Goodness-of-prediction of Zernike polynomial fitting to corneal surfaces.

PURPOSE: To determine the goodness-of-prediction of the fitting routine by measuring the difference between topographic corneal surfaces and their Zernike reconstructions as a function of polynomial order and optical zone size for various corneal conditions. SETTING: Corneal research laboratory in a university eye center. METHODS: Corneal topography maps (N = 253) were obtained from the Louisiana State University Eye Center. A variety of corneal conditions were used: normals; astigmatism; laser in situ keratomileusis, photorefractive keratoplasty (PRK), and radial keratotomy (RK) postoperati

Intraocular Lens Power Calculations in Eyes with Previous Corneal Refractive Surgery: Review and Expert Opinion.

Intraocular lens (IOL) power calculations are less accurate in eyes that have undergone corneal refractive surgery. A wide range of methods have been proposed. We reviewed the methods and outcomes of IOL power calculations in eyes with previous LASIK, excimer laser photorefractive keratectomy (PRK), or radial keratotomy (RK). The PubMed database was searched for articles that (1) discuss methods and outcomes of IOL power calculation in eyes with previous corneal refractive surgery and (2) evaluate the outcomes of toric, multifocal, or extended depth-of-focus (EDOF) IOLs in these eyes. We exclu

Survey of complications and recommendations for management in dissatisfied patients seeking a consultation after refractive surgery.

PURPOSE: To review complications associated with and management options for dissatisfied patients seeking a consultation after refractive surgery performed elsewhere. SETTING: Refractive Eye Surgery Center, The Wilmer Institute, Lutherville, Maryland, USA. METHODS: In this retrospective review, charts of consecutive dissatisfied patients who sought a refractive consultation between June 1999 and January 2003 after refractive surgery performed elsewhere were reviewed and the following parameters were analyzed: visual acuity (uncorrected and best spectacle corrected), prior manifest refraction

Precision in IOL Calculation for Cataract Patients with Prior History of Combined RK and LASIK Histories.

PURPOSE: This study aimed to evaluate the accuracy of 12 intraocular lens (IOL) power calculation formulae for eyes that have undergone both radial keratotomy (RK) and laser assisted in situ keratomileusis (LASIK) surgery to determine the efficacy of various IOL calculations for this unique patient group. Currently, research on this surgical topic is limited. METHODS: In this retrospective study, 11 eyes from 7 individuals with a history of RK and LASIK who underwent cataract surgery at Hoopes Vision were analyzed. Preoperative biometric and corneal topographic measurements were performed. Su

Current keratoconus detection methods compared with a neural network approach.

PURPOSE: Four videokeratographic methods for keratoconus detection were compared with a neural network approach. METHODS: A classification neural network for keratoconus screening was designed to detect the presence of keratoconus (KC) or keratoconus suspects (KCS); a separate cone severity network graded the severity of conelike topography patterns consistent with KC or KCS. Three hundred TMS-1 examinations (Tomey) were randomly divided into training and test sets. Ten topographic indexes were network inputs. Nine categories were used: normal, astigmatism, KC, KCS, contact lens-induced warpa

Corneal melts associated with topically applied nonsteroidal anti-inflammatory drugs.

PURPOSE: Topically applied nonsteroidal anti-inflammatory drugs (NSAIDs) are frequently used to prevent miosis during cataract surgery, to treat ocular allergies, to prevent excessive postoperative inflammation following cataract surgery, and to treat cystoid macular edema following cataract surgery. They have also been used to control pain and photophobia following radial keratotomy and excimer laser photorefractive keratectomy. During August of 1999, severe complications following topical NSAID use including corneal melting, were reported by members of the American Society of Cataract and Re

Femtosecond-Assisted Arcuate Keratotomy for the Correction of Postkeratoplasty Astigmatism: Vector Analysis and Accuracy of Laser Incisions.

PURPOSE: To assess the clinical and refractive outcomes of femtosecond-assisted arcuate keratotomy in postkeratoplasty patients, and the accuracy of the incisions, using optical coherence tomography. METHODS: This is a retrospective study of patients with high postkeratoplasty astigmatism. Patients with a minimum of 4 diopters (D) of postkeratoplasty regular astigmatism were included. The main outcome measures were corrected distance visual acuity, keratometry, corneal topography, and the depth of corneal incisions. Arcuate keratotomy procedures were performed using the IntraLase Femtosecond

Femtosecond LASIK combined with astigmatic keratotomy for the correction of refractive errors after penetrating keratoplasty.

BACKGROUND AND OBJECTIVE: To evaluate the outcomes of femtosecond laser in situ keratomileusis (LASIK) compared to combined LASIK and astigmatic keratotomy in the treatment of refractive errors following penetrating keratoplasty. PATIENTS AND METHODS: A retrospective review was performed on 18 eyes of 16 patients who underwent LASIK for visual rehabilitation after penetrating keratoplasty. Seven eyes (38.8%) had undergone paired relaxing incisions with topographic guidance prior to LASIK performance. RESULTS: Preoperative uncorrected visual acuity was 20/100 or worse in 13 eyes (72.2%) and b

Combined corneal topography and corneal wavefront data in the treatment of corneal irregularity and refractive error in LASIK or PRK using the Carl Zeiss Meditec MEL 80 and CRS-Master.

PURPOSE: To assess the efficacy, safety, and accuracy of TOSCA II (Topography Supported Custom Ablation) treatments for the correction of high corneal aberrations and refractive error following previous ocular surgery. METHODS: Forty-eight eyes of 32 patients were included for study. Indications for TOSCA II treatment included small optical zone, decentered ablation, subjective visually significant higher order aberrations, and irregularly irregular astigmatism induced by LASIK, photorefractive keratectomy, radial keratotomy, conductive keratoplasty, cataract surgery, and deep lamellar kerato

[A review of mathematical descriptors of corneal asphericity].

PURPOSE: Corneal asphericity may be modeled on a conic section which can be described by the apical radius of curvature in the meridian studied and by a measure of the degree of asphericity. MATERIAL AND METHODS: Through an extensive review of the literature, we expose the principles, the population variations and report the application of such corneal modeling. RESULTS: The aspheric anterior corneal surface can be described by a conic section, defined by its radius of curvature and by a parameter measuring asphericity. We analyse the various parameters used in the literature to determine th
