Tuberculin Skin Testing Induces Bilateral Tuberculous Uveitis in a Junior High School Student.

We report a rare case of bilateral tuberculous uveitis associated with tuberculin skin testing (TST) in an otherwise healthy junior high school student. A 12-year-old boy presented with severely decreased vision in both eyes and a history of an intradermal PPD (purified protein derivative) skin test 12 days earlier. The best-corrected visual acuity (BCVA) was 20/1000 in both eyes (OU). Ocular examination showed multifocal grayish-yellow choroidal tubercles. Laboratory examinations, including T-SPOT.TB, revealed no abnormal findings. He was diagnosed with tuberculous uveitis. Combined corticost

Vascular occlusions in the eye from cardiac myxomas.

Vascular occlusion in the eyes from cardiac myxomas was diagnosed in two cases, belatedly in one and not until after death in the other. Suspicion of a myxomatous origin should be aroused by the combination of unexplained retinal (or choroidal) vascular disease occurring with multifocal neurological symptoms and with systemic symptoms suggesting atypical subacute bacterial endocarditis. The first patient had unilateral retinal artery occlusion by embolic material believed to have been myxomatous. Removal of the cardiac myxoma resulted in disappearance of this material (although the eye remaine

Choroid plexus carcinoma in a goat.

A 15-year-old female goat suddenly developed right-sided head tilting with anorexia and depression. Post-mortem examination of the brain revealed a large, unilateral, well-demarcated, intraventricular neoplasm which was diagnosed as a choroid plexus carcinoma. The neoplasm, which occupied about 75% of the left lateral ventricle, led to unilateral obstructive hydrocephalus and invaded the white and grey matter of the left piriform lobe, with focal subarachnoid spread and meningeal implantation. Histopathological examination revealed loss of branching papillary architecture, invasive growth, a h

Idiopathic sclerochoroidal calcification.

PURPOSE: Sclerochoroidal calcification is a rare ocular condition characterized by multifocal, yellow-white elevated fundus lesions typically located in the superotemporal quadrant along the superior arcades. The pathology of calcification can be classified as dystrophic, metastatic, or idiopathic. Vision-threatening complications may arise, such as choroidal neovascular membrane and serous retinal detachment. The pathogenesis of sclerochoroidal calcification remains unclear. CASE REPORT: A 72-year-old Caucasian male patient presented to the clinic for routine examination. Fundus evaluation r

Multiple Evanescent White Dot Syndrome as Epiphenomenon of Infectious Chorioretinopathies.

PURPOSE: Multiple evanescent white dot syndrome (MEWDS) is a rare idiopathic ocular disorder often affecting young adults. Recently, a secondary form, epiphenomenon MEWDS (Epi-MEWDS), has been described in association with pre-existing retinal conditions. However, its correlation with infectious diseases remains poorly documented. METHODS: A retrospective review of six patients with infectious chorioretinopathies who developed Epi-MEWDS was conducted. Clinical presentations, imaging findings, and disease progression were analyzed. RESULTS: This study presents a case series of six patients wh

Syphilitic retinitis and uveitis in HIV-positive adults.

BACKGROUND: The incidence of new infection with syphilis is increasing, particularly in men who have sex with men, with HIV co-infection common. There has been a corresponding increase in ophthalmic manifestations that can be varied in presentation. METHODS: Thirteen consecutive patients with syphilitic uveitis presenting to two ophthalmic departments in Sydney are described. RESULTS: Twelve patients were male, of whom 10 were homosexual and six HIV-positive. Peripheral retinitis with panuveitis was the commonest ophthalmic presentation (n = 7, 54%), and six cases were initially treated with

[Acute zonal occult outer retinopathy (AZOOR)].

BACKGROUND: The "white dot syndromes" are a clinically heterogeneous group of inflammatory chorioretinopathies with an inconsistent classification. It is not yet clear if they represent distinct entities or if they are only different forms of the same basic disease. CASE REPORT: A 53-year-old female patient presented with unilateral photopsia and reduced vision in the left eye. Funduscopy of the left eye showed peripapillary confluent whitish infiltrates in the deep retinal layers with corresponding enlargement of the blind spot. Slow progression of the lesions was observed followed by the de

BULBOSITIES AND INTERVORTEX VENOUS ANASTOMOSIS IN VENOUS OVERLOAD CHOROIDOPATHY MASQUERADING AS POLYPOIDAL CHOROIDAL VASCULOPATHY.

PURPOSE: To describe a patient with venous overload choroidopathy in whom venous bulbosities masqueraded as polyps and intervortex venous anastomosis mimicked a branching vascular network, giving the appearance of polypoidal choroidal vasculopathy. METHODS: The patient had complete ophthalmic examination including indocyanine green angiography and optical coherence tomography. Venous bulbosities were defined on indocyanine green angiography as focal dilations in which the diameter of the dilation is two times that of the host vessel. RESULTS: A 75-year-old woman presented with combined subre

A neuropathologic study of Whipple's disease.

A patient with Whipple's disease developed massive and irreversible CNS involvement after a drug-induced intestinal remission. The multifocal brain lesions were apparent grossly with the heaviest concentration being noted in rhinencephalon, diencephalon, and mesencephalon. This patient exhibited progressive neurologic deterioration, although viable organisms could not be identified in the brain at autopsy. Astrocytes, pericytes, and choroid plexus cells attempted to dispose of the organism, in additional to the traditionally implicated microglial and ependymal cell.

Histologic correlates of "Choroidal abnormalities" in Neurofibromatosis type 1 (NF1).

Neurofibromatosis type 1 (NF1) is a rare autosomal dominant disorder characterized by proliferation of cells from neural crest origin. The most common manifestations are cutaneous, neurologic, skeletal and ocular. The distinction of NF1 from other syndromes with multiple café-au-lait macules may be difficult in the pediatric age group, and ocular findings, especially Lisch nodules (i.e., melanocytic hamartomas on the irides), are a useful, early diagnostic tool. In recent years, novel ocular manifestations descriptively referred to as "choroidal abnormalities", choroidal "hyperpigmented spots"

EDI-OCT as an auxiliary examination for the differential diagnosis of bullous CSC and Vogt-Koyanagi-Harada: A case report.

A 46-year-old male patient visited our clinic with a complaint of blurred vision in the right eye accompanied by headache and insomnia. The fundus examination showed three bullous retinal detachments in the right eye. Considering the prodromal symptoms and other fundus characteristics such as vitreous cells in the posterior pole and multifocal fluorescence leakages on fundus fluorescein angiography (FFA), initial diagnosis was considered as Vogt-Koyanagi-Harada (VKH). However, oral glucocorticoids didn't improve patient's vision. Further enhanced depth imaging (EDI)-optical coherence tomograph

Ultrasonographic findings in endophthalmitis.

Ultrasonographic findings are reported in 7 cases of endophthalmitis (6 of bacterial and 1 of fungal origin) resulting from perforating injuries (2 cases), severe corneal ulcers (2 cases) and open-eye surgery (3 cases). The most frequent ultrasonographic picture was characterized by a series of low reflectivity echoes in the vitreous with a high degree of mobility (7/7), fairly large endovitreal vacuoles (3/7) and hyaloid thickening (3/7). In one case the vitreous involvement was confined to only one quadrant. Choroidal thickening both of a homogeneous-diffuse (2/7) and nodular-multifocal type

Spectrally resolved autofluorescence imaging in posterior uveitis.

Clinical discrimination of posterior uveitis entities remains a challenge. This exploratory, cross-sectional study investigated the green (GEFC) and red emission fluorescent components (REFC) of retinal and choroidal lesions in posterior uveitis to facilitate discrimination of the different entities. Eyes were imaged by color fundus photography, spectrally resolved fundus autofluorescence (Color-FAF) and optical coherence tomography. Retinal/choroidal lesions' intensities of GEFC (500-560 nm) and REFC (560-700 nm) were determined, and intensity-normalized Color-FAF images were compared for bir

Sensitivity of indocyanine green angiography for the follow-up of active inflammatory choriocapillaropathies.

BACKGROUND: Inflammatory choriocapillaropathies (choriocapillaritis) correspond to the clinical spectrum of lesions of the fundus, including acute posterior multifocal placoid pigment epitheliopathy (APMPPE), multiple evanescent white dot syndrome (MEWDS), multifocal choroiditis (MC), and other rarer entities caused by inflammatory disturbances of choriocapillaris perfusion. The aim here was to study the sensitivity of indocyanine green (ICG) angiography in investigating and following inflammatory choriocapillaropathies.
