A Prospective Analysis of the Supine and Sitting Straight-Leg Raise Test and Its Performance in Litigation Patients.

BACKGROUND: The supine straight-leg raise (SLR) test has the potential to be exploited by malingering patients. The sitting SLR is believed to be less recognizable, therefore decreasing the chance of fabrication. The authors aim to compare the supine and sitting SLR test in patients with radiculopathy. METHODS: A total of 107 patients with radiculopathy were included in this study. Two groups were created: patients with workman's compensation (Group 1) and a control group of patients without litigation claims (Group 2). RESULTS: Mean age was 47.3 ± 1.3 years with a 51% female population. Cor

Dissociation and conversion symptoms in dermatology.

Dissociation and conversion (defined as the somatic component of dissociation) can play an important mediating role in the exacerbation of the stress-reactive dermatoses (eg, psoriasis, idiopathic urticaria, atopic dermatitis), dermatoses that are exacerbated by excessive scratching (eg, lichen simplex chronicus, prurigo nodularis) and koebnerization, and the self-induced dermatoses (dermatitis artefacta, acne excoriée, skin picking disorder, trichotillomania, onychotillomania/onychophagia). Dissociative symptoms often coexist with obsessive-compulsive symptoms in the more severe cases of the

Living with the unexplained: coping, distress, and depression among women with chronic fatigue syndrome and/or fibromyalgia compared to an autoimmune disorder.

Chronic fatigue syndrome (CFS) and fibromyalgia are disabling conditions without objective diagnostic tests, clear-cut treatments, or established etiologies. Those with the disorders are viewed suspiciously, and claims of malingering are common, thus promoting further distress. It was hypothesized in the current study that levels of unsupportive social interactions and the coping styles used among those with CFS/fibromyalgia would be associated with perceived distress and depressive symptoms. Women with CFS/fibromyalgia (n=39), in fact, reported higher depression scores, greater perceived dist

"Disturbing phenomenology" in the pain and engagement narratives of Cambodian American survivors of the Killing Fields.

In the clinical literature on trauma, the atrocity survivor's attempt to engage others around the experience of chronic, intractable pain is often viewed as an instance of "help-seeking," logotherapeutic "coherence-making," or-more darkly-"patient malingering." In this article, I challenge the utility of these rubrics through a close examination of the pain and engagement narratives of two survivors of the Cambodian Killing Fields. I demonstrate that survivor narratives can obtain a strategic multivocality, oscillating between phenomenological account and political critique, between clinical d

[Hysterical emergencies].

Most probably common in emergency departments, but still not well studied in this context, the disorders formerly called "hysteria" are now included in the group "Somatization, undifferentiated somatoform, conversion and dissociative disorders" (SSCD disorders) DSM IV. Their common presentation is that of idiopathic somatic symptoms linked with mental disorders. In the emergency department these symptoms confront physicians who generally do not have extensive psychiatric training. The symptoms occur, and disappear, undetermined by the patients, who are genuinely ill and not malingering. Aside

Treatment-refractory Psychosis With Catatonia and Persistent Generalized Retrograde Amnesia: A Case Report of a 41-year-old, Mandarin-speaking Male.

In this report, we describe an unknown patient who presented to the emergency department with severe catatonia and a constellation of symptoms indicative of a primary psychotic disorder. Simultaneously, the patient exhibited severe autobiographical memory impairment suggestive of a comorbid dissociative disorder. Our report offers insight into a rare instance in which catatonia, a primary psychotic disorder, and dissociative disorder may have intersected in a clinical case. While the patient was successfully treated with lorazepam for his catatonia, his psychotic symptoms responded only minima

Neuropsychological evaluation in primary care.

Referring a patient to a neuropsychologist for evaluation provides a level of rigorous assessment of brain function that often cannot be obtained in other ways. The neuropsychologist integrates information from the patient's medical history, laboratory tests, and imaging studies; an in-depth interview; collateral information from the family and other sources; and standardized assessment instruments to draw conclusions about diagnosis, prognosis, and response to therapy. Family physicians can use this information in the diagnosis and treatment of patients with depression, dementia, concussion,

Streff-Like Syndrome in an Adult: A Case Report.

Streff syndrome is a non-malingering visual disturbance commonly affecting near and color vision, which is prevalent in the younger population. This case report presents Streff-like syndrome in a middle-aged woman. A 47-year-old woman with underlying major depressive disorder presented with bilateral reduced vision and a constricted visual field for one week. These symptoms were accompanied by additional neurological complaints of headache, weakness, and numbness triggered after a stressful event that affected her work performance. Examinations revealed reduced vision more toward near, diminis

'You want to show you're a valuable employee': A critical discourse analysis of multi-perspective portrayals of employed women with fibromyalgia.

Background Advice on fibromyalgia, a chronic illness primarily affecting women, often presents it as incompatible with work and rarely covers how to remain employed. Yet many women do. Objectives We aimed to understand how these women, their family members, and workmates portrayed employees with fibromyalgia, and how these portrayals helped women retain employment. Methods We interviewed 22 participants, comprising five triads and three dyads of people who knew each other. Using the methodology of critical discourse analysis, we analysed the interview data within and across the triads/dyads th

Effects of head injury on olfaction and taste.

Traumatic events such as motor vehicle accidents, falls, or assaults can lead to dysfunction in olfaction or gustation. Mechanisms of posttraumatic olfactory dysfunction include direct injury to the sinonasal tract or olfactory epithelium, shearing effect on olfactory fibers at the cribriform plate, or brain contusion or intraparenchymal hemorrhage. Posttraumatic gustatory dysfunction is rare, but may occur as a result of direct injury to the tongue, injury to cranial nerves VII or IX, or brain contusion or hemorrhage. Evaluation of head-injured patients presenting with olfactory or gustatory

A concise overview of using the P300 component of the event-related potential for objective estimation of visual acuity.

Visual acuity (VA) assessment traditionally relies on patient cooperation, which can be unreliable in pediatric cases, cognitive impairment, or suspected malingering. While the visual evoked potential (VEP) provides an objective alternative, it primarily reflects the integrity of the early visual pathways and may overestimate VA in conditions like amblyopia. This concise review explores the P300 event-related potential as a promising cognitively-driven tool for objective VA estimation. A literature search was conducted to identify studies utilizing the P300-a marker of high-level cognitive pro

Reaction time as a performance validity measure in older adults with and without early-stage Alzheimer's disease.

INTRODUCTION: Performance validity assessment is important in evaluating older adults with and without cognitive impairment. Performance validity tests (PVTs) such as the Test of Memory Malingering (TOMM) may inaccurately classify Alzheimer's disease (AD) patients as testing invalid despite genuine effort. We explored an alternative PVT using reaction time (RT). METHODS: Healthy older controls (OCs; N = 20) and AD patients (N = 20) completed an auditory oddball task and the TOMM. Over two counterbalanced sessions, OCs performed honestly and simulated dementia; AD patients completed one honest

Factitious Disorder Masquerading as a Life-Threatening Anaphylaxis.

Factitious disorder is a psychiatric disorder in which sufferers intentionally fabricate physical or psychological symptoms in order to assume the role of the patient, without any obvious gain. We present a case of a 23-year-old female with chronic urticaria who presented with dyspnea, dysphasia, mild generalized erythema, abdominal cramps, and headache. She was tachypneic and hypotensive. This was her third admission with similar symptoms within the last 7 months. Tryptase, complement, anti-SM/RNP, Sjogren, Scl-70, C3, and C4 were negative. Computed tomography-guided bone marrow biopsy showed

Is it Trauma- or Fantasy-based? Comparing dissociative identity disorder, post-traumatic stress disorder, simulators, and controls.

OBJECTIVE: The Trauma Model of dissociative identity disorder (DID) posits that DID is etiologically related to chronic neglect and physical and/or sexual abuse in childhood. In contrast, the Fantasy Model posits that DID can be simulated and is mediated by high suggestibility, fantasy proneness, and sociocultural influences. To date, these two models have not been jointly tested in individuals with DID in an empirical manner. METHOD: This study included matched groups [patients (n = 33) and controls (n = 32)] that were compared on psychological Trauma and Fantasy measures: diagnosed genuine

Chronic dizziness: the interface between psychiatry and neuro-otology.

PURPOSE OF REVIEW: This paper reviews the often-enigmatic relationships between dizziness and psychiatric symptoms. Psychiatric causes of dizziness, neuro-otologic causes of anxiety, underrecognized co-morbid conditions, and medical illnesses that masquerade as 'psychogenic' dizziness are examined. Key clinical features and data from recent treatment trials are presented with potential pathophysiologic mechanisms. RECENT FINDINGS: Investigations at the interface between psychiatry and neuro-otology have identified the distinguishing features of several clinical conditions that present with no

Can we solve the mysteries of the National Vietnam Veterans Readjustment Study?

The National Vietnam Veterans Readjustment Study (NVVRS) researchers reported that 30.9% of all men who served in that conflict developed posttraumatic stress disorder (PTSD) even though only about 15% had been assigned to combat units. Scholars, mainly historians, have questioned the accuracy of the PTSD prevalence rate. The purpose of this article is to evaluate the merits of several hypotheses adduced to explain the high apparent PTSD prevalence in the NVVRS. Empirical and conceptual analysis suggests that malingering is unlikely to account for many cases. Also, deployment to Vietnam in the

Within-Person Score Distribution Measures as Performance Validity Indicators in Service Members and Veterans with and without History of Mild Traumatic Brain Injury.

OBJECTIVE: This study examined within-person score distribution measures (WPSD) as a performance validity indicator using the overall test battery mean (OTBM), within-battery standard deviation (SD-B), coefficient of variation (CV), number of abnormal scores (ABN), and several other measures. The primary aim was comparison of classification accuracy of various WPSD measures. METHOD: The study involved secondary analysis of de-identified data originally collected in the Chronic Effects of Neurotrauma Consortium Study 1. Participants (N = 1431) were categorized into valid and questionable perfo

Symptom attribution is a stronger predictor of PVT-failure than symptom endorsement in treatment-seeking Veterans with remote mTBI history: A pilot study.

OBJECTIVE: To examine relationships between performance validity testing (PVT), neurobehavioral symptom endorsement, and symptom attribution in Veterans with a history of mild traumatic brain injury (mTBI). METHOD: Participants included treatment-seeking Veterans (n = 37) with remote mTBI histories who underwent a neuropsychological assessment and completed a modified version of the Neurobehavioral Symptom Inventory (NSI) to assess symptom endorsement and symptom attribution (the latter evaluated by having Veterans indicate whether they believed each NSI symptom was caused by their mTBI).
