating each problem caused by Dupuytren's disease. A g

[Early functional treatment of intra-articular fractures of the proximal interphalangeal joint of the finger using a modified traction device].

INTRODUCTION: Treatment of intra-articular fractures of the middle phalanx is both difficult and controversial. We report our experience of using a modified dynamic traction device (mTD) utilizing only one pin in the head of the middle phalanx and thus facilitating further open reconstructive surgery at the proximal interphalangeal joint (PIP). The pin does not necessarily have to be inserted in the movement axis of the joint. AIM OF THE STUDY: The aim of the study was to a) determine the effectiveness of the mTD, particularly in comparison with other distraction systems used in the treatment

MR imaging of the metacarpophalangeal joints of the fingers: part I. Conventional MR imaging and MR arthrographic findings in cadavers.

PURPOSE: To demonstrate the normal anatomy of the metacarpophalangeal (MCP) joints of the fingers with magnetic resonance (MR) imaging and MR arthrography in cadavers. MATERIALS AND METHODS: MR images of 20 MCP joints of the fingers of five fresh human cadaveric hands in the extended and flexed positions were obtained before and after arthrography. The MR appearances of all articular and periarticular structures were analyzed and compared with those seen on anatomic sections. Two readers independently graded the visibility of these structures. Interobserver agreement was tested by using the k

Use of an objective measure of articular stiffness to record changes in finger joints after intra-articular injection of corticosteroid.

OBJECTIVE: (1) To measure objectively the subjective improvement in joint stiffness following intra-articular injections of corticosteroids, and (2) to record changes in joint stiffness in the first 24 hours after injection to look for changes in the physical properties of the joint that would be consistent with a crystal synovitis. METHODS: The study population consisted of 15 patients having 17 metacarpophalangeal joints injected as part of their routine care. Measurements were taken before injection, at 24 hours, and after one week. Outcome variables included articular stiffness, strength,

[Treatment of fresh subcutaneous flexor tendon rupture of the distal joint of long fingers with temporary bone wire arthrodesis].

Drop-finger at the distal joint or mallet-finger from injury occurs because of the extensor apparatus is so much weaker than the flexor tendon that it breaks under strain. We report on 100 cases treated by internal splinting with longitudinal applied Kirschner wire in the distal and middle phalanx of the finger. It affords an absolute immobilisation of the distal interphalangeal joint in slight hyperextension with relaxation of the extensor apparatus. The wire was removed after six weeks. The results evaluated in 76 patients by follow-up four to 84 months were classified as excellent in 86.8%

Validation of the spontaneous lateral laxity sign for radial collateral ligament sprain of the metacarpophalangeal joint of the fingers: Anatomical study.

Severe collateral ligament sprain of the metacarpophalangeal joint (MCP) of the fingers is rare. Clinical examination is mandatory to diagnose severity. The purpose of our study was to validate a clinical sign for MCP joint laxity: the spontaneous lateral laxity sign (SLLS), recently described by Meyer et al. to diagnose severe MCP radial collateral ligament (RCL) sprain, with a comparison with the already validated rotational laxity test (RLT). SLLS and RLT were assessed before RCL transection, after RCL transection and after RCL repair on 40 long fingers in 10 fresh thawed cadavers. SLLS was

In finger osteoarthritis, change in synovitis is associated with change in pain on a joint-level; a longitudinal magnetic resonance imaging study.

OBJECTIVE: To investigate determinants of decrease and increase in joint pain in symptomatic finger osteoarthritis (OA) on magnetic resonance (MR) imaging over 2 years. DESIGN: Eighty-five patients (81.2% women, mean age 59.2 years) with primary hand OA (89.4% fulfilling American College of Rheumatology (ACR) classification criteria) from a rheumatology outpatient clinic received contrast-enhanced MR imaging (1.5T) and physical examination of the right interphalangeal finger joints 2-5 at baseline and at follow-up 2 years later. MR images were scored paired in unknown time order, following th

Comparative study on the effectiveness of corticosteroid injections between trigger fingers with and without proximal interphalangeal joint flexion contracture.

UNLABELLED: Trigger fingers with proximal interphalangeal joint flexion contracture are suggested to have a poorer response to corticosteroid injection than those without contracture, though this has not been proven scientifically. We compared the clinical response to corticosteroid injection between trigger fingers with and without proximal interphalangeal joint contracture, and investigated the influence of the injection on the A1 pulley and flexor digitorum tendons using ultrasonography. One month after injection, pain was significantly reduced in the no contracture group, and 56% of trigge

Spontaneous ulnar dislocation of the index, long, ring and small finger extensor tendons at the metacarpophalangeal joints: a case report.

Finger extensor tendon dislocation at the metacarpophalangeal joint is caused by various etiologies, such as trauma, congenital anomaly, or rheumatoid arthritis. When the dislocation occurs with no etiology, this is called spontaneous dislocation. Although spontaneous extensor tendon dislocation in one, two or three fingers has been described, to our knowledge, simultaneous dislocation in four fingers has not been reported. In this paper, we report a spontaneous ulnar dislocation of all the extensor tendons in the index, long, ring, and small fingers. Repair of the radial sagittal bands of the

Biomechanical Comparison of Volar Plate Repair Versus Volar Plate Repair With Suture Tape Augmentation at the Finger Proximal Interphalangeal Joint.

PURPOSE: The purpose of this study was to perform a biomechanical comparison of volar plate repair alone versus volar plate repair with suture tape augmentation in a hyperextension laxity injury model at the proximal interphalangeal (PIP) joint. METHODS: Ten matched cadaveric fingers were obtained from five cadavers (average age, 59 ± 7 years). The specimens underwent 5 and 10 N·cm of extension load at the PIP joint to measure the laxity of the intact joint. Next, an injury model was created by incising the proximal volar plate and loading the specimen on a testing machine to 30° of hyperexte

Management of chronic mallet finger: Indications and long-term results of Fowler central slip tenotomy and distal interphalangeal joint arthrodesis.

INTRODUCTION: Many surgical techniques have been described to correct the sequelae of chronic mallet fingers (MF), but no clear therapeutic strategy has been defined. We have reported the choice of their management according to the severity of the deformities. Two procedures were compared: Fowler's central slip tenotomy (CST) and arthrodesis of the distal interphalangeal joint (DIP). HYPOTHESIS: The use of our decision tree, based on the severity of deformity (flexion deformity at the DIP and recurvatum at the proximal interphalangeal joint), allows good long-term clinical results to be obtai

Study protocol for collecting synchronized multimodal EMG, forearm kinematics and finger joint-angles data in healthy adults and transradial amputees.

Rejection rates of upper-limb prosthetic hands remain high, largely due to limitations in control performance and usability. Most commercially available prosthetic hands rely on a fixed and limited set of predefined control modes, which restrict the execution of complex daily activities and reduce their acceptance by users. Consequently, there is a growing need for advanced control strategies that enable more natural and intuitive prosthetic hand operations. Multimodal sensing approaches that combine surface electromyography with inertial measurement unit and gyroscope data, together with accu

Neuromechanics of finger hangs with arm lock-offs: analyzing joint moments and muscle activations to improve practice guidelines for climbing.

INTRODUCTION: Climbing imposes substantial demands on the upper limbs and understanding the mechanical loads experienced by the joints during climbing movements is crucial for injury prevention and optimizing training protocols. This study aimed to quantify and compare upper limb joint loads and muscle activations during isometric finger hanging exercises with different arm lock-off positions. METHODS: Seventeen recreational climbers performed six finger dead hangs with arm lock-offs at 90° and 135° of elbow flexion, as well as arms fully extended. Upper limb joint moments were calculated usi

Clinical investigation of a digital biomarker for joint swelling in inflammatory arthritis based on automated quantification of dorsal finger fold patterns.

This study aimed to clinically evaluate a digital biomarker, the Finger Fold Index (FFI), derived from the ratio of joint diameter to finger fold surface area in hand photographs, for the assessment of joint swelling in inflammatory arthritis. Smartphone hand photographs from patients with rheumatoid (RA) and psoriatic arthritis (PsA) were analyzed using a machine learning pipeline for automated detection and processing of joint diameter and finger folds at the proximal interphalangeal (PIP) joints. The FFI was clinically evaluated by correlation with joint swelling scores (0-3) and DAS28-CRP.

Finger Joints Reconstructive Coverage with Cross-Arm (Colson) Flaps After Burn Injury: A Literature Review and Our Experience.

Background: Random pattern flaps are widely used in reconstructive surgery when inadequate vascularity precludes skin graft survival or when regional pedicled flaps are unavailable due to local burn injury. Here, thin tissue from the upper arm was utilized to cover exposed cartilage over the proximal interphalangeal (PIP) joints of the contralateral hand. Methods/Technical Note: We report the uncommon application of multiple cross-arm (Colson) flaps to reconstruct dorsal skin defects over the PIP joints of the index, middle, and ring fingers following a high-voltage burn injury, in conjunction
