Endodermal Cyst with a Non-enhancing Nodule in the Quadrigeminal Cistern Developed in an Octogenarian.

BACKGROUND: Intracranial endodermal cysts are congenital lesions that generally develop in the cerebellopontine angle and ventral brainstem of the posterior fossa, whereas endodermal cysts in the quadrigeminal cistern are very rare. We report a rare case of an endodermal cyst in the quadrigeminal cistern with a non-enhancing nodule that developed in patient over 80 years of age. CASE DESCRIPTION: An 85-year-old man presented to our hospital with progressing gait disturbance and urinary incontinence. Preoperative images showed a cystic mass lesion with a nodule in the quadrigeminal cistern and

Acute type A aortic dissection repair in an octogenarian with achondroplasia: a case report.

BACKGROUND: Achondroplasia is an inherited disorder and the most common type of short-limbed dwarfism in human beings, affecting more than 250,000 individuals worldwide. To the best of our knowledge, no study has reported a correlation between achondroplasia and aortic dissection. Here, we report a rare case of acute type A aortic dissection repair in a patient with achondroplasia. CASE PRESENTATION: An 82-year-old Japanese female with achondroplasia was admitted to our hospital because of acute-onset severe chest pain migration to her back accompanied by numbness and pain in the right lower

Successful balloon pulmonary valvuloplasty for congenital valvular pulmonary stenosis in an octogenarian.

While balloon valvuloplasty has been widely used for the treatment of congenital valvular pulmonary stenosis (PS) in children and adults, its use in elderly patients is less common. An 80-year-old woman with congenital valvular PS received valvuloplasty with double-balloon technique. Right ventricle systolic pressure and pulmonary valve systolic pressure gradient decreased from 95 to 44 mm Hg and from 75 to 35 mm Hg, respectively. Follow-up Doppler echocardiography 2 months later showed further decrease in the transvalvular systolic pressure gradient to 29 mm Hg. The patient had symptomatic re

Parathyroidectomy for primary hyperparathyroidism in octogenarians and nonagenarians: a plea for early surgical referral.

BACKGROUND: Surgical referral of elderly patients with PHPT is often deferred because of the perceived risks of general anesthesia, increased age, and comorbidities and also because many of the symptoms of PHPT are difficult to distinguish from the aging process. HYPOTHESIS: Parathyroidectomy for primary hyperparathyroidism (PHPT) in elderly patients is beneficial and safe. DESIGN: Retrospective study of patients with PHPT who were 80 years and older. SETTING: Tertiary academic referral medical center. PATIENTS: Fifty-four consecutive patients (43 women and 11 men) with a mean age of 83.6

Vascular surgical society of great britain and ireland: limb salvage in the octogenarian.

BACKGROUND: Critical limb ischaemia (CLI) presents a unique set of problems in the elderly patient. Foremost among these is the much greater likelihood of loss of independence and reduced quality of life if major amputation is required. For this reason it has been this unit's policy to attempt reconstructive vascular surgery in almost all cases of CLI. The outcome of this policy was examined. METHODS: All patients had surgery performed under one consultant and data were entered prospectively on to a database. RESULTS: Risk factors included diabetes (17 per cent), smoking (78 per cent) and isch

Congenital coronary arteriovenous fistula associated with atrioventricular valvular regurgitation in an octogenarian.

An arteriovenous fistula originating from halfway down the circumflex coronary artery, draining into the coronary sinus was first diagnosed in a patient of 81. For 10 years, she had had dyspnea on exertion, mild mitral regurgitation and permanent ventricular pacing for symptomatic bradyarrhythmia. She was treated medically and despite her age was offered transcatheter balloon embolization, but she refused to undergo such intervention.

Age is Just a Number: Outcomes of Robotic Hiatal Hernia Repair in Octogenarians.

BACKGROUND: Robot-assisted hiatal hernia repair has become increasingly popular in high-risk populations as a relatively low-risk, minimally invasive surgical option. As life expectancy rises, more octogenarians are being considered for surgery despite age-related comorbidities. However, data on the safety and efficacy of robotic repair in this population are limited. This single-center retrospective study evaluates postoperative outcomes and complications after robotic hiatal hernia repair in octogenarians. METHODS: Patients aged ≥80 years who underwent repair between 2017 and 2024 were prop

Reevaluating Internal Thoracic Artery Use in Octogenarians Undergoing Multivessel Coronary Artery Bypass Grafting.

BACKGROUND: Internal thoracic artery (ITA) use is a benchmark of quality in coronary artery bypass grafting (CABG), yet its value in contemporary practice among octogenarians remains uncertain. We compared mid-term major adverse cardiac events and all-cause mortality among octogenarians undergoing CABG with ITA plus saphenous vein grafts (SVGs) vs SVG-only. METHODS: We identified 13,925 octogenarians who underwent isolated first-time elective CABG with 2-4 grafts from 2015 to 2022 in the US Centers for Medicare and Medicaid database validated against individual clinical records: 12,280 receiv

Radical cystectomy in octogenarians: a propensity score-matched analysis of short-term outcomes.

PURPOSE: This study aims to compare the impact of biological versus chronological age on postoperative risk after radical cystectomy (RC) by evaluating whether age ≥ 80 years independently predicts short-term mortality, complications and readmission rates beyond physiological status and comorbidity burden. METHODS: We retrospectively analysed 879 patients undergoing open RC at a high-volume centre between 2015 and 2024. Propensity score matching (PSM; 1:1 nearest neighbor) was performed to balance octogenarians (80-89 years) and patients < 80 years for key covariates including ASA score, CCI

Polatuzumab vedotin and obinutuzumab combination in octogenarians with B-cell non-Hodgkin lymphoma.

The outcomes of elderly patients with lymphoma are poor. There is scarcity of data on outcomes of chemotherapy-free regimens in patients older than 80 years with a B-cell lymphoma. We report three octogenarians with B-cell NHL and the efficacy and safety of a nonchemotherapy regimen using a lower dose of polatuzumab vedotin and anti-CD20 monoclonal antibody. No infusion-related adverse events were noted. No patient developed neutropenia. Peripheral neuropathy was grade 2 in all cases. Recurrent UTI were a concern in two patients. After a median of 6 cycles (range 1-6) of polatuzumab therapy, a

Robotic Totally Endoscopic Mitral Valve Surgery in Octogenarians: Age Is Just a Number!

OBJECTIVE: Octogenarians are sometimes excluded from mitral valve (MV) surgery due to increased morbidity and mortality. Sternal-sparing approaches (e.g., robotic MV surgery) have the potential to reduce these risks. We reviewed the outcomes of robotic totally endoscopic MV surgery in octogenarians. METHODS: Of 600 patients undergoing elective robotic totally endoscopic MV surgery at our institution (September 2013 to December 2024), 40 patients were octogenarians. Early and midterm clinical and echocardiographic outcomes were analyzed. All procedures were performed using the da Vinci Si or X

Tafamidis in octogenarians with wild-type transthyretin cardiac amyloidosis: an international cohort study.

BACKGROUND AND AIMS: In real-world, wild-type transthyretin cardiomyopathy is increasingly diagnosed in patients ≥ 80 years old (octogenarians), although being underrepresented in randomized clinical trials. Specific data on natural course and outcome under tafamidis treatment in octogenarians are therefore scarce. The impact of tafamidis treatment on mortality in real-world wild-type transthyretin cardiomyopathy octogenarians was studied. METHODS: An international, multicentre cohort study of 710 consecutive wild-type transthyretin cardiomyopathy patients with mean follow-up of 2.2 ± 1.8 yea

Investigation of risk factors and outcomes of aortic arch aneurysm repair in octogenarians.

OBJECTIVE: We reviewed the treatment outcomes for aortic arch aneurysms in elderly patients aged > 80 years, and discussed the risk factors for each technique. METHODS: Octogenarians who underwent aortic arch aneurysm repair between 2007 and 2021 were included. Fifty-four patients (23 in the total arch replacement [TAR] group and 31 in the thoracic endovascular aortic repair [TEVAR] group) were included in the study. The early- and mid-term outcomes and risk factors for all-cause mortality were examined in each group. To examine timely surgical outcomes, cases of true aneurysms were included,

Open Repair for Nonruptured Abdominal Aortic Aneurysm in Octogenarians With Prior Abdominal Surgery.

INTRODUCTION: Previous abdominal surgery (PAS) has been shown to potentially affect outcomes of open surgical repair (OSR) for abdominal aortic aneurysm (AAA). Octogenarians have been observed to face significantly higher risks following OSR for AAA, which highlights the need for a more cautious selection of their surgical candidacy. This study aimed to assess the association between PAS and the 30-d outcomes of OSR for nonemergent intact AAA among octogenarians. METHODS: Octogenarians (aged ≥80 y) who underwent OSR for AAA were identified in American College of Surgeons National Surgical Qua

Results of Spine Surgeries in Nonagenarians Compared with Octogenarians: A Propensity-Matched Analysis.

OBJECTIVE: While spinal surgery is common in octogenarians, it is less frequent in nonagenarians. Most research highlights complications and mortality, but few studies compare actual recovery outcomes between these groups. This study examines how spinal surgery outcomes differ between nonagenarians (people in their 90s) and octogenarians (people in their 80s). METHODS: This study included patients aged 80-99 years who underwent spinal surgery between 2017 and 2022 at Kyoto University Hospital and affiliated institutions. Coronary artery bypass grafting in octogenarians: a nomogram for predicting all-cause mortality.

BACKGROUND: The benefits and risks of coronary artery bypass grafting (CABG) in octogenarians remain unclear. This study aimed to identify the predictors of increased risk of all-cause mortality in octogenarian patients after CABG. METHODS: We retrospectively analyzed the data of 1636 octogenarians who underwent isolated elective on-pump CABG between 2007 and 2016. The primary endpoint was mortality from any cause. The Kaplan-Meier curve was generated for mortality. A univariate Cox regression was performed for preprocedural and procedural variables. The Akaike information criterion (AIC) usi
