Acquisition of sensorimotor fMRI under general anaesthesia: Assessment of feasibility, the BOLD response and clinical utility.

We evaluated whether task-related fMRI (functional magnetic resonance imaging) BOLD (blood oxygenation level dependent) activation could be acquired under conventional anaesthesia at a depth enabling neurosurgery in five patients with supratentorial gliomas. Within a 1.5 T MRI operating room immediately prior to neurosurgery, a passive finger flexion sensorimotor paradigm was performed on each hand with the patients awake, and then immediately after the induction and maintenance of combined sevoflurane and propofol general anaesthesia. The depth of surgical anaesthesia was measured and confirm

Bradycardic Arrest During Somatosensory-Evoked Potential Monitoring: A Case Report.

Intraoperative neurophysiological monitoring involves electrical stimulation of nerve pathways and is used to assess the integrity of both motor and sensory pathways. Whereas motor pathway monitoring involves transcranial stimulation and peripheral nerve monitoring, somatosensory monitoring involves peripheral nerve stimulation and cranial monitoring. Arrhythmias are a known complication of motor-evoked potential monitoring but have not been reported with somatosensory monitoring. We describe a case of bradycardic arrest with somatosensory-evoked potential monitoring using median nerve electro

Quantitative Pupillometry for Intracranial Pressure (ICP) Monitoring in Traumatic Brain Injury: A Scoping Review.

The neurological examination has remained key for the detection of worsening in neurocritical care patients, particularly after traumatic brain injury (TBI). New-onset, unreactive anisocoria frequently occurs in such situations, triggering aggressive diagnostic and therapeutic measures to address life-threatening elevations in intracranial pressure (ICP). As such, the field needs objective, unbiased, portable, and reliable methods for quickly assessing such pupillary changes. In this area, quantitative pupillometry (QP) proves promising, leveraging the analysis of different pupillary variables

Filum terminale arteriovenous shunt with nidus structure: a report of rare condition and treatment consideration.

BACKGROUND: In the literature, filum terminale arteriovenous shunts (FTAVSs) always feature a single shunt point. Nidus-type FTAVSs have rarely been reported, and the best treatment strategy is unclear. This is a report of one exceptional case of a nidus-type FTAVS and surgical treatment of the lesion. CASE DESCRIPTION: The patient suffered from cauda equina syndrome for 9 months. Magnetic resonance imaging and spinal angiography revealed a nidus-type FTAVF at the L2 level. Surgical resection was performed in the hybrid operating room, and the nidus was completely resected with the assistance

Noninvasive Neuromonitoring in Pediatric Traumatic Brain Injury: A Scoping Review of Quantitative Pupillometry and the Evidence Gap in Noninvasive Intracranial Pressure Waveform Analysis.

Pediatric traumatic brain injury (TBI) is the leading cause of injury-related death and long-term disability in children worldwide. Current monitoring relies predominantly on invasive techniques that carry procedural risks and are not feasible in all settings or across all severity levels. Noninvasive alternatives, including quantitative automated pupillometry and intracranial pressure (ICP) pulse waveform monitoring, have emerged as promising tools, yet their evidence base in pediatric TBI remains uncharacterized. We conducted a scoping review following the Joanna Briggs Institute methodology

Diagnostic accuracy of intraoperative neuromonitoring during non-tumor thoracic spine surgeries: A systematic review and meta-analysis.

BACKGROUND: Thoracic spine surgeries carry a higher risk of neurological complications due to the region's narrow spinal canal and precarious blood supply, making effective monitoring critical. This study offers the first meta-analysis assessing accuracy and prognostic value of Intraoperative neuromonitoring (IONM) in non-tumor thoracic spinal surgeries, filling a key gap in the literature. METHODS: We adhered to PRISMA-DTA guidelines, conducting a PubMed search for relevant studies. Included studies had over 10 patients and excluded spinal cord tumors. Study quality was evaluated with QUADAS

A Systematic Review and Meta-Analysis to Compare the Diagnostic Accuracy of Direct-Waves and Myogenic Motor-Evoked Potential Neuromonitoring During Intramedullary Spinal Cord Tumor Resection.

BACKGROUND AND OBJECTIVES: Intramedullary spinal cord tumor (IMSCT) resection seeks to maximize tumor removal while preserving neurological function, guided by intraoperative neurophysiological monitoring with myogenic motor-evoked potentials (myogenic MEPs) and direct waves (D-wave) recordings. Despite their widespread use, no systematic review has compared their accuracy for predicting immediate, transient, and persistent postoperative motor deficits in IMSCT surgery. This meta-analysis fills this gap by evaluating their diagnostic accuracy. METHODS: Following Preferred Reporting Items for

Systematic review of alarm criteria and reversibility in motor-evoked potentials during intramedullary spinal cord tumor resection.

BACKGROUND/OBJECTIVE: Myogenic motor-evoked potentials (myogenic MEPs) and Direct waves (D-waves) are essential for monitoring motor pathways during intramedullary spinal cord tumor (IMSCT) resections. However, the diagnostic accuracy of alarm criteria and the prognostic value of reversible versus irreversible changes remain unclear. This is the first study addressing this gap. METHODS: Following PRISMA-DTA guidelines, this review searched PubMed, MEDLINE, and OVID (1985-2024) for studies on intraoperative myogenic MEPs and/or D-waves with postoperative motor outcomes in IMSCT resections.
