Failed Hemispherotomy: Insights from Our Early Experience in 40 Patients.

OBJECTIVE: To study the factors responsible for failure of hemispherotomy and outcomes of revision surgery. The effect of the surgeon's learning curve on failures was also analyzed. METHODS: Forty consecutive patients, who underwent functional hemispherotomy through a 4-year period, from the inception of the single-surgeon epilepsy surgery program, were analyzed. RESULTS: A total of 47 functional hemispherotomies were performed in the study period in 40 patients (7 revision surgeries on 6 patients). Mean age of the cohort was 9.45 ± 14.84 years and it included 7 infants (<2 years). Of the 9

Cognitive outcome of parietooccipital resection in children with epilepsy.

PURPOSE: We followed the neuropsychological development of five children who underwent unilateral neurosurgery of the occipitoparietal lobes as a treatment for epilepsy caused by a developmental lesion (cortical dysplasia). METHODS: The follow-up period ranged from 3-7 years postsurgery. RESULTS: Two participants had a verbal intelligence quotient (IQ) >100 and three had a verbal IQ between 65 and 80. All five children had abnormal nonverbal IQ and exhibited deficits related to visual attention, object recognition, and praxis. Nevertheless, our results suggest that brain plasticity after par

Resective surgery to treat refractory status epilepticus in children with focal epileptogenesis.

Prolonged high-dose suppressive therapy (HDST) is a mainstay in the management of refractory status epilepticus (RSE), albeit with high morbidity and mortality. The authors studied 10 patients who were carefully selected for epilepsy surgery after failing prolonged (>2 weeks) HDST. Status epilepticus was stopped acutely in all of them with no mortality and no substantial morbidity. At follow-up (median 7 months), 7 (70%) of 10 patients were seizure free, and 3 (30%) of 10 had significant improvement in their epilepsy.

[Indications and risk of neurosurgical techniques in the adult presenting with drug-resistant partial epilepsy (radiosurgery included)].

To be considered for resective (curative) surgery, most seizures have to been proved to arise exclusively from one area of the brain that is functionally silent. The drug-resistance must be certain, and the patient must be strongly motivated to undergo surgery. Temporal lobectomy for drug-resistant temporo-mesial epilepsy is now scientifically validated by a randomized controlled trial. Hemispherotomy, which consists in complete disconnection of one hemisphere, is a curative technique, which may be considered where there is a pre-existing hemiplegia associated with a structural abnormality of

Cerebral metabolism following neonatal or adult hemineodecortication in cats: I. Effects on glucose metabolism using [14C]2-deoxy-D-glucose autoradiography.

In the cat, cerebral hemispherectomy sustained neonatally results in a remarkable degree of recovery and/or sparing of function as compared with the effects of a similar lesion but sustained in adulthood. We have proposed that this effect is due to a combination of reduced neuronal loss within partially denervated structures and a lesion-induced reorganization of corticofugal projections arising from the remaining intact hemisphere in the neonatally lesioned animal. The current study was designed to assess the physiological consequences of these anatomical changes utilizing [14C]2-deoxy-D-gluc

The Kempe incision for decompressive craniectomy, craniotomy, and cranioplasty in traumatic brain injury and stroke.

OBJECTIVE: Decompressive craniectomy (DC) is an effective, lifesaving option for reducing intracranial pressure (ICP) in traumatic brain injury (TBI), stroke, and other pathologies with elevated ICP. Most DCs are performed via a standard trauma flap shaped like a reverse question mark (RQM), which requires sacrificing the occipital and posterior auricular arteries and can be complicated by wound dehiscence and infections. The Ludwig Kempe hemispherectomy incision (Kempe) entails a T-shaped incision, one limb from the midline behind the hairline to the inion and the other limb from the root of

A lack of clinically apparent vision loss among patients treated with vigabatrin with infantile spasms: The UCLA experience.

BACKGROUND: Vigabatrin (VGB) is one of two FDA-approved medications for treatment of infantile spasms. Despite demonstrated efficacy, its use has been curtailed by reports indicating a substantial risk of VGB-associated visual field loss (VAVFL). As these reports have conflicted with our clinical observations in routine practice, we systematically reviewed the experiences of patients treated with VGB at UCLA to estimate the prevalence of clinically apparent VAVFL. METHODS: Patients with video-EEG-confirmed infantile spasms evaluated at our center between February 2007 and February 2014 were r

Clinical study of catastrophic infantile epilepsy with focal seizures.

This study investigated clinico-electrical and etiologic characteristics of catastrophic infantile epilepsy with focal seizures developed in early infancy. The patients included 15 children who fulfilled the following criteria: seizure onset before 12 months of age, presence of daily focal or secondarily generalized seizures resistant to antiepileptic drugs for at least 3 months, and exclusion of Ohtahara and West syndromes. Patients were classified into three subgroups. Three patients demonstrated progressively deteriorating neurologic symptoms associated with progressive cerebral atrophy and

Are there false-negative results of motor evoked potential monitoring in brain surgery?

OBJECT: The present study explores the causes of occasional postoperative pareses despite unchanged or fully recovered intraoperative motor evoked potentials (MEPs) in supratentorial brain surgeries. METHODS: In a prospective, observational design, MEP monitoring results, motor outcome, and perioperative imaging were correlated in 200 procedures for brain tumours and cortical dysplasias critically related to motor areas and pathways. RESULTS: Persisting pareses after unchanged or recovered MEPs occurred in four cases due to delayed ischemia, or venous congestive oedema. Transient new deficit

The impact of lesions and epilepsy on personality and mood in patients with symptomatic epilepsy: a pre- to postoperative follow-up study.

PURPOSE: To discern more static and lesion based from more dynamic epilepsy-driven features of depression and personality in focal symptomatic epilepsy, change in personality and mood was examined as a function of focus lateralization and seizure outcome after epilepsy surgery. METHODS: Self-reported personality and depression were assessed before and 1 year after epilepsy surgery in 151 consecutive patients with temporal (N=125) or extratemporal lobe epilepsy (N=26). RESULTS: At baseline only depressed mood (31%) and introversion (38%) appeared elevated, the first being more frequent in rig

United States emergency department visits for children with cerebrospinal fluid shunts.

OBJECTIVE: CSF shunt placement is the primary therapy for hydrocephalus; however, shunt malfunctions remain common and lead to neurological deficits if missed. There is a lack of literature characterizing the epidemiology of children with possible shunt malfunctions presenting to United States emergency departments (EDs). METHODS: A retrospective study was conducted of the 2006-2017 National Emergency Department Sample. The data were queried using an exhaustive list of Current Procedural Terminology and International Classification of Diseases, Ninth Revision and International Classification

Prospective and longitudinal long-term employment outcomes after resective epilepsy surgery.

OBJECTIVE: To investigate long-term employment outcomes after resective epilepsy surgery in a national population-based cohort of adults. METHODS: In the Swedish National Epilepsy Surgery Register, all adults who were operated with resective epilepsy surgery from 1995 to 2010 were identified. Two-year follow-up was available for 473/496, 5-year follow-up for 220/240, 10-year follow-up for 240/278, and 15-year follow-up for 85/109 patients. RESULTS: There were no significant changes in employment outcome over time at group level, but for those with full-time employment at baseline, 79%, 79%,

Significance of the Feuerstein approach in neurocognitive rehabilitation.

BACKGROUND: The theory of Structural Cognitive Modifiability and Mediated Learning Experience of Reuven Feuerstein states that individuals with brain impairment, because of congenital or acquired origin, may substantially and structurally improve their cognitive functioning, by a systematic intervention based on a specific, criteria-based type of interaction ("mediated learning"). Three application systems are based on it: a dynamic-interactive assessment of learning capacity and processes of learning, the LPAD (Learning Propensity Assessment Device); a cognitive intervention program called "I
