A case of mumps associated with acute hemolytic crisis resulting in hemoglobinuria and acute renal failure.

A 13-year-old boy presented with acute hemolytic anemia a few days after mumps infection. The ensuing hemoglobinuria resulted in acute renal failure in this child, which was corrected with fluid and alkali therapy. We draw attention to this uncommon complication of mumps and the need for careful evaluation.

[Paroxysmal nocturnal hemoglobinuria with multiple venous thrombosis and acute renal insufficiency. Description of a case].

Oral anticoagulants were administered to a young woman suffering from P.N.H. after the development of portal vein thrombosis. She interrupted the treatment and developed multiple splanchnic thromboses followed by acute renal failure, from which she recovered four weeks later. Although the pathogenetic mechanism could include renal vascular microthrombosis, the possibility of renal ischemic damage due to massive blood pooling within the splanchnic district is discussed.

A platelet and granulocyte membrane defect in paroxysmal nocturnal hemoglobinuria: usefulness for the detection of platelet antibodies.

The tendency of platelets and leukocytes to lyse after their interaction with antibody and complement was studied by measuring the release of (51)Cr from cells labeled with this isotope. Platelets from six patients with paroxysmal nocturnal hemoglobinuria (PNH) were 15-230 times more sensitive to antibodies and 10-32 times more sensitive to complement than normal platelets or platelets from patients with other types of thrombocytopenic or hemolytic disorders. Mixed white blood cell (WBC) preparations from patients with PNH were 3-20 times more sensitive to anti-WBC antibodies and 5-10 times mo

Iptacopan combined with immunosuppressive therapy for aplastic anemia-paroxysmal nocturnal hemoglobinuria syndrome: A case report.

RATIONALE: Aplastic anemia-paroxysmal nocturnal hemoglobinuria (AA-PNH) syndrome is a rare acquired hematopoietic stem cell disorder. Patients who respond poorly to conventional immunosuppressive therapy (IST) and corticosteroids face limited therapeutic options and a poor prognosis. This article reports on the use of iptacopan combined with IST for AA-PNH syndrome. PATIENT CONCERNS: A previously healthy 24-year-old Han Chinese male presented with dizziness and fatigue and was found to have pancytopenia. He was initially treated with cyclosporine. However, 2 years after discontinuation, he de

Case report: persistent anemia after eculizumab in paroxysmal nocturnal hemoglobinuria: non-dominantly active intravascular hemolysis.

OBJECTIVES: Paroxysmal nocturnal hemoglobinuria (PNH) is a rare condition characterized by intravascular hemolysis (IVH), thrombosis, and organ damage. Although C5 complement inhibitors such as eculizumab have significantly improved the prognosis of patients with PNH, persistent anemia remains in some patients. METHODS: We report a patient with PNH who initially responded to eculizumab but experienced recurrent hemoglobin decrease and became transfusion-dependent after 24 weeks of treatment. Here, we present the clinical features of this patient. In addition, we reviewed the current literatur

Beyond Terminal Blockade: A Mechanism-Based Approach to Complement Inhibitor Selection in Paroxysmal Nocturnal Hemoglobinuria.

Complement inhibitor selection in paroxysmal nocturnal hemoglobinuria (PNH) can no longer be reduced to a binary class-level decision. Terminal C5 inhibitors provide durable control of intravascular hemolysis (IVH) and the most mature evidence for thromboembolic risk reduction, supporting their continued primacy in patients with high thrombotic risk or established venous thromboembolism. Persistent anemia during C5 inhibition is mechanistically heterogeneous. Before it is ascribed to extravascular hemolysis (EVH) or bone marrow failure (BMF), the adequacy of terminal complement inhibition shou

AI-driven diagnostic algorithm enhances early detection of paroxysmal nocturnal hemoglobinuria in real-world settings.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, life-threatening hematologic disease with diagnostic delays exceeding 5 years in 24% of cases. We developed and deployed an artificial intelligence algorithm analyzing structured and unstructured electronic health record data across 14 healthcare organizations in Poland. Screening of 1,307,140 patients identified 356 high-risk individuals; of 119 referred for flow cytometry, 13 were diagnosed (positive predictive value: 10.92%; 95% CI, 9.68%-12.30%), comparing favourably to 6.9% conventional screening hit rate. High-risk patients were signi

Paroxysmal Nocturnal Hemoglobinuria-Driven Hepatic Vein Thrombosis: A Case of Budd-Chiari Syndrome in Disguise.

Paroxysmal nocturnal hemoglobinuria (PNH) is a rare acquired hematopoietic stem cell disorder characterized by complement-mediated intravascular hemolysis and a markedly increased risk of thrombosis, including at atypical splanchnic sites. We report a 26-year-old woman who presented with progressive abdominal distension, right upper quadrant pain, early satiety, and morning dark-colored urine. Investigations revealed normocytic anemia, biochemical evidence of intravascular hemolysis, moderate ascites, hepatomegaly, and elevated D-dimer; direct and indirect antiglobulin tests were negative. Treatment outcomes of complement protein C5 inhibition in 509 UK patients with paroxysmal nocturnal hemoglobinuria.

Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired clonal hematopoietic disorder that occurs on a background of bone marrow failure (BMF). In PNH, chronic intravascular hemolysis causes an increase in morbidity and mortality, mainly because of thromboses. Over the last 20 years, treatment of PNH has focused on the complement protein C5 to prevent intravascular hemolysis using the monoclonal antibody eculizumab and more recently ravulizumab. In the United Kingdom, all patients are under review at 1 of 2 reference centers. We report on all 509 UK patients with PNH treated with eculizumab a

Subacute stent thrombosis with spontaneously resolved secondary thrombi in paroxysmal nocturnal hemoglobinuria: a case report.

BACKGROUND: Stent thrombosis (ST) is a serious complication; however, a method to prevent ST in patients with thrombophilic diseases has not been established. CASE PRESENTATION: We report a case of subacute ST in a patient with paroxysmal nocturnal hemoglobinuria (PNH) who was receiving continuous heparin treatment in addition to the usual dual antiplatelet therapy for contrast defects at the proximal site of the occluded right coronary artery and the proximal site of the left circumflex artery. Despite the resolution of thrombi in secondary lesions, subacute ST occurred. After percutaneous c

Ravulizumab (ALXN1210) in patients with paroxysmal nocturnal hemoglobinuria: results of 2 phase 1b/2 studies.

Ravulizumab (ALXN1210), a humanized monoclonal antibody to complement component C5, was engineered from eculizumab to have a substantially longer terminal half-life, permitting longer dosing intervals for paroxysmal nocturnal hemoglobinuria (PNH) treatment. Two phase 1b/2 multicenter open-label studies evaluated efficacy and safety of multiple doses and regimens of ravulizumab in PNH patients naive to complement-inhibitor treatment. Patients in study 103 (n = 13) received ravulizumab 900 mg (lower trough exposure) or 1800 mg every 4 weeks (higher trough exposure); those in study 201 (n = 26) r

Immune Hemolytic Anemia (Paroxysmal Cold Hemoglobinuria) Preceding Burkitt Lymphoma in a 12-Year-Old Child.

Autoimmune hemolytic anemia (AIHA) in childhood, including paroxysmal cold hemoglobinuria, is an uncommon, potentially life-threatening disorder. AIHA is a recognized complication of several varieties of lymphoproliferative disorders, including high-grade B-cell lymphoma, but it has not been associated with Burkitt lymphoma in people without an underlying immunodeficiency. When AIHA occurs in association with lymphoproliferative disorders, it may precede or accompany the diagnosis of malignant disease or herald relapse. We report a novel case of a previously healthy child diagnosed with paroxy

Chronic treatment of paroxysmal nocturnal hemoglobinuria patients with eculizumab: safety, efficacy, and unexpected laboratory phenomena.

The terminal complement inhibitor eculizumab has become the standard of treatment in patients with symptomatic paroxysmal nocturnal hemoglobinuria (PNH). In this retrospective study, 19 PNH patients received chronic therapy with eculizumab with a median duration of 16 months (range 6-46 months). Parameters of hemolysis, transfusion requirements, and serum iron parameters were analyzed. Lactate dehydrogenase levels were significantly decreased by 85% from a median of 1897 U/l (range 293-3360) to 283 U/l (range 143-667), with an 86% reduction of transfusion requirements, whereas other parameters

Unilateral moyamoya syndrome involving the ipsilateral anterior and posterior circulation associated with paroxysmal nocturnal hemoglobinuria.

Moyamoya refers to a phenomenon of stenoocclusive changes at or around the terminal part of the internal carotid artery (ICA). Moyamoya vessels develop in patients with (moyamoya syndrome) or without (moyamoya disease) various underlying diseases or conditions. Recent evidence shows that stenoocclusive lesions tend to involve the ipsilateral ICA system and posterior cerebral artery (PCA) predominantly in moyamoya disease. A 53-year-old Japanese woman with paroxysmal nocturnal hemoglobinuria presented with cerebral infarction from stenoocclusive involvement in the ipsilateral ICA and PCA associ

COEXISTENCE OF APLASTIC ANEMIA AND PAROXYSMAL NOCTURNAL HEMOGLOBINURIA: DIAGNOSTIC CHALLENGES AND THERAPEUTIC STRATEGIES - CASE REPORT.

Aplastic anemia (AA) and paroxysmal nocturnal hemoglobinuria (PNH) are rare clonal bone marrow disorders. AA is characterized by autoimmune destruction of bone marrow stem cells and pancytopenia. In PNH, acquired genetic mutation and impaired glycoprotein synthesis leads to hemolysis and thrombus formation. The combination of both diseases represents a diagnostic and therapeutic dilemma, since increasing evidence suggests the connection between autoimmunity in AA clonal expansion of PNH. The purpose of this publication is to illustrate the pathogenetic relationship between AA and PNH by review

Hepatic Sinusoidal Obstruction Syndrome Secondary to Aplastic Anemia/Paroxysmal Nocturnal Hemoglobinuria Syndrome: A Rare Case.

Paroxysmal nocturnal hemoglobinuria (PNH) is associated with bone marrow failure disorders and may arise during the long-term follow-up of aplastic anemia (AA), which is named AA/PNH syndrome. Thrombosis is the most frequent clinical complication and is the main cause of mortality in PNH. However, thromboses tend to originate in hepatic and cerebral venous vessels, but rarely in the hepatic microvascular vein in PNH patients. Here, we report on a young man with hepatic sinusoidal obstruction syndrome (HSOS) secondary to AA/PNH syndrome. His main manifestations were hemolytic anemia, renal inju

Management of paroxysmal nocturnal hemoglobinuria in CALR mutated post-essential thrombocythemia myelofibrosis: A case report.

Paroxysmal nocturnal hemoglobinuria (PNH) results from the loss of erythrocyte surface proteins, leading to complement activation and its spectrum of effects. We explore this case of a 57-year-old man with post-essential thrombocythemia (ET) myelofibrosis (MF) who developed symptomatic anemia with evidence of hemolysis on lab work. While hemolysis was localized to be intramedullary based on workup, the exact diagnosis was undetermined, leading to a prolonged course of steroid therapy to control anemia. The hemolysis was eventually attributed to PNH diagnosed on flow cytometry and the patient w

Mycoplasma pneumoniae infection complicated by paroxysmal cold hemoglobinuria with anti-P specificity of biphasic hemolysin.

A 7-year-old boy with Mycoplasma Pneumoniae infection complicated by transitory paroxysmal cold haemoglobinuria (PCH) is described. The Donath-Landsteiner-antibody exhibited anti-P specificity; hemolytic activity was partially inhibited against papainized erythrocytes at 0 degrees C incubation temperature and increased from 8 degrees C upwards. The association of Mycoplasma pneumoniae infection with PCH has been described 4 times only and in one instance where specificity was stated it was anti-I.

Paroxysmal nocturnal hemoglobinuria with a positive Coombs test presenting as acute kidney injury: a case report.

This report presents the case of a 60-year-old woman who was admitted with symptoms of fatigue and poor appetite. Laboratory investigations revealed severe acute kidney injury (AKI), indicated by a serum creatinine level of 2,065 μmol/L, necessitating the initiation of emergency hemodialysis. The patient also exhibited hemolytic anemia (hemoglobin, 69 g/L), thrombocytopenia (platelet nadir, 45 × 109/L), venous thrombosis, a positive direct antiglobulin test (DAT), and a low absolute reticulocyte count. Conventional flow cytometry (red blood cells and neutrophils CD55/CD59) did not detect a par
