Altered tibialis anterior tendon mechanics in hallux valgus: a shear-wave elastography study integrating distal insertion patterns.

BACKGROUND: While distal tibialis anterior tendon (TAT) insertion variants have been associated with hallux valgus, the in vivo mechanical behavior of the tendon has not been adequately investigated. This study aimed to evaluate tibialis anterior tendon stiffness using shear-wave elastography (SWE) in patients with hallux valgus and healthy controls, to examine its relationship with radiographic deformity parameters, distal insertion patterns, and body mass index, and to assess measurement reliability. METHODS: In this prospective case-control study, 105 patients with radiographically confirm

Flexible Fixation in Foot and Ankle Surgery.

Flexible fixation has been described and utilized in various aspects of foot and ankle surgery over the past several decades. In regards to ankle surgery, flexible fixation devices have been used for stabilization of the ankle syndesmosis and augmentation of lateral collateral ankle ligament repair. In the foot, flexible fixation devices have been incorporated into hallux valgus or varus correction, Lisfranc injury repair, and more recently spring ligament repair augmentation. This article reviews the various applications for flexible fixation in foot and ankle surgery, as well as evidence-bas

Relationship between adolescent bunions and flatfeet.

Pes planus has been suggested as a factor in the development of bunions in adolescents. This study objectively evaluated the radiographs of 36 adolescents with 66 hallux valgus deformities for the presence of associated flatfeet. The bunion deformity in these adolescents was also characterized. Only 26.6% of these adolescents had an increased intermetatarsal angle and 8.1% an abnormal metatarsus varus angle as compared with the published norms. Measurements of pes planus including talar pitch, calcaneal plantar angle, dorsoplantar talonavicular angle, and lateral talocalcaneal angle showed an

Evaluating function in the hallux valgus foot following a 12-week minimalist footwear intervention: A pilot computational analysis.

Hallux valgus is a foot pathological condition showing a lateral deviation of the first phalange and medial deviation of the first metatarsal. The purpose of the current study was to evaluate a longitudinal effect of minimalist footwear running protocol for a mild/moderate hallux valgus patient. The computer tomography (CT) images from a male hallux valgus (HV) patient were respectively scanned pre and post 12-week minimalist footwear running intervention. The pre and post -intervention foot finite element (FE) models were developed from the foot three-dimensional geometries manually segmented

Longitudinal Epiphyseal Bracket of the First Metatarsal Bone: Three Case Reports and a Review of the Literature.

Longitudinal epiphyseal bracket (LEB) is a rare bone dysplasia of the tubular bones. Owing to an abnormal secondary ossification center, the affected bones can develop progressive shortening and angular deformity. The aim of our study was to provide an overview of the reported data regarding epidemiology and surgical procedures available for LEB of the first metatarsal bone in a pediatric population combined with a small case series. We report a retrospective case series of 3 nonsyndromic pediatric patients with different ages and with confirmed dysplasia of the first metatarsal bone. All pati

Early Results of Immediate Weightbearing Following First Tarsometatarsal Joint Arthrodesis With Plantar Locking Plate and Dorsal Compression Screw.

Arthrodesis of the first tarsometatarsal joint (TMT1) reduces pathologic angles at the anatomic center of rotation of angulation and presents a substantial correction potential in 3 planes in the treatment of hallux abductovalgus. The optimal fixation method remains unclear since prevailing dorsomedial locking plates and/or compression screws depict elevated implant-associated complications. Medical records of 49 patients that underwent 53 TMT1 arthrodeses in hallux abductovalgus interventions were included. Median average visual analog scale scores decreased (p < .001) from 6.8 (range 4-10) t

Complications after Metatarsal Osteotomies for Hallux Valgus: Malunion, Nonunion, Avascular Necrosis, and Metatarsophalangeal Osteoarthritis.

Avascular necrosis (AVN), nonunion, malunion, and metatarsophalangeal (MTP) osteoarthritis following hallux valgus osteotomies, as well as pathophysiology, diagnosis, prevention strategies, and treatment are discussed in this article. AVN and nonunion are very infrequent, and they can be effectively prevented taking into consideration local anatomy preservation, biomechanics, and patient comorbidities. Shortening, elevation, plantarflexion, varus/valgus, and rotational of the first metatarsal are the most common types of malunion. They can lead to pain, stiffness, deformity recurrence, and tra

Polydactyly of the foot.

One hundred twenty-five patients with 194 supernumerary toes were analyzed. Postaxial polydactyly accounted for 79%. Duplication at the proximal phalanx with either a block metatarsal or a wide metatarsal head was the most common anatomical pattern. Sixty-one patients with 97 duplications evaluated at an average follow-up of 15.1 years postoperatively had 94% excellent or good results. Poor results were often associated with preaxial duplications and persistent hallux varus. Patients with central duplications often had a persistently wide forefoot. Surgical treatment is usually straightforward

Sterkfontein member 2 foot bones of the oldest South African hominid.

Four articulating hominid foot bones have been recovered from Sterkfontein Member 2, near Johannesburg, South Africa. They have human features in the hindfoot and strikingly apelike traits in the forefoot. While the foot is manifestly adapted for bipedalism, its most remarkable characteristic is that the great toe (hallux) is appreciably medially diverged (varus) and strongly mobile, as in apes. Possibly as old as 3.5 million years, the foot provides the first evidence that bipedal hominids were in southern Africa more than 3.0 million years ago. The bones probably belonged to an early member

Infantile tibia vara: correction of recurrent varus deformity following epiphyseolysis.

Infantile tibia vara (infantile Blount's disease) is a condition in which progressive varus of the knee develops due to diminished physeal growth of the medial aspect of the proximal tibia. A 6-year-old boy with severe bilateral infantile tibia vara (Langenskiold stage V) underwent operative correction of his deformities. Surgery consisted of epiphyseolyses of the medial proximal tibiae and valgus-producing corrective tibial osteotomies. Nearly 5 years after symmetric growth, he again developed progressive varus involving his left knee. Usually, a repeat proximal tibial osteotomy is required a

Surgical treatment of adolescent hallux valgus.

Surgical for adolescent hallux valgus (HV) was performed on 32 feet. Five feet (16%) required revision surgery, and technical errors accounted for recurrence in all five. Satisfaction was obtained in 70% of patients. According to a new bunion rating score, 59% of patients had good to excellent results. In a subgroup of patients treated with metatarsal osteotomy for metatarsus primus varus, the Mitchell osteotomy provided an excellent rating in 95% of patients. Surgery for adolescent HV provides satisfying results in most patients. The one factor most highly correlated with both decreased risk

The reliability and validity of radiographic measurements for determining the three-dimensional position of the talus in varus and valgus osteoarthritic ankles.

OBJECTIVE: To assess the most accurate radiographic method to determine talar three-dimensional position in varus and valgus osteoarthritic ankles, we evaluated the reliability and validity of different radiographic measurements. MATERIALS AND METHODS: Nine radiographic measurements were performed blindly on weight-bearing mortise, sagittal, and horizontal radiographs of 33 varus and 33 valgus feet (63 patients). Intra- and interobserver reliability was determined with the intraclass coefficient (ICC). Discriminant validity of measurements between varus and valgus feet was assessed with effec

Long Z-osteotomy: a review and new modification to correct troughing.

The long Z-osteotomy (scarf) has proven to be an effective procedure for correction of the metatarsus primus varus component of the hallux abducto valgus deformity. An historical review and technical considerations of the procedure are described. Although technically demanding, it offers the advantages of superior strength with internal fixation, early range of motion, and early weightbearing. An uncommon but challenging intraoperative complication of the procedure is troughing (channeling). The author describes a technique that involves transfer of the adductor hallucis tendon through the hor

Fixation of metatarsal osteotomies in the treatment of hallux valgus.

Osteotomies as a treatment for hallux valgus require careful preoperative planning and meticulous attention to surgical technique. The procedure selected should be tailored to fit the deformity. For a patient with mild hallux valgus, the chevron osteotomy is the most intrinsically stable of the distal osteotomies and has the least potential for complications. Interest in the Ludloff osteotomy has had a resurgence because of its improved stability compared with more traditional osteotomies for correction of metatarsus primus varus. This osteotomy, however, is less forgiving and more dependent o

Correction of Hallux Valgus Interphalangeus With an Osteotomy of the Distal End of the Proximal Phalanx (Distal Akin Osteotomy).

BACKGROUND: Operative correction of a symptomatic hallux valgus interphalangeus (HVI) deformity is often achieved with an osteotomy at the proximal end of the proximal phalanx (Akin osteotomy). However, the apex of the typical HVI deformity (center of rotation angle) is at the interphalangeal joint of the hallux. This study was done to evaluate the results of performing a medial closing wedge osteotomy at the distal end of the proximal phalanx. METHODS: Thirty-three patients (33 feet) underwent an osteotomy at the distal end of the proximal phalanx for correction of HVI. All of the patients h

A combined classification system for preaxial foot polydactyly and its derived surgical strategies.

BACKGROUND: Pediatric polydactyly is a relatively common congenital condition of hand and foot, affecting physical and mental development of children. Hand polydactyly has been brought into focus recently. By contrast, the studies on foot polydactyly are limited. Among the types, there is little international research on preaxial foot polydactyly since it is a relatively rare type, and the domestic research on it is fewer. The purpose of our study is to propose a new classification for guiding operation. METHODS: We collected all patients with preaxial foot polydactyly who had preoperative ra
