BACKGROUND: The ideal thromboprophylaxis regime following lower limb arthroplasty and proximal femur fractures remains controversial. Guidelines disagree on the type of chemical prophylaxis, its dose or duration. This article describes a method of monitoring venous thromboembolism (VTE) rates following Total Hip (THA), Total Knee Arthroplasty (TKA) and surgery for hip fractures (NOF#).
METHODS: Over 3 years, all patients investigated for VTE were analysed using Picture Archiving Communications System (PACS). All positive scans were then cross-referenced using PACS and local registry data to s

There is no uniform standard of treatment for patients with hip fusion and accompanying symptomatic osteoarthritis of the ipsilateral knee. Fusion takedown is associated with an increased risk of complications, and often the results are not satisfactory for patients. Therefore, each case should be considered individually. We present a case report regarding a 70-year-old patient with hip fusion as a result of tuberculosis at a young age who underwent hip fusion takedown with total hip arthroplasty followed by total knee arthroplasty as a second step. The 70-year-old patient with end-stage renal

PURPOSE: To determine whether patients who reported a discrete traumatic event precipitating the onset of femoroacetabular impingement syndrome (FAIS) reported similar patient-reported outcomes for the modified Harris Hip Score (mHHS) and the Non-Arthritic Hip Score (NAHS) following hip arthroscopy as patients with atraumatic hip pain associated with FAIS alone.
METHODS: A retrospective comparative therapeutic investigation of a prospectively collected database of cases performed by a single surgeon from 2010 to 2015 identified a group of patients who developed FAIS after a discrete traumatic

OBJECTIVE: Recurrent dislocations of a total hip joint replacement are a difficult problem to address. We present a technique for reconstructing the ischiofemoral ligament of the hip to prevent recurrent posterior dislocations.
METHODS: This is a case series of 5 patients with a reconstruction performed using either a bone-patellar tendon-bone graft autograft or allograft.
RESULTS: At an average follow up of 47 months following the reconstruction, there were no redislocations.
CONCLUSION: Ischiofemoral hip ligament reconstruction is a viable technique in addressing recurrent dislocations of

Background. Local plexiform neurofibroma can lead to deformity of the pelvis, valgus deformity of femoral neck, and joint capsule laxity. We report a case of secondary hip osteoarthritis with subluxation and coxa vara deformity resulting from an extra-articular neurofibroma treated with total hip replacement. Case Description. A 39-year-old man had a large benign plexiform neurofibroma at buttock which induced secondary osteoarthritis of the hip. Conservative treatment of tumor was selected because the patient had low chance of malignant transformation due to absence of other neurofibromatosis

BACKGROUND: Ganz surgical hip dislocation is useful in the management of severe hip diseases, providing an unobstructed view of the femoral head and acetabulum. We present our early experience with this approach in pediatric hip diseases.
METHODS: Twenty-three hips of 21 patients with pediatric hip diseases treated using the Ganz surgical hip dislocation approach were the subjects of this study. The average age at the time of surgery was 15.7 years. There were 15 male and 6 female patients who were followed for an average of 15.1 months (range, 6 to 29 months). Diagnoses included hereditary m

INTRODUCTION: Many reports have suggested that Chiari pelvic osteotomy would improve the results of acetabular component placement and fixation in subsequent total hip arthroplasty. However, little is known concerning the biomechanical, radiological, and clinical effects of Chiari pelvic osteotomy on subsequent total hip arthroplasty.
MATERIALS AND METHODS: Ten total hip arthroplasties for developmental dysplasia of the hip after previous Chiari pelvic osteotomy (Chiari group) were compared with 20 total hip arthroplasties for developmental dysplasia of the hip without previous surgery (contr

Nerve lesions occurred in 17 patients who had undergone hip-joint operations (especially total replacement). The femoral nerve was most commonly affected, followed by sciatic and gluteal nerves, with combined pareses in six. As differential diagnosis from pseudoparesis is not always possible, electromyography should be performed in all cases. Stretching and/or squeezing are the most important causes. The operation results are significantly impaired by such nerve lesions. However, long-term results are usually satisfactory in the majority of cases. Early diagnosis and treatment are important in

BACKGROUND: Long-term favorable clinical outcomes of anatomical or high hip center techniques in total hip arthroplasty (THA) are reported in patients with developmental dysplasia of the hip (DDH). However, there is little information about the effect of the hip center location on gait characteristics. The purpose of this study was to compare these surgical techniques with gait analysis, analyze the effect of the hip rotation center location on gait parameters, and discuss the possible problems that may arise.
METHODS: A total of 40 patients who underwent THA due to unilateral coxarthrosis se

INTRODUCTION: Developmental dysplasia of the hip (DDH) poses challenges for component positioning during total hip arthroplasty (THA) secondary to abnormal bone morphology, soft-tissue contractures, and hip center migration. The objective of this study was to evaluate the radiographic and clinical outcomes of THA for DDH performed with robotic assistance versus manual (M) technique.
METHODS: A retrospective review identified 115 patients with Crowe II to IV dysplasia undergoing primary THA at a single institution from 2016 to 2022. There were 59 (51%) M-THAs and 56 (49%) RA-THAs. The median a

Microinstability is an increasingly recognized diagnosis in young athletes presenting with hip pain. Causes of microinstability may include abnormality of the hip bony anatomy, acetabular labral tears, joint capsule laxity or injury, and muscle dysfunction. Borderline hip dysplasia is an increasingly recognized factor predisposing to microinstability. The capsuloligamentous structures of the hip, particularly the iliofemoral ligament, provide important restraints to femoral head motion, and iatrogenic defects can predispose patients to instability after surgery. Injury to the acetabular labrum

The classic Ortolani and Barlow signs are routinely used to diagnose hip instability secondary to severe acetabular dysplasia in the newborn. However, eliciting a positive sign depends largely on the experience of the examiner and the subjective amount of manual pressure the examiner applies on the baby's hips. Furthermore, these signs do not give a clue for the selection of a maturation or immobilization device after reduction of an unstable hip: below-knee hip spica, above-knee hip spica or a Pavlik harness. The aim of this study is to describe a clinical sign that could be useful in detecti

Hip fusion takedown to total hip replacement is a challenging operation. Neck osteotomy and acetabular component placement are technically demanding and often require fluoroscopic guidance. Robotic arm-assisted total hip arthroplasty enhances accuracy of preoperative planning and provides navigated guidance for neck osteotomy and haptic guidance on acetabular reaming and cup implantation. Fluoroscopic guidance is replaced by real-time navigation and on-screen data. This article describes how robotic arm assistance can simplify this complex operation.

The aim of this study is to determine the functional outcome and midterm survival rates of the Birmingham Hip Resurfacing and Birmingham Total Hip Arthroplasty. This retrospective, observational study included 150 surgeries (46 resurfacing procedures and 104 arthroplasty procedures) performed in 127 patients from 2005 to 2012. The Resurfacing and Arthroplasty study groups were evaluated with clinical (Harris Hip Score and Hip Disability and Osteoarthritis Outcome Score) and radiological follow-up. Cobalt and chromium levels were measured via blood samples. No revisions were required in either

INTRODUCTION: Single surgeon studies from specialized centers have suggested that metal-on-metal hip resurfacing in patients with osteonecrosis of hip joint provides good implant survival and function.
METHOD: We tested the hypothesis that multicenter results of hip resurfacing, in terms of function and survival, are similar between patients with osteoarthritis and osteonecrosis.
PATIENTS: 192 patients (202 hips) underwent metal-on-metal hip resurfacing at different centers around the world. We compared the revision risks in 95 patients (101 hips) with osteonecrosis and 97 patients (101 hips

BACKGROUND: The optimal surgical treatment for patients with high congenital dislocation of the hip remains controversial. The purpose of our study was to evaluate the mid-term to long-term results of cementless total hip arthroplasty in such patients.
METHODS: The study included sixty-eight total hip replacements performed between 1989 and 1994 in fifty-six consecutive patients with high congenital hip dislocation at our hospital. The cup was placed at the level of the true acetabulum, and a shortening osteotomy of the proximal part of the femur and distal advancement of the greater trochant
