Screening for late-onset Pompe disease in Internal Medicine departments in Spain.

BACKGROUND: The screening of high-risk populations using dried blood spots (DBS) has allowed the rapid identification of patients with Pompe disease, mostly in Neurology departments. The aim of the study was to determine the prevalence of late-onset Pompe disease (LOPD) among patients not previously diagnosed or tested for this entity despite presenting possible signs or symptoms of the disease in Internal Medicine departments in Spain. METHODS: This epidemiological, observational, cross-sectional, multicenter study included a single cohort of individuals with clinical suspicion of LOPD seen

Sources of primary bloodstream infections in internal medicine patients - a cohort study.

PURPOSE: To describe the sources of bloodstream infections (BSIs) in internal-medicine patients, on admission and during hospitalization, and to determine the proportion of BSIs in which no secondary cause could be defined (i.e., primary-BSI). METHODS: We analyzed all BSIs at the internal-medicine wards of the two campuses of the Hadassah Hebrew-University Medical Center, during 2017-2018. We defined the BSI source of each event (secondary, Central-line associated BSI (CLABSI) or primary non-CLABSI) and compared BSIs present on admission (POA) to hospital acquired (HA). RESULTS: There were 5

[Beyond acute SARS-CoV-2 infection: A new challenge for Internal Medicine].

Infection with the new SARS-CoV-2 coronavirus has reached pandemic proportions, with a very high death toll worldwide. Despite the scientific community's strenuous efforts to address this disease in its acute phase, as well as in prevention through the development of vaccines in record time, there remains another important workhorse: understanding and treating the persistence of symptoms beyond the acute phase, the so-called protracted COVID-19 syndrome or persistent COVID. These persistent manifestations affect several organs and systems and may depend on both the pathogenic mechanisms of the

Timing of Surgery and Internal Medicine Clerkships and Surgery Shelf Examination Scores.

BACKGROUND: Many medical students believe that third-year clerkship rotation sequence affects their success. We hypothesized that students who completed the internal medicine clerkship before the surgery clerkship received higher surgery shelf examination scores compared with the students who did not. MATERIALS AND METHODS: Deidentified academic data including preclinical data and National Board of Medical Examiners shelf examination scores for surgery for all third-year medical students at a single institution from 2012 to 2017 were analyzed. Students who did not complete all six core clerks

Bacterial Infections in Patients with Liver Cirrhosis in an Internal Medicine Department.

BACKGROUND AND AIMS: Liver cirrhosis (LC) is the end stage of multiple processes that lead to hepatic failure and is the 10th most common cause of death in the Western world. Bacterial infections are one of the most important clinical problems in patients with LC, as their underlying immune status is compromised. Approximately 60% of bacterial infections in cirrhotic patients are community acquired (CA) and 40% are nosocomial. The most common infections in cirrhotic patients are spontaneous bacterial peritonitis (SBP) (25%), urinary tract infection (UTI) (20%), pneumonia (15%), bacteremia (12%

[Management of uveomeningitis in internal medicine: Proposal for a diagnostic work-up].

Uveomeningitis relates to an inflammatory state extending from iris and ciliary bodies to the choroid behind the eye. Because of a close contact between eye and brain, and barrier disruption, the inflammation can spread into the central nervous system (CNS). We review the clinical manifestations of uveitis, which are known to provide helpful clues to the diagnosis and describe the infectious, inflammatory, and neoplastic conditions classically associated with the uveomeningitis. Inflammatory or auto-immune diseases are probably the most common clinically recognized causes of uveomeningitis ass

Prevalence, risk factors and prognosis of hypernatraemia during hospitalisation in internal medicine.

INTRODUCTION: Hypernatraemia in hospitalised patients is less common and less studied than hyponatraemia, although it also seems to be associated with a poor prognosis. The present study evaluates its prevalence, risk factors and prognosis in an internal medicine department. METHODS: Full hospital stays over 28 months in a 36-bed internal medicine department were analysed retrospectively. Patients with at least one plasma sodium ≥ 150 mmol/l were compared first with all other patients and then individually with sex- and age-matched normonatraemic controls. RESULTS: Plasma sodium ≥ 150 mmol÷l

[Regular discussion of serious complications during admission to an internal medicine department].

Three patients, two women aged 54 and 84 years, and a man aged 76 years, had serious complications during a stay in an internal medicine ward. The complications were discussed at monthly multidisciplinary complication meetings, which we organise from 2007 and which are aimed at improving care processes. The first patient developed urinary tract infection, fever and delirium and an arm fracture as a result of a fall after she had been given a routine urinary catheter in order to monitor her fluid balance. The complication discussion indicated that a urinary catheter should not be routinely inst

Greater focus on clinical rheumatology is required for training in internal medicine.

Early intervention for rheumatic disease has been shown to improve prognosis, but is dependent on the recognition of pertinent symptoms and signs. This study was undertaken to identify the attitudes of junior doctors toward musculoskeletal assessment. The charts of 100 randomly selected medical patients admitted via the Emergency Department were reviewed for documentation of the musculoskeletal system. In addition, junior doctors were interviewed regarding their views on musculoskeletal assessment. Only 16% of medical admission notes contained a reference to the musculoskeletal system and the

Infections in child day care centers as they relate to internal medicine.

Internists should be aware of the increased frequency of most communicable diseases among parents and child care providers exposed to children attending child day care centers. Episodes of shigellosis and hepatitis A among adults in contact with children in day care centers should be promptly reported to public health authorities, since they may indicate the presence of a day care center outbreak for which effective control measures are available. Although effective treatment or intervention is limited or not available for many other infections acquired by adults from children in day care cent

Iatrogenic illness in a department of general internal medicine: a prospective study.

The incidence and causes of iatrogenic illness in a department of internal medicine were studied prospectively in 1,176 patients admitted from 1 January 1986 to 31 December 1986. A total of 295 patients (25.1%) developed 367 episodes of iatrogenic illness. Phlebitis occurred most frequently (75.2% of all iatrogenic events), followed by drug reactions (10.7%), contusion (4.6%), and urinary tract infections (1.4%). Nineteen patients developed life-threatening events (in 2 it was the cause of death). Etiologic agents included intravenous catheter (79% of all adverse reactions), drugs (9.5%), fall
