Xerophthalmia and Nyctalopia as Presenting Signs of Vitamin A Deficiency in a Patient With Rapid Intentional Weight Loss: A Case Report and Literature Review.

To present a patient with xerophthalmia and night blindness secondary to vitamin A deficiency following rapid weight loss. A 34-year-old man presented with decreased night vision, foreign body sensation, and eye dryness. He reported a rapid intentional weight loss of approximately 34 kg over the past 4 months through a restricted diet. Slit lamp examination revealed severe dry eye and a foamy appearance of the bulbar conjunctiva in both eyes. Retinal inner atrophy was evident on macular optical coherence tomography imaging. This case highlights critical considerations for clinical practice, pa

Variable expressivity in X-linked congenital stationary night blindness.

X-linked congenital stationary night blindness (CSNB) is a well-documented disorder in which the most striking clinical features are impaired night vision, nystagmus and myopia. Recent reports have highlighted differing features between families, and it has been suggested that these discrepancies may be the result of two loci on the X chromosome or of two mutant alleles. We outline the clinical and visual function findings in 42 affected members from 10 families and 1 adopted person. There was a relative unawareness of the disorder in clinical practice. At least one of the main features of CSN

Unique pore architecture underlies constitutive gating of human retinal TRPM1.

Transient receptor potential melastatin 1 (TRPM1), a Ca ²⁺ -permeable nonselective cation channel essential for retinal ON bipolar cell signaling and night vision, and implicated in congenital night blindness, has remained structurally and functionally poorly characterized. Here we report the first cryo-electron microscopy structure of human TRPM1 in conducting state. Although the channel assembles as a tetramer, it adopts an unexpected clockwise domain-swapped pore module with rotational geometry inverse to that observed in previously characterized 6-TM tetrameric channels. This inverted topo

Retinitis Pigmentosa and Bilateral Idiopathic Demyelinating Optic Neuritis in a 6-Year-Old Boy with OFD1 Gene Mutation.

To identify the cause of a sudden binocular vision decrease in patients with retinitis pigmentosa and bilateral idiopathic demyelinating optic neuritis is difficult, but early diagnosis and treatment significantly improve the prognosis. Here, we report a 6-year-old boy with a progressive binocular vision decrease in 38 days. The patient had a history of night blindness, a mottled retina without pigmentation, extinguished electroretinographic response, tritanopia, and an absent ellipsoid zone outside the macula fovea by optical coherence tomography in both eyes. His condition was diagnosed as r

Complete vision-based traffic sign recognition supported by an I2V communication system.

This paper presents a complete traffic sign recognition system based on vision sensor onboard a moving vehicle which detects and recognizes up to one hundred of the most important road signs, including circular and triangular signs. A restricted Hough transform is used as detection method from the information extracted in contour images, while the proposed recognition system is based on Support Vector Machines (SVM). A novel solution to the problem of discarding detected signs that do not pertain to the host road is proposed. For that purpose infrastructure-to-vehicle (I2V) communication and a

Bilateral astrocytic hamartoma with vasoproliferative tumour in retinitis pigmentosa.

We report a rare case of a 32-year-old Indian male who presented to the retina outpatient department with a history of sudden worsening of vision in the left eye. There was a background history of poor vision and deficient night vision since childhood. At the first presentation, the best corrected visual acuity was 6/36 and 6/60 in the right and left eye, respectively. Ocular examination revealed waxy pale disc, bone spicule pigmentation, attenuated vessels and epiretinal membrane in the right eye in keeping with retinitis pigmentosa. An astrocytic harmatoma was also present in the right eye.

Chromophore supply modulates cone function and survival in retinitis pigmentosa mouse models.

Retinitis pigmentosa (RP) is an ocular disease characterized by the loss of night vision, followed by the loss of daylight vision. Daylight vision is initiated in the retina by cone photoreceptors, which are gradually lost in RP, often as bystanders in a disease process that initiates in their neighboring rod photoreceptors. Using physiological assays, we investigated the timing of cone electroretinogram (ERG) decline in RP mouse models. A correlation between the time of loss of the cone ERG and the loss of rods was found. To investigate a potential role of the visual chromophore supply in thi

Juvenile neuronal ceroid lipofuscinosis (Batten disease) CLN3 mutation (Chrom 16p11.2) with different phenotypes in a sibling pair and low intensity in vivo autofluorescence.

BACKGROUND: The neuronal ceroid lipofuscinoses (Batten disease) are a heterogeneous group of autosomal recessively inherited disorders causing progressive neurological failure, mental deterioration, seizures and visual loss secondary to retinal dystrophy. The juvenile type is of special interest to the ophthalmologist as visual loss is the earliest symptom of the disorder. HISTORY AND SIGNS: We present two siblings with severe retinal dystrophy due to juvenile Batten disease. Sibling A (age 10) presented with visual loss, photophobia and night blindness, starting at age 4. His vision was perc

Electronegative electroretinograms in two siblings with bradyopsia, an underdiagnosed pediatric inherited retinal disorder.

In the scotopic flash response of the full-field electroretinogram (ffERG), the downward a-wave reflects photoreceptor response and the upward b-wave is indicative of bipolar cell function. If the b-wave is equal to or smaller than the a-wave, the waveform (and the ffERG) is termed electronegative. For pediatric inherited retinal disorders this finding narrows the differential diagnosis to certain conditions, most commonly congenital stationary night blindness if multimodal imaging is grossly normal. We describe 2 siblings with reduced vision in the setting of normal multimodal imaging. Inheri

Multi-object detection at night for traffic investigations based on improved SSD framework.

Despite significant progress in vision-based detection methods, the task of detecting traffic objects at night remains challenging. Visual information of medium and small stationary objects is deteriorated due to poor lighting conditions. And the visual information is important for traffic investigations. For meeting the needs of night traffic investigations, this study focuses on presenting a nighttime multi-object detection framework based on Single Shot MultiBox Detector (SSD). Considering the need of traffic investigations, the applicable detection framework is presented for detecting traf

A dominant form of inherited retinal degeneration caused by a non-photoreceptor cell-specific mutation.

We have isolated a dominant mutation, night blindness a (nba), that causes a slow retinal degeneration in zebrafish. Heterozygous nba fish have normal vision through 2-3 months of age but subsequently become night blind. By 9.5 months of age, visual sensitivity of affected fish may be decreased more than two log units, or 100-fold, as measured behaviorally. Electroretinographic (ERG) thresholds of mutant fish are also raised significantly, and the ERG b-wave shows a delayed implicit time. These defects are due primarily to a late-onset photoreceptor cell degeneration involving initially the ro

Mice Lacking Gpr179 with Complete Congenital Stationary Night Blindness Are a Good Model for Myopia.

Mutations in GPR179 are one of the most common causes of autosomal recessive complete congenital stationary night blindness (cCSNB). This retinal disease is characterized in patients by impaired dim and night vision, associated with other ocular symptoms, including high myopia. cCSNB is caused by a complete loss of signal transmission from photoreceptors to ON-bipolar cells. In this study, we hypothesized that the lack of Gpr179 and the subsequent impaired ON-pathway could lead to myopic features in a mouse model of cCSNB. Using ultra performance liquid chromatography, we show that adult Gpr17

Effect of sleep deprivation and driving duration on the useful visual field in younger and older subjects during simulator driving.

Nine older subjects (40-51 years) and 10 younger subjects (18-30 years) took part in two one-hour driving sessions. They performed a very monotonous task during which they had to follow a vehicle either after a complete night of sleep or after one night of sleep deprivation. While driving their useful visual field was assessed by introducing signals that would appear on the whole road scene. The analysis of the data indicates that the ability to process peripheral signals deteriorates with age, driving duration and sleep deprivation. However, the effects of these three variables on the periphe

Anterior Segment Findings in Vitamin A Deficiency: A Case Series.

Vitamin A deficiency is a rare but vision threatening disorder in the developed world, which can lead to blindness for severe keratomalacia with cornea scarring and perforation or night blindness due to impaired dark adaptation. Conversely, the disease is quite common in developing countries, as a consequence of chronic malnutrition. The correct diagnosis and therapy with prompt vitamin A supplementation avoid blindness. We report a series of 3 local cases with different age and causes for vitamin A deficiency. The diagnostic workup, therapy, and prognosis are discussed.

Retinal adaptation to dim light vision in spectacled caimans (Caiman crocodilus fuscus): Analysis of retinal ultrastructure.

It has been shown that mammalian retinal glial (Müller) cells act as living optical fibers that guide the light through the retinal tissue to the photoreceptor cells (Agte et al., 2011; Franze et al., 2007). However, for nonmammalian species it is unclear whether Müller cells also improve the transretinal light transmission. Furthermore, for nonmammalian species there is a lack of ultrastructural data of the retinal cells, which, in general, delivers fundamental information of the retinal function, i.e. the vision of the species. A detailed study of the cellular ultrastructure provides a basic

[Disorders of color perception and increase glare sensitivity in phenytoin and carbamazepine therapy. Ocular side effects of anticonvulsants].

Advanced psychophysical tests, performed in 42 epileptic patients, show that the antiepileptic drugs phenytoin and carbamazepine can specifically affect the retinal function, while Valproic Acid and the epileptic seizures do not. The Farnsworth-Munsell 100-Hue and Panel D-15 désaturé tests revealed an accumulation of errors along the tritan/tetartan axis (blue colour vision deficiencies) and a high total error score. The same defect was shown by measurement of the spectral sensitivity functions. The results obtained for mesopic vision and especially glare sensitivity measured by nyktometry wer

Clinical findings and common symptoms in retinitis pigmentosa.

Data analysis was performed in a prospective study of clinical symptoms and findings in 500 patients with retinitis pigmentosa. The symptoms and findings in these patients met the usual definitions of the disease. At initial examination the patients were questioned in a standardized manner; symptoms and associated health problems were reviewed. Some patients were unable to answer all of the questions. Of the patients 274 (55%) were men and 226 (45%) were women, with a race distribution of 21 (4%) black, 47 (9%) Hispanic, 26 (5%) Oriental, three (1%) American Indian, and 403 (81%) white. Sixty-

Generation of a gene-corrected isogenic human iPSC line (CSUASOi006-A-1) from a retinitis pigmentosa patient with heterozygous c.5792C > T mutation in the PRPF8 gene.

Retinitis pigmentosa (RP) is a common inherited retinal disease characterized by progressive degeneration of the retina, leading to night blindness, progressive vision loss, and constriction of the visual field. Previously, we established a human induced pluripotent stem cell line (CSUASOi006-A) from a RP patient carrying heterozygous PRPF8 (c.C5792T) mutation. Here, we corrected the mutation sites in PRPF8 (c.C5792T) using an adenine base editor and then generated an isogenic control (CSUASOi006-A-1), which is a valuable cell resource for research of RP.
