External fixation in multiply injured children.

External fixation treatment for selected fractures can be as readily applied to children as to adults, although only a small percentage of pediatric fractures require this method. The best situations for the use of external fixation in children are in cases of multiple organ system injuries and in fractures associated with skin loss or large wounds. In these situations, while the child's other problems are being treated, external fixation provides excellent temporary stabilization to facilitate fracture management.

[Evaluation of the degree of infundibuliform abnormality of the thorax and technics of the fixation of the sternum].

The author proposes an original method of determination of the degree of deformation in angular values on the basis of his experience in the treatment of 27 patients with infundibulum-like deformation of the chest. In case of surgical treatment the method of thoracoplasty after N. I. Kondrashin is preferable. The design formula of the size of the resected rib area is proposed. To stabilize the framework of the chest fixation by crossing wires on the ring support after G. A. Ilizarov is suggested. Submerged crossing wires with bent supporting ends were used for more continuous fixation.

Conservative management of fractures of the mandibular condyloid process in three cats and one dog.

Three cats and 1 dog with fracture of the mandibular condyloid process and concomitant fracture(s) of the rostral portion of the mandible were treated successfully by wire fixation of the rostral mandibular fracture(s) and by conservative management of the mandibular condylar fracture. All animals regained satisfactory dental occlusion and normal, pain-free motion of the jaw. Fracture of the mandibular condyloid process is an infrequently diagnosed injury that usually occurs with other mandibular fractures. If satisfactory dental occlusion can be achieved by surgical stabilization of the nonco

Cotrel-Dubousset instrumentation in idiopathic scoliosis. A preliminary report.

This is a preliminary report on Cotrel-Dubousset (CD) instrumentation for the surgical management of idiopathic thoracic scoliosis. From September 1985 through April 1986, 37 patients were treated at the authors' hospital, by posterior spinal fusion with CD instrumentation. Twelve patients had surgical treatment of spinal deformity associated with other disorders or had revision surgery. The remaining 25 patients, with no prior surgery, were diagnosed as having juvenile or adolescent idiopathic scoliosis. After operation, this group of patients was routinely noted to have significant improveme

Skeletal stabilization for digital replantation surgery. Use of intraosseous wiring.

Bony fixation is the first priority in any digital replantation procedure. Because of time considerations, however, fracture stabilization is frequently less than optimal and impairs mobilization and rehabilitation significantly. Ordinary methods of digital fracture stabilization have proven inadequate when they are applied to replantation surgery. The technique of intraosseous wiring provides an additional option in the management of bone fixation in certain circumstances, and it may be used alone or in conjunction with a variety of other methods. Familiarity with the advantages and disadvant

Murine Aseptic Surgical Model of Femoral Atrophic Nonunion.

Although bone repair is typically an efficient process, an inadequate healing response can occur, with approximately 5-20% of fractures developing nonunion. Even with improved healing strategies and external fixation devices, overall rate of nonunion has not been significantly reduced, particularly for atrophic nonunion. Atrophic nonunion is characterized by sparse or no callus formation and is difficult to treat clinically, resulting in long-term pain and functional limitation. Reliable preclinical models are needed to study the pathophysiology of atrophic nonunion to create better treatment

Vertebral body clinico-morphological features following percutaneous vertebroplasty versus the conservatory approach.

Most percutaneous vertebroplasty procedures are being performed in order to relieve pain in patients with severe osteoporosis and associated stable fractures of one or more vertebral bodies. In addition, vertebroplasty is also recommended for patients suffering from post-traumatic symptoms associated with vertebral fractures, patients with large angiomas positioned inside the vertebral body, with an increased risk for collapse fracture and also patients presenting with pain associated with vertebral body metastatic disease. On another aspect, it is possible that in isolated cases, an orthopedi

The Difficult Supracondylar Humerus Fracture: Flexion-Type Injuries.

Although flexion-type supracondylar humerus fractures account for a minority of all supracondylar humerus fractures, they warrant special attention because of their relatively high rate of requirement for open reduction and their potential for ulnar nerve injury or entrapment. The severity of flexion-type supracondylar humerus fractures may be difficult to appreciate on initial radiographs; therefore, surgeons must have a high index of suspicion in the evaluation of a patient who has a suspected flexion-type supracondylar humerus fracture. Nondisplaced or minimally displaced flexion-type supra

[Effect of Coflex interspinous stabilization and vertebral arch pedicle screw implantation on the stability of three-dimensional motions of the lumbar spine].

OBJECTIVE: To evaluate the effect of Coflex interspinous stabilization and vertebral arch pedicle screw implant on the stability and motion of the lumbar spine. METHODS: The range of motion (ROM) of 6 fresh adult human cadaver lumbar spine specimens (L(1) approximately S(1)) was biomechanically tested in forward flexion/extension stretch, left/right lateral bending and left/right axial rotation. The ROM and neutral zone (NZ) of the segments L(2/3), L(3/4), and L(4/5) were measured and compared in 3 conditions, namely intact condition (a), rigid fixation of the segment L(4/5) with vertebral ar

A further development in spinal instrumentation. Technical Commission for Spinal Surgery of the ASIF.

The purpose of this paper is to describe the concepts and use of a new, versatile instrumentation system, the universal spine system (USS), that has been designed to have wide application for pathology of the thoracolumbar spine. Many instrumentation systems for thoracolumbar spinal surgery now exist that were each originally designed to address specific areas of spinal pathology. For example, the recent systems designed to treat scoliosis deformities do not provide the instrumentation and implant support to adequately address other spinal disorders, such as fractures. In addition, most poster

Contoured Harrington instrumentation in the treatment of unstable spinal fractures. The effect of supplementary sublaminar wires.

Twenty-nine patients with major fractures and fracture-dislocations of the thoracic and lumbar spine were treated by spinal fusion and contoured Harrington distraction instrumentation. Two groups of patients were identified. Group I (16 patients) had sublaminar wires. Group II (13 patients) had no wiring. Satisfactory alignment was achieved in all patients. There were several complications in both groups. None of the patients in Group I had postoperative cast immobilization. The addition of sublaminar wiring appears to eliminate the need for rigid external immobilization and is beneficial only

Treatment by external fixation of open fractures associated with severe soft tissue damage of the leg. Biomechanical principles and clinical experience.

External fixation is the treatment of choice for fractures of the leg involving severe damage to the soft tissue. Each of the various patterns of application has advantages and disadvantages. Based on various biomechanical studies and clinical experience, the authors use a right-angle arrangement for the screws. If the type of fracture permits, internal and external fixation may significantly increase stability. During the period from 1978 to 1980, 56 patients with second- and third-degree open fractures were treated. A combination of external and internal fixation was used in 42 patients. Of

[Primary treatment of clubfoot].

Our primary club foot therapy consists of a combination of plaster cast manipulation, physiotherapy and surgical correction. The initial plaster cast method of 4 to 6 weeks is followed by a functional mobilisation of the foot. The main aim being the reduction of the malpositioned talus in the ankle mortise. If there is residual deformity surgery is planned after six month. We use the Cincinnati approach with the possibility of the dorsal, medial and lateral release, enabling a correction of the hind-, mid- and forefoot. The main part of postoperative care is seen in the functional rehabilitati

Plating System Design Determines Mechanical Environment in Long Bone Mid-shaft Fractures: A Finite Element Analysis.

Purpose: Internal fixation device failure is rare but unavoidable. Fatigue-related mechanisms are responsible for most mechanical failures of orthopedic plates. Plate design needs to be optimized for both long fatigue life and bone healing. Only in this way can we reduce the occurrence of internal fixation failure. To address this issue, we aimed to provide a theoretical basis for improving the design of orthopedic plates. Material and Methods: The finite element method was used to conduct a computational investigation. Three groups of plate models were designed with varied working lengths and

Arthroscopically assisted stabilization of chronic AC-joint instabilities in GraftRope™ technique with an additive horizontal tendon augmentation.

PURPOSE: A chronic symptomatic acromioclavicular joint (ACJ) instability is a possible consequence of an acute ACJ separation. Besides vertical instability a horizontal component of the instability is common in high-grade ACJ separation and clinically relevant; especially, in chronic cases. A new technique of horizontal biologic ACJ augmentation with a transacromial gracilis tendon loop as an addition to the arthroscopically assisted stabilization with the GraftRope™ device is described and first clinical and sonographical results are shown. HYPOTHESIS: The results after arthroscopically assi

Arthrofibrosis after surgical fixation of tibial eminence fractures in children and adolescents.

BACKGROUND: Tibial eminence fractures are rare injuries in children and adolescents. Displaced fractures require reduction and fixation. Operative stabilization can be accomplished with either open or arthroscopic reduction and fixation. Whereas loss of extension has been reported, there are no reports in the literature that quantify loss of motion or provide guidance for treatment. PURPOSE: To report a series of patients who developed knee stiffness after operative treatment for displaced tibial eminence fractures. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Review of medical

1989 Volvo Award in basic science. Device-related osteoporosis with spinal instrumentation.

An animal model of anterior and posterior column instability was developed to allow in vivo observation of bone remodeling and arthrodesis after spinal instrumentation. After an initial anterior and posterior destabilizing lesion was created at the L5-L6 vertebral levels in 42 adult beagles, various spinal reconstructive surgical procedures were performed--with or without bilateral posterolateral bone grafting, and with or without spinal instrumentation (Harrington distraction; Luque rectangular, or Cotrel-Dubousset transpedicular methods). After 6 months' postoperative observation, there was
