Primary hyperparathyroidism (PHPT) is characterized by an elevation in serum calcium levels, sometimes leading to aggravated clinical conditions, namely nephrolithiasis, nephrocalcinosis, and/or fractures. A 55-year-old patient was admitted to the hospital with acute obstructive pyelonephritis in March 2021, having another episode one year later. Initial blood and urine analysis detected inflammatory markers, namely C-reactive protein, and the presence of leucocytes and blood in the urine. The renal computed tomography scan exhibited renal asymmetry, nephrocalcinosis, and multiple kidney stone

Out of five children with hypercalciuria, four had nephrocalcinosis and one recurrent bladder calculi. Two out of four patients with nephrocalcinosis (patients 1 and 2) had the "hyperprostaglandin E syndrome" and the other two (patients 3 and 4, siblings) had the Royer syndrome (osteopathy, dwarfism, secondary hyperparathyroidism). Treatment with hydrochlorothiazide is effective in patients without increased urinary excretion of prostaglandin, whereas patients with increased prostaglandin excretion respond better to indomethacin therapy.

Hereditary hypophosphatemic rickets with hypercalciuria (HHRH) represents an FGF23-independent disease caused by biallelic variants in the solute carrier family 34-member 3 (SLC34A3) gene. HHRH is characterized by chronic hypophosphatemia and an increased risk for nephrocalcinosis and rickets/osteomalacia, muscular weakness, and secondary limb deformity. Biochemical changes, but no relevant skeletal changes, have been reported for heterozygous SLC34A3 carriers. Therefore, we assessed the characteristics of individuals with biallelic and monoallelic SLC34A3 variants. In 8 index patients and 5 f

Rare kidney stone diseases typically present with nephrolithiasis or nephrocalcinosis in childhood or adolescence. Affected individuals might face kidney injury and even kidney failure related to associated complications. Screening blood and urine tests recommended for patients with nephrolithiasis and/or nephrocalcinosis help guide further evaluation, and the increased availability and decreased costs of genetic testing can facilitate the diagnosis of hereditary stone conditions. Genetic testing should be considered when there are clinical clues of an increased likelihood of an inherited cond

Renal involvement in primary Sjögren's syndrome occurs in 10-60% of cases. Tubulointerstitial nephritis with distal renal tubular acidosis (DRTA) is the main type of involvement. It's generally asymptomatic and revealed by complications of DRTA. We report 4 cases of GJS complicated by nephrocalcinosis, tetapresic hypokaliemia and osteomalacia. In 2 cases, nephrocalcinosis was diagnosed simultaneously with the GJS. The 2 other cases were diagnosed 3 and 15 years after primary GJS. All patients were treated by coticosteroids at the dose of 0.5 mg/Kg/jour, alkaline solution, K Cl in 2 cases and v

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis is an autosomal recessive renal tubular disorder characterized by renal magnesium wasting, hypercalciuria, advanced nephrocalcinosis and progressive renal failure. Mutations in the paracellin-1 (CLDN16) gene have been defined as the underlying genetic defect. The tubular disorders and progression in renal failure are usually resistant to magnesium substitution and hydrochlorothiazide therapy, but hypomagnesemia may improve with advanced renal insufficiency. We present a patient with a homozygous truncating CLDN16 gene mutation (W

Nephrocalcinosis is a rare cause of chronic renal failure. We describe a 64 year-old man, who was admitted to hospital due to chronic renal failure. An ultrasound examination and a computed tomography of the kidneys showed bilateral, severe nephrocalcinosis. He had hypercalcaemia, hyperparathyroidism, and a bone disease caused by an adenoma in one of the parathyroid glands. After removal of the adenoma, the plasma calcium level was in control, but renal function deteriorated. The patient was treated with dialysis and renal transplantation.

Subcutaneous fat necrosis of the newborn (SFN) is a rare form of panniculitis that can be complicated by hypercalcemia and nephrocalcinosis. We conducted a 20-year retrospective cohort study of 30 patients to describe the clinical characteristics of SFN and the prevalence of hypercalcemia and other complications. Hypoxia and therapeutic hypothermia (TH) seem to play an important role in the development of SFN. Hypercalcemia developed in 63% of patients, three of whom developed nephrocalcinosis. There is a need for well-controlled prospective studies to determine the natural history, prevalence

INTRODUCTION: Calcium overload in trisomy 21 is a rare and underrecognized cause of persistent parathyroid hormone (PTH)-independent hypercalcaemia in infants and young children with Down syndrome. Characteristic features include hypercalciuria, nephrocalcinosis, renal impairment, and failure to thrive. Delayed recognition may result in avoidable long-term complications.
CASE PRESENTATION: A 21-month-old boy with trisomy 21 presented with a six-month history of recurrent vomiting, feeding difficulties, and failure to thrive. Bilateral nephrocalcinosis was detected incidentally on abdominal ul

The combination of hypercalcaemia, hypercalciuria, and nephrocalcinosis with and without renal impairment is rare in paediatric clinical practice. However, this constellation of findings has been reported in three children with trisomy 21, but the absence of detailed nutritional data has failed to clarify the underlying pathogenesis. This report describes a 4 year old girl with trisomy 21 who was found coincidentally to have hypercalcaemia, hypercalciuria, nephrocalcinosis, and renal impairment in the absence of metabolic alkalosis, following a prolonged period of excessive calcium intake.

X-linked hypophosphatemia (XLH) is the most common monogenetic cause of chronic hypophosphatemia, characterized by rickets and osteomalacia. Disease manifestations and treatment of XLH patients in the Netherlands are currently unknown. Characteristics of XLH patients participating in the Dutch observational registry for genetic hypophosphatemia and acquired renal phosphate wasting were analyzed. Eighty XLH patients, including 29 children, were included. Genetic testing, performed in 78.8% of patients, showed a PHEX mutation in 96.8%. Median (range) Z-score for height was - 2.5 (- 5.5; 1.0) in

The case of a 41-year-old female patient, with progressive decrease in kidney function, and ultrasound evidencing nephrocalcinosis and nephrolithiasis aims to highlight the importance of interdisciplinary collaboration to diagnose kidney stone diseases and the role of kidney imaging in the evaluation of nephrocalcinosis and nephrolithiasis. Primarily based on abdominal imaging and kidney impairment, the patient was referred to a nephrologist, who diagnosed her with a rare genetic disease, primary hyperoxaluria type 1, with a severe prognosis if not properly and timely treated. The kidney imagi

Nephrocalcinosis, marked by calcium phosphate deposition in the kidneys, is often linked to distal renal tubular acidosis (dRTA). While typically driven by hypercalciuria, hyperphosphaturia can also promote crystal formation in alkaline urine. A 24-year-old female with recurrent fractures and hip pain presented with hypokalemia, metabolic acidosis, and hyperphosphaturia but without hypercalciuria. Imaging revealed medullary nephrocalcinosis and osteomalacia, secondary to dRTA. Potassium citrate improved her condition. This case emphasizes early recognition of dRTA to prevent renal and skeletal

Hypophosphatasia (HPP) is a rare genetic condition associated with poor bone mineralization, low serum alkaline phosphatase, high urinary pyrophosphate excretion, and nephrocalcinosis. Nephrocalcinosis is thought to develop due to the increased filtered loads associated with hypercalcemia and hyperphosphatemia, but the composition of these calcifications is incompletely understood. We report the first ever magnesium pyrophosphate (MgPPi) urinary stone, which prompted the new diagnosis of HPP in a 12-year-old boy. Stone analysis labs should include infrared spectra of PPi salts in their referen

We present a patient with Rabson-Mendenhall syndrome with unsatisfactory glycemic control of diabetes despite the use of metformin, pioglitazone, and sitagliptin. We investigated the long-term effects of treatment with an empagliflozin. After one year of empagliflozin therapy, an improvement in metabolic control of diabetes was detected. However, eleven months after implementation of empagliflozin, the patient was diagnosed with an early form of nephrocalcinosis. The empagliflozin dose was reduced, maintaining metabolic control and reducing hypercalciuria. We discuss the probable causes of nep

Amelogenesis imperfecta (AI) is a disease primarily affecting amelogenesis, but other various aberrations have been reported in association with this entity. Enamel-renal syndrome (ERS) is a very rare disorder associating AI with nephrocalcinosis. It is known by various synonyms such as AI nephrocalcinosis syndrome, MacGibbon syndrome, Lubinsky syndrome and Lubinsky-MacGibbon syndrome. The purposes of this article are to describe other dental anomalies which are rarely associated with AI making the diagnosis of AI a complex disorder and also to report a rare case of MacGibbon syndrome presente

The effect of enterosorbents of natural origin, zoosterol and sodium alginate, on the main types of metabolism and the macro- and microelement composition (calcium, magnesium, zinc, copper) of the renal tissues was studied in animals with experimental nephrocalcinosis. Increased complex-forming ability of zoosterol, related to a higher absorption activity of low-molecular-weight fragments of this enterosorbent, favors retention of large molecules in the form of calcium salts in the parenchyma. Sodium alginate favors the elimination of calcium compounds and compensates for calcification of the

Fifteen cases of hypervitaminosis D in childhood are reviewed. In all of them, vitamin D was given following medical prescription. In four occasions, excessive dosage of vitamine D impaired the evolution of a previous nephropathy. The clinical, analytical, radiological and histological findings as well as the therapeutical aspects are commented. Hypercalcemia, hypercalciuria, polyuria with hypostenuria, renal failure, bone lesions and nephrocalcinosis are the most prominent features of the picture. Occasionally, arterial hypertension and glycosuria were found. Prednisone, thyrocalcitonine and
