A novel "proximal first" Inoue balloon catheter for retrograde aortic valvuloplasty: Initial case report.

The Inoue balloon, invented for percutaneous transseptal mitral commissurotomy for mitral stenosis, is initially dilated in the distal portion and then the proximal portion, forming an hourglass shape that stabilizes the balloon at the mitral valve orifice with a pulling action. The device has been successfully applied to antegrade aortic valvuloplasty; the hourglass shape stabilizes the balloon across the aortic valve without rapid ventricular pacing. Subsequently, an Inoue balloon was developed for retrograde aortic valvuloplasty using the same design as the antegrade balloon. The hourglass-

East meets west: regional impact on agrochemical discovery and innovation.

The efficient production of the food needed to nourish an expanding global population continues to fuel the demand for new crop protection compounds. This task is made all the more difficult by the need to meet increasingly demanding grower, consumer and regulatory constraints. The discovery and development of new synthetic organic crop protection compounds has been largely the responsibility of the agrochemical industry in Europe, Japan and the USA, with government-funded academic research often playing a crucial role in the early stages of the invention and testing of novel activity. The way

[History of injections. Pictures from the history of otorhinolaryngology highlighted by exhibits of the German History of Medicine Museum in Ingolstadt].

BACKGROUND: Injections are part of the arsenal of all medical disciplines. In addition to this common ground, each specialty has its own particular aspects; the historical development of these are presented here with respect to otorhinolaryngology. INTRAVENOUS INJECTIONS: The first experiments with intravenous injections were carried out in 1642 by a gentleman's hunting servant in eastern Germany. Similar experiments were done in 1656 by Christopher Wren, the astronomer, mathematician, and architect in Oxford, England, and a group of scientists around the physicist Robert Boyle. These experim

The role of epistemic reasoning in mutual exclusivity inferences.

When encountering a novel word, adults and children as young as 12 months old often reason that it refers to a novel object rather than one with an existing name - making a 'mutual exclusivity inference.' We explored the mechanism of this inference, aiming to differentiate between three hypotheses: whether mutual exclusivity arises due to reasoning about a specific speaker's knowledge, projection of one's own egocentric knowledge, or reasoning generally about the conventionality of labels. Adults and 3.5-5-year-old children in our experiment heard a label being either taught or invented on the

Ossicular calisthenics: Pneumomassage of the tympanic membrane.

OBJECTIVES/HYPOTHESIS: Throughout the latter portion of the 19th and early 20th centuries, pneumomassage devices were widely used by otologists to treat a variety of ear diseases. The so-called eardrum massagers produced a regular, repetitive, oscillatory movement through modifying the air pressure in the ear canal. The goal of this study was to trace the invention, clinical use, technological diversification, abandonment, and ultimate resurrection of tympanic pneumomassage. METHOD: Review of the 19th- and early 20th-century medical journals, texts, and trade catalogs concerning the tympanic

Spontaneous use of tools as straws in great apes.

Great apes can use multiple tools to extract food embedded in substrates and can invent new ways to exploit those resources. We tested five bonobos, five chimpanzees, and six orangutans in a task in which they had to use (and modify) a tool as a straw to drink the juice located inside a container. Experiment 1 showed that four orangutans and one chimpanzee invented the use of a piece of electric cable to get the juice. Experiment 2 investigated whether subjects could transform a non-functional hose into a functional one by removing blockages that impeded the free flow of juice. Orangutans outp

Unravelling the Antibiotic Resistance: Molecular Insights and Combating Therapies.

Antibiotics, the full-stop of invasive bacteria, have been used in clinical setups from unthreatening fever to massive challenging therapies. Constant dependency on medication upsurges the evasion of microbes from antibiotics contemporarily along with ecological footprint. Thus, the infested pathogen became resilient to antibiotics, disguised as multidrug-resistant bacteria (MDR), pandrug-resistant bacteria (PDR), and extensively drug-resistant bacteria (XDR). The etymology of genetic modifications and horizontal gene transfer played an external influence on the arising resurgence. Also, intri

Evaluating an internal pitch competition for entrepreneurial training in medical innovation: The alligator tank at Mayo Clinic Florida.

There is a general separation between medical and business education, despite the substantial intersection of these fields in hospital and industry settings. This separation hinders medical innovators, who rarely have opportunities to see how entrepreneurial concepts apply to the translation of medical innovation. To address this, the Office of Entrepreneurship at Mayo Clinic Florida provides experiential entrepreneurial training through an internal pitch competition: the alligator tank (AT). This study evaluates the impact and efficacy of the first seven years of the AT. Key metrics include a

Electrosurgery and Other Technical Advancements in Minimally Invasive Gynecological Surgery - An Update.

Electrosurgery in gynecology has changed over the past few decades. The use of energy-based devices, especially in minimally invasive procedures, is extremely important for preparation, short reconvalescence and patient safety. Recently, there have been major advances in energy-based surgical devices that have further shortened OR time and increased patient safety. Although bipolar and monopolar electrosurgery is still very important, the introduction of high-frequency ultrasound in gynecologic surgery has improved cutting and coagulation by lowering thermal damage in the surrounding tissue. Sex and Race-Related DNA Methylation Changes in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is the sixth most common cancer and fourth leading cause of cancer-related death worldwide. The number of HCC cases continues to rise despite advances in screening and therapeutic inventions. More importantly, HCC poses two major health disparity issues. First, HCC occurs more commonly in men than women. Second, with the global increase in non-alcoholic fatty liver diseases (NAFLD), it has also become evident that HCC is more prevalent in some races and/or ethnic groups compared to others, depending on its predisposing etiology. Most studies on HCC in the past ha

Innovations in Post-Transplant Urology: A Systematic Review of Robot-Assisted Ureteral Reconstruction Procedures after Kidney Transplantation.

PURPOSE: Ureteral complications are among the most common types of complications following kidney transplantation (KT). In the past, these complications were often managed using endoscopic and open surgical approaches. However, the robot-assisted approach has been recently implemented. Nevertheless, the available data regarding the potential impact of robotic surgery in this field are sparse. This systematic review (SR) aims to summarize the available evidence of robot-assisted ureteral reconstruction following KT, focusing on postoperative outcomes and complications. METHODS: Studies eligibl

Exploring the future of robotic approaches in neonatal cardiac surgery: opportunities, barriers, and innovation pathways.

Robotic-assisted surgery (RAS) emerged as a transformative approach in various surgical disciplines, particularly in adult cardiothoracic procedures, where it has demonstrated significant benefits such as enhanced precision, improved visualisation, and reduced complication rates. Despite its success in adult surgeries, including mitral valve repairs and coronary artery revascularisations, the application of RAS in neonatal cardiothoracic surgery remains largely unexplored. This narrative review synthesises existing evidence, highlighting the potential of RAS to address the unique challenges po

Digging Deeper into Precision/Personalized Medicine: Cracking the Sugar Code, the Third Alphabet of Life, and Sociomateriality of the Cell.

Precision/personalized medicine is a hot topic in health care. Often presented with the motto "the right drug, for the right patient, at the right dose, and the right time," precision medicine is a theory for rational therapeutics as well as practice to individualize health interventions (e.g., drugs, food, vaccines, medical devices, and exercise programs) using biomarkers. Yet, an alien visitor to planet Earth reading the contemporary textbooks on diagnostics might think precision medicine requires only two biomolecules omnipresent in the literature: nucleic acids (e.g., DNA) and proteins, kn

[The possibilities for the restoration of the laryngeal function: the current state-of-the-art].

The restoration of the functional competence of the larynx following bilateral laryngeal nerve damage and vocal fold paralysis is a serious challenge for the surgeon that has thus far no satisfactory solution. Physiological re-innervation that occurs naturally with time is non-selective and, in the majority of the cases, leads to synkinesis. Laryngeal pacing achieved with the application of the implantable microchips appears to be a promising approach. The animal experiments have demonstrated the possibility of successful restoration of all the functions of the larynx by means of laryngeal pac

Understanding adaptive capacity and capacity to innovate in social-ecological systems: Applying a gender lens.

Development policy increasingly focuses on building capacities to respond to change (adaptation), and to drive change (innovation). Few studies, however, focus specifically on the social and gender differentiation of capacities to adapt and innovate. We address this gap using a qualitative study in three communities in Solomon Islands; a developing country, where rural livelihoods and well-being are tightly tied to agriculture and fisheries. We find the five dimensions of capacity to adapt and to innovate (i.e. assets, flexibility, learning, social organisation, agency) to be mutually dependan
