Cutaneous id reactions: a comprehensive review of clinical manifestations, epidemiology, etiology, and management.

Id reactions are a type of secondary inflammatory reaction that develops from a remote localized immunological insult. To date, id reactions caused by various fungal, bacterial, viral, and parasitic infections have been reported. Superficial fungal infections, especially tinea pedis, are the most common cause of id reactions. Id reactions exhibit multiple clinical presentations, including localized or widespread vesicular lesions, maculopapular or scarlatiniform eruptions, erythema nodosum, erythema multiforme, erythema annulare centrifugum, Sweet's syndrome, guttate psoriasis, and autoimmune

A Hybrid Model for Psoriasis Subtype Classification: Integrating Multi Transfer Learning and Hard Voting Ensemble Models.

Background: Psoriasis is a chronic, immune-mediated skin disease characterized by lifelong persistence and fluctuating symptoms. The clinical similarities among its subtypes and the diversity of symptoms present challenges in diagnosis. Early diagnosis plays a vital role in preventing the spread of lesions and improving patients' quality of life. Methods: This study proposes a hybrid model combining multiple transfer learning and ensemble learning methods to classify psoriasis subtypes accurately and efficiently. The dataset includes 930 images labeled by expert dermatologists from the Dermato

Gamma delta + T cells from patients with psoriatic and rheumatoid arthritis respond to streptococcal antigen.

OBJECTIVE: To investigate cellular immune responses to streptococcal antigens in patients with psoriatic arthritis (PsA). To specifically examine responses of the gamma delta + T cell subset. METHODS: Proliferation of PsA synovial fluid lymphocytes (SFL) and peripheral blood lymphocytes (PBL) cultured with streptococcal antigen was measured using a 3H thymidine (3HTdr) uptake assay system. gamma delta + T cells from PsA PBL and SFL were phenotyped by flow cytometry. Following culture with streptococcal antigen, gamma delta + enriched SFL were sorted by automated flow cytometry and 3HTdr uptak

Role of superantigens in human disease.

Superantigens include bacterial products (mainly of streptococci and staphylococci) that stimulate T cells to proliferate nonspecifically through interaction with class II major histocompatibility complex products on antigen-presenting cells and then with variable regions on the beta chain of the T cell receptor complex. They include pyrogenic toxins (streptococcal scarlet fever toxins of serotypes A, B, and C, toxic shock syndrome toxin 1, and staphylococcal enterotoxin serotypes A, B, Cn, D, E, and G), streptococcal M protein, staphylococcal exfoliative toxin, and recently identified pyrogen

Retinoids and phototherapy for psoriasis.

Retinoids are the most widely used agents for systemic treatment of psoriasis; as structural and functional analogs of vitamin A, they are involved in the regulation of several biologic functions. Acitretin is the oral retinoid currently used, alone or in combination with other treatments, for plaque type, erythrodermic, and pustular psoriasis. Due to its high teratogenic effect, therapeutic contraception is required for women taking the drug. Narrowband ultraviolet B (nbUVB, 311 +/- 2 nm) is effective for guttate and plaque-type psoriasis. At the molecular level, UV light acts (1) directly (t

Anti-PD1/PDL1 induced psoriasis.

BACKGROUND: Immune checkpoint inhibitors are novel agents approved for the treatment of late-stage malignancies. Despite its important clinical benefits, checkpoint inhibition is associated with a unique spectrum of side effects known as immune-related adverse events. Skin toxicities are the most frequent immune-related adverse events during anti-PD1 blockade therapies. Among them, rare cases of psoriasis exacerbation have been reported. METHODS: We present the clinical characteristics of exacerbated psoriasis in 5 patients under anti-PD1/PDL1 therapy. RESULTS: A total of 5 patients were ove

A secondary syphilis rash with scaly target lesions.

A 40-year-old man reported a 5-day history of fever and malaise, followed by a pruritic generalized rash. He had well-demarcated erythematous papules and plaques with scaling. The patient was diagnosed with secondary syphilis. The skin biopsy showed a psoriasiform lichenoid dermatitis with plasma cells. The anti-T. pallidum antibody confirmed the presence of spirochetes. He was also found to be hepatitis C virus and human immunodeficiency virus positive. The characteristic rash of secondary syphilis may appear as maculopapular, evolving initially from macules to small reddish-brown papules wit

[Antibiotic therapy in skin diseases].

Antibiotics are used for systemic and topical skin diseases treatment, taking advantage of their antiseptic, bacteriostatic and also their anti-inflammatory and immunomodulating properties. In case of localized external skin layers infections, a topical treatment is adequate. In case of widespread and deeper infections a systemic treatment is needed. The latter dermatological diseases are mostly treated by beta-lactam antibiotics (penicillins and cefalosporins) macrolides and tetracycline. In topical treatment mostly erythromycin, clindamycin, mupirocyne and fusidic acid are used. Most common

Treatment of psoriasis with light and heat energy (LHE): a preliminary study.

In psoriasis the earliest observable electron microscopic changes are in the papillary dermis vasculature. Selective photothermolysis of the dermal vasculature using a 585-nm pulsed-dye laser (PDL) for the treatment of psoriasis was reported more than a decade ago. Although clinically effective, the pulsed-dye laser has limited practicality because of small spot size (5-10 mm) and lack of cost effectiveness. The objective of this study is to evaluate the safety and efficacy of a novel Light and Heat Energy (LHE) selective-photothermolysis system for the treatment of psoriasis. This system deli

Ichthyosis, psoriasiform dermatitis, and recurrent fungal infections in patients with biallelic mutations in PERP.

BACKGROUND: Germline autosomal dominant and autosomal recessive mutations in PERP, encoding p53 effector related to PMP-22 (PERP), a component of epidermal desmosomes, have been associated with a spectrum of keratodermas. Monoallelic nonsense mutations cause Olmsted syndrome with severe periorificial keratoderma and palmoplantar keratoderma (PPK). Biallelic recessive frameshift and missense mutations are associated with milder forms of the disease, including generalised erythrokeratoderma and PPK. OBJECTIVES: To add new insights into the genotype-phenotype correlations as a consequence of PER

Psoriasis as a spatial immune ecosystem: insights from single-cell and spatial transcriptomics.

Psoriasis is a chronic systemic inflammatory skin disease marked by substantial heterogeneity in both clinical presentation and immunopathology. Bulk RNA sequencing has been instrumental in establishing the IL-23/IL-17 axis as a central pathogenic framework, but tissue-averaged signals may obscure the cell-state heterogeneity and spatial microenvironments that distinguish clinical subtypes at single-cell resolution. Recent advances in single-cell RNA sequencing (scRNA-seq) and spatial transcriptomics (ST) have therefore prompted a conceptual shift from a model centered on individual cytokines

Identifying a Core Domain Set to Assess Psoriasis in Clinical Trials.

IMPORTANCE: There is no consensus on which domains should be measured or which instruments should be used in clinical trials for psoriasis therapies. OBJECTIVE: To achieve international consensus among psoriasis stakeholders on a core set of domains that should be measured in all psoriasis clinical trials. DESIGN, SETTING, AND PARTICIPANTS: Literature review, pre-Delphi survey exercises, nominal group discussions, and audience voting at 4 stakeholder meetings were used to develop candidate domains for 2 rounds of a Delphi survey. Stakeholders were patients or advocates of patients with psori

From Infection to Autoimmunity: S. pyogenes as a Model Pathogen.

Group A β-hemolytic Streptococcus (GAS) is a Gram-positive, coccoid-shaped bacterium that tends to grow in chains; it is a non-spore-forming, facultatively anaerobic, catalase-negative, aerobic bacterium. It is known to cause a wide range of infections in children, ranging from mild upper respiratory tract infections, such as pharyngitis, to severe invasive disease. GAS also notably triggers post-infectious immune sequelae, including acute poststreptococcal glomerulonephritis (APSGN), acute rheumatic fever (ARF), and rheumatic heart disease (RHD), which are major health burdens, especially in

Peripheral blood lymphocytes from psoriatic patients are hyporesponsive to beta-streptococcal superantigens.

The strong association of acute guttate psoriasis and streptococcal throat infection, together with the preferential use of T cells expressing a particular T-cell receptor, has suggested a role for bacterial superantigens in the pathogenesis of psoriasis. We examined the proliferative responses of peripheral blood lymphocytes (PBLs), obtained from patients with psoriasis and from healthy controls, to streptococcal superantigens, cytoplasmic membrane-associated protein (CAP) and secretion-type CAP (SCAP), isolated from group A, beta-haemolytic streptococci. PBLs from patients with psoriasis sho

Purification and characterization of streptococcal proliferative factor.

Group A streptococcal infections are often associated with scarlet fever and flares of guttate psoriasis. Previous investigation has demonstrated the presence of a factor in streptococcal culture filtrates capable of stimulating proliferation of rabbit keratinocytes in vivo and human lymphocytes in vitro. This report outlines an in vivo method for the production of streptococcal proliferation factor, its purification, and characterization of its physical properties. We cultured Group A streptococci (Type 12, Strain NY5) in synthetic media by in vivo incubation within dialysis casing surgically

Decreased IL-10 production by psoriatic peripheral blood mononuclear cells stimulated with streptococcal superantigen.

The strong association of acute guttate psoriasis and streptococcal throat infection has suggested a role for streptococcal antigens in the pathogenesis of psoriasis. We have reported that psoriatic peripheral blood mononuclear cells (PBMCs) showed significantly lower responses to cytoplasmic membrane-associated protein (CAP) isolated from group A beta-hemolytic streptococci, a kind of streptococcal superantigen. The objectives were to evaluate the abnormal cytokine production by psoriatic PBMCs to streptococcal superantigen, CAP. We compared the production of four different cytokines, i.e. IL

The histologic and molecular correlates of COVID-19 vaccine-induced changes in the skin.

A total of 22 patients who had developed an adverse cutaneous reaction to the Moderna or Pfizer vaccine underwent biopsies. Each patient was assessed light microscopically, and, in select biopsies, spike glycoprotein and cytokine assessment were also conducted. The patients developed self-limited cutaneous reactions often described clinically as urticarial or eczematous within 1 day to 4 weeks after receiving the first or second dose of the Pfizer or Moderna vaccine. Classic clinical and morphologic depictions of type IV cutaneous hypersensitivity with features of eczematous dermatitis, interf

Skin manifestations in spondyloarthritis.

Spondyloarthritides (SpA) like psoriatic arthritis, axial spondyloarthritis/ankylosing spondylitis, reactive arthritis and inflammatory bowel disease (IBD)-associated SpA can present with characteristic skin manifestations. These SpA-associated skin disorders may precede joint involvement, reflect a loss of efficacy of a current systemic treatment or can even be treatment associated. Cutaneous manifestations in SpA not only add additional morbidity with physical impact but also impose a psychosocial burden on affected patients. Psoriasis (PsO) - the main skin disease in SpA - has a variety of
