Oral liver-targeted delivery systems for anti-hyperglycemic therapy: From passive to active targeting strategies.

Diabetes is a highly prevalent chronic disease worldwide. Traditional insulin injection is inconvenient, driving oral insulin as a promising research area. However, oral insulin delivery encounters multiple physiological barriers in the gastrointestinal tract. As an indispensable organ for glucose metabolism, the liver serves as one of the key targets for insulin to exert its hypoglycemic effects. Thus, the development of liver-targeted delivery systems provides a novel strategy for improving insulin efficacy and reducing side effects. This technology is expected to promote the preferential ut

Damage control surgery in lung trauma.

Damage control techniques applied to the management of thoracic injuries have evolved over the last 15 years. Despite the limited number of publications, information is sufficient to scatter some fears and establish management principles. The severity of the anatomical injury justifies the procedure of damage control in only few selected cases. In most cases, the magnitude of the physiological derangement and the presence of other sources of bleeding within the thoracic cavity or in other body compartments constitutes the indication for the abbreviated procedure. The classification of lung inj

Hollow viscus perforation after spine surgery: A focused review with illustrative cases.

BACKGROUND: Hollow viscus perforation following spine surgery is uncommon but associated with substantial morbidity and mortality when diagnosis is delayed. Early recognition is challenging because presenting symptoms frequently overlap with expected postoperative bowel dysfunction, and imaging findings may initially be subtle. Despite its clinical significance, the literature remains fragmented across case reports and small series, and a consolidated, mechanism-based framework specific to spine surgery is lacking. METHODS: We performed a single-institution case series of five patients who de

High-Accuracy Lower-Limb Intent Recognition: A KPCA-ISSA-SVM Approach with sEMG-IMU Sensor Fusion.

Accurately decoding human locomotion intention from physiological signals remains a significant hurdle for the seamless control of advanced rehabilitation devices like exoskeletons and intelligent prosthetics. Conventional recognition methods often falter, exhibiting limited accuracy and struggling to capture the complex, nonlinear dynamics inherent in biological data streams. Addressing these critical limitations, this study introduces a novel framework for lower-limb motion intent recognition, integrating Kernel Principal Component Analysis (KPCA) with a Support Vector Machine (SVM) optimize

Ionizing radiation induces cells with past caspase activity that contribute to the adult organ in Drosophila and show reduced Loss of Heterozygosity.

There is increasing recognition that cells may activate apoptotic caspases but not die, instead displaying various physiologically relevant consequences. Mechanisms that underlie the life-or-death decision in a cell that has activated apoptotic caspases, however, are incompletely understood. By optimizing a published reporter for past caspase activity, we were able to visualize cells that survived caspase activation specifically after exposure to ionizing radiation in Drosophila larval wing discs. We found that cells with X-ray-induced past active caspases (XPAC) did not arise at random but we

Alterations of the Plasma Peptidome Profiling in Colorectal Cancer Progression.

Early detection of colorectal cancer (CRC) remains a challenge. It has been highlighted that the pathological alterations within an organ and tissues might be reflected in serum or plasma proteomic/peptidic patterns. The aim of the study was to follow the changes in the plasma peptides associated to colorectal cancer progression by mass spectrometry. This study included 27 adenoma, 67 CRC (n = 33 I-II stage and n = 34 III-IV stage), 23 liver metastasis from CRC patients and 34 subjects disease-free as controls. For plasma peptides analysis, samples purification was performed on the Nanoporous

Cochlear Implant Rate Pitch and Melody Perception as a Function of Place and Number of Electrodes.

Six Nucleus cochlear implant recipients participated in a study investigating the effect of place of stimulation on melody perception using rate-pitch cues. Each stimulus was a pulse train delivered on either a single electrode or multiple electrodes sequentially. Four spatial stimulation patterns were used: a single apical electrode, a single mid electrode, a pair of electrodes (apical and mid), and 11 electrodes (from apical to mid). Within one block of trials, all stimuli had the same spatial stimulation pattern, with pulse rate varying from 131 to 262 pps. An additional pulse rate range of

NLRP1 and NLRP3 inflammasomes are essential for distinct outcomes of decreased cytokines but enhanced bacterial killing upon chronic Nod2 stimulation.

Upon chronic microbial exposure and pattern-recognition receptor (PRR) stimulation, myeloid-derived cells undergo a distinct transcriptional program relative to acute PRR stimulation, with proinflammatory pathways being downregulated. However, other host-response pathways might be differentially regulated, and this concept has been relatively unexplored. Understanding mechanisms regulating chronic microbial exposure outcomes is important for conditions of ongoing infection or at mucosal surfaces, such as the intestine. The intracellular PRR nucleotide oligomerization domain 2 (Nod2) confers th

Tissue- and time-dependent deployment of Toll- and Imd-associated immune responses in Manduca sexta larvae.

Little is known about how the Toll and Imd pathways are deployed across tissues and over time in insects outside the Drosophila lineage. To address this, we performed a systematic analysis of immune gene responses in Manduca sexta using deep RNA-seq of fat body, hemocytes, and midgut after aseptic wounding or microbial injection, identifying 1895 genes strongly induced by injury or infection. Immune challenge triggered extensive transcriptional reprogramming, including induction of pattern recognition receptors, serine protease cascades, cytokines, antimicrobial peptides (AMPs), and core immun

Activation of the Nlrp1b inflammasome by reduction of cytosolic ATP.

The efficacy of the innate immune system depends on its ability to mount an appropriate response to diverse infections and damaging agents. Key components of this system are pattern recognition receptors that detect pathogen-associated and damage-associated molecular patterns (PAMPs and DAMPs). Nlrp1b is a pattern recognition receptor that forms a caspase-1 activation platform, known as an inflammasome, upon sensing the proteolytic activity of anthrax lethal toxin. The activation of caspase-1 leads to the release of proinflammatory cytokines that aid in the clearance of the anthrax infection.

The Cellular Characterization of SARS-CoV-2 Spike Protein in Virus-Infected Cells Using the Receptor Binding Domain Binding Specific Human Monoclonal Antibodies.

A human monoclonal antibody panel (PD4, PD5, PD7, SC23, and SC29) was isolated from the B cells of convalescent patients and used to examine the S protein in SARS-CoV-2-infected cells. While all five antibodies bound conformational-specific epitopes within SARS-CoV-2 spike (S) protein, only PD5, PD7, and SC23 were able to bind to the receptor binding domain (RBD). Immunofluorescence microscopy was used to examine the S protein RBD in cells infected with the Singapore isolates SARS-CoV-2/0334 and SARS-CoV-2/1302. The RBD-binders exhibited a distinct cytoplasmic staining pattern that was primari

Systematic Review of Symptoms of Catatonia in Autism Spectrum Disorder.

Catatonia is a complex neuropsychiatric syndrome characterized by disturbances in mood, motor function, behavior and speech. It is increasingly recognized in individuals with autism spectrum disorder (ASD), although its identification remains challenging due to the overlapping clinical features of the two conditions. Shared characteristics, such as echophenomena, mannerisms, social indifference and repetitive behaviors can obscure accurate diagnosis. Although reports suggest a significant prevalence of catatonia among individuals with ASD, the condition remains poorly understood and frequently

The Role of serine/arginine-rich splicing factors in cardiovascular diseases.

Alternative splicing is a fundamental post-transcriptional mechanism that contributes to cardiovascular development, homeostasis, and disease. Serine/arginine-rich splicing factors (SRSFs), a conserved family of 12 RNA-binding proteins, regulate splice-site selection and other RNA-processing events through their RNA-recognition motifs and arginine/serine-rich domains. Growing evidence implicates SRSF dysregulation in congenital heart disease, cardiac hypertrophy and heart failure, ischemic injury, vascular remodeling, inflammatory myocardial injury, and cardiovascular aging. In this review, we

Proton-Sensitive Receptors/Channels: Therapeutic Target for Ischemic Stroke, Focusing on Their Roles and Mechanisms.

Ischemic stroke is a common cerebrovascular disease. However, its pathophysiological mechanisms and therapeutic targets remain unclear. In physiological states, the brain maintains an acid-base balance through various mechanisms that are crucial for normal brain function. However, during cerebral ischemia, the disruption of this balance leads to acidosis of the ischemic brain tissue, primarily due to the interruption of energy supply and alterations in ion homeostasis. Studies have indicated that proton-sensitive receptors/channels, such as acid-sensing ion channels, protonsensitive G-protein-

A Highly Permeable and Three-Dimensional Integrated Electronic System for Wearable Human-Robot Interaction.

Permeable electronics promise improved physiological comfort, but remain constrained by limited functional integration and poor mechanical robustness. Here, we report a three-dimensional (3D) permeable electronic system that overcomes these challenges by combining electrospun SEBS nanofiber mats, high-resolution liquid metal conductors patterned via thermal imprinting (50 μm), and a strain isolators (SIL) that protects vertical interconnects (VIAs) from stress concentration. This architecture achieves ultrahigh air permeability (> 5.09 mL cm-2 min-1), exceptional stretchability (750% fracture

Vocal quality factors: analysis, synthesis, and perception.

The purpose of this study was to examine several factors of vocal quality that might be affected by changes in vocal fold vibratory patterns. Four voice types were examined: modal, vocal fry, falsetto, and breathy. Three categories of analysis techniques were developed to extract source-related features from speech and electroglottographic (EGG) signals. Four factors were found to be important for characterizing the glottal excitations for the four voice types: the glottal pulse width, the glottal pulse skewness, the abruptness of glottal closure, and the turbulent noise component. The signifi

Electroclinical Features in Epilepsy Surgery Candidates With Epileptic Spasms.

Electroclinical features in surgical candidates with epileptic spasms differ significantly from the other focal epilepsy phenotypes. EEG findings tend to be more diffuse and less localizing in children with epileptic spasms. These are illustrated with five case studies to highlight three different categories of findings on interictal and ictal EEG: lateralizing , nonlateralizing , and false lateralizing . Hemihypsarrhythmia on interictal EEG is the most striking lateralizing abnormality that occurs in a minority of surgical candidates. Persistent focal epileptiform discharges in one region or

Assessment of time-frequency representation techniques for thoracic sounds analysis.

A step forward in the knowledge about the underlying physiological phenomena of thoracic sounds requires a reliable estimate of their time-frequency behavior that overcomes the disadvantages of the conventional spectrogram. A more detailed time-frequency representation could lead to a better feature extraction for diseases classification and stratification purposes, among others. In this respect, the aim of this study was to look for an omnibus technique to obtain the time-frequency representation (TFR) of thoracic sounds by comparing generic goodness-of-fit criteria in different simulated tho
