Atlantoaxial instability in Marfan's syndrome. Diagnosis and treatment. A case report.

A case of Marfan's syndrome is described in which progressive neurological signs and symptoms were produced by flexion of the head. Radiological examinations revealed hypermotility between the atlas and axis, as well as compression of the medulla oblongata by the odontoid process on flexion, since low positioned cerebellar tonsils prevented dorsal shift of the oblongata. The signs and symptoms disappeared after occipitocervical internal transfixation.

Retroflexed dens in paediatric Chiari 1 patients and implications: Single centre retrospective study.

INTRODUCTION: Chiari 1 malformation and hind brain hernia can be associated with skull base and craniocervical anomalies. One of the more recently associated anomalies is a retroverted or retroflexed odontoid process or dens. We conducted a retrospective study of our operated symptomatic and conservatively managed asymptomatic Chiari cohort to assess the impact of dens retroflexion on rate of revision or cerebrospinal fluid diversion following primary foramen magnum decompression (FMD). METHODS: We undertook a retrospective study of all foramen magnum decompression (FMD) cases for Chiari type

Chronic atlanto-axial dislocation with spontaneous bony fusion.

A case of long standing atlanto-axial subluxation following relatively minor trauma is reported. Progressive development of pain and spastic quadriparesis led to clinical and radiological investigation at Groote Schuur Hospital which revealed a spontaneous fusion of C1 and C2 with marked compromise of the spinal cord. The possibility of a pre-existing unfused odontoid process (os odontoideum) is discussed. A brief review of the literature is presented.

[Atloaxoid dislocation with basilar pressure and spinal cord symptoms in rheumatoid arthritis. A case of occipitocervical arthrodesis followed by resection of the odontoid].

The authors report a case of atlanto-axial dislocation in a patient with rheumatoid arthritis. There was some neurological impairment with upper limb pain and hyperreflexia. As a first stage, a cranio-cervical fusion was performed through a posterior approach. Three weeks later the axis was excised through an antero-lateral approach, together with its odontoid process. The patient's condition was much improved and the neurological lesions disappeared.

Management of C2 fractures following multiple classifications, a narrative review.

INTRODUCTION: Classifications are helpful for surgeons as they can be a resource for decision-making, often providing the individual indicators that may deem a case necessary for surgery. However, when there are multiple classifications, the decision-making might be compromised. That is the case with C2 fractures. For this reason, this study was designed to review the different classifications of axis fractures. RESEARCH QUESTION: What are the most commonly used classifications for C2 fractures, and how do these classifications compare in terms of clinical utility? METHODS: A systematic lite

The crowned dens syndrome: a rare form of calcium pyrophosphate deposition disease.

The crowned dens syndrome has been termed as acute neck pain ascribed to CPPD deposits associated with a tomographic appearance of calcification surrounding the odontoid process. This rare entity resulting in cervical cord compression is generally seen in older female patients. We present a 26-year-old woman with cervical cord compression due to massive calcification in the periodontoid area and discuss the X-ray and CT findings of the disease.

Craniofacial and mucopolysaccharide abnormalities in Kniest dysplasia.

Serial roentgencephalograms of a male patient with Kniest dysplasia were obtained between 1 7/12 and 11 3/12 years of age and were analyzed and compared to cephalometric normative data. The patient displayed macrocephaly with increased size of the neurocranium in all three dimensions. The cranial base angle was significantly flattened, partly as a result of anterior displacement of the sella turcica. The odontoid process was short and wide. At 11 years of age there was bony fusion between the anterior arch of the atlas and the odontoid process as well as between the posterior arch of the atlas

A rare case of Parkinson's disease with severe neck pain owing to crowned dens syndrome.

BACKGROUND: Pain is regarded as one of the most common nonmotor symptoms in Parkinson's disease (PD). In particular, musculoskeletal pain has been reported as the most common type of PD-associated pain. Crowned dens syndrome (CDS), related to microcrystalline deposition in the periodontoid process, is the main cause of acute or chronic cervical pain. CASE PRESENTATION: This report describes the case of an 87-year-old woman who had severe bradykinesia, muscle rigidity, gait disturbance and neck pain. Laboratory examination revealed marked elevations of white blood cells (10,100/µl) and C-react

Real-time ultrasound guidance in the endoscopic endonasal resection of a retro-odontoid pannus: Technical note and case illustration.

BACKGROUND AND OBJECTIVES: Odontoidectomy is a surgical procedure indicated in the setting of various pathologies, with the main goal of decompressing the ventral brain stem and spinal cord as a result of irreducible compression at the craniovertebral junction. The endoscopic endonasal approach has been increasingly used as an alternative to the transoral approach as it provides a straightforward, panoramic, and direct approach to the odontoid process. In addition, intraoperative ultrasound (US) guidance is a technique that can optimize safety and surgical outcomes in this context. It is used

Operative versus nonoperative management of type II odontoid fractures in the elderly: A systematic review and meta-analysis of comparative studies.

BACKGROUND: Type II odontoid fractures (OFs) are common cervical spine injuries in elderly patients. Management strategies remain controversial, as both operative and nonoperative approaches carry distinct risks and benefits. METHODS: A systematic search of PubMed, Scopus, Cochrane Library, and Google Scholar was conducted through August 2025. Studies comparing patients aged ≥ 65 years with Type II OFs treated either operatively or nonoperatively were included. Outcomes were analyzed based on follow-up periods and included clinical measures (length of hospital stay (LOS), residual pain), radi

Anterior endoscopically assisted bone grafting for iatrogenic distraction of odontoid fracture after percutaneous anterior screw fixation: A case report.

RATIONALE: The complication of iatrogenic distraction of odontoid fracture after anterior screw fixation has not been reported in the literature. We treated the patient with endoscopically assisted bone grafting with good results. The new technique was not reported in the management of odontoid fracture or nonunion before. PATIENT CONCERNS: A 22-year-old man presented with neck pain after a motorcycle crash. The cervical spine radiograph and computed tomographic scan demonstrated the base of dens displaced 2 mm anteriorly. DIAGNOSES: Radiographic images showed a type II odontoid fracture. I

Atlantooccipital Overlap and Other Craniocervical Junction Abnormalities in Dogs.

The term craniocervical junction abnormality (CJA) is an umbrella term for a variety of malformations that occur in the craniocervical region. These abnormalities include Chiari-like malformation, atlantooccipital overlapping, atlantoaxial instability, occipitoatlantoaxial malformations, atlantoaxial dural bands, and dens abnormalities. Syringomyelia can occur secondary to any of these malformations or a combination. Clinical signs in dogs with a CJA typically include neck pain and varying signs associated with a cervical myelopathy. MRI of CJAs with computed tomography imaging is necessary fo

[A 65-year-old patient with an odontoid fracture caused by 'headbanging'].

A 65-year-old woman who habitually made flexion-extension movements with her cervical vertebral column (so-called 'headbanging') was seen with complaints of the neck. X-rays of the neck revealed an odontoid fracture. Bone densitometry revealed lowered bone mineral density (osteopenia). The diagnosis was odontoid fracture due to headbanging, in the presence of osteopenia. Patient recovered after treatment with a neck collar worn for 6 months.

Pseudogout showing meningoencephalitic symptoms: crowned dens syndrome.

We report a 70-year-old man who presented symptoms resembling those of meningoencephalitis and who was subsequently diagnosed as having a crowned dens syndrome. The patient exhibited severe neck pains, headache, high fever and a pain in his knee joints together with symptoms of the central nervous system. The patient's cerebrospinal fluid was almost clear and showed no sign of viral infection. An analysis of the synovial fluid in the right knee joint revealed typical calcium pyrophosphate dehydrate crystal deposition and a diagnosis of pseudogout was therefore made. A tomographic examination o

Traumatic injuries of the c1/c2 complex: computed tomographic imaging appearances.

Cervical spine fractures occur in 1 to 3% of trauma cases and account for two-thirds of spinal cord injuries. Injury to the C1/C2 complex accounts for 19 to 25% of cervical spine injuries. Fractures of C2 occur most frequently, 55% of which involve the odontoid peg. The unique anatomical relationship between the atlas and axis produces a variety of injury patterns not seen elsewhere in the spine. Numerous developmental anomalies also occur which can mimic trauma. Well-described fracture patterns include posterior arch fractures of the atlas, Jefferson type fractures, fractures of the odontoid

Rare atlantoaxial dislocation secondary to os odontoideum: A comparative study of orthotopic and dystopic variants with two representative cases.

Atlantoaxial dislocation often results in upper cervical spinal cord compression, which may lead to significant neurological impairment. Among these, os odontoideum (OO) is a rare anomaly where the odontoid is replaced by a separate ossicle. Case one describes a 36-year-old male with dystopic OO presented a 4-month history of progressive incomplete quadriparesis. Radiological evaluation confirmed the presence of atlantoaxial dislocation associated with basilar invagination and OO, demonstrating an ossified structure with functional fusion between the free odontoid segment and the basion.
