Hyperostosis Frontalis Interna and a Question on Its Pathology: A Case Report.

BACKGROUND Hyperostosis frontalis interna is a boney overgrowth of the inner side of the frontal bone of the skull caused by overgrowth of the endocranial surface. It is most often found in women after menopause. It is also associated with hormonal imbalance, being overweight, history of headaches, and neurocognitive degenerative conditions. Female gender, advanced age, extended estrogen stimulation, and elevated leptin levels may also play a role. The thickening is usually confined to the frontal bone, but it can spread as far as the anterior parietal and temporal bones. CASE REPORT During a

Is the Supraorbital Notch a Reliable Landmark to Avoid the Frontal Sinus?

BACKGROUND: When performing a craniotomy involving the orbital bar, the supraorbital notch is a potential landmark to localize the lateral extent of the frontal sinus. Avoidance of the frontal sinus is important to reduce the risk of postoperative surgical site infection, epidural abscess formation, and mucocele development. OBJECTIVE: To determine the reliability of the supraorbital notch as a marker of the lateral location of the frontal sinus. METHODS: Cadaveric dissections were used with image guidance software to define the relationship between the frontal sinus and supraorbital foramen

Experimental study of the frontal sinus development on Goettingen miniature pigs.

According to the literature, the development of the frontal sinus cavity is a result of the active immigration of cells from the ethmoidal complex into the os frontale. This migration theory is in contrast to the operative outcome of Apert's syndrome patients, after fronto-orbital advancement. When a fronto-orbital advancement at the age of a few months is performed in these patients while the frontal suture is yet closed, a sinus developed even the distance between nasal root and frontal bone bing up to 2 cm. In order to study the development of the frontal sinus, an animal study on 12 five-w

Radiological evaluation of Hyperostosis frontalis interna: is it of clinical importance?

Hyperostosis frontalis interna (HFI) presents irregular thickening of the frontal bone. Even though HFI is frequently seen during routine radiological imaging, it usually remains unrecorded owing to a common belief that it just represents an incidental finding or anatomical variant. Recent studies implied that HFI may be clinically relevant. Etiology of HFI is still debated, while presumptions are mainly based on altered sex steroids impact on skull bone growth. Some authors implied that frontal bone might be particularly affected by this condition due to specificity of its underlying dura. In

[Application of mucosal flap technique in Draf type Ⅱb/Ⅲ frontal sinus surgery].

Draf type Ⅱb/Ⅲ frontal sinus surgery has been widely used in clinical practice. They can obtain a wide operating space and sufficient drainage pathways. However, the mucosa around the frontal sinus ostium was removed during the surgery, resulting in bone exposure, and sinus ostium stenosis or atresia may occur after the operation. In recent years, rhinologists at home and abroad have applied various mucosal flaps to cover the exposed bone around the frontal sinus orifice after Draf Ⅱb/Ⅲ frontal sinus surgery, in order to reduce scar formation and bone hyperplasia along with the incidence of fr

Autograft frontoplasty. A simple technique for correction of post-osteoplastic flap defects.

A rare but distressing complication of frontal embossment was managed after osteoplastic flap surgery. The technique is relatively simple and predictable, consisting of frontal bone autografts, bone paté, and Silastic sheeting. This technique may have potential application to other posttraumatic defects that require frontal contour restoration.

Anatomical observations on os inca and associated cranial deformities.

The present study describes the presence of os inca, incomplete metopic suture with asymmetrical frontal sinuses and multiple sutural deformities in a skull bone. Os inca has been reported to be associated with other cranial deformities. However, the present study, besides reporting os inca and associated sutural abnormalities, also highlights the presence of an unusual pterion in such cases. The aim is to provide anatomical insight into the morphology of sutures, frontal sinuses and associated cranial abnormalities. These are important findings which may be relevant for surgeons and radiologi

Outcomes of frontal mucoceles treated with conventional endoscopic sinus surgery.

CONCLUSION: Conventional endoscopic sinus surgery (CESS) is useful for frontal mucoceles. A patient with short anterior-posterior distance and bone thickening may need an axillary flap, Draf type IIb, or Draf type III procedure. OBJECTIVE: To present outcomes of frontal mucoceles treated with CESS and predict risk factors for poor outcomes to help define surgical indications. METHODS: A consecutive clinical series was reviewed retrospectively. The authors performed CESS without agger nasi resection (Draf type IIa) for 13 frontal sinus mucoceles in 12 patients between October 2011 and July 20

Postoperative assessment of forehead contouring in facial feminization surgery.

Facial feminization surgery (FFS) is essential for transgender women seeking alignment between facial appearance and gender identity. Frontal bone and sinus reshaping is a key component of FFS to achieve a more traditionally feminine contour. This retrospective study evaluates the effectiveness of frontal debossing by comparing pre- and postoperative volumetric and morphometric data. Sixteen patients who underwent frontal debossing at the University Hospital Basel between 2020 and 2024 were included. Preoperative and postoperative CT or CBCT scans were used to assess volume changes in the fron

Prognosis of Performing Split-Rib Bone Graft for Cranial Bone Defects.

BACKGROUND: Frontal sinus anterior wall defects occur because of various diseases, causing not only aesthetic problems, such as forehead bulging and upper eyelid ptosis, but also exerting physical pressure on the brain or optic nerve. Therefore, this study aimed to evaluate the prognosis of performing split-rib bone graft for frontal sinus anterior wall defects. METHODS: This study included 30 patients who received a split-rib bone graft for a frontal sinus anterior wall defect. The sizes and volumes of the defects and grafts were measured using three-dimensional computed tomography before, a

[Complications of rhinosinusitis].

Rhinogenous complications may manifest in the region of the orbits, bone or soft parts of the wall of the frontal sinus, or endocranially. With regard to orbital complications, a differentiation is made between edema, periostitis, subperiosteal abscess and phlegmon--depending on severity and extent. A possible sequela of frontal sinusitis may be osteomyelitis. If the frontal bone is involved, there is a danger that the infection may spread to the endocranium via medullary spaces and blood vessels. The diagnostic basis for deciding appropriate treatment is CT or MRI. While orbital edema and per

Frontal cranioplasty: risk factors and choice of cranial vault reconstructive material.

A study of patients with large cranial defects involving the frontal bone, frontal sinus, nose, and orbit does not support the contention that there is a clear superiority of reconstructive material despite a history of previous bone infection. No patient with an isolated cranial reconstruction experienced an infection despite location in the area of the frontal sinus or the use of acrylic material. All patients experiencing infection underwent simultaneous reconstruction of the frontal cranium and nose and three- or four-wall reconstruction of the orbit, where the frontal sinus had previously

Cholesterol granuloma of the frontal sinus: a case report.

Cholesterol granulomas are common in the mastoid antrum and air cells of the temporal bone. In the paranasal sinuses, especially in the frontal sinus, they have occasionally been mentioned in the literature. The pathogenesis is unknown, but the majority of the authors support the concept of airway obstruction in the cells well pneumatised of temporal bone and paranasal sinuses. The authors report a case of cholesterol granuloma of the frontal sinus treated with radical surgical techniques, and they also recommend an endoscopic approach to frontal sinus to restore or enlarge the nose-frontal ca
