We report on a spermatic cord liposarcoma arising in a 16-year-old boy. Treatment consisted of radical orchiectomy without retroperitoneal lymph node dissection, and adjuvant radiation and chemotherapy. The patient has survived 14 months without evidence of recurrence. Paratesticular localization of liposarcoma in patients less than 20 years old is rare. The tumor has no particular characteristic. Treatment consists of radical orchiectomy and the role of retroperitoneal lymphadenectomy is controversial.

Two cases of epidermoid cysts are described. Benign tumours of the testis are rare pathology entities accounting for less than 1-3% of all testicular tumours. Epidermoid cysts of the testis are included in this group of tumours and this pathology is most common in young adults. 85% of these cases, reported in the literature, are treated with an orchiectomy. The Authors describe an orchiectomy in an adult and a simple enucleation of the cyst in a young man; in the end the Authors draw their conclusion.

PURPOSE: This case report aims to enhance the understanding of clinical physicians regarding splenogonadal fusion (SGF) and to help them consider SGF as a differential diagnosis when testicular tumors are suspected, thus avoiding unnecessary orchiectomies.

A 32-year-old man presented with severe scrotal pain after vasectomy. Scrotal ultrasound showed the presence of enlargement and decreased echogenicity of more than the upper two-thirds of the right testis. After orchiectomy, scrotal pain disappeared. This is the first report of simple orchiectomy after subtotal testicular infarction due to a suture tie of vasectomy. During vasectomy, it is important to dissect the bare vas to minimize vascular injury.

PURPOSE: We report on 4 patients who presented with nonpalpable testicular masses discovered on scrotal ultrasound, and offer treatment recommendations based on our experience and a review of the literature.
MATERIALS AND METHODS: Four patients underwent intraoperative ultrasound guided localization and excisional biopsy of nonpalpable testicular parenchymal masses. We present a case report of a relapse to the mediastinal lymph node alone, 38 months after orchiectomy on surveillance for stage I typical seminoma. The patient, a 63-year-old man, was treated with high inguinal orchiectomy for left testicular tumor and close follow-up. We report a case of primary testicular carcinoid tumour with mature teratoma metastatic to the para-aortic lymph node and the lymph node around the left gonadal vein, which was treated with radical orchiectomy, bleomycin, etoposide, and cisplatin chemotherapy, and modified retroperitoneal lymph node dissection. Three days after modified retroperitoneal lymph node dissection, bleomycin induced pneumonitis occurred, which was resolved with steroid administration. The patient is alive without recurrence 31 months after radical orchiectomy.

Malignant paratesticular tumors are uncommon. Of these, liposarcoma of the spermatic cord constitutes a rare tumor type with a good prognosis. It has a high rate of survival over 5 years. Inguinal orchiectomy generally suffices, although patients should be followed closely since local recurrence is not uncommon. The present study briefly reviews the literature on spermatic cord liposarcoma and reports an additional case that was treated exclusively by radical orchiectomy. Four years postoperatively the patient continues to be tumor-free.

The majority of solid testicular tumors are treated with orchiectomy given the high risk of malignancy. We present a case of a testicular mass in an adult patient in the setting of recent hand, foot, and mouth disease that was managed conservatively with serial ultrasounds. Even though cases of viral-associated testicular masses are rare, this differential diagnosis should be considered in patients with a new testicular mass in the setting of recent viral infection and negative tumor markers. For these patients, observation may be an option instead of immediate orchiectomy.

A case of primary testicular plasmacytoma complicated with hydrocele of the testis is reported. An 86-year-old man presented with hydrocele of the right testis. High inguinal orchiectomy was performed as the preoperative aspiration cytology of the hydrocele fluid showed atypical cells. Immunohistochemical study of the right testis revealed testicular plasmacytoma positive for IgG. He remained well 9 months after the orchiectomy. This is the second reported case where the preoperative diagnosis of testicular plasmacytoma was made based on the hydrocele fluid cytology.
