One-Stage Arthroscopic Reduction Combined With Reconstruction of Knee Stability for Irreducible Knee Dislocation: A 2-Year Follow-up Study.

BACKGROUND: Irreducible knee dislocation (IKD) is a rare but severe injury, accounting for approximately 4% of all knee dislocations and characterized by soft tissue incarceration that precludes closed reduction. Current management remains debated, particularly regarding the optimal timing of ligament reconstruction (single- vs. two-stage) and the choice of surgical approach. PURPOSE: To investigate the short-term efficacy and outcomes of 1-stage arthroscopic reduction combined with reconstruction of knee stability for irreducible knee dislocation (IKD). STUDY DESIGN: Cohort study; Level of

Establishing the indications for temporising knee-spanning external fixation: A modified Delphi study of the International Knee Dislocation Study Group.

PURPOSE: Knee dislocations (KDs) can be limb-threatening injuries that may require a temporising knee-spanning external fixator (KSEF) for stabilisation. Precise indications for this commonly utilised invasive immobilisation technique remain controversial and poorly defined. The purpose of this study was to establish consensus-driven indications for temporising KSEF use in the initial management of KDs. METHODS: A working group of fellowship-trained orthopaedic surgeons generated clinical scenarios reflecting commonly debated indications for temporising KSEF application. Utilising a modified

Total knee arthroplasty combined with medial patellofemoral ligament reconstruction for osteoarthritic knee with preoperative valgus deformity and chronic patellar dislocation.

We report a challenging case of a 44-year-old woman who had osteoarthritis in the lateral compartment of her right knee with severe valgus deformity and chronic lateral patellar dislocation. Total knee arthroplasty was performed for the knee. However, persistent patellar dislocation remained during the surgery; and therefore, medial patellofemoral ligament (MPFL) reconstruction was additionally performed at the time of the surgery. Stable patellar tracking was obtained after the MFPL reconstruction; and during the 2-year follow-up, her knee functioned well, and no recurrent patellar dislocatio

Mobile Bearing Total Knee Arthroplasty for Valgus Knee Osteoarthritis with Permanent Patellar Dislocation: A Case Report and Review of the Literature.

Permanent patellar dislocation with tibiofemoral joint osteoarthritis is a relatively rare condition. To treat this condition, total knee arthroplasty with proximal or distal realignment of the extensor mechanism has been reported. We report a challenging case of an 80-year-old woman diagnosed with permanent patellar dislocation with tibiofemoral joint osteoarthritis treated by a mobile bearing total knee arthroplasty utilizing navigation system. Lateral retinaculum release was performed to improve patellar tracking; other proximal or distal realignment of the extensor mechanism was not necess

Does age predict outcome after multiligament knee reconstruction for the dislocated knee? 2- to 22-year follow-up.

PURPOSE: The purpose of this study is to determine whether age is a predictor of clinical and functional outcomes in patients who sustained a knee dislocation (KD) and underwent multiligament knee reconstruction. It was hypothesized that increasing age will negatively affect patient outcome. METHODS: In total, 125 multiligament knee injuries (MLKI) associated with KD were surgically reconstructed between 1992 and 2013 and evaluated with IKDC and Lysholm scores at a median follow-up of 5 (range 2-22) years. Patient demographics including age were then analysed with respect to IKDC and Lysholm

Knee Dislocation With Vascular and Nerve Injury in a Professional Football Player: Return to Play.

Traumatic knee dislocations are serious and complex injuries, defined as clinical and/or radiological loss of tibiofemoral congruence, which may represent real emergencies to the integrity of the affected limb. This lesion is responsible for multi-ligament tears but the most serious possible complications are related to vascular and peripheral nerve injuries. Recent studies show that surgical treatment has better functional results and higher return rates to sports practice compared to conservative treatment. However, there is still no consensus on the ideal surgical technique and the timing o

Determining the real site of peroneal nerve injury with knee dislocation: Earlierier is easier.

Common peroneal nerve (CPN) injury is a recognised complication of traumatic knee dislocation with a direct association between the degree of ligamentous injury and the degree of CPN injury. It is essential explore and repair these injuries in good time to reduce morbidity. Often exploration only involves the portion of this nerve associated with the joint as it courses around the fibular head. However, a recent case highlighted the importance of proximal exploration to its branching point from the sciatic nerve, a known point of fragility, even if other defects have been identified.

An analysis of knee anatomic imaging factors associated with primary lateral patellar dislocations.

PURPOSE: Various knee anatomic imaging factors have been historically associated with lateral patellar dislocation. The characterization of these anatomic factors in a primary lateral patellar dislocation population has not been well described. Our purpose was to characterize the spectrum of anatomic factors from slice imaging measurements specific to a population of primary lateral patellar dislocation. A secondary purpose was to stratify these data by sex/skeletal maturity to better detail potential dimorphic characteristics. METHODS: Patients with a history of primary lateral patellar disl

Locked knee from superior dislocation of the patella-diagnosis and management of a rare injury.

Knee locking is often caused by a torn meniscus or loose body. A rare cause of knee locking is a superior dislocation of the patella following trauma with less than 20 reported cases in the English literature. An unusual case of a locked knee secondary to interlocking osteophytes between the medial femoral condyle and the inferior pole of the patella without any history of trauma is presented.

Popliteal artery thrombosis associated with trampoline injuries and anterior knee dislocations in children.

Recent reports have emphasized the benign nature of trampoline-associated injuries. However, this study describes the limb-threatening problem of popliteal artery thrombosis occurring in association with anterior knee dislocation and trampoline injuries. Three children (ages 11, 13, and 17) were referred to the emergency room within the past 12 months with anterior dislocations of the knee, which occurred while jumping on trampolines. All patients had reduction of their dislocations at outside facilities and were referred within 6 to 12 hours after their injuries, with pulse, motor, and sensor

Dislocation of total knee arthroplasty. A report of 6 cases with 2 patterns of instability.

We report 6 cases of dislocation of total knee arthroplasty (TKA). We identified 2 patterns of instability in the sagittal plane, one with posterior translation of the tibia, occurring mainly in the postoperative period and usually resulting from a trauma, and the other with anterior translation of the tibia, occurring 6 months to 7 years postoperatively and with no preceding trauma. In the latter group, instability was manifested gradually as a lengthening of the posterior cruciate ligament and the posterior capsule, leading to a recurvatum deformity of the knee.

MRI in acute knee dislocation. A prospective study of clinical, MRI, and surgical findings.

We treated 17 knees in 15 patients with severe ligament derangement and dislocation by open repair and reconstruction. We assessed the competence of all structures thought to be important for stability by clinical examination, MRI interpretation, and surgery. Our findings showed that in these polytrauma patients clinical examination was not an accurate predictor of the extent or site of soft-tissue injury (53% to 82% correct) due mainly to the limitations of associated injuries. MRI was more accurate (85% to 100% correct) except for a negative result for the lateral collateral ligament and pos

The Hauser operation for patellar dislocation. 3-32-year results in 63 knees.

The Hauser operation for patellar dislocation was performed in 34 women and 20 men, median age 18 (3-55) years; one leg was amputated because of wound infection with chronic septic arthritis. At the time of follow-up, 8 (3-32) years after the operation 57 knees had normal or almost normal patellar stability, but only 26 knees were free from pain. Only 16 knees had both normal patellar stability and were without pain. Patellar arthrosis had developed in 16 knees and femorotibial arthrosis in 23 knees. Eleven patients operated on before the age of 15 years showed varying grades of axial deformit
