Cerebellar mutism after spontaneous intratumoral bleeding involving the upper cerebellar vermis: a contribution to the physiopathogenic interpretation.

BACKGROUND: Transient mutism as a consequence of posterior fossa surgery is a well-known phenomenon. However, it has rarely been reported after focal nonsurgically induced cerebellar damage. CASE REPORT: We describe a 7-year-old child affected by a tumor arising from the quadrigeminal plate who developed transient cerebellar mutism after a spontaneous bleeding which extended to the upper cerebellar vermis. The recovery from mutism started about a week after the bleeding. At the time of the surgical treatment, 8 weeks after the spontaneous intratumoral bleeding, she was only dysarthric. DISCU

Risk factors associated with cerebellar mutism syndrome and late postoperative hydrocephalus following pediatric posterior fossa medulloblastoma surgery.

OBJECTIVE: To investigate the risk factors for postoperative complications in pediatric patients with posterior cranial fossa medulloblastoma(MB). METHODS: A retrospective analysis was performed based on the clinical data of 53 pediatric patients with posterior fossa medulloblastoma treated in Tangdu Hospital, from Dec, 2016 to May, 2024. Various data points, including age, gender, tumor maximum diameter, surgical approach, and complications, were collected and statistically evaluated for an association with cerebellar mutism syndrom(CMS) and late postoperation hydrocephalus (LPH). RESULTS:

Acute psychogenic mutism following total hip arthroplasty under general anesthesia in an elderly patient: a case report.

Postoperative neuropsychiatric complications most commonly manifest as delirium or cognitive impairment; however, acute isolated mutism in the absence of frank delirium or structural brain injury-referred to in some literature as acute psychogenic mutism (APM)-represents an exceedingly rare and poorly understood postoperative phenomenon. This report presents a case of APM in a 69-year-old woman following elective total hip arthroplasty under general anesthesia. The patient developed acute selective aphonia upon emergence from anesthesia, despite preserved comprehension and nonverbal communicat

Beyond the triad: akinetic mutism in idiopathic normal pressure hydrocephalus, with response to shunt placement. Illustrative case.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) typically manifests with the classic Hakim-Adams triad of gait disturbance, cognitive impairment, and urinary incontinence. While akinetic mutism represents a rare and severe neurological presentation characterized by profound reduction in voluntary movement and speech, its association with iNPH remains underrecognized in clinical practice. This case illustrates both the diagnostic challenges and remarkable therapeutic potential when encountering this unusual manifestation of iNPH. OBSERVATIONS: A previously healthy 52-year-old femal

Case Report: Treatment of Akinetic Mutism after Unilateral Anterior Cerebral Artery Infarction with Methylphenidate and Levodopa/Benserazide.

INTRODUCTION: Akinetic mutism is a severe state of impaired volition that can result from a stroke. Its therapeutic evidence relies on single case reports that used atomoxetine or levodopa with variable latency and efficacy. CASE PRESENTATION: We present the case of a 54-year-old woman who developed akinetic mutism after infarction in the territory of the right anterior cerebral artery, successfully treated with methylphenidate and levodopa/benserazide. Clinical examination showed a patient lacking any spontaneous speech and movement while opening her eyes and fixating. Suspecting akinetic mu

A pilot study of an online behavioral parent training program for children with selective mutism: feasibility and preliminary effectiveness.

BACKGROUND: Recent approaches to treating selective mutism (SM) have increasingly emphasized parent involvement, and several parent-mediated programs have shown promising efficacy. However, in regions where SM specialists are scarce, families have limited access to appropriate support. In this context, online behavioral parent training (BPT) may serve as a promising and scalable option that can overcome geographic barriers. Nevertheless, empirical studies specifically targeting SM-focused BPT remain limited. This pilot study evaluated the feasibility and preliminary effectiveness of an online

Switching to aripiprazole for the treatment of residual mutism resulted in distinct clinical courses in two catatonic schizophrenia cases.

OBJECTIVES: The efficacy of a partial agonist for the dopamine D2 receptor, aripiprazole, for catatonia in schizophrenia has been reported. METHODS: We report distinct clinical courses in challenging aripiprazole to treat residual mutism after severe catatonic symptoms improved. RESULTS: In the first case, mutism was successfully treated when the patient was switched from olanzapine to aripiprazole. In contract, switching to aripiprazole from risperidone aggravated auditory hallucinations in the second case. CONCLUSIONS: We will discuss the benefits and risks of using aripiprazole for the t

[Neuroleptic Intolerance and Residual Mutism in a Young Woman with Anti-N-Methyl-D-Aspartate Receptor (NMDAR) Encephalitis].

We report a case of anti-NMDAR encephalitis and residual mutism in a 23-year-old woman who presented with neuroleptic intolerance. Admission to our department for investigation of her abnormal behavior revealed cerebrospinal fluid (CSF) positivity for anti-NMDAR antibodies, and the patient underwent immunotherapy. However, generalized tonic seizures developed, requiring mechanical ventilation in the intensive care unit. Antipsychotic drugs were also administered for involuntary movements and insomnia. Thereafter, a malignant syndrome of severe hyperCKemia (Max: 191,120 IU/L) and shock develope

System degeneration in an MM1-type sporadic Creutzfeldt-Jakob disease case with an unusually prolonged akinetic mutism state.

Methionine/methionine type 1 (MM1-type) sporadic Creutzfeldt-Jakob disease (sCJD), known as the 'classic type,' shows typical clinicopathological sCJD findings. In general, patients reach an akinetic mutism state within a few months of disease onset and die soon after if supportive therapies are not administered. Here, we describe remarkable neuropathologic observations of MM1-type sCJD in a 48-year-old, Japanese man with an unusually prolonged akinetic mutism state. In the early disease stages, the patient exhibited abnormal behaviour with gait disturbance and rapidly progressive cognitive dy

Tracing volitional recovery in post-stroke akinetic mutism using longitudinal microstructure imaging: Insights from a single case study.

Lesions in the frontal-subcortical circuitry can lead to akinetic mutism (AM) characterized by diminished volition. However, the microstructural changes in the damaged network underlying its recovery remain unknown. Clinical examination and neuropsychological assessment were performed on a patient with post-stroke AM. Multimodal MRI scans were performed at baseline and follow-ups. We used diffusion MRI and biophysical models, specifically utilizing neurite orientation dispersion and density imaging for assessing gray matter microstructure, and fixel-based analysis for the evaluation of white m

Fluorodeoxyglucose positron emission tomography/computed tomography findings in a patient with cerebellar mutism after operation in posterior fossa.

Cerebellar mutism is a transient period of speechlessness that evolves after posterior fossa surgery in children. Although direct cerebellar and brain stem injury and supratentorial dysfunction have been implicated in the mediation of mutism, the pathophysiological mechanisms involved in the evolution of this kind of mutism remain unclear. Magnetic resonance imaging revealed dentatothalamocortical tract injuries and single photon emission computed tomography showed cerebellar and cerebral hypoperfusion in patients with cerebellar mutism. However, findings with 18F-fluorodeoxyglucose positron e

Effectiveness of selective serotonin reuptake inhibitors in inpatients with selective mutism compared to social anxiety disorder: an observational study.

Selective Mutism (SM) is a rare childhood anxiety disorder characterized by a persistent inability to speak in specific social settings despite adequate speaking ability in others, sharing etiological and clinical overlap with Social Anxiety Disorder (SAD). While evidence for pharmacological treatment in SM is limited, selective serotonin reuptake inhibitors (SSRIs) have demonstrated potential efficacy, but studies on effectiveness in routine clinical practice are lacking. This single-center, retrospective observational study analysed data from 2011 to 2021 on children aged 4-14 years with SM

A case of postoperative cerebellar mutism with hyperphagia in a child following gross total resection of medulloblastoma occupying the cerebellar vermis.

INTRODUCTION: Cerebellar mutism syndrome is a well-known complication following posterior fossa tumor resection. Its incidence is markedly increased among patients with medulloblastoma. Patients typically present with an inability to communicate verbally due to disruption of the dentato-thalamocortical pathway. CASE DESCRIPTION: We present a unique case of cerebellar mutism in a three-year-old girl who underwent gross total resection of medulloblastoma occupying the cerebellar vermis. In addition to mutism, the patient developed hyperphagia. DISCUSSION: This case report aims to contribute to

The controversy around the diagnosis of selective mutism - a critical analysis of three cases in the light of modern research and diagnostic criteria.

The position of selective mutism disorder - SM - has been modified in the last edition of the classification of mental disorders DSM-5. It was removed from "Disorders of childhood and adolescence" and placed in "Anxiety disorders". This caused two important changes in the interpretation of the symptoms of selective mutism. It highlighted anxious etiology of the disorder and also open the possibility to diagnose selective mutism in adults as a special category of anxiety disorders. The aim of this study was to present three different cases concerning the diagnostic difficulties of selective mut
