Staghorn calculi of the kidneys. A. Clinical review.

A series of 91 staghorn calculi are reviewed. Possible aetiological factors are analysed. A high incidence of hypercalciuria was found, and the significance of this is discussed. The rates of complete stone clearance after conservative surgery are considered for different types of staghorn calculus and it is felt that local circulatory arrest with kidney cooling may be of value in removing stones due to Proteus. The place of elective nephrectomy in the management of staghorn calculi is discussed. Nephrectomy may be the treatment of choice in the management of large, unilateral stones in older

Bone metastases in Wilms' tumour.

In a series of 81 patients with a diagnosis of Wilms' tumour seen at the Hospital for Sick Children, Great Ormond Street, between 1960-1972, two patients developed histologically proven bone metastases. In one patient a lump on the head was the presenting symptom of his Wilms' tumour. In the second patient an isolated metastasis developed in the iliac bone 2 3/4 years after complete removal of an encapsulated Wilms' tumour. The bone lesion was outside the previously irradiated volume. Response to treatment was poor and both patients died with further widespread disease. A review of other serie

Extracorporeal shock wave lithotripsy in ureteral and kidney malformations.

INTRODUCTION: Extracorporeal shock wave lithotripsy (SWL) has long been accepted worldwide in the treatment of kidney stone disease. Upper ureter calculi in ureteral and kidney malformations are rather frequent (10-25%). The aims of this retrospective study were to determine whether malformations might impair fragment expulsion. MATERIAL AND METHODS: From 1986 to 1995, 203 patients with ureteral and kidney malformations were treated and followed up for a minimum of 1 year to a maximum of 9 years. Malformations included: ureteropelvic junction stenosis, horseshoe kidney, renal malrotation, ren

[Transurethral endoscopic pyelolithotripsy in combined treatment of complex forms of nephrolithiasis].

The article presents results of retrograde endoscopic pyelolithotripsy (REP) in 75 patients with large and coral nephroliths. Retrograde contact pyelolithotripsy was made with application of Swiss Lithoclast unit, semirigid ureteroscopes R. Wolf with a cone 8-9 Ch and working channel 5.3 Ch. This enables maximal removal of the destroyed concrement by hydraulic litholapaxia with consequent insertion of the internal stent. Extracorporeal lithotripsy with low-energy shock-wave impulses was performed on demand (1-3 sessions). The stent was removed in 3-5 days in cases of complete one-stage sanatio

[Urinary stones and urinary tract abnormalities. Is the stone composition independent of the anatomical abnormality?].

INTRODUCTION: More than ten per cent of stones are associated with a urinary tract abnormality. To verify whether the malformation influences stone composition, we studied the composition of stones observed in fifteen urological abnormalities. MATERIAL AND METHOD: This study is based on 1,461 stones associated with a clearly defined malformation analysed by infrared spectroscopy plus 402 bladder stones in men with benign prostatic hyperplasia. RESULTS: In this series of 1,863 abnormalities, 732 (39.3%) involved the kidney, 561 (30.1%) involved the ureter and 570 (30.6%) involved the lower tr

Cadaveric study of arterial renal anatomy and its surgical implications in partial nephrectomy.

PURPOSE: Partial nephrectomy is gaining, nowadays, more interest in oncologic kidney surgery. This type of surgery requires the good knowledge of vascular renal anatomy to make it safe and to guarantee good functional and oncological outcomes. This paper exposes the clinical implication of the arterial renal anatomy in nephron-sparing surgery. METHODS: This is a cadaveric study of 71 human kidneys performed at Charles Nicolle mortuary. The right and left kidneys with surrounding tissues were removed en bloc with the adjacent part of the aorta and inferior vena cava, cleared and studied. Color

Urothelial cancer of the renal pelvicaliceal system: unusual imaging manifestations.

Unusual imaging features of urothelial cancer of the renal pelvicaliceal system can be challenging for any radiologist. These manifestations include noncalcified and calcified focal infiltrative parenchymal masses, incidentally detected tumors in a hydronephrotic kidney due to ureteropelvic junction obstruction, transpelvic infiltrating solid masses extending through the retroperitoneum, and tumors primarily invading the perirenal fat. Less common manifestations include massive papillary frond-like projections within a large hydronephrotic sac, large multiloculated cystic masses with thick and

Tc-99m MAG3 scintigraphy in unusually dilated bilateral extrarenal pelves.

The authors report the findings of a radionuclide diuretic renogram in a patient with markedly dilated pelves bilaterally that filled most of the abdominal cavity. With the patient in the sitting position, at the end of the furosemide injection, there was good delineation and filling of the dilated calyces. These findings are unusual because of the extensive pelvicaliceal dilatation, and they emphasize the importance of the sitting position.

Endoscopic percutaneous drainage of a renal echinococcal cyst during percutaneous nephrolithotomy.

In Morocco, hydatid disease is endemic and constitutes a major problem of public health. Renal echinococcal cyst ranks third after pulmonary and hepatic locations. We report a case of renal echinococcal cyst diagnosed during percutaneous nephrolithotomy which was treated endoscopically. To our knowledge, this situation has not been reported previously in the literature.

Effect of furosemide and intravenous normal saline fluid load upon the renal resistive index in nonobstructed kidneys in children.

Recent literature has shown that relative to baseline the renal resistive index remains unchanged in nonobstructed kidneys and increases in obstructed kidneys after administration of furosemide. To our knowledge the effect upon the renal resistive index of furosemide administered in conjunction with intravenous normal saline fluid load has not been reported. We evaluated the renal resistive index in 13 nonobstructed kidneys in 8 children 6 to 18 years old before and after furosemide and intravenous normal saline fluid load. The mean resistive index decreased from baseline (mean decrease was 0.

Calyceal rupture with perirenal urinoma in a patient with cervical carcinoma.

A patient is presented with calyceal rupture and perirenal urinoma formation secondary to distal ureteral obstruction by cervical carcinoma. Preoperative diagnosis was established with computed tomography and renal scan. Surgical placement of an indwelling ureteral stent resolved the urinoma. Causes of perirenal urine extravasation and approaches to diagnosis and management are reviewed.

Percutaneous removal of caliceal and other "inaccessible" stones: results.

Percutaneous removal of renal stones (percutaneous nephrolithotomy) is becoming an established procedure, especially for stones lying free in the renal pelvis. However, some renal stones, particularly caliceal stones, are less accessible and, therefore, more difficult by the percutaneous route. We removed percutaneously 95 caliceal or otherwise poorly accessible renal stones from 53 patients with a variety of techniques, including percutaneous puncture above the 12th rib, double or Y percutaneous nephrostomy tracts, rigid and flexible endoscopy, and intrarenal cutting with diathermy. Computed tomography in the diagnosis of renal parapelvic cysts.

The appearances of 34 parapelvic cysts diagnosed by computed tomography (CT) in 29 patients are described. The majority were found as an incidental finding but some caused hydrocalycosis and, in one instance, hypertension. Their etiology, CT features, and differential diagnosis are discussed. The specificity of CT in these cases eliminates the need for invasive diagnostic procedures.

Cause of upper urinary-tract essential hematuria: Veno-caliceal fistula or contralateral coexistent upper-tract transitional-cell carcinoma? Case report.

A 65-year-old woman was admitted with intermittent gross hematuria of 6 months' duration without other significant urinary-tract symptoms. At bilateral ureterorenoscopy under general anesthesia, a connection from an upper calix to the main left renal vein through a small venous channel was apparent, but there was no evidence of bleeding. In the right upper calix, a bleeding small papillary lesion was seen and ablated with low-power thulium laser coagulation. The histologic examination revealed a grade 3 stage pT1 TCC. Where no definite bleeding is seen from a veno-caliceal fistulous opening at

Extracorporeal shock wave lithotripsy of radiolucent urinary calculi using the Dornier HM-3 and HM-4 lithotriptors.

Between September 1992 and June 1995, 33 patients with radiolucent urinary calculi were treated in our hospital using Dornier HM-3 and HM-4 lithotriptors. There were 11 caliceal stones, 13 in the renal pelvis, and 9 in the ureter. Stone localization was achieved in only 30 patients using intravenous urography or retrograde urography through a ureteric catheter during the procedure. All patients were treated on an outpatient basis, without anesthesia. During the 1st month following extracorporeal shock wave lithotripsy, the patients received alkalization. The overall stone-free rate at 3 months

[Para-pelvic cysts: difficulties in differential diagnosis].

According to their different etiology, parapelvic cysts must be differentiated from simple renal cysts, pararenal pseudocysts, intramural renal pelvis cysts and suprarenal pseudocysts. An important criterium of parapelvic cysts is their epithelial lining. According to their radiological features, these cysts must be described as "parapelvically located cysts" until the definite histological diagnosis is made by the pathologist. The exact differentiation between a parapelvic cyst and a pararenal pseudocyst may be of forensic importance in posttraumatic patients.

Carcinoma of colon after ureterocolic anastomosis: implantation on calyceal mucosa.

Two cases are reported of the development of mucoid adenocarcinoma of the colon in young patients, nineteen and thirty-four years after ureterocolic anastomosis for benign conditions. One patient showed implantation of the colonic neoplasm on the mucosa of an upper calyx in the obstructed kidney. Patients who have undergone ureterocolic anastomosis have a greatly increased risk of the development of large-bowel neoplasms. When suspected, radiologic examination of the colon using a water-soluble contrast material, together with rectosigmoidoscopy or colonoscopy, is essential. Because of the ser

Late sequelae of adult-onset acute bacterial nephritis.

The late manifestations of adult-onset acute bacterial nephritis were studied in four patients from 4 months to 5.6 years following initial infection. An unusual combination of generalized (global) wasting of the kidney and focal calyceal clubbing developed within a few weeks of the acute infection. The pattern of calyceal deformity strongly suggests papillary necrosis; it may develop during the acute phase, but go unrecognized during excretory urography at that time because of impaired contrast material excretion. The combination of a small, smooth kidney and papillary necrosis previously has

Uretero-calicostomy.

Implantation of the ureter into the calyx is indicated in cases of uretero-pelvic stricture when anastomosis of the ureter with the pelvis is impossible. Results of this procedure performed in ten cases are presented. In two cases the same kidney was operated upon twice; in five cases the results were satisfactory; in two cases the results were poor, and in one case the time of observation is too short for proper evaluation.

Perirenal extravasation with blunt trauma from rupture of a calyceal fornix.

Perirenal extravasation of urine with an intact renal parenchyma and with dye visualized in the ureter was seen on an excretory urogram (IVP) and computed tomographic (CT) scan in two patients (bilateral in one) with multiple injuries following blunt trauma. Expectant management with no invasive procedures resulted in disappearance of the extravasation within 3 to 5 days. Traumatic rupture of a calyceal fornix is thought to be the cause of the extravasation similar to the perirenal extravasation seen with renal colic from a ureteral calculus.
