Individualized blood pressure management and postoperative organ dysfunction in older hip fracture patients: a study protocol for a single-center, randomized, controlled trial.

BACKGROUND: Postoperative organ dysfunction is a leading cause of death and disability following hip fracture surgery in older patients. Intraoperative hypotension is a major modifiable risk factor for this complication, yet the optimal management strategy to prevent it remains controversial. We hypothesize that an individualized blood pressure management strategy is superior to standard management in reducing postoperative organ dysfunction. METHODS: This single-center, randomized, controlled trial will enroll 180 patients aged 65-85 years with hip fractures under general anesthesia. Eligibl

Effect of phenylephrine versus ephedrine on the incidence of postoperative delirium in elderly patients undergoing non-cardiac and non-neurosurgical procedures: protocol for a multicentre, double-blind, randomized, controlled trial.

BACKGROUND: Whether ephedrine, when used to maintain intraoperative blood pressure, is superior to phenylephrine in reducing postoperative delirium remains uncertain. This study aims to determine whether the intraoperative management of hypotension with ephedrine, compared with phenylephrine, reduces the incidence of postoperative delirium in elderly patients undergoing major non-cardiac, non-neurosurgical surgery. METHODS: This multicentre, randomized, double-blind, controlled trial will enrol 1,084 elderly patients scheduled for elective non-cardiac and non-neurosurgical major surgeries und

Effects of varying inhaled oxygen concentrations on lung function in older adult patients undergoing laparoscopic gastrointestinal surgery under general anesthesia: protocol of a prospective multicenter clinical study in China.

BACKGROUND: Postoperative pulmonary complications (PPCs) are severe and are of particular concern in older adult patients undergoing laparoscopic gastrointestinal surgery. Both 40% and 80% fraction of inspired oxygen (FiO2) are commonly used for anesthesia. Presently, whether 40% FiO2 can increase the oxygenation index of patients 48 h postoperatively and reduce PPCs remains controversial. Moreover, no clear consensus exists for older adult patients. Therefore, this study aims to compare the effects of low FiO2 (40%) and high FiO2 (80%) levels on postoperative pulmonary function in older adult

Preoperative frailty and chronic pain after cardiac surgery: a prospective observational study.

BACKGROUND: Chronic pain after cardiac surgery, whether or not related to the operation, is common and has negative impact on health related quality of life (HRQL). Frailty is a risk factor for adverse surgical outcomes, but its relationship with chronic pain after cardiac surgery is unknown. This study aimed to address the association between frailty and chronic pain following cardiac surgery. METHODS: This sub-study of the Anesthesia Geriatric Evaluation study included 518 patients ≥ 70 years undergoing elective cardiac surgery. Pain was evaluated with the Short-Form 36 questionnaire prior

Cognitive decline in the elderly after surgery and anaesthesia: results from the Oxford Project to Investigate Memory and Ageing (OPTIMA) cohort.

Concerns have been raised about the effects on cognition of anaesthesia for surgery, especially in elderly people. We recorded cognitive decline in a cohort of 394 people (198 women) with median (IQR) age at recruitment of 72.6 (66.6-77.8) years, of whom 109 had moderate or major surgery during a median (IQR) follow-up of 4.1 (2.0-7.6) years. Cognitive decline was more rapid in people who on recruitment were: older, p = 0.0003; male, p = 0.027; had worse cognition, p < 0.0001; or carried the ε4 allele of apoliprotein E (APOEε4), p = 0.008; and after an operation if cognitive impairment was alr

Risk-Stratified Use of Topical and Infiltrative Local Anesthetics in High-Risk Dermatologic Surgery.

Local anesthetics are fundamental to dermatologic practice, yet their safety profile requires nuanced understanding in high-risk contexts including end-arterial sites, barrier-compromised skin, and scenarios predisposing to systemic toxicity. This narrative review synthesizes contemporary evidence across these three interacting domains to provide an integrated, risk-stratified framework for clinical decision-making. Regarding end-arterial territories, over two decades of clinical evidence encompassing more than 200,000 digital and acral injections has effectively dismantled the historical dogm

Early incident and subsyndromal delirium in older patients undergoing elective surgical procedures: a randomized clinical trial of an avoid delirium protocol.

BACKGROUND: Pharmacological avoidance guidelines for preventing delirium have been suggested; however, there are limited pragmatic studies of these strategies. Early (<24 h) delirium can be observed in the postoperative care unit and is associated with an increased risk of subsequent delirium. We examined the effectiveness of an avoid delirium protocol (ADP) in older (>65 years) patients undergoing elective surgeries. METHODS: The randomized controlled trial assessed an ADP developed using the American Geriatric Society's Clinical Practice Guidelines for Postoperative Delirium in Older Adults

Risk factors for postoperative mortality at 30 days in elderly Chinese patients with hip fractures.

UNLABELLED: Arrhythmia, pneumonia, cardiac insufficiency, a high leukocyte count, and low albumin concentrations were associated with increased 30-day mortality in elderly hip fracture patients after surgery. It is important to improve short-term survival rates by optimizing the respiratory and cardiac function of geriatric patients before they undergo surgery. INTRODUCTION: This study aims to investigate the 30-day mortality and related risk factors for elderly patients following surgery for hip fractures. METHODS: This retrospective study examined chart reviews for evaluating associations

The relationship between intraoperative hypotension and acute kidney injury in elderly and super elderly patients undergoing noncardiac surgery: a retrospective cohort analysis.

BACKGROUND: Intraoperative hypotension has been identified as a significant factor associated with postoperative renal dysfunction. While numerous studies have reported on the relationship between incidence of postoperative acute kidney injury (AKI) and intraoperative hypotension, limited research has specifically focused on elderly and super elderly patients. METHODS: This retrospective cohort study comprised subjects aged 60 years and older who underwent elective non-cardiac surgery under general anesthesia. AKI was defined in accordance with established clinical criteria based on changes in

DENTAL DISEASES AND OTHER ORAL PATHOLOGIES OF CAPTIVE JAGUARS (PANTHERA ONCA) FROM BELIZE, CENTRAL AMERICA.

Dental and oral diseases are prevalent in many mammalian species including wild felids. Determining the dental and oral health status of captive animal populations can help establish preventive and therapeutic strategies, leading to improved welfare and conservation efforts. The aim of this study was to assess the prevalence of periodontal disease, endodontic disease, tooth resorption, and other clinically relevant dental and maxillofacial abnormalities in a population of captive jaguars (Panthera onca) using clinical, radiographic, and histopathological findings. Fifteen jaguars, ranging from

Different MMSE Score Is Associated with Postoperative Delirium in Young-Old and Old-Old Adults.

BACKGROUND: Postoperative delirium is one of the most common postoperative complications in geriatric patients. Mini-mental state examination (MMSE) assesses cognitive function in patients and is associated with postoperative delirium. However, whether there is an age-dependent relationship between preoperative MMSE score and postoperative delirium remains unknown. METHODS: We therefore set out to investigate the association between preoperative MMSE score and postoperative delirium in young-old (≤80 year-old, 75.46±4.69 years, 27.0% male, n = 63) and old-old (>80 year-old, 84.51±3.46 years,

Epidural versus subpleural analgesia for pulmonary resections: a comparison of morbidities.

There are several methods to mitigate the pain that results from thoracic surgery. All of them may be associated with complications. We analyzed the morbidities associated with epidural and subpleural catheter analgesia in patients undergoing pulmonary resections for lung cancer. We conducted a retrospective review of our prospective lung cancer outcomes database for all patients undergoing lobectomy for lung cancer through a thoracotomy or thoracoscopy. All patients had either an epidural or subpleural catheter placed for pain control. One hundred twenty-nine patients met the inclusion criter

Efficacy and safety of endoscopic retrograde cholangiopancreatography in the very elderly by using a combination of intravenous midazolam, ketamine and pethidine.

AIM: The number of therapeutic endoscopic procedures in elderly individuals keeps increasing and this population has a high risk of adverse events related to sedation and general anesthesia. However, there is a paucity on data about the efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) in this population. METHODS: In total, 417 consecutive ERCP procedures were performed in 362 patients between September 2018 and January 2020. Of these, 59 patients (74 sessions) were aged ≥80 years (Group A) and 173 patients (193 procedures) were aged ≤65 years (Group B). We analyzed

Renal disease in the elderly.

With advancing age and in the absence of any disease, there is a significant alteration in kidney structure and a gradual decline in renal function. While RPF, GFR, and tubular reabsorptive capacity decrease with age, abnormal signs or symptoms cannot be detected under ordinary circumstances. The aging kidneys, however, have limited capacity to cope with rapid hemodynamic changes or disturbances in fluid and water balance, and this results in signs and symptoms of renal dysfunction. In the elderly, the incidence of primary renal diseases declines while the incidence of renal diseases secondary

Postoperative Delirium Severity and Recovery Correlate With Electroencephalogram Spectral Features.

BACKGROUND: Delirium is an acute syndrome characterized by inattention, disorganized thinking, and an altered level of consciousness. A reliable biomarker for tracking delirium does not exist, but oscillations in the electroencephalogram (EEG) could address this need. We evaluated whether the frequencies of EEG oscillations are associated with delirium onset, severity, and recovery in the postoperative period. METHODS: Twenty-six adults enrolled in the Electroencephalography Guidance of Anesthesia to Alleviate Geriatric Syndromes (ENGAGES; ClinicalTrials.gov NCT02241655) study underwent major
