Takotsubo Syndrome Mimicking Acute Myocardial Infarction: A Case Report.

Takotsubo syndrome is an acute, stress-related cardiomyopathy characterized by transient left ventricular systolic dysfunction in the absence of obstructive coronary artery disease. Although often clinically indistinguishable from acute coronary syndrome, Takotsubo syndrome represents a distinct form of myocardial injury within the Myocardial Infarction with Non-Obstructive Coronary Arteries (MINOCA) spectrum. Prompt recognition is essential, as early complications may include hemodynamic instability and clinically significant arrhythmias. We present the case of a 77-year-old woman with type 2

Spontaneous Coronary Artery Dissection in Clinical Practice: Pathophysiology and Therapeutic Approaches.

Spontaneous coronary artery dissection (SCAD) is a cause of myocardial infarction without obstructive coronary artery disease (MINOCA). It is determined by a coronary artery wall layers separation, which occurs regardless of traumatic or iatrogenic injuries. Even if it is often a missed diagnosis, its incidence is growing along with the improvement of intracoronary imaging techniques that allow for better detection. The main angiographical classification distinguishes three different forms, with slightly different prognoses at long-term follow up. SCAD is a recurrent condition, severely hamper

Transient Left Ventricular Dysfunction from Cardiomyopathies to Myocardial Viability: When and Why Cardiac Function Recovers.

Transient left ventricular dysfunction (TLVD), a temporary condition marked by reversible impairment of ventricular function, remains an underdiagnosed yet significant contributor to morbidity and mortality in clinical practice. Unlike the well-explored atherosclerotic disease of the epicardial coronary arteries, the diverse etiologies of TLVD require greater attention for proper diagnosis and management. The spectrum of disorders associated with TLVD includes stress-induced cardiomyopathy, central nervous system injuries, histaminergic syndromes, various inflammatory diseases, pregnancy-relat

Spontaneous coronary artery dissection (SCAD): The underdiagnosed cardiac condition that plagues women.

Coronary heart disease (CHD) continues to be understudied, underdiagnosed, and undertreated in women. Gender and age bias complicate the evaluation of women with acute coronary syndrome (ACS). As a result, conditions like spontaneous coronary artery dissection (SCAD) are often missed. SCAD is an infrequent yet important cause of myocardial infarction (MI) with a predilection for young to middle aged women. The condition is thought to be under-reported, likely a result of both low index of suspicion as well as an unfamiliarity with SCAD's angiographic variants. Recently, the European Society of

[Intracoronary imaging in women].

BACKGROUND: Coronary artery disease remains the leading cause of mortality among women, despite a persistent underestimation of its clinical impact. Intracoronary imaging techniques (ICI), such as intravascular ultrasound (IVUS) and optical coherence tomography (OCT), have emerged as valuable tools to complement angiography, especially in women, whose pathophysiological mechanisms are often atypical. AIMS: To analyse current data on the use of ICI in women and highlight its benefits, limitations, and sex-specific considerations. METHODS: Narrative review of recent literature, including obser

A case of Sudden Cardiac Death with myocardial infarction due to iatrogenic coronary dissection in a patient with coronary vasospasm.

Coronary vasospasm and coronary artery dissection are two recognized causes of Sudden Cardiac Death [SCD] in association with Myocardial Infarction in Non-Obstructive Coronary Artery [MINOCA]. This is a case of post procedure death in a 50-year-old female during cardiac angiography, who had a family history of coronary spasm with sudden cardiac death in her mother. She went into cardiac arrest during the procedure with iatrogenic acute coronary dissection which was treated with LMS/LCx vessel stenting. However, she died in 4 days post procedure. Postmortem cardiac examination demonstrated a tr

Invasive Functional Coronary Assessment in Myocardial Ischemia with Non-Obstructive Coronary Arteries: from Pathophysiological Mechanisms to Clinical Implications.

Despite ischemic heart disease (IHD) has been commonly identified as the consequence of obstructive coronary artery disease (OCAD), a significant percentage of patients undergoing coronary angiography because of signs and/or symptoms of myocardial ischemia do not have any significant coronary artery stenosis. Several mechanisms other than coronary atherosclerosis, including coronary microvascular dysfunction (CMD), coronary endothelial dysfunction and epicardial coronary vasospasm, can determine myocardial ischemia or even myocardial infarction in the absence of flow-limiting epicardial corona

Acute coronary syndrome from scombroid poisoning: a narrative review of case reports.

BACKGROUND: Scombroid syndrome constitutes a toxic illness caused by ingestion of improperly stored fish, usually contaminated by Gram-negative bacteria producing histamine and other toxins. Scombroid currently accounts for approximately 5% of food toxicities in the United States. Though cardiovascular complications are infrequent, some patients experienced acute coronary syndrome (ACS) on admission to hospital. This article focuses on the main pathophysiology, clinical features, therapy, and outcomes in scombroid-related ACS. METHODS: Starting from the consolidated knowledge on histaminergic

Cardiac magnetic resonance in patients with acute myocardial infarction: The added clinical value.

Cardiovascular magnetic resonance (CMR) has emerged as the reference noninvasive modality for a comprehensive assessment of myocardial injury following acute myocardial infarction (AMI). Beyond accurate quantification of ventricular volumes and ejection fraction, CMR uniquely enables in vivo characterization of the infarct size, the ischemic area at risk, microvascular obstruction, intramyocardial hemorrhage, and interstitial remodeling-pathophysiological processes that are all tightly linked to adverse left ventricular remodeling and long-term clinical outcomes. Advances in late gadolinium en

State of the art: Evaluation and prognostication of myocarditis using cardiac MRI.

Myocarditis encompasses both primary and secondary processes causing inflammation of the myocardium. Viral infections are a common secondary cause of myocarditis with important clinical relevance. Viral myocarditis has a varied clinical presentation, potentially resulting in significant morbidity and mortality. Acutely, systolic dysfunction and sudden cardiac death may ensue; chronically, myocarditis may result in a dilated cardiomyopathy requiring heart transplantation. Myocarditis is thought to be one of the most common causes of myocardial infarction with nonobstructive coronary arteries (M

Cardiac Magnetic Resonance-Supported Diagnosis of Acute Myocarditis Mimicking Acute Coronary Syndrome in a Patient With Sepsis Due to a Massive Perianal Abscess.

Acute myocarditis is a rare but potentially life-threatening inflammatory disease that may mimic acute myocardial infarction, particularly in septic patients where myocardial injury may have other causes. We report the case of a 46-year-old man with sepsis secondary to a massive perianal abscess who underwent emergency surgical drainage under general anesthesia. Postoperatively, persistent respiratory deterioration and elevated cardiac biomarkers prompted further cardiac evaluation. Echocardiography showed newly reduced left ventricular (LV) systolic function, with an ejection fraction (EF) of

[Cardiac MRI in nonischemic cardiomyopathies].

BACKGROUND: The classification of cardiomyopathies used in Germany goes back to the European Society of Cardiology (ESC) classification of 2008. The cardiomyopathies are subdivided according to the phenotype, so that magnetic resonance imaging (MRI) is able to differentiate between the various cardiomyopathies. IMAGING AND DIFFERENTIAL DIAGNOSTICS: The strength of MRI is the ability to differentiate nonischemic cardiomyopathies from other diseases with similar morphofunctional aspects, based on the possibilities of tissue differentiation. In the case of dilated cardiomyopathy (DCM), for examp

[Acute Coronary Syndrome (ACS) in Preclinical Emergency Medicine].

Acute coronary syndrome (ACS) is a common diagnosis in preclinical emergency medicine. The term summarizes the acute manifestations of coronary artery disease. It ranges from unstable angina pectoris via cardiogenic shock to sudden cardiac death. The leading key symptom is chest pain. With this trigger symptom, a clinical diagnostic algorithm is initiated, acting quickly on the suspected diagnosis of acute myocardial infarction. Due to the potentially life-threatening course, rapid diagnosis and initiation of therapeutic measures is crucial. Pre-clinical antithrombotic medication and therapy f

Endothelin: 30 Years From Discovery to Therapy.

Discovered in 1987 as a potent endothelial cell-derived vasoconstrictor peptide, endothelin-1 (ET-1), the predominant member of the endothelin peptide family, is now recognized as a multifunctional peptide with cytokine-like activity contributing to almost all aspects of physiology and cell function. More than 30 000 scientific articles on endothelin were published over the past 3 decades, leading to the development and subsequent regulatory approval of a new class of therapeutics-the endothelin receptor antagonists (ERAs). This article reviews the history of the discovery of endothelin and it

Prognostic value of stress hyperglycemia ratio in patients with acute myocardial infarction: A systematic review with Bayesian and frequentist meta-analysis.

The present systematic review and meta-analysis aimed to investigate the prognostic value of stress hyperglycemia ratio (SHR) in patients with acute myocardial infarction (AMI). A total of 26 cohort studies, involving 87,974 patients, were analyzed. The frequentist meta-analysis showed that AMI patients with SHR in the upper quantile had a significantly higher hazard of major adverse cardiovascular and cerebrovascular events (MACCE, HR = 1.7; 95 % CI= [1.42, 2.03]; P < 0.001; I2 = 71 %; P <0.01), long-term (HR = 1.64; 95 % CI= [1.49, 1.8]; P < 0.001; I2 = 16 %; P = 0.29) and in-hospital all-ca

Kounis Syndrome in the Modern Era: A Comprehensive Review of Allergic Acute Coronary Syndromes.

Kounis syndrome (KS) is a largely underdiagnosed cause of acute coronary syndromes triggered by allergic or hypersensitivity reactions. The syndrome results from complex immune cell activation with the release of vasoactive and prothrombotic mediators leading to coronary vasospasm (type I KS) or thrombosis of plaque (type II KS), stent (type III KS) or coronary artery bypass graft (type IV KS). Despite increasing recognition over the past few decades, its pathophysiological mechanisms, diagnostic boundaries, and therapeutic implications remain incompletely understood.
