OBJECTIVE: High-frequency ventilation (HFV) is increasingly used in preterm infants, but data on weaning and extubation are limited. We aimed to establish if weaning the continuous distending pressure (CDP) below 8 cm H2O and the Fio2 below 0.30 is feasible in preterm infants on open lung HFV and if these settings result in successful extubation.
DESIGN: Retrospective cohort study.
SETTING: Neonatal intensive care unit in a university hospital.
PATIENTS: Preterm infants ventilated and directly extubated from HFV between January 2003 and August 2005.
MEASUREMENTS AND MAIN RESULTS: Data on p

Ventilation using high-frequency oscillation (HFO) has become a standard care for the ventilatory management of critically ill newborns. In recent years, there has been growing recognition that maintenance of an optimal lung volume during high-frequency oscillation plays an important role in minimizing ventilator-induced lung injury. The primary variable affecting lung volume is the mean airway pressure (MAP). To effectively maintain lung recruitment and optimal gas exchange without overstretching (or collapsing) the lung, MAP should be set between two well defined points in the pressure-volum

UNLABELLED: In order to determine the response to high-frequency oscillatory ventilation (HFOV), used as an "early rescue" therapy, in a cohort of paediatric patients with acute respiratory distress syndrome (ARDS), a prospective clinical study was performed in a tertiary care paediatric intensive care unit. Ten consecutive patients, aged 12 days to 5 years with ARDS and hypoxaemic respiratory failure on conventional ventilation (CV), using a lung protective strategy, were managed with HFOV early in the course of the disease process (median length of CV 4 h). Arterial blood gases, oxygenation

A 22-year-old man was admitted to a district general hospital with chest injuries, a ruptured spleen and limb fractures, sustained in a road traffic accident. After an emergency splenectomy, the patient developed unilateral pulmonary oedema with hypoxaemia which was resistant to both conventional controlled mechanical ventilation (CMV) and independent lung ventilation (ILV). He was transferred to a specialist cardiothoracic unit where high frequency jet ventilation (HFJV) also failed to achieve adequate oxygenation. Combined high frequency ventilation (CHFV), using high frequency pulses from a

UNLABELLED: Inhaled nitric oxide (iNO) improves oxygenation in near-term and term newborns with persistent pulmonary hypertension of the newborn (PPHN), and decreases the need for treatment with extracorporeal membrane oxygenation. However, some patients with PPHN either do not respond or have only transient improvements in oxygenation during iNO therapy. Extrapulmonary shunting associated with high pulmonary vascular resistance in PPHN can cause critical hypoxaemia; however, the syndrome of PPHN is often associated with severe parenchymal lung disease (e.g., meconium aspiration pneumonitis, b

BACKGROUND: Effective gas exchange can be maintained in animals by using external high-frequency oscillation (EHFO). The present study evaluates the effect of relatively long-term duration EHFO combined with pressure support ventilation (PSV) in patients with acute respiratory failure.
METHODS: Twelve patients were ventilated with EHFO combined with PSV for 8 h at 60 oscillations.min-1, with a cuirass pressure of 36 cm H2O: -26 to +10 cm H2O (27 mm Hg: -19.5 to +7.5 mm Hg) and an inspiratory-to-expiratory ratio of 1:1. Blood gas values and hemodynamic parameters were measured.
RESULTS: Signi

The Hayek Oscillator is a recent development in external high frequency ventilation and is a useful mode of ventilatory support during anaesthesia, in chronic respiratory failure and weaning in intensive care. The Hayek Oscillator is new and its application is growing, as are the number of patients who have benefited from its use. However, there are no clear guidelines on how best to adjust the oscillator to achieve optimum ventilation. A simple method of predicting changes in tidal volume and minute ventilation following adjustment would make the oscillator more useful. We present nomograms f

OBJECTIVE: To identify clinical factors in term neonates with severe respiratory failure that predict which neonates are unlikely to respond to high-frequency oscillatory ventilation (HFOV).
DESIGN: This was a retrospective review of patient charts and medical records.
PATIENTS: We studied 190 newborns treated with HFOV between July 1985 and December 1992. All patients were at least 35 weeks' estimated gestational age and had severe respiratory failure, defined as arterial to alveolar oxygen ratio (a/A ratio) of less than 0.2 or the need for peak inspiratory pressure greater than 35 cm H2O o

A child presented with hydrocarbon ingestion leading to pneumonitis and adult respiratory distress syndrome (ARDS). Treatment with conventional ventilation in this child led to very high pressures and pulmonary air leaks. Treatment with high frequency jet ventilation (HFJV) resulted in less barotrauma, resolution of air leak, and clinical improvement. Thus, HFJV is an acceptable alternative to both conventional ventilation and extracorporeal membrane oxygenation in the treatment of hydrocarbon pneumonitis leading to ARDS.

Five very low birthweight infants manifested unilateral tension pulmonary interstitial emphysema (PIE). All infants required intermittent positive pressure ventilation for respiratory distress syndrome. PIE was managed by positioning the infant on his (or her) side with PIE and by shortening the inspiratory time, in four patients also by shortening exspiratory time resulting in a higher frequency ventilation. PIE resolved within 3-6 days after institution of this treatment. This was paralleled by a reduction of FiO2 as well as peak inspiratory pressures. All infants could successfully be extub

We tested the hypothesis that high frequency oscillatory ventilation (HFOV) would result in decreased pulmonary barotrauma in infants with hyaline membrane disease by comparing HFOV at 10 Hz to conventional positive pressure ventilation with continual distending airway pressure (PPV/PEEP) in premature baboons with hyaline membrane disease. Nineteen baboon fetuses were randomized to one of two treatment groups, delivered at 140 +/- 2 days, and, after stabilization and instrumentation of PPV/PEEP, placed in their respective ventilator group. Animals on conventional ventilation were managed by ad

One-lung and two-lung high frequency ventilation (HFV) through a 2-mm internal diameter catheter was evaluated in 22 patients during endoscopic laser excision of stenotic lesions of larynx, trachea, and bronchi. High frequency ventilation at 80 to 250 breaths per minute using air during two-lung HFV and using air-oxygen at an inspired oxygen concentration of 25% during one-lung HFV maintained adequate alveolar ventilation and oxygenation in all patients. The use of HFV through a catheter allowed continuous control of ventilation and provided maximal surgical exposure for endoscopic laser surge

The role of high-frequency jet ventilation (HFJV)/continuous positive airway pressure (CPAP) and HFJV/intermittent mandatory ventilation (IMV) in the treatment of surgical patients with the adult respiratory distress syndrome were evaluated. To compare the efficacy of HFJV to IMV at a constant FiO2 and positive end-expiratory pressure, patients in surgical intensive care were randomized to receive IMV/CPAP therapy or one of three modes of HFJV: (1) HFJV/CPAP alone, (2) HFJV/CPAP + IMV (1), or (3) HFJV/CPAP + IMV (2). Each patient served as his own control. During comparison of HFJV/CPAP + IMV

To examine the correlation DCO2/PaCO2 on high-frequency oscillatory ventilation (HFOV) combined with volume guarantee (VG) throughout increasing frequencies in two different respiratory conditions, physiological and low compliance. Neonatal animal model was used, before and after a bronchoalveolar lavage (BAL). HFOV combined with VG was used. The frequency was increased from 10 to 20 Hz, and high-frequency tidal volume (VThf) was gradually decreased maintaining a constant DCO2. Arterial partial pressure of carbon dioxide (PaCO2) was evaluated after each frequency and VThf change. Six 2-day-old

INTRODUCTION: Because of the high mortality, potential limitations, and inherent adverse effects associated with conventional therapies, as well as extracorporeal membrane oxygenation, for persistent pulmonary hypertension of the newborn (PPHN), alternative modes of ventilatory support have been researched. There is anecdotal evidence that high-frequency flow interruption ventilation (HFFI) benefits neonates with severe air leak and lung diseases unresponsive to conventional ventilation, so we conducted a study to compare the hospital course, survival rate, and incidence of chronic lung diseas

OBJECTIVE: Mechanical ventilation may impair cardiovascular function if the transpulmonary pressure rises. Studies on the effects of high-frequency oscillatory ventilation (HFOV) on cardiovascular functions have yielded conflicting results. This study was done to compare alterations in left ventricular output and blood flow velocities in the anterior cerebral artery, internal carotid artery, and celiac artery using a Doppler ultrasound device before and 2 h after initiating HFOV in neonates with respiratory distress syndrome (RDS) or pulmonary interstitial emphysema (PIE).
DESIGN: Prospective

OBJECTIVE: Nasal high-frequency oscillatory ventilation (nHFOV) and synchronized nasal intermittent positive pressure ventilation (sNIPPV) were the new modes of noninvasive ventilation. This study's aim was to clarify as to which of the nHFOV and sNIPPV modes was superior in preventing postextubation failure or reintubation in neonates.
STUDY DESIGN: An open-label parallel randomized study was performed. Extubated preterm and term neonates were randomly allocated into nHFOV or sNIPPV modes; the reintubation rate was evaluated within 7 days after extubation between the two modes. Subgroup anal

We report the case of a 33-yr-old patient with adult respiratory distress syndrome (ARDS) complicated by bilateral pneumothoraces, who was successfully treated with high-frequency oscillatory ventilation following failure to respond to conventional ventilation. The role of high-frequency ventilation in the management of ARDS and air leaks is discussed.
