Cortisol and insulin behaviors during an ultramarathon event: are they real markers of extreme exertion?

BACKGROUND: The current work aimed to describe and compare the cortisol and insulin concentrations behavior and rate of perceived exertion (RPE) during a 115 km ultramarathon race. METHODS: Nine ultrarunners (eight males) were evaluated six times (0, 37, 60, 76, 89 and 115 km). At each moment, saliva samples (for cortisol and insulin assessment) and RPE (CR10 scale) were collected. Statistical analysis included correlation, one-way repeated measure ANOVA, and Statistical Parametric Mapping to define discrete and continues changes and compare cortisol, insulin and RPE profiles. RESULTS: Our m

Data driven shoe design improves running economy beyond state-of-the-art Advanced Footwear Technology running shoes.

BACKGROUND: Advanced Footwear Technology (AFT) has enabled remarkable improvements in running performance over traditional marathon racing shoes. However, reported differences between state-of-the-art AFT models are small, and vary across individuals. To assess if the benefits of AFT have been fully realized or if further running economy improvements can be unlocked using modern computational design and optimization techniques, we compared a prototype AFT shoe developed using a data-driven computational design process (PUMA Fast-R 3 (FR3)) against its traditionally developed predecessor model

Resilience against exercise-related coronary atherosclerosis: A case study in a master athlete participating in 500 marathons.

BACKGROUND AND PURPOSE: Lifelong endurance exercise is generally associated with cardiovascular health benefits. However, recent studies suggest that prolonged high-volume training may contribute to coronary atherosclerosis, even in athletes with low traditional cardiovascular risk. This case report aims to explore the cardiovascular status of a master endurance athlete with an exceptionally high lifetime training volume, in light of recent concerns raised in the literature. METHODS: We present the case of a 60-year-old recreational male marathon runner with no history of cardiovascular disea

Test-Retest Reliability of Running Economy and Other Physiological Parameters During 90 min of Running in Well-Trained Male Endurance Runners.

Running economy (RE) is highly reliable when measured in an unfatigued state; however, its reproducibility during prolonged exercise has not been investigated. RE is known to worsen during prolonged exercise (referred to as RE "durability"), and quantifying the reliability of RE durability measurements will determine our sensitivity to detect subtle differences between groups or conditions. This study aimed to assess the reliability of RE and other physiological parameters throughout a 90-min run in the heavy-intensity domain among well-trained runners. Fourteen male marathon runners (maximal

Cardiovascular magnetic resonance with parametric mapping in long-term ultra-marathon runners.

PURPOSE: There is a direct reverse dose-effect relationship between the amount of physical activity and cardiovascular risk. It is unknown whether this is true for extreme, persistent endurance training. The aim of the study was to assess structural changes of the heart in long-time ultra-marathon runners with special focus on myocardial fibrosis using parametric mapping. METHOD: We studied a group of 30 healthy, male ultra-marathon runners (mean age 40.9 ± 6.6 yrs, median 9 yrs of running with frequent competitions) and 10 matched controls not engaged in any regular activities. All of them u

Win Your Race Goal: A Generalized Approach to Prediction of Running Performance.

We introduce a novel approach for predicting running performance, designed to apply across a wide range of race distances (from marathons to ultras), elevation gains, and runner types (front-pack to back of the pack). To achieve this, the entire running logs of 15 runners, encompassing a total of 15,686 runs, were analyzed using two approaches: (1) regression and (2) time series regression (TSR). First, the prediction accuracy of a long short-term memory (LSTM) network was compared using both approaches. The regression approach demonstrated superior performance, achieving an accuracy of 89.13%

Total Energy Expenditure and Intake During a 161-km Mountain Ultramarathon.

UNLABELLED: High total energy expenditure (TEE) and rate of water turnover (rH2O) using doubly labeled water (2H218O) have been reported during single-stage and multistage ultraendurance events. At the 161-km distance, less is known about the interaction of TEE, rH2O, and ad libitum energy/fluid intake. This is especially true during the postrace recovery window. PURPOSE: To measure TEE, rH2O, and ad libitum energy/fluid intake during a 161-km ultramarathon while extending the TEE and rH2O measurement window to a 7-day postrace period. METHODS: Doubly labeled water was used to monitor TEE an

Reversible ischemic colitis in a high endurance athlete.

Ischemic colitis is a rare but serious consequence of long distance running. Herein we report a well documented case of severe ischemic colitis in an elite class marathon athlete in whom an extensive diagnostic evaluation, including magnetic resonance angiography and hypercoagulation profile, was performed. Our results suggest that physiologic shunting due to splanchnic vasoconstriction, intravascular volume depletion due to chronic dehydration, and possibly some element of hypercoaguability due to secondary polycythemia may have been causative factors. The patient's full recovery, both clinic

Running Performance Decline in IRONMAN-Distance Triathlon: A Scoping Review of Existing Evidence and Research Gaps.

In an IRONMAN-distance triathlon, run performance is a particularly strong correlate of overall race outcome, alongside cycling. Although individual research areas such as pacing, performance prediction, and laboratory physiology have been reviewed separately, they have not been integrated to determine factors contributing to running performance decline or to identify key knowledge gaps. This scoping review aims to map the current evidence on running performance decline in IRONMAN-distance triathlon and identify key gaps, with a particular focus on age-group athletes. Following the Arksey and

Characteristics of sudden cardiac arrest during endurance racing: a decade of the Paris registry.

AIMS: The rising popularity of endurance races underscores the need to explore the risks of sports-related sudden cardiac arrest (Sr-SCA). Although rare, Sr-SCA is significantly more prevalent in men than in women. The mechanisms underlying these sex differences remain unclear. We aimed to investigate the incidence rates, clinical characteristics, aetiologies, sex differences, and exercise performances among SCA cases during major endurance races in Paris over a 10-year period. METHODS AND RESULTS: We Analysed the Paris Sudden Death Expertise Centre Registry Data (Covering 2011-2024, excluding

Nutritional Dimensions of Sports Tourism: Runners' Encounters with Polish Local Food Cultures.

Background/Objectives: Although nutrition is widely recognized as a key factor in post-event recovery in sports, little attention has been given to how its cultural and social dimensions-embodied in local cuisine-intersect with the needs of traveling athletes, for whom food often also serves as a medium of cultural immersion and sensory exploration. Poland, with its rich regional culinary traditions and numerous international running events, offers a compelling context in which to explore these interactions. This study aims to understand the role of local cuisine in the experiences of foreign

Contribution of medical resonance imaging in the return to physical activity after exertional rhabdomyolysis a case report.

INTRODUCTION: Exertional rhabdomyolysis (ER) is the breakdown of muscle cells that occurs after intensive physical activity. Although a cautious and gradual return to physical activity (RTPA) is recommended after an episode of ER, physical sequelae are reported long afterwards. CASE REPORT: We present the case of a 31-year-old untrained amateur runner with no prior medical history who developed heat stroke, ER, and acute kidney failure on the occasion of a half-marathon. Three months later, whereas the patient was symptom-free and CK levels were normal, an MRI revealed typical signs of rhabdo

Correlation of Pre-Race Anxiety Using the Generalized Anxiety Disorder 2-Item Scale.

An estimated 40 million adults in the United States have been diagnosed with an anxiety disorder, making it the most common psychiatric disorder in the country. Although the data are conflicting and limited, engaging in or increasing exercise has been proposed for the management of anxiety and other mental health disorders. The purpose of this study was to determine if there is a correlation between pre-race anxiety and running experience, sex, body mass index, age, and mental health history using the validated Generalized Anxiety Disorder 2-Item screening tool for anxiety. This study was a pr

Mechanisms, Economy, and Performance of Advanced Footwear Technology in Endurance Running-A Review.

Since the introduction of advanced footwear technology (AFT) in 2017, numerous world records from 5 km to the marathon have been broken. Among these innovations, carbon-plated shoes have received particular attention. Previous research indicates improvements of 2-4% in running economy (RE), which translates into an approximate 1-2% improvement in running performance when running in these shoes. The rapid progression of performance has generated significant scientific interest; however, a clear understanding of the mechanisms driving the effectiveness of AFT remains limited. Despite widespread

The New York City Marathon: A systematic review of performance, participation, pacing, and health-related outcomes.

The 'New York City Marathon' is one of the world's largest and most influential mass-participation marathons. Although numerous studies have examined performance trends, participation patterns, pacing behavior, environmental influences, and physiological aspects of runners in this event, no review has synthesized the evidence specific to this race. This study aimed to systematically summarize the scientific literature on the 'New York City Marathon'. A systematic search of Scopus, PubMed, Web of Science, Embase, and Cochrane was conducted using terms related to the 'New York City Marathon' to

Aborted sudden cardiac death associated with an anomalous right coronary artery.

Coronary artery anomalies arising from the opposite sinus of Valsalva and having an interarterial course between the aorta (AO) and pulmonary artery (PA) are the second most common cause of sudden cardiac death among young athletes, after hypertrophic cardiomyopathy. The right coronary artery (RCA) originating from the AO above the left sinus of Valsalva (LSV) is an extremely rare anomaly. We report the first case of a RCA arising from the AO above the LSV that subsequently runs between the AO and the PA, discovered by a 64-slice multidetector coronary CT, in a patient who was successfully res

Hypertrophy of the heart; electrocardiographic distinction between physiologic and pathologic enlargement.

Electrocardiograms of marathon runners were examined to study hypertrophy of the heart due to prolonged physical exertion and to differentiate this from hypertrophy due to various disease states, especially essential hypertension, aortic valvular disease and coarctation of the aorta. The electrocardiogram of the marathon runners was characterized by a slow cardiac rate, high voltage of the QRS complexes and T waves in the standard and/or precordial leads with normal R/T ratios. There was moderate enlargement of the heart as observed on teleoroentgenogram. These findings are characteristic of p

Spontaneous oesophageal perforation secondary to renal colic.

Boerhaave syndrome is a rare but potentially life-threatening condition that involves a full-thickness tear of the oesophagus. It accounts for around 15% of all cases of oesophageal perforations and is associated with up to 40% of mortality. Vomiting has been found to be associated with the development of Boerhaave syndrome. However, the aetiology of vomiting varies broadly in the available literatures from alcohol indulgence to marathon running, and from panic attack to radiotherapy for cancer. We present here an unusual case of Boerhaave syndrome where the patient developed spontaneous oesop
