Effects of Low-Dose Acetylsalicylic Acid on Spontaneous Pain, Pain Sensitivity, and Central Pain Modulation in Response to a Sleep Restriction Challenge in Healthy Individuals.

BACKGROUND: Poor sleep and pain perpetuate each other through a feed-forward cycle. While mechanisms driving this cycle remain poorly understood, they appear to involve inflammatory pathways. We investigated whether pre-emptive administration of low-dose acetylsalicylic acid (ASA, aspirin), which targets multiple inflammatory pathways, attenuates the pain response to a sleep restriction challenge in healthy adults. METHODS: Forty-six participants completed a randomized, double-blind crossover protocol consisting of three conditions: sleep restriction/ASA, sleep restriction/placebo, and contro

Phenotyping Interstitial Cystitis/Bladder Pain Syndrome: Clinical and Mechanistic Evidence, Candidate Biomarkers, and Implications for Management.

Interstitial cystitis and bladder pain syndrome are clinically overlapping conditions that should be distinguished according to Hunner lesion status. IC with Hunner lesions is a distinct inflammatory bladder disease characterized by urothelial denudation, lymphoplasmacytic infiltration, adaptive immune activation, and progressive structural impairment. The intensity and distribution of these inflammatory and structural abnormalities may vary among patients with IC, but the underlying pathological pattern is comparatively coherent. BPS without Hunner lesions is substantially more heterogeneous.

Construction of the chronic temporomandibular disorder patients: the association between neural and psychological pathways.

Chronic temporomandibular disorder (cTMD) as a term based on the diagnostic criteria for temporomandibular disorders (DC/TMD) classification refers, in this paper, to the condition listed that has a non-mechanical association without any obvious organic cause. Specifically, this is the condition that falls under the International Classification of Diseases 11th revision (ICD-11) classification of chronic primary and chronic secondary pains. This implies that there is increased responsiveness of nociceptive neurons in the central nervous system, a phenomenon known as central sensitisation. cTMD

Regenerative Medicine Approaches to Craniofacial and Corneal Neuropathic Pain.

Craniofacial and corneal neuropathic pain are disabling conditions characterized by persistent pain that is frequently refractory to conventional pharmacologic and interventional therapies. These disorders arise from complex interactions between peripheral nerve injury, neuroinflammation, and maladaptive central sensitization within trigeminal pathways, features that span neuropathic and nociplastic pain mechanisms as defined by the International Association for the Study of Pain, thus emphasizing the need for mechanism-based, patient-stratified treatment strategies. Regenerative medicine offe

AAV-Mediated Overexpression of Prdm12 in Knee-Innervating Afferents Reduces Inflammatory Joint Pain and Neuronal Hyperexcitability in Female Mice.

Inflammatory joint pain features in numerous musculoskeletal disorders that affect millions globally. The Prdm12 gene encodes a conserved zinc finger transcriptional regulator expressed selectively in the nervous system. In humans, PRDM12 mutations can cause congenital insensitivity to pain (CIP) or midface toddler excoriation syndrome (MiTES). Prdm12 is prominently expressed in developing somatosensory ganglia, where it plays a crucial role in nociceptive neuron development, its expression being maintained in mature C-LTMRs (C-low-threshold mechanoreceptors) and nociceptive neurons. Despite e

Low-dose naltrexone for treatment of pain in patients with fibromyalgia: a randomized, double-blind, placebo-controlled, crossover study.

INTRODUCTION: Fibromyalgia (FM) is a chronic fluctuating, nociplastic pain condition. Naltrexone is a µ-opioid-receptor antagonist; preliminary studies have indicated a pain-relieving effect of low-dose naltrexone (LDN) in patients with FM. The impetus for studying LDN is the assumption of analgesic efficacy and thus reduction of adverse effects seen from conventional pharmacotherapy. OBJECTIVES: First, to examine if LDN is associated with analgesic efficacy compared with control in the treatment of patients with FM. Second, to ascertain the analgesic efficacy of LDN in an experimental pain m

Preliminary safety and effectiveness of psilocybin-assisted therapy in adults with fibromyalgia: an open-label pilot clinical trial.

INTRODUCTION: Fibromyalgia (FM) is the prototypical nociplastic pain condition, characterized by widespread pain and issues with cognition, mood, and sleep. Currently, there are limited treatment options available that effectively treat FM symptoms. Psilocybin-assisted therapy (PAT) is an emerging combined drug-therapy intervention, but no studies to-date have investigated PAT for FM. METHODS: Here, we report findings from an open-label, pilot clinical trial of PAT for FM (N = 5). In conjunction with psychotherapy (two preparatory, four integration sessions), participants received two doses o

Validation of ICD-9 Codes for Identification of Chronic Overlapping Pain Conditions.

Chronic overlapping pain conditions (COPCs) are a collection of chronic pain syndromes that often co-occur and are thought to share underlying nociplastic pathophysiology. Since they can manifest as seemingly unrelated syndromes they have historically been studied in isolation. Use of International Classification of Diseases (ICD) codes in medical records has been proposed as a means to identify and study trends in COPCs at the population level, however validated code sets are needed. Recently, a code set comprising ICD-10 codes as proxies for 11 COPCs was validated. The goal of this project w

Implementation of Ultrasound-Guided Musculoskeletal Injections in a South Trinidad Medical Practice: A Perspective From a Resource-Limited Setting.

Musculoskeletal pain (MSP) is a general term for pain varying by source, characterized into nociceptive (tissue pain), neuropathic (nerve pain), and nociplastic (centralized pain). It affects muscles, bones, joints, tendons, and ligaments due to multiple etiologies, including trauma, degenerative changes, or autoimmune and inflammatory disorders, presenting as either acute or chronic pain with symptoms such as sharp, dull, tingling, or burning sensations. It is a leading cause of disability globally, particularly in low and middle-income countries. We want to highlight a real-world implementat

The Microbiome-Brain Axis in Burning Mouth Syndrome and Its Comorbidities: An Integrated Perspective.

Burning mouth syndrome (BMS) is a chronic nociplastic pain condition of unknown etiology, predominantly affecting women and highly comorbid with depression and anxiety. This narrative review proposes a comprehensive model integrating the significant role of the oral and intestinal microbiomes in BMS pathology. We explore how microbial dysbiosis compromises barrier functions, leading to systemic and neuroinflammation, and subsequently modulates key brain networks involved in pain processing and emotional regulation. Evidence suggests that dysbiosis within the oral microbiome (e.g., increased St

Altered serum bile acid profile in fibromyalgia is associated with specific gut microbiome changes and symptom severity.

Alterations in the composition and function of the gut microbiome in women with fibromyalgia have recently been demonstrated, including changes in the relative abundance of certain bile acid-metabolizing bacteria. Bile acids can affect multiple physiological processes, including visceral pain, but have yet to be explored for association to the fibromyalgia gut microbiome. In this study, 16S rRNA sequencing and targeted metabolomic approaches were used to characterize the gut microbiome and circulating bile acids in a cohort of 42 women with fibromyalgia and 42 healthy controls. Alterations in

Differences Among Veterans With Chronic Overlapping Pain Conditions and Other Chronic Pain: Baseline Results From the LAMP Pain Management Trial.

BACKGROUND: Chronic overlapping pain conditions (COPCs) are a constellation of disorders posited to share an underlying pain mechanism (nociplastic pain). Unfortunately, individuals with COPCs are under-represented in clinical research. The current study aimed to determine COPC prevalence among participants enrolled in a pragmatic clinical trial and compare those with COPCs to those with non-COPCs across several domains. METHODS: Learning to Apply Mindfulness to Pain (LAMP) study baseline data of veterans with chronic pain (N = 811) were utilised. COPC diagnoses were determined using ICD-10 c

A biopsychosocial approach to phenotyping people with knee osteoarthritis awaiting total knee arthroplasty: A secondary cohort analysis.

BACKGROUND: Previous research showed chronic post-total knee arthroplasty (TKA) pain in 20% of people with knee osteoarthritis (KOA). Various preoperative biopsychosocial-related factors have been described, but phenotyping people with KOA awaiting TKA based on these factors is still lacking. This could be relevant to understanding differences in TKA surgery responses. OBJECTIVE: To identify phenotypes in people with KOA awaiting TKA and differences in post-TKA pain based on preoperative biopsychosocial factors. METHODS: People with KOA awaiting TKA in 4 hospitals in Belgium and the Netherla

16. Pain in chronic pancreatitis.

INTRODUCTION: Chronic pancreatitis is defined as a disease of the pancreas in which recurrent inflammatory episodes result in replacement of the pancreatic parenchyma by fibrous connective tissue in individuals with genetic, environmental, and other risk factors. Pain is one of the most important symptoms of chronic pancreatitis and, in many cases, has chronic visceral nociceptive, nociplastic, and even neuropathic components, with evidence of both central and peripheral sensitization, neuroplasticity, and neurogenic inflammation. METHODS: The literature on the diagnosis and treatment of pain

Fibromyalgia and Central Sensitization in Rheumatoid Arthritis, Psoriatic Arthritis, and Spondyloarthritis: A Systematic Review.

Fibromyalgia (FM) and central sensitization are increasingly recognized as clinically relevant in adults with inflammatory arthritis and spondyloarthritis, particularly when persistent pain and patient-reported symptoms complicate disease activity assessment. This systematic review synthesized clinical evidence on FM and central sensitization in adults with rheumatoid arthritis (RA), psoriatic arthritis (PsA), axial spondyloarthritis (axSpA), ankylosing spondylitis (AS), and other spondyloarthritis (SpA) populations. FM and central sensitization were treated as the primary concepts of interest

Association Between Diagnostic Imaging, Medication Intake, and Health Outcomes in Chronic Whiplash-Associated Disorders: An Observational Study.

Purpose: Studies have demonstrated that medication and diagnostic imaging (DI) use and, more broadly, health care resource utilization, are not necessarily concordant with clinical practice guidelines. However, these studies did not evaluate the concurrent presence of clinical manifestations. This study therefore aimed to investigate the association between medication intake and DI, and health outcomes including pain, disability, physical, and mental health-related quality of life in people with chronic whiplash-associated disorders (WADs). We also aimed to evaluate whether medication intake a

Personalized assessment and management of non-specific low back pain.

BACKGROUND AND OBJECTIVE: Low back pain (LBP), and in particular non-specific low back pain (NSLBP), which accounts for approximately 90% of LBP, is the leading cause of years lived with disability worldwide. In clinical trials, LBP is often poorly categorized into 'specific' versus 'non-specific' and 'acute' versus 'chronic' pain. However, a better understanding of the underlying pain mechanisms might improve study results and reduce the number of NSLBP patients. DATABASES AND DATA TREATMENT: Narrative review. RESULTS: NSLBP is a multi-dimensional, biopsychosocial condition that requires al
