Mental health of UK Armed Forces medical personnel post-deployment.

BACKGROUND: The mental health effects of deployment vary widely, and personnel in both combat and combat support roles, including medical personnel, may be adversely affected. AIMS: To compare the mental health of deployed UK military medical staff in both forward and rear locations and to compare these two groups with other deployed military personnel. METHODS: Participants were medics who had deployed to Iraq or Afghanistan and provided information about their deployed role, experiences during and on return from deployment and demographic and military factors. Health outcomes included comm

Child maltreatment in enlisted soldiers' families during combat-related deployments.

CONTEXT: Parental stress is believed to play a critical role in child maltreatment, and deployment is often stressful for military families. OBJECTIVE: To examine the association between combat-related deployment and rates of child maltreatment in families of enlisted soldiers in the US Army who had 1 or more substantiated reports of child maltreatment. DESIGN AND SETTING: Descriptive case series of substantiated incidents of parental child maltreatment in 1771 families of enlisted US Army soldiers who experienced at least 1 combat deployment between September 2001 and December 2004. U.S. Army combat operational stress control throughout the deployment cycle: a case study.

As military conflicts around the world persist, a comprehensive approach in managing behavioral health issues will continue to be a key component of military healthcare. Deployed military personnel frequently exposed to trauma are well-known to be at high risk for developing behavioral health disorders, including combat stress reactions and posttraumatic stress disorder. In the U.S. Army, members of combat operational stress control (COSC) units have unique skills to assist soldiers and their families not only throughout all phases of a deployment, but also throughout a soldier's entire career

The past and present threat of rickettsial diseases to military medicine and international public health.

Morbidity and mortality caused by rickettsioses have had a major influence on military activities and public health for >2000 years. The threat posed by the rickettsioses is reviewed, focusing on the impact and epidemiology of those that have adversely influenced wartime operations and the current challenges posed by these diseases. With their uneven worldwide distribution, the discovery of drug-refractory strains of Orientia tsutsugamushi, the increased threat of their use in acts of bioterrorism, frequent deployment of troops to regions of endemicity, and exposures due to increased humanitar

Prevalence of environmental and other military exposure concerns in Operation Enduring Freedom and Operation Iraqi Freedom veterans.

OBJECTIVE: This study examined the prevalence of self-reported exposures in returning Operation Enduring Freedom (OEF)/Operation Iraqi Freedom (OIF) veterans and the relationship of exposure reports to current physical symptoms. METHODS: Using self-reports obtained immediately after return from deployment in a cohort of 760 enlisted Army reserve component military personnel, we assessed prevalence rates of environmental and other exposures and the association of these exposures to severity of physical symptoms. RESULTS: Reporting of environmental exposures was relatively low in veterans of O

Military-civilian partnerships on the west coast: Differing models producing combat-ready trauma teams.

Military-civilian partnerships have emerged as essential platforms for preparing military trauma surgeons and health care teams for deployment. These programs, many of which were developed following congressional initiatives to enhance military trauma readiness, address the critical issue of sustaining trauma skills during peacetime avoiding a phenomenon known as the "Walker Dip," when skills and proficiency in treating battlefield injuries decline during peacetime after periods of intense conflict. They also provide military surgeons and other providers with training in research methodology a

Amputations of upper and lower extremities, active and reserve components, U.S. Armed Forces, 2000-2011.

Traumatic amputations of limbs profoundly alter the lives of affected service members. Service members are at risk for traumatic amputations of limbs during combat deployments as well as from other hazards such as motor vehicle accidents. From 2000 to 2011, there were 6,144 incident cases of traumatic amputations among 5,694 service members. Over one-third of these service members (n=2,037) had major amputations (i.e., loss of a hand or foot or more). Male, junior enlisted members of the Army and Marine Corps in combat-specific military occupations have been most affected by major amputations.

Interventional pain management for a patient with chronic post-traumatic headaches after a traumatic brain injury.

Traumatic brain injuries (TBI) and chronic post-traumatic headaches (PTH) have affected many military personnel. The increasing use of improvised explosive devices have made blast- and explosive-related head injuries more common than in previous wars [1, 2]. Data from Operation Enduring Freedom, Operation Iraqi Freedom, and Operation New Dawn demonstrate that a significant portion of military TBIs are caused by blast-related injuries [3, 4]. From 2000 to 2016 more than 350,000 military service members were diagnosed with a TBI [4]. Military troops are at greater risk of developing PTH from hea

Psychosocial predictors of military misconduct.

The objective of this longitudinal study was to determine psychosocial predictors of military misconduct in a cohort of Marine Corps war veterans. The study included data from 20,746 male Marines who completed a life history questionnaire during initial basic training and were subsequently deployed to a combat zone. Associations between psychosocial variables, psychiatric diagnoses, and subsequent misconduct outcomes were analyzed using Cox proportional hazards regression. The strongest predictors of misconduct outcomes (bad conduct discharges and military demotions) were psychiatric diagnoses

Humanitarian Engagement by the US Military During Natural Disasters and Armed Conflict.

This article of the role of the US military in humanitarian crises focuses on the utilization of medical resources, concentrating on the impact on the pediatric age group. The US military possesses unique capabilities that enable rapid delivery of supplies, deployment of medical teams, infrastructure support, and security operations, all supporting the needs of complex operational environments. Emergencies range from rapid responses to domestic and global natural disasters, to those involving complex humanitarian emergencies involving armed conflict. The employment of these resources requires

Previously Treated or Remitted Posttraumatic Stress Disorder and Renewed Military Trauma Exposure: Evidence Gaps and a Provisional Fitness-for-Duty Framework.

INTRODUCTION: Military clinicians may be required to assess personnel with previously treated or remitted posttraumatic stress disorder (PTSD) for return to duty, deployment, weapons-related responsibilities, or other safety-sensitive assignments involving renewed trauma exposure. Direct evidence after verified remission and unrestricted military return is sparse. This review does not develop a new PTSD screening instrument or actuarial fitness score. Its aim is to synthesize the available evidence and translate it into a provisional, structured fitness-for-duty assessment and documentation fr

Pre-deployment insomnia is associated with post-deployment post-traumatic stress disorder and suicidal ideation in US Army soldiers.

STUDY OBJECTIVES: Insomnia is prevalent among military personnel and may increase risk of mental disorders and suicidal ideation. This study examined associations of pre-deployment insomnia with post-deployment post-traumatic stress disorder (PTSD) and suicidal ideation among US Army soldiers. METHODS: Soldiers from three Brigade Combat Teams completed surveys 1-2 months before deploying to Afghanistan in 2012 (T0), on return from deployment (T1), 3 months later (T2), and 9 months later (T3). Logistic regression was performed to estimate associations of pre-deployment (T0) insomnia with post-

An Analysis of the Presence of Coagulopathy on Arrival to Emergency Departments in Deployed Military Treatment Facilities.

INTRODUCTION: Acute traumatic coagulopathy remains a high predictor of mortality; however, the incidence and outcomes in the military population have limited data. The goal of our study is to characterize coagulopathy on arrival to deployed Role 2 and Role 3 Military Treatment Facilities (MTFs). MATERIALS AND METHODS: We utilized the Department of Defense Trauma Registry to identify United States (U.S.) military, Coalition forces, and U.S. contractors with documentation of international normalized ratio (INR) in the emergency department. We defined coagulopathy as INR of ≥1.6 based on Youden'

The Role of Military Identity in Substance Use and Mental Health Outcomes among U.S. Army Reserve and National Guard Soldiers.

We investigated how military identity (i.e., veteran identity centrality, the extent to which military service is central to an individual's sense of self) relates to substance use and mental health among U.S. Army Reserve and National Guard (USAR/NG) soldiers. Data were drawn from Operation: SAFETY, a longitudinal survey study of USAR/NG soldiers. Regression models (n=413 soldiers) examined relationships between military identity and substance use (i.e., alcohol problems, past 3-months non-medical use of prescription drugs (NMUPD), illicit drug use, tobacco use), and mental health (i.e., gene

Schistosomiasis: Traverers in Africa.

Schistosomiasis is a parasitic infection acquired through freshwater exposure in the tropics. It is an infection that can have devastating implications to military personnel if it is not recognized and treated, especially later in life. While there is an abundance of information available about schistosomiasis in endemic populations, the information on nonendemic populations, such as deployers, is insufficient. Definitive studies for this population are lacking, but there are actions that can and should be taken to prevent infection and to treat patients. This literary review presents a case s

Associations Between Sleep Disorders and Treatment Response in Service Members With Post-traumatic Stress Symptoms: A Secondary Outcome Analysis.

INTRODUCTION: Compared with the civilian population, a higher rate of reported sleep apnea exists among military service members resulting in inadequate sleep. Those who experience chronic sleep deprivation may suffer from debilitating problems that may compromise military mission readiness and unit safety. The purpose of the study on which this secondary outcome analysis was based was to evaluate the effect of manual standardized stress acupuncture as an adjunct therapy to an abbreviated form of cognitive behavioral therapy for insomnia for sleep disturbances in post-deployment service member

A cost-effectiveness analysis of typhoid fever vaccines in US military personnel.

Typhoid fever has been a problem for military personnel throughout history. A cost-effectiveness analysis of typhoid fever vaccines from the perspective of the US military was performed. Currently 3 vaccine preparations are available in the US: an oral live Type 21A whole cell vaccine; a single-dose parenteral, cell subunit vaccine; and a 2-dose parenteral heat-phenol killed, whole cell vaccine. This analysis assumed all vaccinees were US military personnel. Two pharmacoeconomic models were developed, one for personnel who have not yet been deployed, and the other for personnel who are deploye
