Combined Effect of Pre-Operative Antibiotics and Enema for Surgical Site Infection Prevention in Anorectal Surgery: A Retrospective Study.

BACKGROUND: There is no consensus on the optimal strategy for surgical site infection (SSI) prevention in anorectal surgery. Pre-operative antibiotic agents and enema are commonly used, but their effectiveness in SSI prevention has not been studied. The objective of this study was to determine the combined effect of antibiotic agents and enema on SSI rates in anorectal surgery for benign disease. METHODS: A retrospective study was conducted between July 2015 and June 2024 in an academic institution on adult patients who underwent either elective excisional hemorrhoidectomy, lateral internal s

Management of chronic anal fissures: should fissurectomy be performed with botulinum toxin injection?

BACKGROUND: Despite the superior success rate of lateral internal sphincterotomy for managing chronic anal fissure (CAF), there is a trend towards sphincter-preserving treatment due to the increased risk of incontinence. Botulinum toxin (BT) and fissurectomy are two sphincter-preserving options for CAF. We aim to assess if combining fissurectomy with botox treatment is superior to botulinum toxin alone in the management of CAF. METHODS: This retrospective cohort study was conducted across two Sydney hospitals over 7 years. All patients with a CAF managed with either BT and fissurectomy (group

Three Years of Experience Using a Diode Laser in the Management of Common Anal Benign Pathologies: A Retrospective Study.

Background The management of anorectal conditions continues to evolve with newer techniques for colorectal surgeons. Recently, diode laser technology has gained attention in proctology as a less invasive alternative to traditional surgery. The mechanism involves tissue shrinkage and degeneration and depends on the power of the laser and the duration of exposure. This method is associated with reductions in postoperative pain, bleeding, discharge, and hospital stays. Patients and methods This retrospective study involved 456 patients with hemorrhoids, chronic anal fissure, and perianal fistula.

The 5-year outcomes and predictors of healing in chronic anal fissure treated with botulinum toxin: a retrospective analysis of 199 cases.

BACKGROUND: Botulinum toxin (BT) is a nonsurgical alternative to lateral internal sphincterotomy (LIS). While there are promising results, there is still a gap in knowledge regarding long-term outcomes and the predictors for healing after BT. METHODS: Chronic anal fissure (CAF) patients treated with 100 IU BT with a minimum 5-year follow-up were analyzed retrospectively. Patients with persistent or recurrent fissures after their first BT injection were offered either a second BT injection or LIS. Healing was defined as complete symptom resolution with fissure epithelization. The primary outco

Effects of neuromodulation on treatment of recurrent anal fissure: A systematic review.

BACKGROUND: Anal fissure is a linear tear in the distal anoderm most frequently occurring in the posterior midline. Lateral internal sphincterotomy is considered the gold standard for the treatment of this condition. To this date, several pharmacological and neuromodulatory therapies have been tried out as alternative non-invasive treatments for anal fissures, however, the efficacy of some of these methods remains to be understood. OBJECTIVE: the primary aim of this study was to review available literature on neuromodulatory treatments for recurrent anal fissure as a potentially effective cur

Internal sphincterotomy in post-pull-through Hirschsprung's disease.

Despite proper technique, pull-through operations for Hirschsprung's disease sometimes fail to deliver normal or effective bowel evacuation. Ten patients, described herein, had had a pull-through procedures. The procedures had been performed by various pediatric surgeons. Five cases had been diagnosed in the newborn period and had undergone colostomies. The remainder had been diagnosed later (at 2 months to 2 years of age). They too had undergone colostomy initially, and all had an elective pull-through procedure. The techniques varied; two had Soave procedures, seven had Duhamel procedures, a

High-Intensity Laser Therapy - An Option for Managing the Pain in Anal Fissures.

Introduction: Anal fissures (AFs) are a prevalent pathology. Although internal lateral sphincterotomy is still the gold-standard surgery for treating chronic AFs, this procedure is associated with a considerable risk of anal incontinence. This study describes an alternative and minimally invasive technique for treating AFs using photobiomodulation and a high-power diode laser-fissure treatment (LFT) and highlights initial results pertaining to pain. Methods: This retrospective study focused on 38 patients treated with LFT on an outpatient basis at three different hospitals in different states

Surgical management of chronic pancreatitis.

The management of chronic pancreatitis continues to achieve only limited success. A lack of understanding of the basic pathogenic mechanism of this disease limits our therapy to treatment of symptoms, sequelae, and complications. The diagnosis of chronic pancreatitis usually is based on a history of classic pain plus some objective findings of pancreatic disease. Imaging techniques, such as ultrasonography or CT, are helpful in defining the size of the gland and the presence of masses and collections of fluid. Endoscopic pancreatography, however, remains the most helpful tool for diagnosis. Th

Cost-saving effect of treatment algorithm for chronic anal fissure: a prospective analysis.

Evidence-based medicine suggests that in the management of chronic anal fissure (CAF), lateral internal sphincterotomy (LIS) is far more effective than medical treatment in lowering the anal sphincter tone and curing the fissure. In the current study, we developed a treatment algorithm from topical nitroglycerin (NTG) to botulinum toxin type A (Botox [BTX]) to LIS and analyzed its cost benefit by calculating the effective and potential costs based on the treatment success and the rate of avoided surgeries. Patients presenting between November 2003 and December 2004 with CAF and symptoms for gr

Anorectal disorders.

Anorectal disorders are commonly encountered in the practice of emergency medicine. Most can be diagnosed and treated in the emergency department setting. Almost all anorectal disorders once diagnosed and treated in the emergency department need appropriate follow-up to ensure adequacy of treatment, for further possible diagnostic procedures (e.g., endoscopy, biopsy), or for definitive treatment. Hemorrhoids are the most prevalent anorectal disorder and are the most common cause of hematochezia. Treatment is dependent on the degree of hemorrhoid prolapse and symptoms. Most cases can be treated

Nonsurgical management of an extensive spontaneous spinal epidural hematoma causing quadriplegia and respiratory distress in a choledocholithiasis patient: A case report.

RATIONALE: Spontaneous spinal epidural hematoma (SSEH) manifests from blood accumulating in the epidural space, compressing the spinal cord, and leading to acute neurological deficits. The disease's cloudy etiology and rarity contribute to dangerously suboptimal therapeutic principles. These neural deficits can be permanent, even fatal, if the SSEH is not treated in a timely and appropriate manner. Standard therapy is decompressive laminectomy, though nonsurgical management is a viable course of action for patients who meet a criterion that is continuously being refined. PATIENT CONCERNS: A 7

A systematic review and meta-analysis of the treatment of anal fissure.

BACKGROUND: Anal fissure has a very large number of treatment options. The choice is difficult. In an effort to assist in that, choice presented here is a systematic review and meta-analysis of all published treatments for anal fissure that have been studied in randomized controlled trials. METHODS: Randomized trials were sought in the Cochrane Controlled Trials Register, Medline, EMBASE and the trials registry sites clinicaltrials.gov and who/int/ictrp/search/en. Abstracts were screened, full-text studies chosen, and finally eligible studies selected and abstracted. The review was then divid

AGA Clinical Practice Update on Management of Ampullary Neoplasms: Expert Review.

DESCRIPTION: Although ampullary neoplasia is a rare entity and represents 0.6%-0.8% of gastrointestinal cancers, the incidence among young adults has risen over the past 20 years. Most ampullary tumors are sporadic, but a few genetic syndromes substantially increase the risk of ampullary neoplasia. Compared with other neoplastic pancreaticobiliary entities, there is a relative lack of guidance on best practices for management of ampullary lesions. Ampullary cancer is associated with a 5-year survival rate ranging from 20% to 75%, depending on the stage of diagnosis. Ensuring high-quality endos

Waldenstrom Macroglobulinemia Leading to Cholestatic Liver Injury and Rapid Progression to Liver Failure.

Waldenstrom macroglobulinemia (WM) is an indolent B-cell neoplasm with generally good prognosis, and while hepatic involvement in WM has been documented in the literature, it is rarely clinically significant. We present a case of a 77-year-old man with WM who presented with a cholestatic liver injury and rapid progression to decompensated liver disease. The patient ultimately transitioned to comfort care and died from liver failure. This case not only highlights the rarity of this poor outcome from WM, but also underscores the importance of early recognition of infiltrative malignancy as a pot
