Glaucoma is the second cause of blindness worldwide. In the next years, approximately one-fourth of all glaucoma cases will be angle-closure types. Older age, female sex, and Asian ethnicities are described risk factors as well as shorter axial length, shallow anterior chamber depth, lens position, and thickness. New findings suggest iris volume and choroidal thickness to have a key role in the mechanisms of angle closure. Classical clinical conditions are acute, intermittent, and chronic forms. Ultrasound biomicroscopy and anterior segment optical coherence tomography contribute to a better u

Primary angle-closure glaucoma is a major cause of blindness worldwide. It is a disease of ocular anatomy that is related to pupillary-block and angle-crowding mechanisms of filtration angle closure. Eyes at increased risk for primary angle-closure are small with decreased axial length, anterior chamber depth, and filtration angle width, associated with a proportionately large lens. Angle-closure glaucoma afflicts Asian and Eskimo eyes more frequently than eyes in other races with similar predisposing dimensions. The treatment of primary angle closure addresses its causal mechanisms. Laser per

ISGEO classification was proposed as the diagnostic criterion of primary angle closure glaucoma in epidemiological prevalence survey, which is preferable to be used in epidemiology rather than employing as a clinical diagnostic criterion. A different classification means a different definition. To unify the different point-views on the ISGEO classification, a further understanding of glaucoma definition is discussed and reviewed.

PURPOSE: Certain types of glaucoma are linked to nuclear genetic mutations or to mitochondrial disturbances. In this study, patients with primary angle-closure glaucoma (PACG) were examined for mutations in nuclear genes reported to be associated with glaucoma and for possible mitochondrial abnormalities.
METHODS: In patients with PACG, the nuclear genes MYOC, OPTN, CYP1B1, WDR36, OPA1, and OPA3 were sequenced, the entire mitochondrial (mt)DNA coding region was sequenced, relative mtDNA content was measured, and mitochondrial respiratory activity (MRA) was assessed.
RESULTS: No novel or prev

Primary angle-closure glaucoma is a leading cause of blindness worldwide. However, the terminology used for angle-closure in the literature is inconsistent, with inappropriate emphasis on symptomatology. A new nomenclature for primary angle-closure glaucoma focuses on the presence of end-organ damage and limits the use of the term "glaucoma" only for people who have suffered injury to the optic nerve. This review describes the various modalities of treatment for primary angle-closure glaucoma. The role and limitations of laser peripheral iridotomy in the management of the different forms of th

BACKGROUND: Angle-closure glaucoma is a well-known sequel to syphilitic interstitial keratitis. This study describes angle-closure glaucoma in the absence of corneal opacity as a presumed presenting sign of syphilis.
CASES: Two patients presented with angle-closure glaucoma with high peripheral anterior synechiae to the level over Schwalbe's line extending the whole circumference of the angle. Neither corneal opacity nor aqueous inflammation was present. Diffuse or localized retinochoroidal degeneration with pigmentation was found in 1 eye of 1 patient and in both eyes of the other patient.

Iris specimens obtained during the peripheral iridectomy in 12 cases (18 eyes) of primary angle closure glaucoma were studied histologically. The iris specimens taken during the senile cataract extraction in otherwise normal eyes were used as controls. The remarkable differences of the two groups are the changes of the quality and quantity of the mitochondria in the cells of iris tissue. The changes in primary glaucoma, especially in the intermittent stage of the angle closure glaucoma, were found in the mitochondria of the cells.

BACKGROUND: Carotid cavernous fistulas are abnormal communications between the cavernous sinus and the external or internal carotid arteries. Although rare, closure of carotid cavernous fistulas can lead to immediate ocular complications. To our knowledge, our case represents the first report of worsening angle closure glaucoma and choroidal detachments over an extended period of two months subsequent to closure of a carotid cavernous fistula.
CASE PRESENTATION: A 70-year-old female with a history of primary angle closure glaucoma presented with 4 mm of proptosis, resistance to retropulsion,

This retrospective study evaluated differences in Corvis ST tonometry (CST) parameters between eyes with primary open-angle glaucoma (POAG) and primary angle-closure glaucoma (PACG) using propensity score matching. We analyzed 48 eyes with POAG and 48 eyes with PACG. Twelve CST parameters were compared using linear mixed-effects models, with group as a fixed effect and subject ID as a random intercept to account for inter-eye correlation, adjusting for age, sex, axial length, intraocular pressure, and central corneal thickness. Multiple comparisons were controlled using the Benjamini-Hochberg

PURPOSE: To compare lamina cribrosa thickness (LCT) of primary angle-closure glaucoma (PACG) and primary open-angle glaucoma (POAG) using the enhanced depth-imaging mode of the Heidelberg Spectralis spectral-domain optical coherence tomography (EDI-OCT).
PATIENTS AND METHODS: A comparative cross-sectional study was conducted. We enrolled 34 patients with PACG, 38 with POAG, and 62 controls, testing only one eye of each participant. Lamina cribrosa thickness was determined at the center of the optic nerve head using EDI-OCT. Nine points of LCT were measured, and LCT averages were analyzed.

PURPOSE: To evaluate utility values in Chinese glaucoma patients.
PATIENT AND METHODS: Singapore Chinese residents (n = 213) with primary open-angle glaucoma or primary angle-closure glaucoma were recruited from a single tertiary ophthalmic center. Standard face-to-face interviews were conducted to ask about utility values (time trade-off and standard gamble for both death and blindness). Ocular information, including current visual acuity, intraocular pressure, visual field defect, and cup-disc ratio were also obtained.
RESULTS: The mean time trade-off utility value was 0.88 (95% confidence
