Virilizing ovarian Leydig cell tumor in a woman with subclinical Cushing syndrome.

OBJECTIVE: To report the case of a patient with a virilizing ovarian Leydig cell tumor and subclinical Cushing syndrome attributable to an adrenal adenoma. METHODS: Detailed clinical, laboratory, radiologic, and pathologic findings are presented, and the pertinent literature is reviewed. RESULTS: A 49-year-old woman was referred for evaluation of a left adrenal mass (3.0 by 2.4 cm), which had been diagnosed by computed tomographic scan 4 years previously during a work-up for hirsutism. On examination, she had central obesity, facial hirsutism, and male pattern baldness. Work-up showed elevat

[Follow-up in a boy with Leydig cell tumor after selective surgery].

A 8(6/12) year-old-boy presented with precocious puberty and a slightly enlarged left testis. After a detailed examination a Leydig cell tumour was diagnosed. Surgical exploration revealed an encapsulated tumour, 2.7 cm in length, which was selectively removed without orchidectomy. Within one year the clinical signs of pubertal precocity disappeared, the bone age did not further advance and height velocity declined from 8.2 cm / year (+3.9 SDS) to 4.1 cm/year (-1.0 SDS). Physiologically, he entered puberty at the chronological age of twelve years, presenting at that age, in comparison to his p

Effects of bismuth citrate on the viability and function of Leydig cells and testicular macrophages.

Bismuth is present in several popular over-the-counter drugs for nausea and diarrhea and is occasionally abused by patients with chronic gastrointestinal disorders. The most common consequence of bismuth overdose is neurological dysfunction. In experimental animals, bismuth overdose results in lowered serum testosterone levels, suggesting that reproductive dysfunction may be an additional component of bismuth toxicity. Although the precise mechanisms responsible for the lowered testosterone levels are unknown, it has been shown that bismuth accumulates within testicular macrophages. This may b

[Male infertility in an adult with unilateral Leydig cell tumor of the undescended testis].

We report a case of Leydig cell tumor of the postpubertal cryptorchid testis with the chief complaint of male infertility. A 36-year-old man had consulted another clinic and his semen analysis revealed oligospermia. A solid mass was palpated in the right inguinal undescended testis. He was referred to our hospital for treatment of the testicular tumor. Right inguinal orchiectomy was performed and pathological diagnosis was Leydig cell tumor with no malignant findings. After the surgery improvement was seen in his semen analysis. There was no evidence of recurrence 9 months after surgery.

Evidence for genetic heterogeneity in male pseudohermaphroditism due to Leydig cell hypoplasia.

Leydig cell aplasia or hypoplasia is a rare form of male pseudohermaphroditism resulting from inadequate fetal testicular Leydig cell differentiation. Affected individuals presented a wide phenotypic spectrum, ranging from complete female external genitalia to males with micropenis. Recessive mutations in the LH receptor gene have been identified as responsible for the condition. The majority of these mutations are point mutations and have been located in exon 11 of the gene. In this study, we report the molecular characterization of the LH receptor gene in three siblings with Leydig cell hypo

Hormonal profile of patients with Leydig cell tumors: a urologic cause of gynecomastia.

It is possible to hypothesize an alternative role for estrogens as a predisposing factor for testicular abnormalities: estrogen exposure during development in perinatal life may initiate cellular changes which would require estrogen and/or androgen later in life for promotion to hyperplasia or neoplasia. We reviewed the literature on Leydig cell tumors and the hormonal modifications they induce. In adult patients with Leydig cell tumors, although the serum estrogen (E2) and testosterone (T) varied, the T/E2 ratio was constantly low, and the chorionic gonadotropin administration produced an hig

A paradoxical inhibition of androgenic hyperproduction by a Sertoli-Leydig cell tumour ovary.

A 17 year old woman was evaluated for amenorrhoea and severe hirsutism (Ferriman-Gallway index = 31). Pelvic ultrasound demonstrated a right unilateral ovarian mass (6 x 5 cm), whereas the computed tomography for the adrenal gland was normal. Endocrinological findings revealed normal concentrations of oestradiol, progesterone, dihydroepiandrosterone sulphate, cortisol, prolactin, follicle-stimulating hormone, luteinizing hormone and adrenocorticotrophic hormone (ACTH). Total testosterone, free testosterone, androstenedione and 17-hydroxy-progesterone concentrations, already elevated at basal c

[Mid-term and long-term outcome in Leydig cell estrogen tumors].

A study has been realized in six patients with estrogen secreting Leydig cell tumors from 18 to 120 months after hemicastration. Testicular contralateral volume is normalized, gynecomastia can be completely reduced, sex steroids are normalized while gonadotropins can be temporary increased. Spermogram which is abnormal can be normalized. These results confirm the heterogeneity of these tumors.

[Leydig cell tumor: presentation of 3 new cases with a benign course].

Three cases of Leydig cell tumor are described. Patient ages were 27, 44 and 55 years, respectively. The first two patients consulted for a testicular mass and the third patient presented with gynecomastia, an incidental finding during the ultrasound work up of a hydrocele. Hormonal evaluation revealed a marked serum testosterone level in one case, whereas in another case it fell within the lower limits of normal ranges. No evidence of dissemination was observed in all patients. Treatment was by radical inguinal orchiectomy in all patients. The pathological analysis of the surgical specimens r

Ovarian and peripheral steroid hormones in a case of Sertoli-Leydig cell tumor.

The clinical course, histology, and steroid secretion of a 59-year-old postmenopausal woman with a 4-yr history of virilizing well-differentiated Sertoli-Leydig cell tumor of the right ovary are reported. Hormone secretion was examined by measuring peripheral and ovarian venous gradients and pre-and postoperative levels of some delta 4 and delta 5 steroids, estrogens, gonadotropins and Sex Hormone Binding Globulin levels. The preoperative responsiveness to ACTH, dexamethasone and hCG is also reported. The results are consistent with a Sertoli-Leydig cell tumor producing mainly testosterone and

Atrophy of Leydig cells in the testes of men with longstanding chronic bronchitis and emphysema.

The total volume of Leydig cells in the testes of men coming to necropsy with a history of chronic bronchitis and emphysema of at least 15 years' duration, and with morphological evidence at necropsy of the cardiopulmonary effects of hypoxia, was significantly less than the volume in matched controls (0.98 (SD 0.39) and 0.93 (0.42) ml v 1.66 (0.54) and 1.72 (0.52) ml for left and right testes respectively; p less than 0.005). This atrophy may be a consequence of hypoxic inhibition of pituitary synthesis or release of luteinising hormone.

Stimulation of Leydig cell function by a polypeptide present in testicular interstitial fluid.

Rat testicular interstitial fluid (IF) contains a polypeptide factor(s) which when added to Percoll-purified rat Leydig cells in vitro, enhances both basal and hCG-stimulated production of testosterone in a dose-dependent manner. To assess the possible physiological significance of this factor(s), its levels in testicular IF from individual rats were assessed following the suppression of intratesticular levels of testosterone by either (a) treatment with anti-LH, (b) induction of unilateral cryptorchidism, or (c) treatment with ethane-1,2-dimethane sulphonate (EDS). These treatments suppressed

Leydig cell hyperplasia in fetal mice treated transplacentally with ethinyl estradiol.

Pregnant female mice were given ethinyl estradiol on days 11 through 17 of gestation. On day 18 the dams were killed and the male fetuses were examined for testicular differentiation. Three of 12 males from dams treated with the highest dose of ethinyl estradiol showed cryptorchid testes with uterine tubes. Light and electron microscopic evaluation of the testes, both cryptorchid and normal, demonstrated foci of hyperplasia of Leydig cells showing cytoplasmic and nuclear pleomorphism, increase in lipid droplets, and decrease in smooth endoplasmic reticulum and ribosomes when compared to testes

Ovarian Sertoli-Leydig cell tumors. A clinicopathological analysis of 207 cases.

The clinical and pathological features of 207 ovarian Sertoli-Leydig cell tumors from our consultation and hospital files were reviewed. The patients ranged in age from 2 to 75 (average 25) years. Seventy-five percent of them were 30 years of age or younger and less than 10% were over 50 years of age. One-third of the patients presented because of unequivocal evidence of androgen excess, and an additional 10% had a history suggesting androgen excess; most of the remaining patients complained of abdominal swelling or pain. At operation, 97.5% of the tumors were Stage I, 1.5% were Stage II, and

Leydig's and sertoli cells. Their fine structures in three cases of Klinefelter's syndrome.

Testicular biopsy specimens obtained from three patients with Klinefelter's syndrome were studied by light and electron microscopy. Of note in the ultrastructural findings were the absence of annulate lamellae in the Sertoli cells and the abnormal mitochondria, the presence of microcrystalline formations, and the absence of the crystals of Reinke in Leydig's cells. It is suggested that the impaired hormonal function of the testes in Klinefelter's syndrome may be related to the absence of annulate lamella in the Sertoli cells and the presence of microcrystal formations in Leydig's cells.

A boy with Leydig cell tumour and precocious puberty: ultrasonography as a diagnostic aid.

A 7 1/2 year-old boy developed pseudoprecocious puberty. The diagnosis of Leydig cell tumour was suggested by clinical and hormonal findings and greatly facilitated by ultrasonographic investigation of the testes. Surgical exploration was in keeping with the diagnosis and the tumour was removed. Orchidectomy was not necessary. It appears that ultrasonography may be very useful in diagnosing a testicular tumour. It facilitates surgical intervention since it permits localization of the lesion which is usually very small and limited to a small part of the testis, as in this case.

Leydig cell dysfunction and gynaecomastia in adult males treated with alkylating agents.

Three adult males presented with gynaecomastia, small testes and clinical features of androgen deficiency after treatment with alkylating agents. Laboratory evaluation revealed elevated serum follicle stimulating hormone and luteinizing hormone. Serum testosterone oenanthate relieved the symptoms of androgen deficiency in addition to reducing the size and tenderness of the gynaecomasia. It is postulated that in addition to the well documented effects of alkylating agents on the seminiferous tubules, a state of 'compensated Leydig cell dysfunction' in induced. The relative decrease in serum tes

Leydig cell responsiveness to single and repeated human chorionic gonadotropin administration.

A single im injection of 1500 IU hCG significantly increased plasma testosterone levels for at least 96--120 h in normal men (n = 7), patients with isolated gonadotropin deficiency (n = 6), and boys with delayed puberty (n = 7); the maximum values [1315 +/- 309, 370 +/- 177, and 963 +/- 249 ng/100 ml (mean +/- SD), respectively] were achieved after 72 h in each group. Repeated daily injections of 1500 IU hCG for 3 days increased plasma testosterone levels in the same subjects at 72 h after the start to levels (1342 +/- 412, 407 +/- 199, and 1052 +/- 449 ng/100 ml, respectively) similar to thos

Identification and isolation of human testicular peritubular myoid cells and Leydig cells by a combination of ITGA9 and NGFR.

BACKGROUND: Testicular somatic cells play an important role in supporting spermatogenesis. Leydig cells (LCs) and peritubular myoid cells (PTMs) originate from a common progenitor population and show similar expression signatures in adulthood, making it difficult to distinguish and isolate the two in vitro. METHODS: In this study, new surface markers for identifying adult LCs (ALCs) and PTMs were discovered by reanalyzing testicular single-cell dataset. Differential expressions of ITGA9 and NGFR were confirmed through immunofluorescence staining of human testes. A novel Fluorescence activated

Role of immunohistochemistry in the fertility-sparing management of Sertoli Leydig cell tumour.

Ovarian Sertoli Leydig cell tumours (SLCTs) are rare and occur predominantly in young females. Categorised as less common ovarian cancers, they have a favourable prognosis, and fertility preservation is an option in early-stage disease, where fertility is desired. Here, we describe a case of a late adolescent girl, who presented after tumour excision of left ovarian mass. Her initial histopathology report showed mucinous adenocarcinoma of left ovary. Slide review and immunohistochemistry suggested stage IC, intermediate grade Sertoli Leydig cell tumour and the patient was kept on observation.
