Persistent calcium dependence after total thyroidectomy: a hypothesis of bone hunger syndrome with parathyroid reserve insufficiency in the setting of normalized parathyroid hormone - a case report and literature review.

BACKGROUND: Restoration of parathyroid hormone (PTH) to the normal range following total thyroidectomy is conventionally regarded as an indicator of functional recovery of the parathyroid glands. Nevertheless, a subset of patients develop hypocalcemia upon withdrawal of supplementation. This report describes a case in which PTH normalized postoperatively yet calcium supplements could not be discontinued for three years, and discusses the underlying mechanisms and management strategy. CASE SUMMARY: A 40-year-old woman with multifocal papillary thyroid carcinoma harboring the BRAF V600E mutatio

Vitamin B-12 Concentrations in Breast Milk Are Low and Are Not Associated with Reported Household Hunger, Recent Animal-Source Food, or Vitamin B-12 Intake in Women in Rural Kenya.

BACKGROUND: Breast milk vitamin B-12 concentration may be inadequate in regions in which animal-source food consumption is low or infrequent. Vitamin B-12 deficiency causes megaloblastic anemia and impairs growth and development in children. OBJECTIVE: We measured vitamin B-12 in breast milk and examined its associations with household hunger, recent animal-source food consumption, and vitamin B-12 intake. METHODS: In a cross-sectional substudy nested within a cluster-randomized trial assessing water, sanitation, hygiene, and nutrition interventions in Kenya, we sampled 286 women 1-6 mo post

Rapid Emergence of Appetite and Hunger Resulting in Weight Gain and Improvement of Eating Disorder Symptomatology during and after Short-Term Off-Label Metreleptin Treatment of a Patient with Anorexia Nervosa.

Off-label treatment of a 15-year-old female patient with anorexia nervosa (AN) with human recombinant leptin (metreleptin) for nine days was associated with self-reported increments of appetite and hunger resulting in rapid weight gain and substantial improvement of eating disorder cognitions and of depression. The results further substantiate the effects of metreleptin on both AN and depression. We contrast these results with the widespread view that leptin is an anorexigenic hormone. Randomized controlled trials are warranted to confirm the described effects.

Links between mothers' and children's disinhibited eating and children's adiposity.

Few studies have examined relationships between parents' and children's specific disinhibited eating behaviors. We investigated links among mothers' and children's binge/loss of control eating, eating in the absence of hunger, and children's adiposity in 305 non-treatment-seeking youth, aged 8-17 years (13.62±2.65 years; 49.8% female) and their mothers. Youths' loss of control eating and eating in the absence of hunger were assessed by interview and self-report questionnaire. Children's adiposity was assessed with BMI-z and air displacement plethysmography. Maternal binge eating, eating in the

Trial Protocol: randomised controlled trial of the effects of very low calorie diet, modest dietary restriction, and sequential behavioural programme on hunger, urges to smoke, abstinence and weight gain in overweight smokers stopping smoking.

BACKGROUND: Weight gain accompanies smoking cessation, but dieting during quitting is controversial as hunger may increase urges to smoke. This is a feasibility trial for the investigation of a very low calorie diet (VLCD), individual modest energy restriction, and usual advice on hunger, ketosis, urges to smoke, abstinence and weight gain in overweight smokers trying to quit. METHODS: This is a 3 armed, unblinded, randomized controlled trial in overweight (BMI > 25 kg/m2), daily smokers (CO > 10 ppm); with at least 30 participants in each group. High-fat diet exposure, internal state, and reward value influence decision making during motivational conflict.

Survival depends on adaptive decision-making wherein the benefits of seeking food are weighed against the risk of threats, a process influenced by internal states like hunger. Highly palatable, calorie-dense foods, such as a high-fat-containing diet (HFD), potently drive feeding even in the absence of hunger. However, it remains unclear whether approach-avoidance conflict is shaped by the interaction between hunger state, motivation for rewards of differing palatability, and the lasting effects of chronic high-fat diet exposure (cHFD). Here, we examined how hunger, reward value, and cHFD expos

Role of vitamin D supplements in prevention of hungry bone syndrome after successful parathyroidectomy for primary hyperparathyroidism: A prospective study.

BACKGROUND: We postulated that the preoperative correction of vitamin D levels can significantly reduce the incidence of hunger bone syndrome among patients undergoing parathyroidectomy for primary hyperparathyroidism. METHODS: We performed a prospective, randomized, open-label study on 102 patients with primary hyperparathyroidism and coexisting vitamin D deficiency who were scheduled to undergo parathyroidectomy. Patients were divided into the following two groups: group I which included 52 patients who did not receive preoperative vitamin D supplementation; and group II which included 50 p

Beneficial effects of leptin substitution on impaired eating behavior in lipodystrophy are sustained beyond 150 weeks of treatment.

AIM: Metreleptin treatment in lipodystrophy patients improves eating behavior with increased satiety and reduced hunger. However, no data are available whether effects are maintained beyond 52 weeks of treatment. METHODS: A prospective study with measurements at baseline and at >150 weeks of metreleptin treatment was performed. Five female lipodystrophy patients with indication for metreleptin were included. Behavioral aspects of hunger- and satiety regulation were assessed by validated eating behavior questionnaires and visual analog scales assessing hunger and satiety feelings before and af

Effects of different forms of dyspnoea on pain perception induced by cold-pressor test.

Although dyspnoea has been shown to attenuate pain, whether different forms of dyspnoea exert a similar inhibitory effect on pain has never been tested. We examined the effects of two different forms of dyspnoea, i.e., "air hunger" sensation (AIR HUNGER) and "work/effort" sensation (WORK/EFFORT), on pain induced by a cold-pressor test. Dyspnoea was induced by two different dyspnoea stimuli (i.e., AIR HUNGER and WORK/EFFORT stimuli) and the magnitudes of both sensations were evaluated by using a visual analogue scale (VAS). At equi-dyspneic VAS levels of two different forms of dyspnoea, pain wa

Mindfulness and laboratory eating behavior in adolescent girls at risk for type 2 diabetes.

Mindfulness-based intervention has become increasingly popular to address disinhibited eating in obesity and type 2 diabetes (T2D). Theoretically, present-moment attention promotes the ability to recognize and respond to internal hunger cues and to differentiate physiological hunger from other stimuli. Yet, there is limited research describing the relationship of mindfulness with disinhibited eating patterns in adolescents. In this study, we evaluated the relationship of dispositional mindfulness to laboratory eating in 107 adolescent (12-17 years) girls at risk for T2D. Adolescents reported d

Food reward. What it is and how to measure it.

We investigated the contribution of hunger and food liking to food reward, and the relationship between food reward and food intake. We defined liking as the pleasantness of taste of food in the mouth, and food reward as the momentary value of a food to the individual at the time of ingestion. Liking and food reward were measured, respectively, by ratings of the pleasantness of the taste of a mouthful, and ratings of desire to eat a portion, of the food in question. Hunger, which we view as primarily the absence of fullness, was rated without food being present. Study 1 provided evidence that

Beta and gamma synchronous oscillations in neural network activity in mice-induced by food deprivation.

Food deprivation is known to trigger hunger sensation and motivation to eat for energy replenishing. However, brain mechanisms associated with hunger and neural circuitries that mediate hunger driven responses remained to be investigated. To understand neural signaling of hunger, local field potentials (LFPs) in the lateral hypothalamus (LHa), nucleus accumbens (NAc), dorsal hippocampus (HP) and olfactory bulb (OB) and their interconnectivities were studied in freely moving adult male Albino mice during 18-20 h food deprivation and fed periods. Raw LFP signals were recorded and analyzed for me

The endocrine effects of bitter tastant administration in the gastrointestinal system: intragastric versus intraduodenal administration.

Bitter tastants are recently introduced as potential hunger-suppressive compounds, the so-called "Bitter pill." However, the literature about bitter administration lacks consistency in methods and findings. We want to test whether hunger ratings and hormone plasma levels are affected by: 1) the site of administration: intragastrically (IG) or intraduodenally (ID), 2) the bitter tastant itself, quinine hydrochloride (QHCl) or denatonium benzoate (DB), and 3) the timing of infusion. Therefore, 14 healthy, female volunteers participated in a randomized, placebo-controlled six-visit crossover stud

The effect of Khat (Catha edulis) as an appetite suppressant is independent of ghrelin and PYY secretion.

BACKGROUND: The leaves of the khat plant (Catha edulis) are chewed for their pleasurable effects. Chewing releases cathinone which may decrease appetite through an unknown mechanism. Levels of the peptide ghrelin increase with hunger and decrease immediately post-prandially, while peptide YY is released following a meal. We hypothesised that the anorexigenic effects of khat may be mediated through changes in these hormones. MATERIALS AND METHODS: Six habitual khat chewers attended on two separate occasions. For a period of 3h they chewed either khat leaves or lettuce. Blood pressure (BP) and

Assessment of the brain ischemia during orthostatic stress and lower body negative pressure in air force pilots by near-infrared spectroscopy.

CASE: We present the case of a 49-year-old woman who underwent a 237-day hunger strike on the streets of Pittsburgh, Pennsylvania. Despite medical supervision and appropriate micronutrient supplementation, including higher-than-recommended dosing of oral thiamine, the patient developed Wernicke's Encephalopathy and subsequent Korsakoff Syndrome. She is now permanently impaired. INTRODUCTION: Hunger strikers are subject to numerous sequelae of micronutrient deficiency. Among these are the paired conditions of Wernicke's Encephalopathy and Korsakoff Syndrome, conditions secondary to prolonged t

Genetic technologies to enhance crop nutritional value under climate change.

At present, more than 700 million people live with caloric hunger, and more than two billion suffer from micronutrient deficiencies, known as 'hidden hunger'. From an agricultural viewpoint, three major objectives need to be worked towards simultaneously to achieve zero hunger (the United Nations Sustainable Development Goal 2): (1) enhanced yield; (2) higher vitamin and mineral density to sustain recommended daily intake (multi-biofortification); and (3) enhanced climate-change resilience. Although the Green Revolution increased global calorie production, it exacerbated hidden hunger by prior

Relation between postprandial satiation and antral area in normal subjects.

The factors influencing appetite in humans are poorly understood. There is a weak relation between appetite and gastric emptying in normal subjects. Recent studies have shown that fasting and postprandial antral areas increase in patients with functional dyspepsia compared with normal subjects. We evaluated the hypothesis that antral area, and hence antral distention, is a significant determinant of postprandial fullness. Fourteen normal subjects had simultaneous measurements of gastric emptying by scintigraphy and antral area by ultrasound after ingestion of 350 mL 20% glucose. Fullness and h
