Determination of pelvic limb alignment in the large-breed dog: a cadaveric radiographic study in the frontal plane.

OBJECTIVES: To report a method for radiographic determination of the mechanical axis of the canine pelvic limb and its relationship to the joints and bone axes. To report reference ranges for the relationships between the axes of the pelvic limb and for joint position relative to the pelvic limb axis. STUDY DESIGN: Cadaveric radiographic anatomic study. ANIMALS: Pelvic limbs (n=101) from normal midsized to large breed dogs; tibiae (n=105) from dogs with cranial cruciate ligament disease (previous study). METHODS: Extended full-limb radiography was performed and images analyzed to determine:

An Update on Pediatric Flatfoot.

The pediatric flatfoot can include multiple planes of deformity and concomitant concerns such as metatarsus adductus and equinus. Each aspect of the deformity must be carefully evaluated before any surgical planning. The goal of surgery should be an improvement in symptoms by creating a controllable foot with a reduction of deforming forces. There are multiple procedures that can be used for the pediatric flatfoot, including the Evans calcaneal osteotomy, the Cotton medial cuneiform osteotomy, the medial calcaneal slide osteotomy, and arthroereisis implants. Each contributes in a specific way

IMAGING DIAGNOSIS--RADIOGRAPHIC, ULTRASONOGRAPHIC, COMPUTED TOMOGRAPHIC, AND FLUOROSCOPIC APPEARANCE OF A DISTAL PELVIC LIMB ARTERIOVENOUS MALFORMATION IN A YOUNG GERMAN SHEPHERD DOG.

A German shepherd puppy presented for evaluation of a suspected arteriovenous fistula on the distal aspect of the right pelvic limb. Radiographs demonstrated expansion and resorption of the tarsal and metatarsal bones, and ultrasound detected a vascular abnormality. Using computed tomographic angiography, a complex arteriovenous malformation (AVM) involving the distal tibia, tarsus, and the metatarsus and an osteochondritis dissecans (OCD) lesion of the talus were identified. Based on these findings, therapeutic limb amputation was performed. Fluoroscopic angiography, vascular casting, and dis

Percutaneous sagittal plane closing wedge osteotomy of the first metatarsal.

Osteotomy of the first metatarsal in the sagittal plane is useful in correction of numerous deformity of the foot. Plantarflexion osteotomy of the first metatarsal can be used to treat hallux rigidus, hallux limitus, forefoot varus in flatfoot deformity and iatrogenic metatarsus primus elevates. Dorsiflexion osteotomy of the first metatarsal is an important component in surgical correction of pes cavus. It is also indicated in recalcitrant diabetic neuropathic ulcers at the first metatarsal head. We described a minimally invasive technique of sagittal plane corrective osteotomy of the first me

Mesomelic shortening of the upper extremities with spur formation and cutaneous dimpling.

We report a boy with mesomelic dysplasia of the forearms, skin dimples, mildly bowed tibiae, metatarsus adductus, congenital cataracts, sensorineural hearing loss, hypotonia, and mildly dysmorphic features. Prominent radiologic findings include bony spurs of the diaphyses of the radii bilaterally with angulated, significantly shortened radii and ulnae and elbow dislocations. Changes in the lower extremities are confined to bowing of the tibiae and elongation of the thinned fibulae. To our knowledge this case represents a unique combination of findings not previously reported, although some res

Painless stress fractures in diabetic neuropathic feet.

We describe two patients with diabetes mellitus and associated neuropathy, who presented with painless foot swelling and no history of trauma. X-Rays revealed recent underlying fractures-in one of a metatarsus, and the other of a proximal phalanx. These were assumed to be 'stress' fractures unassociated with pain because of the severe sensory neuropathy. Though spontaneous fractures in neuropathic feet have been previously described, they almost always occur in association with Charcot joints, and are usually painful. The differential diagnosis of acute swelling in the foot of a diabetic patie

Parent satisfaction comparing two bandage materials used during serial casting in infants.

Plaster of Paris was compared with semirigid fiberglass casting material during serial casting in 17 infants with clubfoot or rigid metatarsus adductus. Semirigid fiberglass was statistically superior in its durability, convenience, performance, and ease of removal. The average amount of time for home cast removal by the parents was 55 minutes for plaster of Paris and 21 minutes for semirigid fiberglass. Complications such as skin abrasions and cast slip-off were similar for both casting materials. Ninety-four percent of parents strongly preferred semirigid fiberglass rather than plaster of Pa

Basal osteotomy of the fifth metatarsal with intermetatarsal pinning: a new approach to tailor's bunion.

The direct approach to the fifth metatarsophalangeal joint for the treatment of metatarsus quintus valgus often has been unsuccessful, mainly because of technical difficulties in the osteotomy of the narrow fifth metatarsal neck and the fixation of the unstable head fragment. Complications such as delayed union, nonunion, or recurrence have been reported. This paper describes a surgical technique for optimal correction of this deformity. The technique has a biomechanically sound fixation and a shortened bone healing time with minimum complications. Results of 12 patients presenting severe defo

The plantar proximal cortex of the third metatarsal bone shows raised longitudinal ridges at the suspensory ligament enthesis in normal equine isolated limbs - a radiographic, computed tomography, and MRI study.

INTRODUCTION: Knowledge of normal radiographic appearance is essential to avoid misinterpretation of radiographs. This study aimed to assess the computed tomographic (CT) appearance of the plantar surface of the proximal metatarsus and evaluate the influence of the radiographic angle on the trabecular/cortical interface of the proximal plantar metatarsal cortex on lateromedial and slightly oblique radiographs. METHODS: Eight hindlimbs were collected from six horses with no known history of lameness and euthanized for reasons unrelated to the study. Limbs underwent computed tomographic (CT) an

The obliquity of the first metatarsal base.

BACKGROUND: The degree of obliquity of the base of the first metatarsal is thought to predispose to metatarsus primus varus deformity. Radiographs currently are used to interpret the obliquity of the proximal first metatarsal but are subject to error. A 'normal' obliquity of the first metatarsal base has not yet been established. METHODS: Direct measurements of 77 human first metatarsals were used to establish mean values for the proximal first metatarsal obliquity angle with respect to age, gender, and ethnicity in this sample population. The overall length of the first metatarsal and the wi

[Hallux valgus--an atavism?].

In biology, atavism is generally understood as a biological phenomenon which brings to sight a recurrent phenotypic character which was lost for extinct generations. Phylogenic evolution of homo sapiens demonstrates today that the first plantar ray in tetrapods has always been a very stable ray and the lateral foot experienced a progressive adduction to the talo-metatarsal axis, together with a remodeling of the lateral column of the foot and a progressive valgus of the calcaneus beneath the talus. The foot appears as a strongly modified extremity, starting at a prehensile foot organ, common t

Stress fracture of the fourth metatarsal in a relapsed clubfoot of a 5.5-year-old child.

Stress fractures are uncommon in skeletally immature patients and usually affect adolescents involved in competitive sports, whereas they are very rare in young children. Involvement of the fourth metatarsal is very infrequent and has been associated with metatarsus adductus deformities. The authors describe a stress fracture of the proximal fourth metatarsal occurred in a 5.5-year-old child with a relapsed clubfoot. Operative correction of the deformity and cast immobilization provided consolidation of the fracture. Stress fractures of lateral metatarsals may represent a possible source of pa

Hallux rigidus: etiology, biomechanics, and nonoperative treatment.

Hallux rigidus is a degenerative osteoarthritic process characterized by progressive loss of metatarsophalangeal joint range of motion and notable dorsal or periarticular osteophyte formation. Documented factors associated with hallux rigidus are a flat or chevron-shaped joint, hallux valgus interphalangeus, metatarsus adductus, bilaterality in persons with a positive family history, trauma history in unilateral cases, and female gender. Elevation of the first ray noted radiographically is thought to be a sign of worsening metatarsophalangeal joint function. Nonoperative care is aimed at impro

Tibial spastic varus foot caused by os calcaneus secundarius: A case report.

Peroneal spastic flatfoot caused by tarsal coalition is well known; however, tibial spastic varus foot is a rare clinical entity also caused by tarsal coalition in most cases. The os calcaneus secundarius is a rare accessory bone between the anterior process of the calcaneus and the navicular bone. The case of a 29-year-old woman with tibial spastic varus foot caused by os calcaneus secundarius is presented. Operative excision of the os calcaneus secundarius completely resolved the varus deformity. This is the first case report involving tibial spastic varus foot caused by os calcaneus secunda

A case of polydactyly in the hind-limbs of a West African Dwarf goat in South-West Nigeria.

This report describes a case of polydactyly in the hind-limb of a West African Dwarf goat kid in South West Africa. Physical examination revealed the presence of four digits in each of the hind limbs. Radiological examination and macerated bones of the animal showed a bifid shape of each metatarsal that was more prominent from the distal half of the diaphysis. This resulted in the presence of four articulating surfaces per limb at the distal extremity. Though this condition is rare in goats, we advise that continuous reporting by researchers can give a better prevalence statistics of these occ

Brachymetatarsia with brachymesodactyly.

A case of brachymetatarsia with congenital absence of all middle phalanges was presented. This is believed to be the first such case reported. Many surgical options are available for the correction of microdactyly. Satisfactory surgical results were achieved via bunionectomy, the implantation of a double-stemmed prosthetic joint in the fourth metatarsophalangeal joint and osteotomy of the second metatarsal bone.

The anterior heel for metatarsalgia in the adult foot.

A conservative approach to foot problems is especially useful in patients with diabetes, rheumatoid arthritis, diminished circulatory changes, and those who are too debilitated for surgical treatment. If one will start with either the medial heel wedge or the anterior heel correction, some response should be evident within 4 to 6 weeks. If the response after a trial period of approximately two to three months for a heel wedge or another two to three months for the anterior heel doesn't relieve pain, then perhaps some other problem might now become easier to localize, i.e. Morton's toe syndrome
