Possible role of anastrozole-induced hormonal alterations in pathogenesis of mammary apocrine carcinoma and follicular lymphoma: a case report and review of the literature.

BACKGROUND: In postmenopausal women, aromatase inhibitors decrease estrogen levels and increase local dihydrotestosterone concentrations. In this case report, we describe interesting associations between aromatase-inhibitor-induced hormonal changes and the development of apocrine mammary carcinoma and follicular lymphoma. CASE PRESENTATION: Here we report an 83-year-old Caucasian female patient who initially presented with Paget's disease of the right nipple and associated small focus of invasive ductal carcinoma (ERα + PR + HER2-). The patient did not pursue surgical resection and was treate

Diagnosis of Extensive DCIS with Progressive Microcalcifications over a Period of 11 Years.

A 53-year-old woman presented complaining of enlargement of her right breast; 12 years earlier she was treated elsewhere for an invasive ductal carcinoma of the right breast with breast-conserving surgery, adjuvant chemotherapy, anti-HER2 therapy, radiotherapy and endocrine therapy. One year postoperatively, a group of microcalcifications was seen in the ipsilateral breast on mammography. In the following years, the area of microcalcifications progressively increased, but instead of vacuum-assisted biopsy, the treating physicians performed core needle biopsies that were negative for malignancy

Gynecomastia: the horizontal ellipse method for its correction.

BACKGROUND: Gynecomastia is an extremely disturbing deformity affecting males, especially when it occurs in young subjects. Such subjects generally have no hormonal anomalies and thus either liposuction or surgical intervention, depending on the type and consistency of the breast, is required for treatment. If there is slight hypertrophy alone with no ptosis, then subcutaneous mastectomy is usually sufficient. However, when hypertrophy and/or ptosis are present, then corrective surgery on the skin and breast is mandatory to obtain a good cosmetic result. METHODS: Most of the procedures sugges

Tuberculosis of the breast masquerading as carcinoma: a study of 100 patients.

One hundred patients with tuberculous mastitis were referred to the Tata Memorial Hospital, a cancer center, with a clinical diagnosis of malignancy. This study identifies the possible causes of misdiagnosis and reviews the management of these patients. A lump in the breast with or without ulceration was the commonest presentation, the others being diffuse nodularity and multiple sinuses. Concomitant axillary lymph nodes were found in one-third of the patients. Tuberculosis lesions such as nodular mastitis, disseminated mastitis, and sclerosing lesions clinically mimicked a fibroadenoma, carci

Surgeon-delivered non-ultrasound-guided TAP blocks enhance recovery in DIEP flap breast reconstruction: A prospective comparative cohort study.

BACKGROUND: Donor-site pain remains a major barrier to early recovery after deep inferior epigastric perforator (DIEP) flap breast reconstruction, despite advances in enhanced recovery after surgery (ERAS) pathways. Ultrasound-guided transversus abdominis plane (TAP) blocks delivered by anaesthetists perioperatively have shown benefit, but the role of a simple, surgeon-delivered, non-ultrasound guided TAP block during DIEP surgery, has not been systematically evaluated. METHODS: A prospective comparative cohort study was conducted by including 88 patients undergoing immediate DIEP flap recons

Case Report: Metachronous bilateral primary and secondary angiosarcoma of the breast.

BACKGROUND: Breast angiosarcoma (AS) is an extremely rare and aggressive malignancy with no standardized treatment guidelines. We report an exceptional case of metachronous bilateral primary and secondary AS in a single patient, highlighting the diagnostic and therapeutic challenges posed by this disease. CASE PRESENTATION: A patient underwent surgical dissection for primary AS of the right breast. Seven months later, a primary AS was identified in the left breast. Four months after that, a secondary angiosarcoma also developed within the previously irradiated left chest wall and axilla. Post

Radiation-induced angiosarcoma: case report.

Angiosarcoma of the breast accounts for less than 1% of breast tumors. This tumor may be primary or secondary to previous radiation therapy and it is also named "radiogenic angiosarcoma of the breast", which is still a rare entity with a poor prognosis. So far, there are only 307 cases reported about these tumors in the literature. We present a case of a 73-year-old woman with a prior history of breast-conserving treatment of right breast cancer, exhibiting mild pinkish skin changes in the ipsilateral breast. Her mammography was consistent with benign alterations (BI-RADS 2). On incisional bio

Malignant Phyllodes tumor in a young female: A Case Report.

Phyllodes tumor is a rare fibroepithelial neoplasm of the breast and constitutes 0.3-0.5% of all breast tumors. They are usually benign and only few of them undergo malignant transformation. Benign or borderline phyllodes tumor typically never recur however, malignant phyllodes tumors do have a recurrence and a metastatic potential. Differentiating these two groups is of prime importance for proper management of the patient. We report a rare case of Malignant Phyllodes tumor in an 18 year old female. She presented with a left breast lump which was gradually increasing in size. There was no sig

Adrenal metastasis of a phyllodes tumor of the breast: Case report and review of the literature.

INTRODUCTION: A phyllodes tumor is a neoplasm of mixed mesenchymal and epithelial origin affecting the breast. It may pursue a benign or malignant evolution with distant metastases in the latter case. Sites most commonly affected by metastases are the lungs and bones. Simple mastectomy is the mainstay of treatment. This article presents the first described case of metastasis to the adrenal gland after sarcomatous transformation of a phyllodes tumor. A review of the literature is presented afterwards. PRESENTATION OF CASE: A 57-year old female patient presented with a voluminous breast mass wh

Multiple intraductal papillomas of breast clinically masquerading as malignancy.

BACKGROUND: Intraductal papilloma is characterized by proliferation of epithelial and myoepithelial cells overlying fibro-vascular stalks creating an arborescent structure within the lumen of duct. Some times multiple papillomas with florid proliferation of epithelium may be confused with malignancy. A case of multiple intraductal papillomas of breast with ulceration of overlying skin and large lump leading to clinical diagnosis of malignancy is documented here. CASE REPORT: A 45-year-old female presented with ulcerated mass of six months duration in the left breast. On examination, a firm, i

Breast reconstruction by local flaps after conserving surgery for breast cancer: an added asset to oncoplastic techniques.

This study describes and evaluates the results of a proposed simple technique of volume replacement by local flaps to reconstruct the breast after conserving surgery for breast cancer. Twenty-five patients with breast cancer were enrolled in the study between 1998 and 2004. All patients were surgically treated by wide local excision and axillary dissection of level I & II. The primary closure was not feasible because of resulting large defect in proportion to the breast size. The defect was constructed by local flap raised from adjacent skin and subcutaneous tissue with or without glandular br

Case Report: Primary breast leiomyosarcoma in an 84-year-old male.

Primary breast leiomyosarcoma is an extremely rare malignancy originating from mesenchymal tissue. Fewer than 10 male cases have been reported globally. This paper reports an 84-year-old male patient. This represents the oldest reported case in the current literature. A painless, slowly enlarging mass was present in his right breast. The mass had a 10-year history. This contrasts sharply with the typically rapidly progressive pattern documented in previous literature. Clinical examination revealed a mobile mass measuring 12 cm × 10 cm in the right breast. No lymphadenopathy was detected. Ultra

Primary Small Cell Neuroendocrine Carcinoma of the Breast in a 75-Year-Old Woman With Aggressive Recurrence Despite Multimodal Therapy: A Case Report and Literature Review.

BACKGROUND Neuroendocrine breast tumors represent an uncommon subtype of breast cancer, accounting for less than 1% of all breast malignancies and fewer than 0.1% of all neuroendocrine tumors. Their histological and immunophenotypic features closely resemble neuroendocrine neoplasms of other organs, and diagnosis requires careful exclusion of extramammary primaries. CASE REPORT We present the case of a 75-year-old woman diagnosed with primary small cell neuroendocrine breast carcinoma following an incidental detection of a right breast mass. She underwent a simple mastectomy with sentinel lymp

Immediate breast reconstruction for breast carcinoma using the periareolar approach.

Skin-sparing mastectomy with immediate breast reconstruction has shown to be oncologically safe while providing dependable aesthetic results. However, flap inset into the skin defect of the excised biopsy site and nipple-areola complex often results in a patchlike effect and transverse scars. By keeping the mastectomy incision solely around the areola, all breast skin can be preserved. Thus, in immediate breast reconstruction with replacement of the nipple and areola by a small skin island from a deepithelialized TRAM flap or latissimus dorsi muscle flap, the scar is kept at the natural border

[Metastatic breast melanoma in a man: Case report].

BACKGROUND: Malignant melanoma is a skin neoplasm that typically does not invade the breast, making its spread to the breast tissue in a male patient uncommon. When it metastasizes to the breast, it can present as a palpable mass or even be asymptomatic. Ultrasound allows for the detection of round hypoechoic lesions, while mammography detects well-defined nodules with increased density. The aim of this article is to describe the approach to a male patient with a breast mass and suspected melanoma, as well as the importance of immunohistochemistry for proper differentiation. Cystosarcoma phyllodes: epidemiology, pathohistology, pathobiology, diagnosis, therapy, and survival.

Cystosarcoma phyllodes is a very rare tumor which maybe difficult to diagnose clinically. The epidemiology and pathobiology are different from those of breast carcinoma. Risk factors, multicentricity, bilaterality, as associated with breast carcinoma, are not observed in patients with cystosarcoma phyllodes. Although the term "sarcoma" indicates a malignant tumor, only 10%-30% of cystosarcomas are histologically diagnosed as malignant; clinical diagnosis of malignancy does not exceed 10%. Axillary node involvement is rare, but hematogenous spread of cystosarcoma occurs into lung, pleura, bone,

Measuring upper limb function and patient reported outcomes after major breast cancer surgery: a pilot study in an Asian cohort.

BACKGROUND: Breast cancer is the most common cancer in women worldwide. Major breast cancer surgery especially with axillary lymph node dissection (ALND), is associated with upper limb functional decline. Majority of studies are conducted in Western population and may not be applicable to Asians. This pilot study aims to evaluate whether major breast surgery results in upper limb functional impairment in a cohort of Asian women with breast cancer. METHODS: This is a prospective cohort study of 41 patients who underwent 44 major breast surgeries from April 2018 to August 2019. Main inclusion c

Lymphaticovenular Anastomosis for Lymphorrhea after Expander-based Breast Reconstruction: A Case Report.

Refractory axillary lymphorrhea is a postoperative complication of breast cancer with no established standard treatment. Recently, lymphaticovenular anastomosis (LVA) was used to treat not only lymphedema but also lymphorrhea and lymphocele in the inguinal and pelvic regions. However, only a few reports have been published on the treatment of axillary lymphatic leakage with LVA. This report presents a case of successful treatment of refractory axillary lymphorrhea after breast cancer surgery with LVA. A 68-year-old woman underwent nipple-sparing mastectomy for right breast cancer, axillary lym

Basosquamous Cell Carcinoma of the Nipple-Areola Complex-Report of a Case.

Basosquamous cell carcinoma (BSCC) is a rare malignancy usually arising on sun-exposed areas of the skin. BSCC is described as a rare variant of Basal cell carcinoma (BCC) which shows clinical and microscopic features of both BCC and of Squamous cell carcinoma (SCC). We report the case of a 70-year-old male with a cutaneous lesion of the nipple-areola complex (NAC); to the best of our knowledge, this is the first ever reported patient with BSCC in this area. The lesion had a fast growth, but, due to the COVID19 crisis, the patient only came to our observation one year after onset of this condi
