Improving oral care practice in long-term care.

At Deer Lodge Centre, oral care practice for adult dependent patients often included the use of sponge swabs and liquid mouth rinse, but the facility had no formal policy outlining best practice. The authors sought to develop such a policy by answering two main questions: Are sponge swabs effective in cleaning the oral cavity? What oral care is required for individuals with dysphagia and those who depend on others for oral care? After a review of the literature for pertinent guidance, a new protocol for oral care, based on tooth brushing and use of antibacterial gel, was implemented for one ca

Interprofessional education for oral-systemic preventive care in older adults: evaluation of a nursing-dental educational session.

BACKGROUND: The oral-systemic connection is a critical yet under-addressed component of geriatric care, despite the high burden of oral disease and preventable morbidity among older adults. Interprofessional education (IPE) offers a strategy to strengthen team-based communication, clarify professional roles, and support collaborative preventive care. OBJECTIVES: To describe the development and implementation of a joint nursing-dental IPE session integrating geriatric oral health, oral cancer screening, and vaccination content, and to evaluate students' post-session perceptions of interprofess

Interdisciplinary Strategies for Improving Oral Health in Older Adults: A Comprehensive Review.

Oral health in older adults is a critical component of overall well-being requiring integrated, interdisciplinary approaches to address its complex interplay of medical, functional, and psychosocial challenges. The aim of this is to examine strategies to enhance interdisciplinary collaboration among dental professionals, physicians, nurses, nutritionists, and caregivers to improve oral health outcomes in aging populations. Older adults commonly face dental problems such as periodontal disease which can be exacerbated by polypharmacy, systemic diseases, and barriers to accessing care. These mul

The importance of diagnostic procedures to ensure quality of health care in geriatric medicine. Examples from recent studies.

Within the health care of the elderly with prevention, diagnosis, therapy, rehabilitation, nursing care and social service, diagnostic procedures are of great importance to avoid under- and over-diagnosis. Many diagnostic difficulties exist in elderly patients such as changed reference values, changed normal values and changed signs and symptoms. Well-known examples of conditions which are likely to be under-diagnosed include depression and urinary incontinence. Examples are given from the cardiopulmonary field where e.g. dyspnoea showed to be very common, but in only 36% of males and 52% in f

Does the degree of limitation in activities of daily living in geriatric patients influence the amount of care and case management required? An analysis from the RubiN project.

BACKGROUND: The number of people in Germany over the age of 70 will increase significantly over the next 10 years. This will be accompanied by an increase in geriatric diseases and disabilities. An important goal for many geriatric patients is to remain in their own homes for as long as possible, while making use of support services. To achieve this, patients with limitations in their daily activities and in need of geriatric care should be identified as early as possible. The RubiN project implemented assessment-based care and case management for geriatric patients in physician networks in Ge

Nurses' general attitudes and caregiving-specific perceptions toward the oldest-old: A nationwide survey.

BACKGROUND: Despite the growing population of older adults worldwide, nurses may harbor negative attitudes toward older patients that might risk patient safety. Most studies have investigated general attitudes toward older adults. Only a few have focused on nurses' perceptions of caring for older patients, and there has been little focus on the oldest-old (aged ≥80 years), an age group that is particularly affected by negative attitudes. Emerging models to combat this issue suggest that education about aging and interaction with older adults are key factors. These models have primarily been ev

The blocked bed: a prospective study.

A prospective study of all patients (15 years and over) admitted to acute medical beds at a medium sized general hospital was carried out. For each patient, demogaphic data, day and time of admission, consultant in charge and the main reason for admission, were recorded. The patients hospital stay was analysed to determine the extent of the bed blocking problem. Sixty-two percent of total days stay required acute medical intervention, 32% was spent receiving nursing care only, 4% paramedical activities and 3% awaiting outside medical consultations. The three major factors found to influence to

Predictors of Dependency in Geriatric Trauma Patients with Rib Fractures: A Population Study.

The geriatric trauma population is unique. These patients are at risk of being discharged to rehabilitation or a skilled nursing facility, instead of being returned to their homes, placing a significant burden on both the patient families and society. This study evaluated which patient characteristics increase the likelihood of a previously independent geriatric blunt trauma becoming functionally dependent and being discharged to a location other than home. Data were extracted from the National Trauma Data Bank from 2012 to 2014 for blunt trauma patients ≥65 years old, admitted from home, with

[Artificial intelligence in the care of geriatric patients-more of a curse or more of a blessing?].

BACKGROUND: Artificial intelligence (AI) is conquering medicine in many fields. With geriatric patients, it is important not only to understand the decision tree of, for example, a tumor disease, but also to assess their functionality and functional deficits. METHODS: For potential (urological) care of geriatric patients, AI-supported tumor boards, AI-simulated human interaction, and AI-based polypharmacy tools were identified. The advantages and disadvantages, risks and benefits, and practical aspects are weighed against each other. RESULTS: The specific characteristics of geriatric patient
